Introduction: Respiratory distress (RD) is a major emergency to which infants are particularly vulnerable. It can lead to neurological sequelae and even death when treatment is not adequate and rapid. Objective: To evaluate the management of RD in infants at the CNHU-HKM in Cotonou. Methods: The study was of a transversal and analytical nature and took place over a period of 06 months, from 1st January to 30th June 2015. Included in the study were all infants hospitalised for DR. The therapeutic modalities were analysed according to the recommendations of the World Health Organization. Results: A total of 96 infants were included. The hospital frequency of DR was 38%. The average age was 12 months. One in 3 infants had SaO2 less than 90% on admission, and in 58.3% of cases, the infant showed at least 3 signs of struggle. The initial assessment was as recommended. The systematic use of oxygenrequires a revision of the criteria for oxygentherapy in the unit. The three main causes were severe pneumonia (31.3%), severe malaria (18.8%) and bronchiolitis (15.6%), and their treatment was correct. However, none of the infants had been able to benefit from ventilatory support. Mortality was high (31.2%) linked to the intensity of DR (p = 0.04) and sepsis (p = 0.006). Background In order to make posterior fossa decompression for the management of Chiari I malformation simple and less invasive while using direct visualization, a novel solely endoscopic procedure has been employed for the decompression of Chiari malformation type I. The objective of this study was to present neural endoscopic posterior fossa decompression and atlas laminectomy for Chiari type I patients. Methods Twenty-one patients with Chiari type I underwent neural endoscopic posterior fossa decompression and atlas laminectomy. We described the procedure for neural endoscopic posterior fossa decompression and atlas laminectomy. All patients in this series demonstrated cerebellar tonsil herniation below the foramen magnum in addition to syringomyelia. All patients in the reviewed study underwent preoperative MRI as well as 3-month postoperative MRI. Additional follow-up MRI varied but was usually repeated 12 months to 18 months after surgery. Postoperative MRI studies were retrospectively reviewed and compared with preoperative studies. Results All patients showed clinical improvements, and none had any complications. Patients with syringomyelia had symptoms entirely disappear. Eleven patients （52.4%） experienced radiographic improvement in syringomyelia （decreased size or resolution） during the follow-up period. Nine patients （42.8%） demonstrated decreased syrinx size and four （19%） demonstrated resolved syrinx. Of the 15 patients with symptomatic syringomyelia, 11 （73.3%） experienced symptomatic improvement. The median time to symptom improvement was four months after surgery. Post surgical MRI examinations indicated complete and sufficient decompression of foramen magnum region. Conclusions Endoscope atlanto-occipital decompression surgery is an innovative, safe and effective surgical procedure. Background We reviewed the time taken for post-portoenterostomy(PE)biliary atresia(BA)patients to obtain jaundice-clearance(total bilirubin≤1.2 mg/dL;JC)post-PE to determine if JC time(JCT)is prognostic for survival of the native liver(SNL).Methods The subjects were 66 BA patients treated with PE at our institute between 1989,the year when liver transplan-tation(LTx)became available in Japan,and *** was used to create three groups(≤30 days:n=14;31-60 days:n=31;≥61 days:n=21).Medical records were reviewed retrospectively to evaluate:age at onset of symptoms,duration of symptoms pre-PE,age and weight at PE,serum liver function tests,incidence of cholangitis,and micro-bile duct size at *** Age at onset of symptoms,age and weight at PE,duration of symptoms pre-PE,and micro-bile duct size were similar for all patients in all three *** and SNL appeared to correlate because preoperative total bilirubin(7.1,9.6,10.2 mg/dL;P<0.05)was significantly lower in the JCT≤30 days group(P<0.05)while there was a significant decrease in SNL(P<0.03)and a significant increase in LTx(P<0.01)in the JCT≥61 days *** LTx subjects who achieved JC were found to have developed cholangitis within 3 months of *** During the follow-up of post-PE subjects,longer JCT and cholangitis occurrence within 3 months of PE would appear to be negative prognostic factors for SNL while preoperative total bilirubin would appear to be a positive prognostic factor for SNL.

AIM: To show that aluminum potassium sulfate and tannic acid(ALTA) sclerotherapy has a high success rate for Grade Ⅱ and Ⅲ ***: This study was based on the clinical data of 604 patients with hemorrhoids who underwent ALTA sclerotherapy between January 2009 and February 2015. The objective of this study was to assess the efficacy of this treatment for Grades Ⅱ and Ⅲ hemorrhoids. Preoperative and postoperative symptoms, complications and success rate were all assessed retrospectively. Follow-up consisted of a simple questionnaire, physical examination and an anoscopy. Patients were followed-up at one day, one week, two weeks, one month, one year, two years, three years, four years and five years after the ALTA ***: One hundred and sixty-nine patients were diagnosed with Grade Ⅱ hemorrhoids and 435 patients were diagnosed with Grade Ⅲ hemorrhoids. The one year, three year and five year cumulative success rates of ALTA sclerotherapy for Grades Ⅱ and Ⅲ hemo-rrhoids were 95.9% and 93.1%; 89.3% and 83.7%; and 89.3% and 78.2%, respectively. No significant differences were observed in the cumulative success rates after ALTA sclerotherapy between Grades Ⅱ and Ⅲ hemorrhoids(P = 0.09). There were forty-seven post-operative complications(low grade fever; anal pain; urinary retention; rectal ulcer; and others). No serious or life-threatening complications occurred and all cases improved through conservative treatment. At univariate analysis there were no predictive factors of ***: ALTA sclerotherapy has had a high success rate for Grade Ⅱ and Ⅲ hemorrhoids during five years of post-operative treatment. However, additional studies are needed to evaluate the efficacy of this ALTA sclerotherapy in the management of hemorrhoidal disease.

AIM. To develop a new experimental model of esophagitis that serves a complementary tool to clinical investigation in an insight into the mechanism of the damage to the esophagus mucosa by aggressive factors, and role of COX inhibitors in this process. METHODS： The study was conducted in 56 male mice. Animals were divided into seven groups：（1） perfused with HCI, （2） perfused with HCI and physiologic concentration of pepsin （HCI/P）, （3） perfused with similar HCI/P solution enriched with conjugated bile acids （glycho- and tauro-sodium salts） designated esophageal infusion catheter under the general anesthesia, （4） perfused as in group 2 treated with indometacin, （5） perfused as in group 2 treated with NS-398, （6） perfused as in group 3 treated with indometacin, and （7） perfused as in group 3 treated with NS-398. The esophagus was divided into 3 parts： upper, middle and lower. The PGE2 concentration was measured in all parts of esophagus using RIA method. Esophagus of sacrificed animals was macroscopically evaluated using a low power dissecting microscope （20x）. Specimeris, representing the most frequently seen changes were fixed, stained with H＆E and assessed microscopically using the damage score, and inflammatory score. RESULTS： The macroscopic changes were significantly severer in HCI/P than those in HCI animals （77%） and in HCI/P/BA group （43%）. In HCI/P NS-398 group we noticed significantly less changes than those in not treated group （42%） and in analogical group treated with indometacine （45%）. In HCI/P/BA INDO group we observed significantly severer changes than that in not treated group （52%）. We noticed less changes in HCI/P NS-398 than that in group with indometacine （46%）. In HCI/P/BA NS-398 group we had less changes than that in indometacin group （34%）. The microscopic changes observed in HCI/P/BA INDO group were severer than that in not treated group （48%）. Is Caries in the Early Mixed Dentition Associated with Caries Development during Orthodontic Treatment?

Objective: To investigate if caries in the pre-treatment early mixed dentition is associated with caries development in the permanent dentition during orthodontic treatment. Material and Methods: We included 41 consecutive patients (29 girls, 12 boys) with a pre-treatment documentation in the mixed (mean age 9.4 years) and a post treatment documentation in the permanent dentition (age 14.9 years) (two-phased treatment time 4.5 (±1.6) years). The DMFT/S indices were calculated. High-risk and low-risk groups were defined according to dmfs + DMFS score before treatment. Results: Initial dmft/s + DMFT/S (SD) was 5.15 (3.60) and 8.32 (6.64);final DMFT/S was 2.76 (2.84) and 3.01 (3.20). The missing (because of decay) second deciduous molars were most powerfully associated with caries increment during treatment, showing significant correlations to second premolars (r = 0.47, p = 0.003), while fillings on second deciduous molars seem to influence the prevalence of fillings on permanent molars (r = 0.44, p = 0.001). The increment at surface level was 2.01 (2.61) in the whole sample and 2.60 (3.81) in the high-risk group (4 boys, 6 girls). Compared to the low-risk group (10 girls), post-treatment caries experience was significantly higher in the high-risk group (p = 0.029). Boys were more at risk than girls (p = 0.005). Conclusions: Children with elevated caries experience in the early mixed dentition are exposed to higher caries risk during orthodontic treatment. Thus, in prevision of treatment, caries should already be assessed in the mixed dentition, so that an extended prophylaxis program can be initiated.
