Association of physical activity with risk of chronic kidney disease in China:A population-based cohort study

Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is limited.We aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in China.Methods:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at baseline.An interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on quartiles.Cox regression was used to analyze the association between PA and CKD risk.Results:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)cases.Total PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest quartile.Similar results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN risk.Increased nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,respectively.Conclusion:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.

Summary What is already known about this topic?Noncommunicable diseases(ncds)are a major public health problem in the world.ncds are the leading cause of premature deaths and disabilities among Chinese residents,resulting in heavy economic and health burdens.What is added by this report?This study conducted a quantitative analysis of the policy texts on ncds prevention and control in China from 1990 to 2020,based on the perspective of policy instruments.It was discovered that China’s ncds prevention and control policies developed rapidly from the ground up over the 30 years from 1990 to 2020 and that the majority of China’s ncds prevention and control policies were environment-oriented,while supply-oriented and demand-oriented policies were insufficient.What are the implications for public health practice?The findings of this study suggested that increasing supply-oriented and demand-oriented strategies should be prioritized in the future formulation and revision of ncds prevention and control policies.

Noncommunicable diseases(ncds)and related risk factors are one of the major global challenges of the 21st century and are responsible for 71%of all deaths worldwide(1).More than 36 million people die annually as a result of ncds,including 15 million people who die at a young age,between 30 and 70 years old(2).The burden continues to rise and has been one of the major public health challenges for all countries,including China.Behavioral risk factors interventions are considered the cornerstone of the prevention and control of chronic diseases.Substantial reduction of NCD mortality requires policies that considerably reduce tobacco and alcohol use and blood pressure,and provide equitable access to efficacious and high-quality preventive and curative care for acute and chronic ncds(3).

Based on the major findings of our studies,we found that in the past two or three decades,China’s noncommunicable diseases(ncds)prevention and control policies experienced rapid development.The service mode of ncds management was transformed,the service capabilities at primary medical facilities were comprehensively improved,and the health literacy and action capability of residents were improved.Remarkable achievements have been made in the prevention and control of ncds,and a lot of experience has been accumulated.However,unhealthy lifestyles among Chinese residents are still common,and the control of ncds and related behavioral risk factors faces great challenges.Therefore,in order to implement the Healthy China strategy,early prevention and strengthening of the health management of ncds in high-risk groups are essential.

The tobacco epidemic is one of the biggest public health threats the world has ever faced,killing more than 8 million people a year,mainly through noncommunicable diseases(ncds)(1).World Health Organization(WHO)set nine voluntary global targets for ncds to be attained by 2025,including a 30%relative reduction in prevalence of current tobacco use by 2025(2),which requires a strong commitment by governments to strengthen the national health system responses and to take action against the leading risk factors for ncds including tobacco use,the single most preventable cause of death.Commitment to strengthening full implementation of the WHO Framework Convention on Tobacco control(WHO FCTC)is a crucial component of action by governments.

Summary What is already known about this topic?Since the launch of China Healthy Lifestyle for All(CHLA),each action area has been evaluated at the local level and effective results have been achieved in most areas.What is added by this report?Based on the Reach,Effectiveness,Adoption,Implementation,and Maintenance(RE-AIM)model,the study found that there is an imbalance in the development of CHLA,and some of the action goals and indicators are not satisfactory.What are the implications for public health practices?A national action evaluation index system should be built to ensure the sustainability and scientific nature of this strategy.In addition,the government needs to attach great importance to CHLA to effectively help residents take health actions.

Global trends in diabetes-related mortality with regard to lifestyle modifications,risk factors,and affordable management:A preliminary analysis

Background:According to the World Health Organization (WHO),a global reduction of 17% has been achieved in the major noncommunicable disease-associated mortality rate since 2000.This decline was due to the decreasing mortality associated with cardiovascular and chronic respiratory diseases. The WHO has not made any comments on diabetes-related mortality thus far.The objective of this study was to demonstrate trends in diabetes-related mortality associated with country-wide interventions.Methods:The WHO statistics were used to assess trends in diabetes-related mortality from 2000 to 2016.Different types of community-based interventions in 49 countries were compared and assessed.Results:The baseline mortality decreased by 7%.Mortality in middle-income countries was higher than that in high-income countries. The prevalence of obesity showed a gradual increase in all countries. After implementation of the WHO "best buy" in 2010, mortality increased in 17 countries and decreased in 32 countries. Regarding the smoking prevalence trend, 87% countries with decreasing diabetes-related mortality had a gradual decline in tobacco usage since 2000.The decline was observed only in 43% countries with increasing diabetes-related mortality. The prevalence of hypertension increased in 19% countries with declining diabetes-related mortality and in 35% countries with increasing diabetes-related mortality. Physical activity measures tended to be better implemented in countries with declining diabetes-related mortality than in countries with increasing diabetes-related mortality.Conclusion:Smoking cessation and better blood pressure control are associated with declining diabetes-related mortality. Longer implementation periods are needed for other lifestyle interventions.

Incidence,Prevalence,and Mortality of Four Major Chronic Non-communicable Diseases—China,1990-2017

Introduction Chronic and non-communicable diseases(ncds)have become an important public health concern in China.We aimed to estimate the incidence,prevalence,and mortality of four major ncds including cardiovascular disease(CVD),cancer,chronic respiratory diseases(CRD),and diabetes in China during 1990-2017.Methods The general analytic framework of Global Burden of Diseases Study 2017(GBD 2017)was applied.Data from Disease Surveillance Point System,censuses,and Chinese Center for Disease control and prevention Cause-of-Death Reporting System were used for mortality estimates.National surveys,cancer registries,and published studies were used for incidence and prevalence estimates.Results In 2017,new cases for CVD,cancer,and diabetes were 13.5,4.6,and 3.3 million,an increase of 120.4%,159.7%,and 57.3%respectively compared with 1990.There were 9.6 million incident cases for CRD in 2017,a 4.9%decrease compared with 1990.A total of 8.1 million people died from cancer,CVD,CRD,and diabetes in China in 2017.There was a substantial decline in age-standardized mortality rate for CRD and a steady but slow decline for CVD and cancer.During 1990-2017,the age-standardized prevalence rate increased by 7.4%,135.2%,and 17.0%for CVD,cancer,and diabetes respectively.Conclusions and Implications for Public Health Practice The accumulated prevalence of the four major ncds are increasing but the age-standardized mortality rate for CVD,cancer,and CRD has been on a decline during 1990-2017.More effective intervention strategies should be developed to deal with the continuously increasing burden caused by ncds in China.

BACKGROUND In recent decades,China has faced a double burden of infectious diseases and noncommunicable diseases(ncds).The number of deaths due to ncds accounts for 88.5%of total deaths in China in 2019.To address these public health issues and improve the accessibility and affordability of primary health care,a National Basic Public Health Service Program(NBPHSP)was introduced as part of China’s healthcare reform initiated in 2009.Subsided by national and local government budgets,NBPHSP is mainly implemented by community and township health service centers,as well as health stations in urban areas and village clinics in rural areas(1).Health management of patients with hypertension and diabetes is an integral and pivotal part of the NBPHSP.
