Association between axial length and toric intraocular lens rotation according to an online toric back-calculator

目的：报道特发性中心凹旁毛细血管扩张症（IJFT）伴新生血管膜（NVM）患者的视力结果。方法：我们对16例（26只眼）双眼IJFT伴NVM的患者进行了回顾性的无对照分析（Gass分类为2A组）。将眼分为2组：WO组（n=11）IJFT而不伴NVM；W组（n=15)则指入选时诊断为IJFT伴NVM。对WO组，观察了最初视力和从最初检查到诊断NVM的时间。回顾了2组患者特征性的眼底改变，包括脉络膜视网膜吻合存在与否，视网膜内的色素斑块，结晶样沉着以及最终的视力结果。结果：最初视力WO组为20／20－20／70（平均：20／30），W组为20／20－4／200（平均：20／70）。从最初诊断IJFT到NVM产生的平均时间为73个月（5-142个月）。也对WO组NVM产生之前脉络膜视网膜吻合和同时发生的血管旁视网膜色素上皮过度增生进行了观察。所有眼的最终视力为20／40－2／200（平均20／200）。81％的眼（21／26）最终视力≤20／200。结论：对IJFT伴NVM患者而言，通常最终的视力较差，其中80％眼最终视力（120／200。IJFT伴有脉络膜视网膜吻合和视网膜色素上皮增殖斑通常将加速NVM的产生。

Sensitivity,specificity and predictive values of anterior chamber tap in cases of bacterial endophthalmitis

Background:To assess the sensitivity,specificity,positive predictive value and negative predictive value of anterior chamber tap for the diagnosis of bacterial endophthalmitis on a population with high ***:Retrospective,single centre,case series *** reviewed all medical records with clinical diagnosis of bacterial endophthalmitis in our hospital from January 1st,2000 to December 31st *** each record,we documented general demographic data,best corrected visual acuity and vitreous and aqueous tap microbiological *** cases were further divided according to the endophthalmitis aetiology to perform individual calculations of sensitivity,specificity,positive predictive value,negative predictive value,accuracy and *** used the results of the vitreous tap as the gold standard for diagnosis of bacterial *** excluded those records in which the aqueous and vitreous samples were not taken simultaneously or had an incomplete microbiological *** were assessed with chi squared statistics,with an alpha value of 0.05 for statistical ***:A total of 190 cases fulfilled the inclusion/exclusion *** culture rate from vitreous samples was 64.74%.Positive culture rate from aqueous sample was 32.11%.Bacteria isolated from aqueous samples matched those isolated from vitreous samples 78.68%of the *** overall sensitivity was 38.21%,specificity:75.51%,positive predictive value:79.66%,negative predictive value:32.74%(p=0.08).Subgroup analysis showed that anterior chamber taps in cases of post-surgical endophthalmitis had a moderate to low sensitivity(37.73%),high specificity(93%)and high positive predictive value(95%)(p<0.04).Conclusion:The sensitivity and specificity of anterior chamber tap are low and should not be used for critical therapeutic decisions in patients with suspected bacterial *** cases of post-surgical endophthalmitis,the result of an anterior chamber tap

Purpose: To determine the feasibility and side effect profile of 2-octyl cyanoacrylate with parabens (Liquid Bandage) as an adjunct wound sealant and barrier against the inflow of ocular surface fluid after clear corneal cataract surgery. Design: Prospective interventional case series. Participants: Fifty-one eyes of 51 patients undergoing clear corneal cataract surgery. Methods: Fifty-one eyes of 51 patients who underwent cataract surgery by one surgeon (RSK) were enrolled. A 2.75-mm clear corneal, triplanar, temporal incision was used in each case, followed by routine phacoemulsification. At the conclusion of each case, the wound was dried with a cellulose sponge. A modified microsponge with 1 to 2 drops of 2-octyl cyanoacrylate with parabenswas then applied in a smooth layer to the wound. Allwoundswere rechecked for leakagewith a cellulose sponge applied at the wound lip. The following criteria were evaluated on the first 2 postoperative visits: (1) visual acuity; (2) patient complaints; (3) wound integrity; (4) presence of tissue adhesive; and (5) any adverse events, including abnormal increase in local tissue reaction to the adhesive. Main Outcome Measures: Side effect profile and clinical course of 2-octyl cyanoacrylate with parabens applied to clear corneal wounds. Results: All 51 eyes demonstrated intraoperative watertight wound closure after the application of 2-octyl cyanoacrylate with parabens. Forty-five eyes (88%) had tissue adhesive still completely covering the wound on postoperative day 1. Two eyes (4%) had tissue adhesive over portions of their wound, and 4 eyes (8%) had no adhesive noted on their wound. All wounds were watertight on all postoperative visits. Thirtyone of 51 patients (61%) complained of mild transient foreign body sensation on postoperative day 1. Twenty-six patients (51%) had trace diffuse bulbar conjunctival hyperemia.
