Different plasma levels of interleukins and chemokines： comparison between children and adults with aids in China

Background The immunological differences between children and adults with aids in China are not well documented. Th1/Th2 cytokines and chemokines are two types of immune factors intimately involved in disease progression of HIV-1 infection. This study aimed to identify changes in plasma levels of Th1/Th2 cytokines inerleukin （IL）-18, IL-16, IL-10 and chemokines regulated on activation, normal T cell expressed and secreted （RANTES）, stromal cell-derived factor-1 （SDF-1） and monocyte chemoattractant protein-1 （MCP-1） in HIV-l-infected children and adults in China. Methods Seventy-five children with aids and 35 adult aids patients were recruited and clinical data were collected. CD4^＋ T lymphocyte counts were measured by flow cytometery and plasma HIV RNA levels were measured by quantitative RT-PCR. Plasma levels of IL-18, IL-10, IL-16, RANTES, MCP-1, SDF-1a and SDF-1β were quantified by enzyme-linked immunosorbent assay. The levels of β2-microglobulin （β2-MG） and soluble Fas （sFas） were measured to validate the level of humoral and cellular immune activation. Results The mean levels of all cytokines in pediatric and adult aids patients were significantly higher than in their healthy controls （P 〈0.01）. The mean levels of these cytokines were higher in pediatric patients than in adult patients （P 〈0.05, except for SDF-la and β2-MG）. Some of the cytokine levels in patients younger than 6 years old was higher than in older children and adults with aids （IL-10, IL-18, SDF-la, MCP, RANTES and sFas, P 〈0.05）. Levels of IL-18, IL-10, RANTES and β2-MG of pediatric patients increased as the levels of viral load increased （P 〈0.05）. Conclusions Abnormal immune activation can be measured in Chinese pediatric and adult patients with aids, and is higher in children than in adult patients. The cytokines levels coincide with disease progression of aids, but have no direct relationship with total CD4^＋ T cell count.

The aim of this study was to investigate epidemiological characteristics of co-infection of HIV and intestinal parasites in a HIV/aids highly endemic area in China.A cross-sectional survey was carried out in two villages where HIV/aids prevalence in residents was over 1%.Stool samples of all residents in the two targeted villages were examined for the presence of intestinal *** examination was performed for the HIV infection detection and anemia test.A questionnaire survey was carried out in all residents participating in the study.A total of 769 individuals were enrolled in the investigation,720 of whom were involved in stool examination of intestinal *** infection rates of parasites in the residents of the targeted villages were as follows:0.56%for Ascaris lumbricoides,4.03%for Hookworm,0.28%for Trichuris trichiura,0.42%for Clonorchis sinensis,21.39%for Blastocystis hominis,3.89%for Giardia lamblia,1.67%for Entamoeba spp.,4.44%for Cryptosporidium spp.,and no infection for Strongyloides *** overall infection rate of intestinal worms was 4.72%,intestinal parasite infection rate was 24.31%,the anemia prevalence rate was 34.68%,the co-infection rate of HIV and intestinal helminthes 2.17%,the co-infection rate of HIV and intestinal protozoa 28.26%,of which the co-infection rates of HIV and Blastocystis hominis,and HIV and Cryptosporidium *** 19.57%and 13.04%,respectively.A signiﬁcant difference of Cryptosporidium *** rate was found between HIV-positive group(13.04%)and HIV-negative group(4.70%)(P50%)and women(>39%).It was concluded that the infection rate of intestinal protozoa in the residents was higher than that of intestinal helminthes in the local setting,and a same pattern appeared in the co-infection rate of HIV and *** interestingﬁnding is that the infection rate of Cryptosporidium *** the HIV-positiv

Assessing Tuberculosis among Smokers: Secondary Analysis of an Active Case Finding Project

Background: Previous case-control studies and a small number of cohort studies in high-risk populations have found an association between smoking and tuberculosis, but limited studies are available in the general population that predicts association of smoking and TB. Objectives: To investigate the association between smoking and tuberculosis in a cohort of a general population. Methods: Four districts of Punjab province (Lahore, Rawalpindi, Faisalabad and Islamabad) were randomly selected. In routine, household contact investigation is practiced by the national TB control programme. For two years from July 2013-June 2015, all people who were living within 50 metres from the household of smear positive TB patients were screened for tuberculosis. Those found presumptive TB were investigated through smear microscopy and those found smear negative;the Xpert MTB/RIF test was done. All the diagnosed TB patients were referred to nearest basic health unit for further treatment and care. Results: A total of 783,043 contacts were screened for tuberculosis, of whom 19,815 (2.53%) were smokers. Smoking was common among men, in diabetic & teenage, elderly age population and in household contact of smear positive TB patients. Smoking was associated with an increased risk of tuberculosis (odds ratio [OR], 2.43;95% confidence interval, 2.27 - 2.60) in household contact of smear positive TB patients. The association was stronger among those greater than 45 years of age (OR, 11.09) than those between 25 - 44 years of age (OR, 5.83) and diabetic persons (OR, 2.0). Conclusions: Smoking was associated with a twofold increased risk of active tuberculosis in a cohort of general population.

Emerging HIV Epidemic in the North-Eastern State of Assam, India: An Exploratory Study Using Mixed-Methods Approach

Background: The national HIV scenario in India has improved impressively during past two decades. Contrastingly, the State-level HIV sentinel surveil-lance and routine programme data from various testing facilities indicated an increasing HIV prevalence among antenatal clinic attendees and female sex workers (FSWs) in three districts of Assam. Objectives: 1) To explore the reasons behind increasing adult HIV prevalence and 2) To understand the role of FSW and Migrants/long distance truckers as drivers of the HIV epi-demic in these districts. Methodology: Three sources of data were utilized in this concurrent mixed methods study—1) existing programme data for past three years;2) questionnaire-based data for a case-control study, where dur-ing past six months at designated testing facilities HIV(+) married women were “cases” and age-matched HIV(?) women from the same district were “controls”. Spouses of these women were also interviewed separately;and 3) a qualitative study, where focus group discussions were conducted among FSWs, outgoing male migrant labourers and long-distance truckers. Results: The study revealed high levels of unsafe sexual practices among the FSWs. Sometimes in poverty-struck areas, women, not identifying themselves as sex workers, sold sex on a part-time basis to earn extra money for financial sup-port and often remained invisible to the programme. The clients of the sex workers, male migrants and truckers also revealed various risk behaviours for contracting and transmitting HIV infection. Conclusion: Population and context specific programme strategies are required to halt the rising trend of HIV infection in the general population in Assam.

Gonorrhoea among Sex Workers in China: Prevalence, Risk Factors, and Resistance to Antibiotics

Objective To study gonorrhoea among sex workers in China as to prevalence, risk factors, and resistance to antibiotics Subjects & Methods From March 1998 to October 1999, 966 sex workers from Guangzhou were recruited into a sexually transmitted diseases (STD) care and prevention programme through various outreach activities. Data collection was standardized. Cervical samples were tested for the presence of Neisseria (N.) gonorrhoea by culture. The agar dilution method was performed to determine susceptibilities of 85 isolates to penicillin, ciprofloxacin, spectinomycin and ceftriaxone. The production of B-lactamase was determined by paper acidometric testing. Results The prevalence of gonorrhoea was 9%. Independent predictors for the disease were inconsistent condom use, lack of regular salary, absence of a previous STD check-up in the preceding 12 months, and a concurrent trichomoniasis or chlamydial infection. Dysuria, vaginal malodour, and the presence of (muco) purulent discharge at genital examination were also independently associated with gonorrhoea. Of the 85 N. gonorrhoea isolates, 3.5% was penicillinase producing (PPNG) and 7% plasmid-mediated tetracycline resistant (TRNG). Of the non-PPNG strains 74% was insensitive to penicillin, while 52% and 47% were resistant or less susceptible to ciprofloxacin, respectively. One strain exhibited decreased susceptibility to ceftriaxone. All strains were susceptible to spectinomycin. Conclusion A high level of resistance to penicillin and ciprofloxacin was seen, but strains were sensitive to ceftriaxone and spectinomycin. Several personal or medical characteristics were independently associated with the presence of gonorrhoea. Women diagnosed with gonorrhoea should receive concurrent treatment for chlamydial infection.

Impact of TI programmes Intervention under national aids control programme among Female Sex Workers in India: Evidence from Integrated Biological and Behavioural Survey, 2014-15

Background: A better understanding of the impact of Targeted Interventions (TI) services on Female Sex Worker (FSW) behaviours can help in informing and strengthening future TI efforts under national aids control programme (NACP). Methods: national Integrated Biological and Behavioural Survey (IBBS) 2014-15 has been analysed in the paper. Descriptive statistics and logistic regression analysis were used to understand factors affecting condom use. Propensity score matching (PSM) was done to understand the impact of the TI services on condom use at the last act and consistent condom use in the last three months among FSWs. Results: Younger FSWs aged 15 - 25 years were 1.4 times (95% CI: 1.04 - 1.77) more likely to use consistent condoms in the last three months compared to older over 45 years after controlling for other socio-economic and programme variables. The matched samples estimate ATET i.e. the average treatment effect on treated for consistent condom use in the last three months shows that a 6.0% (CI: 4.7 - 7.3) increase in consistent condom use in the last three months in the FSWs who received information on STI/HIV from peer educator and outreach worker. There is also a 6.0% (4.7 - 7.2) increase in condom use in the last act for FSWs who received the referral services at STI clinics, HIV testing, and detox centres. Conclusions: It is evident that the TI programme has a positive impact on behavior change among FSWs which can prove beneficial to curtail the spread of HIV to the partners and onward transmission to the general population.
