Prevalence and intensity of urinary schistosomiasis and soil-transmitted helminths among women of reproductive age in Mwaluphamba,Kwale

Objective:To assess the epidemiology of urinary schistosomiasis and soil-transmitted helminthiasis among women of reproductive age in Mwaluphamba,Kwale County,kenya.Methods:A community-based cross-sectional study design was employed to randomly sample 422 women of reproductive age(15-<50 years)from four villages in Mwaluphamba location.Stool specimens were collected and examined using the Kato-Katz method,while filtration technique was used to analyze urine specimens.Participants’sociodemographic details were obtained using a standardized questionnaire.Results:Urinary schistosomiasis prevalence was at 4.7%(20/422,95%CI 2.8%-6.9%)while the prevalence of soil-transmitted helminthiasis infection was 4.5%(19/422,95%CI 2.6%-6.7%).The infection intensities of urinary schistosomiasis among the study participants ranged from 1 to 120 eggs/10 mL of urine with median egg count of 18.45 eggs/10 mL.The patients were diagnosed with light infection,of 56.16 egg/gram and 48.48 egg/gram for Trichuris trichiura and hookworms,respectively.Women without latrines had 15.7 times higher risk of having urinary schistosomiasis compared to those with a latrine.Similarly,use of surface water(aOR=1.0,95%CI 0.2-1.4,P=0.010)and crossing the river to go to a place(aOR=1.1,95%CI 0.3-1.6,P=0.009)were statistically significant risk factors for getting urinary schistosomiasis.In bivariable regression analysis,defecating around the water source(OR=4.3,95%CI 1.5-12.9)had a statistically significant association with the prevalence of soil-transmitted helminthiasis(P=0.008).Conclusions:This study has given an insight on the prevalence and intensity of urinary schistosomiasis and soil-transmitted helminthiasis in Mwaluphamba location that form a basis for strengthening the control and elimination programmes for these neglected tropical diseases.

Progress in experimental models to investigate the in vivo and in vitro antidiabetic activity of drugs

Diabetes mellitus is one of the world's most prevalent and complex metabolic disorders,and it is a rapidly growing global public health issue.It is characterized by hyperglycemia,a condition involving a high blood glucose level brought on by deficiencies in insulin secretion,decreased activity of insulin,or both.Prolonged effects of diabetes include cardiovascular problems,retinopathy,neuropathy,nephropathy,and vascular alterations in both macro-and micro-blood vessels.In vivo and in vitro models have always been important for investigating and characterizing disease pathogenesis,identifying targets,and reviewing novel treatment options and medications.Fully understanding these models is crucial for the researchers so this review summarizes the different experimental in vivo and in vitro model options used to study diabetes and its consequences.The most popular in vivo studies involves the small animal models,such as rodent models,chemically induced diabetogens like streptozotocin and alloxan,and the possibility of deleting or overexpressing a specific gene by knockout and transgenic technologies on these animals.Other models include virally induced models,diet/nutrition induced diabetic animals,surgically induced models or pancreatectomy models,and non-obese models.Large animals or non-rodent models like porcine(pig),canine(dog),nonhuman primate,and Zebrafish models are also outlined.The in vitro models discussed are murine and human beta-cell lines and pancreatic islets,human stem cells,and organoid cultures.The other enzymatic in vitro tests to assess diabetes include assay of amylase inhibition and inhibition ofα-glucosidase activity.

The performance of bridge projects in kenya is poor in terms of completion by schedule, cost, and quality (scope). Yet, there is less evidence of empirical research on what factors contribute to this outcome. This study aimed to bridge this gap by examining the effects of contractor-related factors on the performance of bridge construction projects in kenya through a case study of the Bridge projects Implemented by the kenya National Highway Agency (KeNHA). The theory of constraints (TOC) was adopted as its theoretical framework. Descriptive research was used, and the target population was 18 bridge construction projects, which were the units of analysis from 2012 to 2022. In each of these projects, 18 respondents, namely clients, consultants, contractors, engineers, environment and social guards, project managers, stakeholders, subcontractors, technical advisors, and inspectors, were included in a target population of 144 respondents. A census was conducted and a structured questionnaire was administered from which a response rate of 68% was achieved. The information was analyzed using descriptive, correlation, and multiple linear regression analysis. The contractor-related factors considered in the study were staff and management factors. The findings indicated that staff and management factors had a positive and significant outcome on performance of bridge construction projects. The study recommends continuous training and a safe learning environment for staff to improve their skills and performance in future projects. The study also recommends that a special category for bridge contractors be created in kenya’s National Construction Authority rankings to ensure that only qualified contractors implement the Bridge projects.

Characterization of Small-Scale Farmers and Assessment of Their Access to Crop Production Information in Selected Counties of kenya

Small-scale farming accounts for 78% of total agricultural production in kenya and contributes to 23.5% of the country’s GDP. Their crop production activities are mostly rainfed subsistence with any surplus being sold to bring in some income. Timely decisions on farm practices such as farm preparation and planting are critical determinants of the seasonal outcomes. In kenya, most small-scale farmers have no reliable source of information that would help them make timely and accurate decisions. County governments have extension officers who are mandated with giving farmers advisory services to farmers but they are not able to reach most farmers due to facilitation constraints. The mode and format of sharing information is also critical since it’s important to ensure that it’s timely, well-understood and usable. This study sought to assess access to geospatial derived and other crop production information by farmers in four selected counties of kenya. Specific objectives were to determine the profile of small-scale farmers in terms of age, education and farm size;to determine the type of information that is made available to them by County and Sub-County extension officers including the format and mode of provision;and to determine if the information provided was useful in terms of accuracy, timeliness and adequacy. The results indicated that over 80% of the farmers were over 35 years of age and over 56% were male. Majority had attained primary education (34%) or secondary education (29%) and most farmers in all the counties grew maize (71%). Notably, fellow farmers were a source of information (71%) with the frequency of sharing information being mostly seasonal (37%) and when information was available (43%). Over 66% of interviewed farmers indicating that they faced challenges while using provided information. The results from the study are insightful and helpful in determining effective ways of providing farmers with useful information to ensure maximum benefits.

Glaucoma,an irreversible optic neuropathy,primarily affects retinal ganglion cells(RGC)and causes vision loss and blindness.The damage to RGCs in glaucoma occurs by various mechanisms,including elevated intraocular pressure,oxidative stress,inflammation,and other neurodegenerative processes.As the disease progresses,the loss of RGCs leads to vision loss.Therefore,protecting RGCs from damage and promoting their survival are important goals in managing glaucoma.In this regard,resveratrol(RES),a polyphenolic phytoalexin,exerts antioxidant effects and slows down the evolution and progression of glaucoma.The present review shows that RES plays a protective role in RGCs in cases of ischemic injury and hypoxia as well as in ErbB2 protein expression in the retina.Additionally,RES plays protective roles in RGCs by promoting cell growth,reducing apoptosis,and decreasing oxidative stress in H_(2)O_(2)-exposed RGCs.RES was also found to inhibit oxidative stress damage in RGCs and suppress the activation of mitogen-activated protein kinase signaling pathways.RES could alleviate retinal function impairment by suppressing the hypoxia-i nducible factor-1 alpha/vascular endothelial growth factor and p38/p53 axes while stimulating the PI3K/Akt pathway.Therefore,RES might exert potential therapeutic effects for managing glaucoma by protecting RGCs from damage and promoting their survival.

Optimization of Methylene Blue Dye Adsorption onto Coconut Husk Cellulose Using Response Surface Methodology: Adsorption Kinetics, Isotherms and Reusability Studies

In this study, coconut husk cellulose was employed as a cost-effective and environmentally friendly adsorbent to eliminate methylene blue (MB) dye from aqueous solutions. The successful development of response surface methodology paired with a central composite design (RSM-CCD) enabled the optimization and modelling of the adsorption process. The study investigated the individual and combined effects of three variables (pH, contact time, and initial MB dye concentration) on the adsorption of MB dye onto coconut husk cellulose. The developed RSM-CCD model exhibited a remarkable degree of precision in predicting the removal efficiency of MB dye within the specified experimental parameters. This was demonstrated by the strong regression parameters, with an R2 value of 99.79% and an adjusted R2 value of 99.6%. The study depicted that the optimal parameters for attaining a 98.8827% removal of MB dye using coconut husk cellulose were as follows: an initial MB dye concentration of 30 mg∙L−1, contact time of 120 minutes, and pH 7 at a fixed adsorbent dose of 0.5 g. The Freundlich isotherm model provided the most satisfactory description of the equilibrium adsorption isotherms, suggesting that MB dye adsorption onto coconut husk cellulose occurs on a heterogeneous surface. The experimental results demonstrated a strong agreement with the pseudo-second-order kinetics model, indicating that the number of active sites present on the cellulose adsorbent predominantly influences the adsorption process of MB dye. Additionally, the adsorbent made from coconut husk cellulose exhibited the potential to be reused, as it retained its efficiency for a maximum of three cycles of adsorption of MB dye. The results of this study show that coconut husk cellulose has the potential to be an effective and sustainable adsorbent for removing MB dye from aqueous solutions.

Climate change poses a serious long-term threat to biodiversity.To effectively reduce biodiversity loss,conservationists need to have a thorough understanding of the preferred habitats of species and the variables that affect their distribution.Therefore,predicting the impact of climate change on speciesappropriate habitats may help mitigate the potential threats to biodiversity distribution.Xerophyta,a monocotyledonous genus of the family Velloziaceae is native to mainland Africa,Madagascar,and the Arabian Peninsula.The key drivers of Xerophyta habitat distribution and preference are unknown.Using 308 species occurrence data and eight environmental variables,the MaxEnt model was used to determine the potential distribution of six Xerophyta species in Africa under past,current and future climate change scenarios.The results showed that the models had a good predictive ability(Area Under the Curve and True Skill Statistics values for all SDMs were more than 0.902),indicating high accuracy in forecasting the potential geographic distribution of Xerophyta species.The main bioclimatic variables that impacted potential distributions of most Xerophyta species were mean temperature of the driest quarter(Bio9)and precipitation of the warmest quarter(Bio18).According to our models,tropical Africa has zones of moderate and high suitability for Xerophyta taxa,which is consistent with the majority of documented species localities.The habitat suitability of the existing range of the Xerophyta species varied based on the climate scenario,with most species experiencing a range loss greater than the range gain regardless of the climate scenario.The projected spatiotemporal patterns of Xerophyta species help guide recommendations for conservation efforts.

Effect of Gum Arabic from Acacia senegal var. kerensis as an Improver on the Rheological Properties of Wheat Flour Dough

Dough improvers are substances with functional characteristics used in baking industry to enhance dough properties. Currently, the baking industry is faced with increasing demand for natural ingredients owing to increasing consumer awareness, thus contributing to the rising demand for natural hydrocolloids. Gum Arabic from Acacia senegal var. kerensis is a natural gum exhibiting excellent water binding and emulsification capacity. However, very little is reported on how it affects the rheological properties of wheat dough. The aim of this study was therefore, to determine the rheological properties of wheat dough with partial additions of gum Arabic as an improver. Six treatments were analyzed comprising of: flour-gum blends prepared by adding gum Arabic to wheat flour at different levels (1%, 2% and 3%), plain wheat flour (negative control), commercial bread flour and commercial chapati flour (positive controls). The rheological properties were determined using Brabender Farinograph, Brabender Extensograph and Brabender Viscograph. Results showed that addition of gum Arabic significantly (p chapati. These findings support the need to utilize gum Arabic from Acacia senegal var. kerensis as a dough improver.

Low-Level Antibiotic Resistance among Staphylococcus aureus and Gram-Negative Pathogens from Infected Skin and Soft Tissues in Rural kenya

Introduction: Bacterial skin and soft tissue infections (SSTIs) are a cause of frequent inpatient and outpatient care visits whose causative agents are associated with a high antimicrobial resistance burden. For insights on antimicrobial susceptibilities in a rural setting, we examined specimens from suspected SSTIs from two public health facilities in kenya. We additionally assessed antibiotic use, appropriateness of empiric therapy and risk factors for SSTI. Methodology: Between 2021 and 2023, 265 patients at Kisii and Nyamira County Referral hospitals were enrolled. Wound swabs/aspirates were collected and processed following standard microbiological procedures. Identification and antimicrobial susceptibility were performed using the VITEK 2 Compact platform. Demographic, clinical, and microbiological data were analyzed with R Statistical software. Results: S. aureus was isolated in 16.2% (43/265) of patients with a methicillin resistance (MRSA) proportion of 14% (6/43). While 13/15 drugs elicited susceptibilities ranging from 84% - 100%, penicillin (16%) and trimethoprim-sulfamethoxazole [TMP-SXT] (23%) yielded the lowest susceptibilities. Escherichia coli (n = 33), Klebsiella pneumoniae (n = 8), Pseudomonas aeruginosa (n = 8), and Citrobacter species (n = 4) were the most commonly isolated gram-negative species. Gram-negative strains showed high susceptibilities to most of the tested drugs (71% - 100%) with the exception of ampicillin (18%), TMP-SXT (33%), and first and second generation cephalosporins. Conclusions: The low MRSA prevalence and generally high antibiotic susceptibilities for S. aureus and gram-negative bacteria present opportunities for antibiotic stewardship in the study setting. Diminished susceptibilities against penicillin/ampicillin and TMP-SXT accord with prevailing local data and add a layer of evidence for their cautious empiric use.

Climate Trends and Their Impact on Sorghum Production in Marigat, Baringo County: A Historical Analysis

Climate is subject to fluctuations in the majority of the world, mainly caused by rainfall as well as temperature variations. Climate fluctuations in kenya have resulted in the spread of desert-like conditions in the ASALs region, such as Marigat in Baringo County. As a county, Baringo experiences great variations in climate annually, as well as uncertainty in expected rains, thereby negatively impacting the production of crops such as sorghum. This study applied the rainfall anomaly index (RAI), standardised precipitation evapotranspiration index (SPEI), standard precipitation index (SPI), and Mann-Kendall (MK) statistical test for trends on historical climatic data in analysing both temperature and precipitation data over the period 1990 to 2022 to determine their trend, patterns and how they affect the production of sorghum crops. The machine learning method (R studio) with inputs was used to calculate the SPI, SPEI, RAI and MK trend test. The rainfall varied from below average to above average during the study period with no clear pattern in the RAI, SPEI and SPI values. The years 2020 and 2000 stood out as they had higher and lower rainfall than usual, respectively. The Marigat area generally experienced more rainfall during the high/long rainfall season (AMJJ). The MK trend test on average monthly rainfall, SOND, AMJJ, and annual precipitation confirmed a positive trend in precipitation. However, the short rainy season (SOND) was found to be the most variable period for rainfall, and there was a slight increase in daily average temperatures during this season.
