Objective: To report the experience of surgical caravans for urogenital fistulas care. Methodology: This was a retrospective study covering the period from January 2014 to December 2014 and which took place on 7 sites of fistula care. The epidemiological, anatomoclinical, therapeutic and evolutionary aspects were studied. Results: 346 patients were operated during 14 caravans. Their average age was 33.11 years (range: 12 to 70 years). Most of these patients were without remunerative activities (80%) and without education (63.3%). The average duration of fistula progression was 6.08 years (range: 0 to 42 years). Obstetrical etiology was predominant (87.9%). According to the classification of Kees Waaldijk, fistulas were divided into type I (67.4%), type II (21.1%), and type III (11.5%). The most common surgical approach was the transvaginal route (82.1%). The vesicovaginal splitting with separated suture of the bladder and the vagina was the basic technique (94.7%). The therapeutic results were judged after a follow-up of 1 month and 3 months. Across the cohort, 80 patients (23.1%) were lost of sight for follow-up at 1 month and 245 (70%) at 3 months. The success rates evaluated in patients reviewed at 1 month and 3 months were respectively 70% and 64%. Conclusion: The incidence of urogenital fistulas is still high in Cote d'Ivoire. Various anatomoclinical varieties have been identified and treated with satisfactory results in poorly equipped local structures.

Climate change is described as the most universal and irreversible environmental problem facing the planet Earth. While climate change is already manifesting in Ethiopia through changes in temperature and rainfall, its magnitude is poorly studied at regional levels. The objective of this paper was to assess and quantify the magnitude of future changes of climate parameters using Statistical Downscaling Mode (SDSM) version 4.2 in Amhara Regional State which is located between 8°45‘N and 13°45‘N latitude and 35°46‘E and 40°25‘E longitude. Daily climate data (1979- 2008) of rainfall, maximum and minimum temperatures were collected from 10 observed meteorological stations (predictand). The stations were grouped and compared using clustering and Markov chain model, whereas the degree of climate change in the study area was estimated using the coupled HadCM3 general circulation model (GCM) with A2a emission scenarios (Predictors). Both maximum and minimum temperatures showed an increasing trend;the increase in mean maximum temperature ranges between 1.55°C and 6.07°C and that of the mean minimum temperature ranges from 0.11°C and 2.81°C. While the amount of annual rainfall and rainy days decreased in the study Regions in the 2080s. The negative changes in rainfall and temperature obtained from the HadCM3 model in the current study are alarming and suggest the need for further study with several GCM models to confirm the current results and develop adaptation options.

The impact of low-quality irrigation water on plant development has garnered significant attention from researchers. In light of this, two field experiments were conducted to evaluate the performance, yield, oil production and composition, as well as active constituents of Rocket(Eruca sativa Mill) cultivated in calcareous soil under saline water irrigation. Foliar sprays containing condensed molasses soluble(CMS), zinc(Zn), and boron(B) alone or in combination were used for irrigation. The data obtained from measuring various parameters of Rocket following foliar spraying with CMS, Zn, B or their combinations demonstrated that most treatments resulted in a significant increase in these parameters. The highest values for most measurements were observed when foliar application included all three components(CMS + Zn + B), resulting in a seed yield of 184.6 g/m2and an oil content of 675.3 kg/ha. Compared to the control group, the macronutrient content of N, P, K, Mg, and Ca increased by 34.4%, 56%, 42%, 45%, and 39% respectively in the seeds treated with these ***, carbohydrates, proteins, phenolics flavonoids, and antioxidants showed increases of 24%, 34%,21%, 43%, and 28% respectively compared to the control group. Gas-liquid chromatography analysis identified ten components present in the seed oil characterized by higher unsaturated fatty acids ranging from 81.28% to92.28% and lower saturated fatty acids ranging from 6.72% to 8.21%. Therefore, foliar spray application including CMS, zinc, and boron can help alleviate salinity effects on Rocket plants grown under saline water irrigation conditions while improving growth, yield, oil production, and nutritional content such as total carbohydrates, proteins, and macronutrients levels.

Humans and animals can be exposed to Dioxins and Furans through ingestion of fatty food, skin contact, and breathing contaminated air. Resulting health problems include skin disease, immune problems, and cancers. Managing the release of these chemicals is therefore important. The Stockholm Convention on Persistent Organic Pollutants requires parties to adopt measures that reduce sources of these chemicals. Sierra Leone developed its national Implementation Plan (NIP) in 2008, in compliance with this requirement. However, no known further steps have been taken, particularly at community level. In 2013, the UNDP’s GEF/SGP funded a pilot project to manage the release of Dioxins and Furans from two dumpsites in Freetown, Sierra Leone. This work was sponsored with funds from that project. The work sought to delineate the exposure of communities within and around the Bormeh-Kingtom dumpsite (Kingtom, Kolleh town, Ascension town, Crab town, and Congo town) to the chemicals. The study considered sources of exposure, relative quantities generated per year, and the routes of exposure. This exposure study is a first step in managing the release from those sources. Enumerators deployed at the dumpsite every day, 7:00 AM to 6:00 PM, for three months, quantifying and recording source materials. The amount of source materials dumped per day informed calculation of Dioxin/Furan releases in grams toxicity equivalence per year. The results revealed a release of 128.914 g TEQ/year in air and residue. This implies that the communities are at high risk of inhalation and dermal exposure. Livestock, mainly pigs, are also exposed as they feed on the waste deposited in the dumpsite. Humans in turn feed on the livestock, a recipe for biomagnification. The project team has been working on developing Best Management Practices to suppress the release of the chemicals.

Double layered self-expanding metal stents for malignant esophageal obstruction, especially across the gastroesophageal junction

AIM: To evaluate the clinical outcomes of double-layered self-expanding metal stents (SEMS) for treatment of malignant esophageal obstruction according to whether SEMS crosses the gastroesophageal junction (GEJ). METHODS: Forty eight patients who underwent the SEMS insertion for malignant esophageal obstruction were enrolled. Patients were classified as GEJ group (SEMS across GEJ, 18 patients) and non-GEJ group (SEMS above GEJ, 30 patients) according to SEMS position. Double layered (outer uncovered and inner covered stent) esophageal stents were placed. RESULTS: The SEMS insertion and the clinical improvement were achieved in all patients in both groups. Stent malfunction occurred in seven patients in the GEJ group and nine patients in the non-GEJ group. Tumor overgrowth occurred in five and eight patients, respectively, food impaction occurred in one patient in each group, and stent migration occurred in one and no patient, respectively. There were no significant differences between the two groups. Reflux esophagitis occurred more frequently in the GEJ group (eight vs five patients, P = 0.036) and was controlled by proton pump inhibitor. Aspiration pneumonia occurred in zero and five patients, respectively, and tracheoesophageal fistula occurred in zero and two patients, respectively. CONCLUSION: Double-layered SEMS are a feasible and effective treatment when placed across the GEJ for malignant esophageal obstruction. Double-layered SEMS provide acceptable complications, especially migration, although reflux esophagitis is more common in the GEJ group.
