Renal aspergillosis in a liver transplant patient: A case report and review of literature

BACKGROUND Aspergillosis is a frequent invasive fungal infection in liver recipients(affecting 1%-9.2% of all patients), second only to candidiasis. Significant risk factors for invasive aspergillosis in liver recipients include corticosteroid therapy, neutropenia, T-cell dysfunction, renal failure and requirement for renal replacement therapy. Aspergillus infection usually affects the lungs of liver recipients, with hematogenous dissemination occurring in 50%-60% of cases. Renal involvement is rare and is considered to occur in 0.4% of all cases of invasive *** SUMMARY This paper describes a case of a liver recipient presenting with a newly formed renal mass a year after liver transplantation. The patient underwent liver transplantation due to alcoholic liver cirrhosis, with preoperative corticosteroid therapy and postoperative immunosuppressants(tacrolimus and mycophenolate mofetil). His 1-year follow-up was uneventful, with a satisfying graft function and lack of any symptoms. During a routine follow-up abdominal ultrasound, he was diagnosed with a renal tumor. The renal ima-ging findings were inconclusive(with a differential diagnosis to renal cell carcinoma), while the computed tomography(CT) of the chest showed scar tissue in the lungs suggestive of previous inflammation. The patient underwent radical nephrectomy, with histopathological analysis showing renal aspergilloma, yielding postoperative treatment with voriconazole. His follow up was uneventful, and the chest CT did not show any change in pulmonary lesions. This case illustrates the possibility of aspergillosis affecting the lungs of liver recipients, subsequently affecting the kidney and forming an *** Clinicians should be aware of aspergilloma mimicking solid organ tumors in organ recipients.

Early mortality and safety after transcatheter aortic valve replacement using the SAPIEN 3 in nonagenarians

Background Transcatheter aortic valve replacement （TAVR） has been performed for many elderly patients with severe aortic stenosis （AS）. The SAPIEN 3 is one of the latest balloon-expandable prosthesis. This study aimed to investigate the early clinical outcomes after TAVR using the SAPIEN 3 in nonagenarians. Methods A total of 97 consecutive patients underwent TAVR for severe AS between De- cember 2015 and December 2016. Of these, 85 consecutive patients who underwent TAVR using the SAPIEN 3 were included. According to the age, patients were classified into age 〉 90 years group （17 patients） or age 〈 90 years group （68 patients）. The clinical outcomes in- cluding all-cause mortality and composite endpoint of early safety at 30 days were evaluated. Results The Society of Thoracic Surgeons score in age 〉 90 years group was higher than age 〈 90 years group （12.3 ±6.1% vs. 8.5 ± 5.1%, P 〈 0.01）. There was no significant difference in 30-day mortality between the two groups. However, the life-threatening bleeding and major vascular complications in age 〉 90 years group were greater than age 〈 90 years group （11.8% vs. 1.5%, P = 0.04 and 11.8% vs. 1.5%, P = 0.04, respectively）. The composite endpoint of early safety at 30 days was similar between the two groups. Multivariate logistic regression analysis showed that prior myocardial infarction was an independent predictor of the composite endpoint of early safety （odds ratio： 4.76, 95% confidence interval： 1.02-22.21, P = 0.047）. Conclusions The early mortality and safety after TAVR using the SAPIEN 3 in nonagenarians were similar and acceptable despite of higher operative risk.

Review of health risks of low testosterone and testosterone administration

Hypogonadism is prevalent in older men and testosteronereplacement therapy(TRT) for older hypogonadal men is a promising therapy. However, a number of important clinical concerns over TRT safety remain unsolved due to a lack of large-scale randomized clinical trials directly comparing the health risks of untreated hypogonadism vs long-term use of TRT. Meta-analyses of clinical trials of TRT as of 2010 have identified three major adverse events resulting from TRT: polycythemia, an increase in prostate-related events, and a slight reduction in serum high-density lipoprotein cholesterol. There are other purported health risks but their incidence can be neither confirmed nor denied based on the small number of subjects that have been studied to date. Furthermore, subsequent literature is equivocal with regard to the safety and utility of TRT and this topic has been subject to contentious debate. Since January 2014, the United States Food and Drug Administration has released two official announcements regarding the safety of TRT and clinical monitoring the risks in TRT users. Additionally, the health risks related to the clinical presentation of low or declining testosterone levels not been resolved in the current literature. Because TRT is prescribed in the context of putative risks resulting from reduced testosterone levels, we reviewed the epidemiology and reported risks of low testosterone levels. We also highlight the current information about TRT utilization, the risks most often claimed to be associated with TRT, and current or emerging alternatives to TRT.

Differentiation renders susceptibility to excitotoxicity in HT22 neurons

HT22 is an immortalized mouse hippocampal neuronal cell line that does not express cholinergic and glutamate receptors like mature hippocampal neurons in vivo. This in part prevents its use as a model for mature hippocampal neurons in memory-related studies. We now report that HT22 cells were appropriately induced to differentiate and possess properties similar to those of mature hippocampal neurons in vivo, such as becoming more glutamate-receptive and excitatory. Results showed that sensitivity of HT22 cells to glutamate-induced toxicity changed dramatically when comparing undifferentiated with differentiated cells, with the half-effective concentration for differentiated cells reducing approximately two orders of magnitude. Moreover, glutamate-induced toxicity in differentiated cells, but not undifferentiated cells, was inhibited by the N-methyi-D- aspartate receptor antagonists MK-801 and memantine. Evidently, differentiated HT22 cells expressed N-methyI-D-aspartate receptors, while undifferentiated cells did not. Our experimental findings indicated that differentiation is important for immortalized cell lines to render post-mitotic neuronal properties, and that differentiated HT22 neurons represent a better model of hippocampal neurons than undifferentiated cells.

Content-adaptive robust error concealment for packet-lossy H.264 video streaming

In this paper, we present a spatio-temporal post-processing error concealment (EC) algorithm designed initially for a H.264 video-streaming scheme over packet-lossy networks. It aims at optimizing the subjective quality of the restored video under the constraints of low delay and computational complexity, which are critical to real-time applications and portable devices having limited resources. Specifically, it takes into consideration the physical property of motion field in order to achieve more meaningful perceptual video quality, in addition to the improved objective PSNR. Further, a simple bilinear spatial interpolation approach is combined with the improved boundary-match (B-M) based temporal EC approach according to texture and motion activity analysis. Finally, we propose a low complexity temporal EC method based on motion vector interpolation as a replacement of the B-M based approach in the scheme under low-computation requirement, or as a complement to further improve the scheme's performance in applications having enough computation resources. Extensive experiments demonstrated that the proposal features not only better reconstruction, objectively and subjectively, than JM benchmark, but also robustness to different video sequences.

On the material constants measurement method of a fluid-saturated transversely isotropic poroelastic medium

The fluid-saturated transversely isotropic poroelastic medium could be widely found in nature, e.g., the sedimentary rocks underground. To determine the eight independent material constants for the transversely isotropic poroelastic medium, a series of tests are discussed. Two undrained tests and one drained test are suggested as a set of tests of the least amount. For the verification purpose, two additional drained tests are also introduced as an option. The atmospheric dried test is discussed as a replacement of the traditional infiltrated drained test to save the time waiting for an equilibrium state. Some microscopic material constants, i.e.,the unjacketed bulk coefficients, the porosity, and the compressibility of porous fluid, are measurable but unnecessary to determine the independent material constants of a poroelastic medium.

Optimizing feeding regimen of replacement gilts to improve their reproductive performance and retention rate of their first 2 parities

The effects of age,body weight(BW),and backfat thickness(BF)of replacement gilts at first estrus and first mating on their subsequent reproductive performance and retention of their first 2 parities as sows was evaluated.A total of 3,025 Danish replacement gilts were categorized by farm(allocated to 4 farms),cross combination,age,BW and BF at first estrus and first mating,estrous cycle number at first mating,and flush feeding before first *** result shows that all the factors mentioned above were significantly associated with reproductive performance and retention rates of the first 2 *** 3 had more piglets born alive per litter(BA)(Preplacement gilts had the most total piglets born per litter(TB),BA,HP,PW and a higher retention rate of the 2 parities than Landrace×Yorkshire(L×Y)replacement gilts(Preplacement gilts rearing parameters on reproductive performance traits differed,we used 100 replacement gilts as a unit and the total number of weaned piglets from the first 2 parities as a new *** gilts undergoing their first estrus between 180 and 210 d of age at 115 to 124.9 kg BW and 14 to 15 mm BF had significantly higher reproductive indexes for their first 2 parities per 100 replacement *** gilts that mated between 210 and 230 d of age at 140 to 149.9 kg BW and 15 to 16 mm BF had optimal reproductive *** results provide a new insight into the complex relationships among these reproductive performance traits and may help guide successful management of replacement gilts as a pivotal starting point for future fertility and longevity of rearing herds.

Physico-chemical Characteristics of Wet-milled Ultrafine-granulated Phosphorus Slag as a Supplementary Cementitious Material

The phosphorus slag（PS） can be used as a supplementary cementitious material due to its potential hydrating activity. However, its usage has been limited by its adverse effects, including prolonged setting and lowered early-stage strength. In this study, we achieved ultrafine granulation of PS using wetmilling（reducing d50 to as low as 2.02 μm） in order to increase its activity, and examined the physico-chemical properties of the resulting materials, including particle-size distribution, slurry pH, zeta potential, and activity index, as well as how their replacement level and granularity affect the setting time and mechanical performance of PS-cement mixture systems. The results suggested that as the granularity increases, there are significant boosts in the uniformity of particle sizes, slurry pH, and activity index, and the effects on cement paste, including setting times, and early-and late-stage strengths, are significantly mitigated. When d（50）=2.02 μm, the slurry becomes strongly alkaline（pH=12.16） compared to the initial d（50）=20.75 μm（pH=9.49）, and the activity is increased by 73%; when used at 40% replacement, the PS-cement mixture system can reach a 28 d compressive strength of 93.2 MPa, 36% higher than that of the pure cement control group.
