A highly practical asymmetric cpoxidation procedure for the synthesis of optically pure epoxychromans has been developed which employs 30% hydrogen peroxied as the oxidant and the Jacobsen-catalyst as the catalyst. The presence of imidazole has a significant effect on epoxidation rates and total turnovers. A variety of chromene derivatives were converted into corresponding epoxides of 89～100% ee in 72～92% isolated yields.

AIM: To generate recombinant adenoviral vector con-taining calreticulin (CRT)-hepatitis B surface antigen (HBsAg) fusion gene for developing a safe, effective and HBsAg-specific therapeutic ***: CRT and HBsAg gene were fused using polymerase chain reaction (PCR), endonuclease diges-tion and ligation methods. The fusion gene was cloned into pENTR/D-TOPO transfer vector after the base pairs of DNA (CACC) sequence was added to the 5′ end. Adenoviral expression vector containing CRT-HBsAg fusion gene was constructed by homologous recombinan-tion. The human embryo kidney (HEK) 293A cells were transfected with linearized DNA plasmid of the recombi-nant adenoviral vector to package and amplify recombi-nant adenovirus. The recombinant adenovirus titer was characterized using the end-dilution assay. The expres-sion of the CRT/HBsAg fusion protein in Ad-CRT/HBsAg infected 293A cells was detected by Western ***: The CRT-HBsAg fusion gene was char-acterized by PCR and sequencing and its length and sequence were confirmed to be accurate. The CRT-HB-sAg fusion gene recombinant pENTR/D-TOPO transfer vector was constructed. The recombinant adenoviral vector, Ad-CRT/HBsAg, was generated successfully. The titer of Ad-CRT/HBsAg was characterized as 3.9 × 1011 pfu/mL. The CRT-HBsAg fusion protein was ex-pressed by HEK 293A cells correctly. CONCLUSION: CRT/HBsAg fusion gene recombinant replication-defective adenovirus expression vector is constructed successfully and this study has provided an experimental basis for further studies of Hepatitis B vi-rus gene therapy.

BACKGROUND: During the past years, the number of liver transplantation has increased greatly, but the number of available organs has not increased. In view of the critical shortage of organs, the indications for living-related liver transplantation (LRLT) have broadened since experience with the procedure has been achieved. This study was undertaken to assess the value of multi-slice spiral CT (MSCT) angiography in evaluating the hepatic arterial and veinous anatomy of potential donors for LRLT. METHODS: MSCT was performed after intravenous injection of contrast material at 3 ml/s. The total dose was calculated as 2 ml/kg. Twenty LRLT donors (2 men and 18 women) were subjected to MSCT angiography of hepatic blood vessels. These were generated by volume rendering and maximum intensity projection, while curved planar reformation was added in 5 patients. RESULTS: We identified 10 important hepatic vascular variants in 9 of the 20 donors (4 arterial, 4 venous, and 2 portal venous variants). In hepatic arterial variants, two had a replaced right hepatic artery arising from the superior mesenteric artery, an accessory right hepatic artery from the superior mesenteric artery and a replaced left hepatic artery arising from the left gastric artery. In hepatic venous variants, three had an accessory inferior right hepatic vein and one had two accessory inferior right hepatic veins. In hepatic portal venous variants, two had trifurcation of the main portal vein. CONCLUSIONS: As a non-invasive and reliable method, MSCT angiography is of value in the clinical evaluation of LRLT donors. MSCT angiography should be recommended as a routine preoperative examination for potential LRLT donors.

Background Frequent premature ventricular complexes from the right ventricular outflow tract （RVOT-PVCs） are associated with left ventricular *** study adopted two-dimensional speckle tracking imaging to evaluate global and regional left ventricular myocardial function in patients with frequent *** This study included 30 patients with frequent RVOT-PVCs and 30 healthy *** systolic velocity-time integral （AoVTI） and myocardium strain in circumferential （CS）,radial （RS） and longitudinal （LS） directions were evaluated by conventional echocardiography and speckle tracking *** values of patients with RVOT-PVCs were recorded during sinus （PVC-S） and PVC beats （PVC-V）.Results Significant differences were demonstrated in global CS,RS and LS between the control subjects and the PVC-V （CS：（17.46±2.48）% vs.（11.52±3.28）%,RS：（48.26±10.20）% vs.（20.92±9.78）%,LS：（19.89±2.62）% vs.（11.79±3.66）%,P 〈0.01）,and in segmental RS and LS of nearly all the left ventricular *** differences in segmental CS between the PVC-V and the control subjects were only observed in anterior,anteroseptal and septal segments （only seen in anteroseptal and septal segments at apex）.Furthermore,V/S AoVTI （AoVTI during the PVC beat divided by AoVTI during the sinus beat,then multiplied by 100%） correlated with coupling interval （r=0.67,P 〈0.001） and global strain （CS：r=0.48,P=0.007; RS：r=0.65,P 〈0.001; LS：r=0.65,P 〈0.001）.Conclusions Frequent RVOT-PVCs can induce global and regional left ventricular systolic *** reduction of hemodynamic parameters relates to the coupling interval and the global systolic function.

Effectiveness and Safety of Chinese medicine for Idiopathic Pulmonary Fibrosis: A Systematic Review and Meta-Analysis

Objective: To evaluate the effectiveness and safety of Chinese medicine(CM) for Idiopathic pulmonary fibrosis(IPF) patients. Methods: To screened relevant articles, Pub med, Cochrane Library, Excerpta medica Datase(EMBASE), China National Knowledge Infrastructure(CNKI), Chinese VIP Information(VIP), Wanfang Database and Chinese Biomedical Database(CBM) were searched in English or Chinese until December 2015 for randomized controlled trials, which compared CM treatment(CM group) with Western medicine or placebo(control group) on IPF. The outcome measures included acute exacerbation, pulmonary function, the St George’s respiratory questionnaire(SGRQ) scores, 6-minute walk test(6 MWT) distance, adverse events and mortality. Results: This meta-analysis included 25 randomized controlled trials involving 1,471 patients. Compared with the control group, CM group was superiori in reducing the risk of exacerbation [relative risk(RR)=0.40, 95% CI 0.22 to 0.72, P0.05). This meta-analysis also revealed that CM therapy significantly decreased the SGRQ score(SMD=–0.60, 95% CI –1.14 to –0.05, P0.05) between CM and control groups. Conclusions: The pooled outcomes suggest that CM treatment appears benefit in reducing the risk of exacerbation, improving lung function and decreasing the incidence of adverse effects and enhancing the quality of life. AIM：To determine the dominant predictive factors of postoperative visual recovery for patients with pituitary ***：Pub med,Google Scholar,Web of Science and Cochrane Library were searched for relevant human studies,which investigated the prediction of the postoperative visual recovery of patients with pituitary adenoma,from January2000 to May ***-analyses were performed on the primary *** the related data were extracted by two independent investigators,pooled weighted mean difference（WMD）and odds ratio（OR）with 95%confidence interval（CI）were estimated using a random-effects or a fixed-effects ***：Nineteen studies were included in the literature review,and nine trials were included in the Metaanalysis,which comprised 530 patients（975 eyes）with pituitary *** the primary outcomes,there was a significant difference between preoperative and postoperative mean deviation（MD）values of the visual field（WMD-5.85;95%CI：-8.19 to-3.51;P〈0.00001）.Predictivecharacteristics of four factors were revealed in this Metaanalysis by assigning the patients to sufficient and insufficient groups according to postoperative visual field improvements,including preoperative visual field defect（WMD 10.09;95%CI：6.17 to 14.02;P〈0.00001）,patient age（WMD-12.32;95%CI：-18.42 to-6.22;P〈0.0001）,symptom duration（WMD-5.04;95%CI：-9.71 to-0.37;P=0.03）,and preoperative peripapillary retinal nerve fiber layer（p RNFL）thickness（OR 0.1;95%CI：0.04 to 0.23;P〈0.00001）.CONCLUSION：Preoperative visual field defect,symptom duration,patient age,and preoperative p RNFL thickness are the dominant predictive factors of the postoperative recovery of the visual field for patients with pituitary adenoma.

BACKGROUND: A simultaneously transplanted liver shields a bowel graft from immunologic attack in small animals, while the possible immuno-tolerance induced by the liver in liver and small bowel transplantation (LSBT) is uncertain in large animal models. To investigate the clinically suspected beneficial effect of the liver on small bowel allograft, we developed a new model of composite LSBT in the pig. METHODS: Seventy outbred long-white pigs were randomized into four groups. LSBT without immunosuppressive treatment (n=10, group A); LSBT with routine immunosuppressive treatment (n=10, group B); LSBT with a lower dose of immunosuppressive treatment (n=10, group C); and small bowel segment allotransplantation without immunosuppressive treatment (n=10, group D). RESULTS: There was no remarkable difference in survival time between groups A and D (10.33 vs. 12.89 days, P>0.05), but the initial time of acute rejection of the intestinal graft in group A was clearly delayed when compared to group D (8.22 vs. 4.33 days, P0.05), initial acute rejection time (25.40 vs. 22.13 days, P>0.05) or rejection score did not differ between groups B and C within one postoperative month. CONCLUSIONS: Compared to isolated segment small bowel allotransplantation, the intestinal graft in LSBT (group A) had a delayed initial time of acute rejection and a lower postoperative acute rejection score, and a lower dose of immunosuppressive treatment led to persistent graft immuno-tolerance in LSBT. OBJECTIVE: To conduct a review of the systematic reviews and meta-analyses on the Traditional Chinese medicine(TCM) treatment of type 2 diabetes mellitus(T2DM).METHODS: We searched Pub med, Cochrane, Web of Science, Chinese Biomedical Literature Database(CBM), China National Knowledge Infrastructure(CNKI), China Science and Technology Journal Database(VIP), Wanfang, and other databases from database inception to May 2014 for systematic reviews and meta-analyses on TCM treatment of T2 DM. Manuscripts were read by two investigators if they met the inclusion criteria, and data were extracted. A Measurement Tool to Assess Systematic Reviews(AMSTAR) was used to classify research quality, and the evidence quality was graded by the Grade of Recommendation, Assessment, Development, and Evaluation(GRADE) ***: Eighteen systematic reviews and meta-analyses were considered. Fifteen analyzed the efficacy of Chinese herbal medicines, and three investigated the efficacy of acupuncture. AMSTAR evaluation ranged from 3-10, and re-evaluation of the main results implied that treatment of T2 DM with TCM has certain advantages when compared with conventional Western medicine. However, the evidence quality was generally ***: This work shows favorable evidence for the clinical treatment of TCM on T2 DM. However, it is recommended that evidence-based decisions are made based on clinical trials because of the GRADE scores of the studies. To achieve higher quality of clinical research, clinical research on TCM requires specific and suitable research methods. Further trials may increase the quality of evidence to evaluate the clinical efficacy of TCM for T2 DM.
