Hepatitis B virus(HBV)/Hepatitis C virus(HCV)coinfection is frequently observed because of the common infection *** the reciprocal inhibition exerted by HBV and HCV genomes,the coinfection of HBV and HCV is associated with more severe forms of liver ***,the complexity of viral interference and underlying pathological mechanism is still *** the demonstration of absence of direct viral interplay,some in vitro studies suggest the indirect effects of viral-host interaction on viral dominance ***,we comprehensively investigated the viral replication and host immune responses which might mediate the interference between viruses in HBV/HCV coinfected Huh7-NTCP cells and immunocompetent HCV human receptors transgenic ICR *** found that presence of HCV significantly inhibited HBV replication in vitro and in vivo irrespective of the coinfection order,while HBV did not affect HCV *** alteration was coincidently reproduced in coinfected *** addition to the participation of innate immune response,an involvement of HCV in up-regulating HBV-specific immune responses was described to facilitate HBV *** systems partially recapitulate HBV/HCV coinfection and unveil the uncharacterized adaptive anti-viral immune responses during coinfection,which renews the knowledge on the nature of indirect viral interaction during HBV/HCV coinfection.

Biomarkers and a tailored approach for immune monitoring in kidney transplantation

A literature review on immune monitoring in kidney transplantation produced dozens of research articles and a multitude of promising biomarkers,all in the quest for the much sought after-but perennially elusive-"holy grail" of kidney biomarkers able to unequivocally predict acute transplant rejection vs *** methodologies and study designs were many and *** the motivation for this editorial,which espouses the notion that in today's kidney transplantation milieu,the judicious use of disease classifiers tailored to specific patient immune risks may be more achievable and productive in the long run and confer a greater advantage for patient treatment than the pursuit of a single "omniscient" *** addition,we desire to direct attention toward greater scrutiny of biomarker publications and decisions to implement biomarkers in practice,standardization of methods in the development of biomarkers and consideration for adoption of "biomarker-driven" *** propose "biomarkerdriven" biopsies as an adjunctive to and/or alternative to random surveillance(protocol) biopsies or belated indication *** discovery of a single kidney transplantation biomarker would represent a major breakthrough in kidney transplantation practice,but until that occurs-if ever it does occur,other approaches offer substantial potential for unlocking prognostic,diagnostic and therapeutic *** conclude our editorial with suggestions and recommendations for productively incorporating current biomarkers into diagnostic algorithms and for testing future biomarkers of acute rejection in kidney transplantation.

Prognostic factors for a favorable outcome after varicocele repair in adolescents and adults

The effect of varicocele repair on male fertility remains controversial. It would be helpful to determined which men would benefit most from varicocele repair, and target repair efforts at those individuals. A detailed review of the literature on prognostic factors for varicocele repair was performed using the PubMed NLM database. We found that the best predictor of postvaricocelectomy semen parameters is the preoperative semen parameters. The greatest improvements in semen parameters were found in men with larger varicoceles. While there is controversy, higher testosterone, younger age and larger testis size, in some studies predict for improvements in semen parameters postvaricocelectomy. A nomogram has been developed to predict the postvaricocelectomy semen parameters based on the preoperative semen parameters, varicocele grade and the age of the man （***. com）. Limited data consistently demonstrates the greatest improvements in DNA fragmentation rates in men with higher baseline DNA fragmentation rates. With respect to reproductive outcomes, higher baseline sperm density consistently predicts for natural pregnancy or assisted reproductive technology （ART） pregnancy rates. In addition, varicocele repair does seem to reduce the need for more invasive modalities of ART. In conclusion, we can now start to use specific parameters such as baseline semen quality, varicocele grade and patient age to predict post-repair semen oualitv and fertility potential following varicocelectomy.

Stretch-induced human myometrial cytokines enhance immune cell recruitment via endothelial activation

Spontaneous term labour is associated with amplified inflammatory events in the myometrium including cytokine production and leukocyte infiltration; however, potential mechanisms regulating such events are not fully understood. We hypothesized that mechanical stretch of the uterine wall by the growing fetus facilitates peripheral leukocyte extravasation into the term myometrium through the release of various cytokines by uterine myocytes. Human myometrial cells （hTERT-HM） were subjected to static mechanical stretch; stretch-conditioned media was collected and analysed using 48-plex Luminex assay and ELISA. Effect of stretch-conditioned media on cell adhesion molecule expression of human uterine microvascular endothelial cells （UtMVEC-Myo） was detected by quantitative polymerase chain reaction （qPCR） and flow cytometry; functional assays testing leukocyte-endothelial interactions： adhesion of leukocytes to endothelial cells and transendothelial migration of calcein-labelled primary human neutrophils as well as migration of THP-1 monocytic cells were assessed by fluorometry. The current in vitrostudy demonstrated that mechanical stretch （i） directly induces secretion of multiple cytokines and chemokines by hTERT-HM cells （IL-6, CXCL8, CXCL1, migration inhibitory factor （MIF）, VEGF, G-CSF, IL-12p70, bFGF and platelet-derived growth factor subunit B （PDGF-bb）, P〈0.05）; stretch-induced cytokines （ii） enhance leukocyte adhesion to the endothelium of the surrounding uterine microvasculature by （iii） inducing the expression of endothelial cell adhesion molecules and （iv） directing the transendothelial migration of peripheral leukocytes. （vi） Chemokine-neutralizing antibodies and broad-spectrum chemokine inhibitor block leukocyte migration. Non-alcoholic fatty liver disease(NAFLD),is a disease spectrum characterized by fat accumulation in hepatocytes presenting as hepatic steatosis to advance disease with active hepatic inflammation,known as nonalcoholic *** steatohepatitis will lead to progressive hepatic fibrosis causing cirrhosis and increased risk for developing hepatocellular carcinoma(HCC).Fatty liver disease prevalence has increased at alarming rates alongside obesity,diabetes and metabolic syndrome to become the second most common cause of cirrhosis after alcohol related liver disease *** this rise in prevalence,it is becoming increasingly more important to find non-invasive methods to diagnose disease early and stage hepatic *** clinicians with the tools to diagnose and treat the full spectrum of NAFLD will help prevent known complications such as cirrhosis and HCC and improve quality of life for the patients suffering from this *** article discusses the utility of current noninvasive liver function testing in the clinical progression of fatty liver disease along with the imaging modalities that are ***,we summarize available treatment options including targeted medical therapy through four different pathways,surgical or endoscopic intervention.

Xanthogranulomatous cholecystitis mimicking gallbladder cancer and causing obstructive cholestasis

BACKGROUND: Xanthogranulomatous cholecystitis (XGC) is a destructive inflammatory disease of the gallbladder that can mimic gallbladder carcinoma. METHODS: We present the case of a 35-year-old Hispanic male complaining of right upper quadrant pain and jaundice for 2 months prior to admission. He denied a history of fever, nausea/ vomiting, and weight loss. The past medical history was relevant only for diabetes. He had no previous history of jaundice or previous operations. RESULTS: CA19-9 was slightly elevated (52 U/mL). Abdominal ultrasonography showed an irregular thickening of the gallbladder wall and no gallstones were detected. CT scan also revealed an irregular thickening of the wall of the gallbladder body suggestive of malignancy. At laparotomy, the mass was adherent to the duodenum and colon, and although the frozen section biopsy was negative, the intraoperative findings were suggestive of malignancy, and the patient underwent left liver trisegmentectomy, resection of the common bile duct and Roux-en-Y hepaticojejunostomy. Pathological examination unexpectedly revealed XGC without malignancy. CONCLUSIONS: Preoperative and intraoperative differential diagnosis of XGC from gallbladder carcinoma remains a challenge when it is associated with inflammatory involvement of surrounding tissues. Since gallbladder carcinoma and XGC may coexist, radical resection is justified when malignancy cannot be completely ruled out.

目的本研究旨在确定护士对家属在场时实施复苏的自信心与实践之间的关系,并描述护士对相关情形的偏好。方法采用横断面调查方法,研究对象来自肯尼亚西阿亚县转诊医院内外科不同单元。采用匿名自填问卷进行调查,主要测量工具为Twibel等设计的家属在场自信量表(Family Presence Self-confi-dence Scale)。采用χ^(2)检验和二元回归分析进行统计学分析。结果护士对家属在场时实施复苏操作的自信心与实践存在关联(χ^(2)=8.060,P=0.01)。非常自信的护士在临床实践中实施家属在场情况下复苏操作的可能性是有点自信的护士的4.9倍(OR=4.94,95%CI 1.07~22.71)。结论护士对实施家属在场复苏操作的自信心差异较大。为成功实施家属在场复苏实践,护士还需通过更多专门培训和实践获得更高水平的自信心。
