Matrix metalloproteinase-9 contributes to parenchymal hemorrhage and necrosis in the remnant liver after extended hepatectomy in mice

AIM： To investigate the effect of matrix metallopro- teinase-9 （MMP-9） on the remnant liver after massivehepatectomy in the mouse.METHODS： Age-matched, C57BL/6 wild-type （WT）, MMP-9（-/-）, and tissue inhibitors of metalloprotein- ases （TIMP）-1（-/-） mice were used. The mice received 80%-partial hepatectomy （PH）. Samples were obtained at 6 h after 80%-PH, and we used histology, immuno- histochemical staining, western blotting analysis and zymography to investigate the effect of PH on MMP-9. The role of MMP-9 after PH was investigated using a monoclonal antibody and MMP inhibitor.RESULTS： We examined the remnant liver 6 h after 80%-PH and found that MMP-9 deficiency attenuated the formation of hemorrhage and necrosis. There were significantly fewer and smaller hemorrhagic and ne- crotic lesions in MMP-9（-/-） remnant livers compared with WT and TIMP-1（-/-） livers （P 〈 0.01）, with no dif- ference between WT and TIMP-1（-/-） mice. Serum ala- nine aminotransaminase levels were significantly lower in MMP-9（-/-） mice compared with those in TIMP-I（-/-） mice （WT： 476± 83 IU/L, MMP-9（-/-）： 392 ± 30 IU/L, TIMP-I（-/-）： 673 ± 73 IU/L, P 〈 0.01）. Western blot- ting and gelatin zymography demonstrated a lack of MMP-9 expression and activity in MMP-9（-/-） mice, which was in contrast to WT and TIMP-1（-/-） mice. No change in MMP-2 expression was observed in any of the study groups. Similar to MMP-9（-/-） mice, when WT mice were treated with MMP-9 monoclonal antibody or the synthetic inhibitor GM6001, hemorrhagic and necrotic lesions were significantly smaller and fewer than in control mice （P 〈 0.05）. These results suggest that MMP-9 plays an important role in the development of parenchymal hemorrhage and necrosis in the small remnant liver.CONCLUSION： Successful MMP-9 inhibition attenuates the formation of hemorrhage and necrosis and mightbe a potential therapy to ameliorate liver injury after massive hepatectomy.

The application of the strip-shaped cymba conchae orthosis in the nonsurgical correction of complex auricular deformity

Objective: This study aims to evaluate the efficacy and safety of using a strip-shaped cymba conchae orthosis for the nonsurgical correction of complex auricular deformities. Methods: clinical data were collected from 2020 to 2021 for 6 patients who underwent correction using a stripshaped cymba conchae orthosis. The indications, corrective effects, and complications associated with use of the orthosis were analyzed. Results: There were four indications for treatment: cryptotia with helix adhesion;cryptotia with grade I microtia;cryptotia with excessive helix thickness;and auricular deformity beyond the treatment time window(≥6 months). Excellent corrective effects were observed in all 6 patients. Complications occurred in one patient, who recovered after symptomatic treatment. Conclusion: The use of a strip-shaped cymba conchae orthosis alone or combined with a U-shaped helix orthosis presents a feasible approach for correcting complex auricular deformities or deformities beyond the treatment time window in pediatric patients.

Postoperative intestinal obstruction caused by Meckel’s diverticulitis after appendectomy

Surgical treatment of the complications associated with Meckel’s diverticulum is widely accepted, but surgical management of an asymptomatic diverticulum found incidentally during laparotomy for other reasons is controversial. Appendectomy is the most common reason for laparotomy in many pediatric surgery clinics. The most common causes of an intestinal obstruction in the early period after appendectomy are an intra-abdominal abscess, adhesive small bowel obstruction and invagination. Obstruction due to Meckel’s diverticulum is reported rarely. In this study, we present 2 cases, five and eleven years old, with an asymptomatic Meckel’s diverticulum where appendectomy was performed due to acute and perforated appendicitis and obstruction developed due to Meckel’s diverticulitis in the postoperative early stage. Meckel’s diverticulitis must be considered in intestinal obstruction cases developing in the early stages after appendectomy although quite rare.

Lymphocytic esophagitis: Still an enigma a decade later

Lymphocytic esophagitis (LE) is a clinicopathologic entity first described by Rubio et al in 2006. It is defined as peripapillary intraepithelial lymphocytosis with spongiosis and few or no granulocytes on esophageal biopsy. This definition is not widely accepted and the number of lymphocytes needed to make the diagnosis varied in different studies. Multiple studies have described potential clinical associations and risk factors for LE, such as old age, female gender and smoking history. This entity was reported in inflammatory bowel disease in the pediatric population but not in adults. Other associations include gastroesophageal reflux disease and primary esophageal motility disorders. The most common symptom is dysphagia, with a normal appearing esophagus on endoscopy, though esophageal rings, webs, nodularities, furrows and strictures have been described. Multiple treatment modalities have been used such as proton pump inhibitors and topical steroids. Esophageal dilation seems to be therapeutic when dysphagia is present along with esophageal narrowing secondary to webs, rings or strictures. The natural history of the disease remains unclear and needs to be better delineated. Overall, lymphocytic esophagitis seems to have a chronic and benign course, except for two cases of esophageal perforation in the literature, thought to be secondary to this entity.

Efficacy of oral magnesium therapy in the treatment of chronic constipation in spastic cerebral palsy children:a randomized controlled trial

Background Constipation is a common problem in children with spastic cerebral palsy(sCP)with a prevalence that reaches 75%.We hypothesized that treating constipation in those children will improve their health and shorten time spent in daily *** aim was to evaluate the efficacy and safety of oral magnesium sulfate for treating chronic constipation in children with *** A prospective,double-blinded randomized control trial was carried out involving 100 children aged 2-12 years with sCP(level Ⅲ-Ⅴ of the Gross Motor Functional Classification system)and chronic *** were followed up in the pediatric neurology clinic,Children's hospital,Ain Shams University,May 2017-January *** intervention group(O-Mg)received oral magnesium sulfate 1 mL/kg/day daily for 1 month compared to the *** measures were constipation improvement and decrease in bowel evacuation time after 1 *** Initially,weekly bowel movements,constipation scores and stool consistency were comparable in both *** 1 month of regular administration of oral magnesium sulfate,the constipation score,stool frequency and consistency improved compared to the placebo group(P<0.001).Effective safe treatment was achieved in 31(68%)and 4(9.5%)patients in the O-Mg and placebo groups,respectively(RR,2.95;95%CI 2.0-4.5)(P<0.001).Painful bowel evacuation attempts spent by mothers decreased from 25(55.6%)of the cases initially to 10(22%)cases after one month in the O-Mg group(P=0.001).In contrast,in the placebo group,the decrease went from 21(50%)cases initially to 18(42.9%)after 1 month and was not significant(P=0.5).Conclusions Oral magnesium sulfate seems effective in alleviating chronic constipation and pain experience in children with ***,saving maternal time spent in daily bowel evacuation attempts.

Impact of Rotavirus Vaccination in Severe Rotavirus Gastroenteritis Outpatient Visits at Three pediatric Primary Care clinics in Shibata City, Niigata Prefecture, Japan

The impact of rotavirus (RV) vaccination in reducing severe rotavirus gastroenteritis (RVGE) in outpatient settings was prospectively surveyed in three pediatric clinics in Shibata City. In children younger than 3 years of age, the occurrence of severe RVGE among all acute gastroenteritis (AGE) was found to be significantly lower in three seasons after introduction of RV vaccines, compared to that in 2011, before introduction of RV vaccines. The incidence rates of severe RVGE among children younger than 3 years of age were found to be reduced by 71.2%, 47.7%, and 81.1% for 2012, 2013, and 2014, respectively, compared to that in 2011. These results suggest that the RV vaccination is effective for the prevention of severe RVGE in Japanese voluntary RV vaccination settings with estimated coverage rates of 32.5%, 40.5% and 47.1% for 2012, 2013 and 2014, respectively. It is expected that the reducing effect on severe RVGE would be persistently established by increasing the vaccine coverage rates.

Crowdfunding refers to the online gathering of finance via numerous small donations from individual supporters （the ＂crowd＂） in order to fund a service, project or cause from various fields including business, arts, medicine and science （Sisler, 2012; Cameron et al., 2013; Weigmann, 2013; Wheat et al., 2013）. Crowdfunding platforms facilitate the interaction between organizations soliciting funding for their projects and the people who wish to support them. As prices of molecular testing plummet, utilizing crowdfunding in order to support genetic research becomes increasingly feasible （Cameron et al., 2013）. Whole Exome Sequencing （WES） has been used extensively for the purpose of identifying the genetic cause of rare mendelian diseases, uncovering novel mutations in previously implicated genes or identifying new disease- causing genes （Ng et al., 2009; Bamshad et al., 2011; Gilissen et al., 2011）.

clinical and Pathohistological Profiles of Chronic Odontogenic Infections with Microvascular Proliferation in Children with Permanent Dentition

The apical periodontitis as an infection is the result of endodontic infections and is manifested when the immunological system of the host is activated from microorganisms which penetrate in to the dental pulp through the root canals. The aim of this study is to compare the clinical diagnosis and the pathohistological identification of chronic odontogenic infections with microvascular proliferation. The study was carried out in University Dental clinical Center of Kosova in cooperation with Histopathology Departement. In this study were included 36 children between 10 - 15 years old, 20 girls and 16 boys, with permanent dentition and chronic apical changes. After the clinical and x-ray examination, the tooth extraction was done, and then the tissue of periapical region was taken, by the exvaction of the periapical granulomas and the other pathological tissues. From a total of 36 children with chronic apical changes and granulomas, 9 (25.0%) of the children had chronic apical periodontitis, from which 6 (16.67%) of the children did not have microvascular proliferation, 1 (2.78%) child had a medium level of microvascular proliferation and 2 (5.56%) children had signification microvascular changes (proliferation). A total of 6 (16.67%) children had chronic apical periodontitis with a fistula from which 5 (13.89%) children did not have microvascular proliferation and 1 (2.78%) child had a medium level of microvascular proliferation. A total of 5 (13.89%) children had chronic apical periodontitis with parulis, from which 3 (8.33%) children did not have microvascular proliferation, 1 (2.78%) child had a low level of microvascular proliferation and 1 (2.78%) child had severe microvascular proliferation.

Bacteriological Identification of Selected Pathogens in Infected Primary and Young Permanent Teeth Associated with clinical Symptoms

Background: The common cause of dental infections is necrosis of the pulp of a tooth, which is followed by bacterial invasion through the pulp chamber into the deeper tissues. The bacterial infections are caused by host indigenous bacteria primarily aerobic gram positive cocci, anaerobic gram negative and gram negative rods. The aim of this study is to identify the presence of different cultivating microorganisms which cause dental infections in primary and young permanent teeth manifested by as abscesses and apical parodontitis. Material and Methods: Seventy kids ages 5 - 15 years ( 36 females, 34 males) were studied prospectively (I assume this) at the University Dental Clincal Center of Kosovo, Department of pediatric Dentistry in cooperation with Microbiological Department of Faculty of Medicine. All patients who had clinical, and radiological confirmation of thier dental infections, followed by extraction of their teeth. Once the tooth was extracted blood and pus sample from the empty alveoli were cultured. The analysis of the sample for bacteria identification was done using the automated system VITEK 2 cards (Biomeriux, France). Results: Of 70 children’s with deciduous 73.5% of them had acute infection and 26.5% chronic infection. On the other hand 75% of children with permanent dentitions had chronic and 25% acute infection. In acute infections the most common aerobic bacteria was Streptococcus group with 47% (Streptococcus mitis and oralis) followed by the Actonomyces group (Actinomyces mayeri and Actinomyces odontoliticus) and anaerobic bacteria in 34%. In chronic infections streptococcus group was the prevalent aerobic group and in anaerobic infection Actinomyces mayeri and Actinomyces naeslundi was the most representative aerobes in 21% of samples. Conclusion: The dental infections in pediatric population (5 - 15 years old) are polymicrobial predominantly anaerobic bacteria over aerobic.
