AIM: To extend the knowledge of the dynamic interaction between Helicobacter pylori (H. pylori) and host mucosa. METHODS: A time-series cDNA microarray was performed in order to detect the temporal gene expression prof iles of human gastric epithelial adenocarcinoma cells infected with H. pylori. Six time points were selected to observe the changes in the model. A differential expression prof ile at each time point was obtained by comparing the microarray signal value with that of 0 h. Real-time polymerase chain reaction was subsequently performed to evaluate the data quality. RESULTS: We found a diversity of gene expression patterns at different time points and identifi ed a group of genes whose expression levels were significantly correlated with several important immune response and tumor related pathways. CONCLUSION: Early infection may trigger some important pathways and may impact the outcome of the infection.

AIM:To determine the association of hepatitis B virus (HBV) genotypes with probable cirrhosis and fatty liver in community-based ***: A multi-stage cluster probability sampling method was applied to recruit 10 167 subjects aged between 6 and 72 years from our epidemiological bases in Eastern China. After excluding the subjects co-infected with hepatitis C or hepatitis D viruses, the hepatitis B surface antigen (HBsAg)-positive subjects were examined for HBV genotype, serum viral load, alanine aminotransferase (ALT), hepatitis B e antigen (HBeAg) status, and ultrasonographic changes. Logistic regression models were used to determine the factors associated with probable cirrhosis and fatty ***: Of 634 HBsAg-positive subjects with HBV genotype determined, 82 had probable cirrhosis (ultrasonographic score ≥5), 42 had ultrasonographic fatty liver. Probable cirrhosis was only found in the HBeAg-negative subjects, and more frequently found in the subjects with genotype C than in those with genotype B (14.8% vs 8.0%, P=0.018). In HBeAg-negative subjects, high viral load was frequently asso-ciated with abnormal ALT level, while ALT abnormality was more frequent in those with probable cirrhosis than those without (19.5% vs 7.8%, P=0.001). Univariate analysis showed that age, sex, HBV genotypes, and viral load were not significantly associated with ultrasonographic fatty liver, whereas ALT abnormality was significantly related to ultrasonographic fatty liver (OR=4.54,95% CI:2.11-9.75, P<0.001). Multivariate analysis demonstrated that HBV genotype C, age (≥45 years), male sex, and ALT abnormality were independently associated with probable cirrhosis (AOR=2.30,95% CI:1.26-4.19; AOR =1.81,95% CI:1.10-2.99;AOR=1.74,95% CI:1.03-2.95; AOR=2.98,95% CI:1.48-5.99, respectively).CONCLUSION: A crude prevalence of probable cirrhosis is 12.9% in the community-based HBV-infected subjects. Background Six provinces in China accounted for 70%-80% of all reported HIV/AIDS cases in the country in 2009 and five provinces accounted for 78% of all reported mother-to-child transmission （MTCT） of HIV cases. Because Guangxi belonged to both groups, the prevention of Mother-to-Child Transmission （PMTCT） Plus program was established there to understand better low birth weight （LBW） and preterm delivery （PD） birth outcomes and their associated risk factors better. Methods Pregnancy outcomes were examined among HIV-infected pregnant women who enrolled in the PMTCT Plus program from June 2006 to February 2009 in Guangxi, China. Multivariate Logistic regression analysis was used to explore the risk factors associated with LBW （〈2500 g） and PD （gestational age 〈37 weeks）. Results The prevalence of LBW and PD among 194 HIV-positive mothers was 19.6% （38/194） and 9.8% （19/194）, respectively. Multivariate Logistic regression analysis showed that CD4 cell count 〈100 cell/IJI （multivariate-adjusted odds ratio （AOR） 5.52; 95% Cl 1.11-25.55） and CD4 cell count 100-199 cells/IJI （AOR3.40; 95% Cl 1.03-11.25, compared to CD4 cell count 〉350 cells/IJI）, gestational age 〈37 weeks （AOR 4.38; 95% Cl 1.29-14.82, compared to 〉37 weeks）, maternal weight 〈45 kg （AOR 5.64; 95% Cl 1.09-29.07） and maternal weight 45-54 kg （AOR 3.55; 95% CI 1.31-9.60, compared to 〉55 kg） at enrollment, and H IV RNA 〉100 000 copies/ml at enrollment （AOR 4.22; 95% CI 1.24-14.32） and 20 000-99 999 （AOR 2.77; 95% Cl 1.01-7.77, compared to 〈20 000 copies/ml） were associated with a higher risk of LBW. For PD, only maternal injection drug use as the route of HIV transmission （AOR 5.30; 95% Cl 1.33-21.14, compared to those infected with HIV through sexual transmission） was significantly associated with a higher risk of PD. Background Optimal mineral intakes are important for infant growth and development. However, data on mineral compositions of breast milk in Chinese women are scarce, and most were acquired before 1990. The objectives of this study were three-fold：（1） to investigate the mineral compositions of Chinese healthy mothers＇ breast milk in different lactation stages; （2） to explore correlations among mineral concentrations in breast milk; and （3） to explore the associated factors affecting mineral compositions in breast milk. Methods The inductively coupled plasma mass spectrometry （ICP-MS） was used to analyze mineral concentrations in breast-milk of 444 healthy lactating women from three cities in China. A questionnaire was used to survey socio- demographic characteristics and pregnancy history. Food intakes by lactating women were measured using both food frequency questionnaire and one cycle of 24-hour dietary recall. Results Mineral compositions of breast milk varied in different regions. Concentrations of most minerals were higher in the first one or two months of lactation, and then decreased with time, except for magnesium and iron. Inter-mineral correlations existed among several minerals. The calcium-to-phosphorus ratio was above 2：1 in each lactation stage. Women with caesarean section had higher concentration of iodine in the transitional milk （349.9 pg/kg） compared to women with natural delivery （237.5 pg/kg, P〈0.001）. Dietary mineral intakes, supplements, food intake frequencies in the recent 6 months, maternal age and maternal BMI did not show significant correlations with concentrations of milk minerals （all P 〉0.05）. Conclusions Milk minerals decreased with time, and changed most rapidly in the first one or two months of lactation. Caesarean section might affect the iodine level in transitional milk.
