Reliability and concurrent validity of postural asymmetry measurement in adolescent idiopathic scoliosis

AIM To investigate the reliability and concurrent validity of the Baseline~? Body Level/Scoliosis meter for adolescent idiopathic scoliosis postural assessment in three anatomical *** This is an observational reliability and concurrent validity study of adolescent referrals to the Orthopaedic department for scoliosis screening at KarolinskaUniversity Hospital, Stockholm, Sweden between MarchMay 2012. A total of 31 adolescents with idiopathic scoliosis(13.6 ± 0.6 years old) of mild-moderate curvatures(25°± 12°) were consecutively recruited. Measurement of cervical, thoracic and lumbar curvatures, pelvic and shoulder tilt, and axial thoracic rotation(ATR) were performed by two trained physiotherapists in one day. The intraclass correlation coefficient(ICC) was used to determine the inter-examiner reliability(ICC2,1) and the intra-rater reliability(ICC3,3) of the Baseline~? Body Level/Scoliosis meter. Spearman's correlation analyses were used to estimate concurrent validity between the Baseline~? Body Level/Scoliosis meter and Gold Standard Cobb angles from radiographs and the Orthopaedic Systems Inc. *** There was excellent reliability between examiners for thoracic kyphosis(ICC2,1 = 0.94), ATR(ICC2,1 = 0.92) and lumbar lordosis(ICC2,1 = 0.79). There was adequate reliability between examiners for cervical lordosis(ICC2,1 = 0.51), however poor reliability for pelvic and shoulder tilt. Both devices were reproducible in the measurement of ATR when repeated by one examiner(ICC3,3 0.98-1.00). The device had a good correlation with the Scoliometer(rho = 0.78). When compared with Cobb angle from radiographs, there was a moderate correlation for ATR(rho = 0.627).CONCLUSION The Baseline~? Body Level/Scoliosis meter provides reliable transverse and sagittal cervical, thoracic and lumbar measurements and valid transverse plan measurements of mild-moderate scoliosis deformity.

Brain Processing of Fearful Facial Expression in Mentally Disordered Offenders

Emotional facial expressions are important cues for interaction between people. The aim of the present study was to investigate brain function when processing fearful facial expressions in offenders with two psychiatric disorders which include impaired emotional facial perception;autism spectrum disorder (ASD) and psychopathy (PSY). Fourteen offenders undergoing forensic psychiatric assessment (7 with ASD, and 7 psychopathic offenders) and 12 healthy controls (HC) viewed fearful and neutral faces while undergoing functional magnetic resonance imaging (fMRI). Brain activity (fearful versus neutral faces) was compared both between HC and offenders and between the two offender groups (PSY and ASD). Functional co-activation was also investigated. The offenders had increased activity bilaterally in amygdala and medial cingulate cortex as well as the left hippocampus during processing fearful facial expressions compared to HC. The two subgroups of offenders differed in five regions compared with each other. Results from functional co-activation analysis suggested a strong correlation between the amygdala and anterior cingulate cortex (ACC) in the left hemisphere only in the PSY group. These findings suggest enhanced neural processing of fearful faces in the amygdala as well as in other facial processing brain areas in offenders compared to HC. Moreover, the co-activation between amygdala and ACC in the PSY but not the ASD group suggested qualitative differences in amygdala activity in the two groups. Since the sample size is small the study should be regarded as a pilot study.

Continuity of Care during Care Transition: Nurses’ Experiences and Challenges

The aim of this study was to gain increased knowledge about nurses’ experiences of care transition of older patients from hospital to municipal health care, based on two research questions: How is nurses’ experience continuity during care transition of older patients from hospital to municipal health care? How would nurses describe an optimal care transition? Nurses have a pivotal role during care transitions of older patients. More knowledge about their experiences is necessary to develop favorable improvements for this important period in the older patient’s treatment and care. The study has a qualitative explorative design with follow-up focus group interviews. Nurses (N = 30) working in hospital (n = 16) and municipal (n = 14) health care were organized in five mixed focus groups during the period October-January 2014/2015. The focus groups met twice, answering the research questions following a previously circulated semi-structured interview guide. The interview analysis was inspired by content analysis. The analysis resulted in the themes “Administrative demands challenge terms for collaboration” and “Essentials for nursing determine optimal care transitions for older patients”. Administrative demands may prevent nurses’ professional dialogue and collaboration across health care levels. Older patients’ best interests should be ensured through a collaborative relationship between hospital and municipal nurses, to form continuous care across health care levels. Clinical practice should be aware of essentials for nursing, which could influence and facilitate a more individualized and continuous transition for older patients.

Road Side Units(RSUs)are the essential component of vehicular communication for the objective of improving safety and mobility in the road *** are generally deployed at the roadside and more specifically at the intersections in order to collect traffic information from the vehicles and disseminate alarms and messages in emergency situations to the neighborhood vehicles cooperating with the ***,the development of a predominant RSUs placement algorithm for ensuring competent communication in VANETs is a challenging issue due to the hindrance of obstacles like water bodies,trees and *** this paper,Ruppert’s Delaunay Triangulation Refinement Scheme(RDTRS)for optimal RSUs placement is proposed for accurately estimating the optimal number of RSUs that has the possibility of enhancing the area of coverage during data *** RDTRS is proposed by considering the maximum number of factors such as global coverage,intersection popularity,vehicle density and obstacles present in the map for optimal RSUs placement,which is considered as the core improvement over the existing RSUs optimal placement *** is contributed for deploying requisite RSUs with essential transmission range for maximal coverage in the convex map such that each position of the map could be effectively covered by at least one RSU in the presence of *** simulation experiments of the proposed RDTRS are conducted with complex road traffic *** results of this proposed RDTRS confirmed its predominance in reducing the end-to-end delay by 21.32%,packet loss by 9.38%with improved packet delivery rate of 10.68%,compared to the benchmarked schemes.

Panretinal photocoagulation versus panretinal photocoagulation plus intravitreal bevacizumab for high-risk proliferative diabetic retinopathy

AIM： To evaluate the effects of panretinal photocoagulation （PRP） compared with PRP plus intravitreal bevacizumab （IVB） in patients with high-risk proliferative diabetic retinopathy （PDR） according to the Early Treatment Diabetic Retinopathy Study ***： The data were collected retrospectively from the eyes of high-risk PDR patients, which were divided into two groups. After treated with standard PRP, the eyes were randomly assigned to receive only PRP （PRP group） or PRP plus intravitreal injection of 1.25 mg of bevacizumab （PRP-Plus group）. Patients underwent complete ophthalmic evaluation, including best corrected visual acuity （BCVA）, intraocular pressure （IOP）, and new vessel size in fluorescein angiography （FA） and optical coherence tomography for the assessment of central subfield macular thickness （CSMT） at baseline and at weeks 12 （±2）, 16 （±2）, 24 （±2） and 48 （±2）. Main outcome measures also included vitreous clear-up time and neovascularization on the disc （NVD） regression time. Adverse events associated with intravitreal injection were ***： Thirty consecutive patients （n=36 eyes） completed the 48-week follow-up. There was no significant difference between the PRP and PRP-Plus groups with respect to age, gender, type or duration of diabetes, area of fluorescein leakage from active neovascularizations （nvs）, BCVA or CSMT at baseline. The mean vitreous clear-up time was 12.1±3.4wk after PRP and 8.4±3.5wk after PRP combined with IVB. The mean time interval from treatment to complete NVD regression on FA examination was 15.2±3.5wk in PRP group and 12.5±3.1wk in PRP-Plus group. No significant difference in CSMT was observed between the groups throughout the study period. However, the total area of actively leaking nvs was significantly reduced in the PRP-Plus group compared with the PRP group （P〈0.05）. Patients received an average of 1.3 injections （range： 1-2）.
