Factors affecting response rates to mailed preoperative surveys among arthroplasty patients

AIM: To identify factors that affect patient response rates to preoperative functional surveys in hip and knee arthroplasty ***: From May 2008 to March 2009, 247 patients were scheduled more than 4 wk in advance for hip or knee arthroplasty by one of two participating surgeons at our center. A personalized questionnaire comprised of the Short Form 12(SF-12) and Western Ontario and Mc Master Universities(WOMAC) Index was mailed to patients at random time points ranging from 7 to 101 d prior to surgery. Nine independent factors were documented prospectively, including age, gender, ethnicity, marital status, type of surgery, surgeon, days prior to surgery(DPS) of survey mailing, WOMAC score and SF-12 score. The date of the completed survey receipt was also documented. For non-responders, the surveys were completed with the research team at the hospital upon admission. Multivariate regression and χ2 analysis were performed with Statistical Analysis Software ***: DPS was the only factor that affected patient response. Mailing surveys 26 d to 31 d prior to surgery dates led to a peak response rate of 80% that was significantly higher(P < 0.023) than response rates for patients who were mailed their surveys ≤ 16 d(62.5%), 17 d to 25 d(70%) or ≥ 32 d prior to surgery(55%). No other factors, including preoperative WOMAC and SF-12 scores, significantly influenced response ***: The DPS was independently the most significant predictor of response rates for pre-operative functional data among patients scheduled for hip and knee arthroplasty.

The ulnar collateral ligament(UCL) is a vital structure to the overhead athlete,especially the baseball *** reasons not completely understood,UCL injurieshave become increasingly more common in major league baseball(MLB) pitchers over the past 10 *** reconstruction(UCLR) is the current gold standard of treatment for these injuries in MLB pitchers who wish to return to sport(RTS) at a high level and who have failed a course of non-operative *** following UCLR in MLB pitchers have been encouraging,with multiple RTS rates now cited at greater than 80%.Unfortunately,with the rising number of UCLR,there has also been a spike in the number of revision UCLR in MLB *** to primary UCLR,the etiology of the increase in revision UCLR,aside from an increase in the number of pitchers who have undergone a primary UCLR,remains *** current literature has attempted to address several questions including those surrounding surgical technique(method of exposure,graft choice,management of the ulnar nerve,concomitant elbow arthroscopy,etc.),post-operative rehabilitation strategies,and timing of RTS following *** some questions have been answered,many remain *** literature surrounding UCLR in MLB pitchers will be reviewed,and future directions regarding this injury in these high level athletes will be discussed.

Duodenal gangliocytic paraganglioma with lymph node metastases: a case report and comparative review of 31 cases

Gangliocytic paraganglioma(GP) is a rare tumor of uncertain origin most often located in the second portion of the duodenum. It is composed of three cellular components: Epithelioid endocrine cells, spindlelike/sustentacular cells, and ganglion-like cells. While this tumor most often behaves in a benign manner, cases with metastasis are reported. We describe the case of a 62-year-old male with a periampullary GP with metastases to two regional lymph nodes who was successfully treated with pancreaticoduodenectomy. Using Pub Med, EMBASE, EBSCOhost MEDLINE and CINAHL, and Google Scholar, we searched the literature for cases of GP with regional lymph node metastasis and evaluated the varying presentations, diagnostic workup, and disease management of identified cases. Thirty-one cases of GP with metastasis were compiled(30 with at least lymph node metastases and one with only distant metastasis to bone), with age at diagnosis ranging from 16 to 74 years. Ratio of males to females was 19:12. The most common presenting symptoms were abdominal pain(55%) and gastrointestinal bleeding or sequelae(42%). Twenty-five patients underwent pancreaticoduodenectomy. Five patients were treated with local resection alone. One patient died secondary to metastatic disease, and one died secondary to perioperative decompensation. The remainder did well, with no evidence of disease at follow-up from the most recent procedure(except two in which residual disease was deliberately left behind). Of the 26 cases with sufficient histological description, 16 described a primary tumor that infiltrated deep to the submucosa, and 3 described lymphovascular invasion. Of the specific immunohistochemistry staining patterns studied, synaptophysin(SYN) stained all epithelioid endocrine cells(18/18). Neuron specific enolase(NSE) and SYN stained most ganglion-like cells(7/8 and 13/18 respectively), and S-100 stained all spindle-like/sustentacular cells(21/21). The transformation of a Palladium-based metal-organic cage to a structurally similar one by direct ligand replacement usually leads to unwanted ligand scrambling. In this work, an intermediate ligand with different shape and basicity from the initial/final ones was introduced to avoid ligand scrambling to achieve the efficient indirect cage-to-similar-cage transformation. Compared with the direct transformation, the stepwise conversion has the advantages of high efficiency(93%) and simple workup.

背景：肥胖患者金属对金属髋关节表面置换术疗效如何，目前尚不清楚。本研究通过比较体重指数≥30患者和体重指数〈30患者的临床疗效，评估体重指数对于金属对金属髋关节表面置换假体生存率的影响。方法：回顾性研究本院所有接受过金属对金属髋关节表面置换术并经两年以上随访的病例。根据患者手术时的体重指数，将这些患者分为研究组（体重指数≥30）、对照组（体重指数〈30）。研究组125例患者（144侧髋关节），平均体重104．6kg，平均体重指数33．4；对照组531例患者（626侧髋关节），平均体重7813kg，平均体重指数25．4。根据UCLA（University of Califomiaat Los Angeles，加州大学洛杉矶分校）评分、Harris髋关节评分、SF-12（ShortForm-12，生活质量12项问卷调查）、并发症发生率、影像学检查以及假体生存率比较两组临床疗效。结果：术后UCLA疼痛、行走评分以及SF-12问卷心理方面评分两组间无显著性差异。然而，研究组UCLA功能评分（9．2）低于对照组（9．6，p=0．001），活动能力评分（7．1）也低于对照组（7．6，p-0．002）。对照组SF-12术后身体方面评分（51．4）明显高于研究组（49．3，p=0．01），术后Harris评分（93．8）也明显高于研究组（90．6，p=0．0003）。研究组2例髋关节（占1．4％）翻修。与之相比，对照组31例髋关节（5．0％）转换成全髋关节置换，其中20例由于股骨假体松动而翻修。研究组髋关节假体5年生存率为98．6％，对照组为93．6％（p=0．0401）。根据体重指数，将所有病例分为〈25、25-29、≥30三组，结果显示随着体重指数增加，相邻两组患者翻修风险减少两倍（p=0．013）。无一例患者发生髋臼假体松动。股骨假体平均直径，研究组为4813mm，对照组为46．8mm（p=0．0001）。27例体重指数≥35的患者，无一例翻修，亦未见X线片上假体周围有透明带改变。结论：尽管关节功能评分较体重指数〈30的患者为低，但金属对金属髋关节表面置换在高体重指数患者中效果满意。这种高体重指数对假体生存率的作用可能源于此类患者往往活动较少以及选用假体型号较大。

Rotator Cuff Reconstruction and Humeral Head replacement Using Smaller Humeral Prostheses in Cuff Tear Arthropathy Patients under 70 Years of Age

Background: Most reverse shoulder arthroplasty (RSA) studies have shown good improvement in arm elevation without improvements in external rotation (ER). In addition, high rates of complications after long-term RSA have been reported, suggesting that RSA should be limited to elderly patients, especially those who are older than 70 years old. Since 2001, we have developed a strategy of rotator cuff reconstruction with muscle transfer and humeral head replacement (HHR), using smaller humeral prostheses, in cuff tear arthropathy patients. The aim of the present study was to investigate the clinical outcome of our strategy in patients under 70 years of age who had irreparable rotator cuff tears and osteoarthritis. Materials and Methods: A total of 25 shoulders of 25 patients under 70 years of age (males, 15;females, 10) with irreparable cuff tears were treated with HHR and cuff reconstruction. The average age at the time of surgery was 64.3 years (range, 55 - 69) and the average follow-up period was 38.7 months (range, 24 - 72). The cuff defect was repaired using a partial subscapularis transfer in 14 shoulders;nine shoulders required a latissimus dorsi transfer, one required a pectoralis major transfer, and one required both a latissimus dorsi and a pectoralis major transfer. Clinical outcomes were assessed with the range of motion (ROM), UCLA score, Japanese Orthopaedic Association (JOA) score and postsurgical complications. Results: Shoulder pain was diminished in all patients after surgery. The preoperative UCLA and JOA scores were 13.1 and 47.0 respectively, improving to 28.6 and 81.5 respectively after surgery. Active forward flexion has improved from an average of 89.0° to 138.8°, and the ER improved from an average of 16.2° to 33.2°. No complications occurred after surgery. Electronic Structure and Stability of Lead-free Hybrid Halide Perovskites:A Density Functional Theory Study

The most commonly used and studied hybrid halide perovskite is ABX_3, where A usually stands for CH_3NH_3, B for Pb, and X for I. A lead-free perovskite with high stability and ideal electronic band structure would be of essence, especially considering the toxicity of lead. In this work, we have considered 11 metal elements for the B site and three halide elements(Cl, Br, and I) including various combinations among the three halides for the X site. A total number of 99 hybrid perovskites are studied to understand how the crystal structure, band gap and stability can be tuned by the chemistry modification, i.e., the replacement of toxic element, Pb in the original MAPbX_3, with non-toxic metal elements. We find that the favorable substitutes for Pb in MAPbI_3 are Ge and Sn.
