目的肝素和华法林是外科手术中预防静脉血栓栓塞症（venous thromboembolism，VTE）的常用药物。抑制Xa因子（factorXa，FXa）系基于一种特异性的抗凝机制，为改善效益风险比提供了可能。方法评估FXa的口服强效直接抑制剂阿哌沙班在全膝关节置换（total knee replacement，TKR）术后患者中的疗效和安全性，并研究其剂量一反应关系。患者／方法1238例患者随机分配至6个不同剂量的双盲阿哌沙班组[5、10或20rog／d，分别按每日1次（QD）或每日2次（BID）给药]、依诺肝素组（30mgBID）或开放标签的华法林组[剂量滴定至国际标准化比值（International Normalized Ratio，INR）1．8～3．0]。治疗持续lO～14d，术后12～24h开始应用阿哌沙班或依诺肝素治疗，华法林组于手术当晚以华法林治疗。主要疗效结局指标为治疗期间VTE（静脉造影）和全因死亡的复合终点，主要安全性结局指标是严重出血事件。结果共1217例患者可进行安全性分析，856例患者可进行疗效分析。所有阿哌沙班组患者的疗效终点事件发生率均低于对照组。随着阿哌沙班剂量的增加，主要终点事件发生率降低（QD／BID组联合分析P=0．09；QD组P=0．19；BID组P=0．13）。无论在QD组（P=0．01）还是BID组（P=0．02），总出血事件发生率均表现出随剂量上升的趋势；但QD组与BID组间差异无统计学意义。结论与现有的TKR术后标准治疗方案相比，阿哌沙班2．5mgBID或5mgQD方案的效益风险比较高。

目的研究心脏外科术后肾脏替代治疗患者资料,分析影响其预后的危险因素. 方法回顾性分析阜外医院2011年5月～2014年10月期间第二成人外科恢复室收治的心脏外科术后肾脏替代治疗患者资料,包括患者一般资料、基础疾病、手术类型及体外循环时间,术中及术后各项临床指标.终点指标包括机械通气时间、ICU治疗时间、死亡人数、围术期并发症发生率. 结果全组216名患者死亡68例,死亡率31.5％.死亡率最高为冠脉搭桥联合瓣膜置换术死亡32人,死亡率56.1％（32/57）;心脏移植术后死亡5例,死亡35.7％（5/15）;冠脉搭桥术后死亡23人,死亡率28.0％（23/82）;瓣膜置换术死亡7人,死亡率15.2％（7/46）;先心病矫治术死亡1例,死亡率9.1％（1/11）.术前心功能分级、术中低血压、术后低心排综合征及体外膜氧合（ECMO）辅助是心脏术后肾脏替代治疗患者死亡的重要危险因素.住院期间肾功能恢复者73例（33.8％）,转为慢性肾功能不全需要长期血液透析者41例（18.9％）.术前基础血肌酐水平、术中低血压、二次开胸探查、多尿期出现时间是肾功能恢复的重要危险因素.结论心脏外科术后肾脏替代治疗患者死亡率高、与围术期多种危险因素密切相关.

To the Editor:In sepsis,bacterial endotoxins play a central role by inducing a dysregulated and exaggerated release of inflammatory mediators and cytokines,often described as the“cytokine storm”or“cytokinemia.”[1]Both endotoxins and cytokines have been implicated in the development of organ dysfunction,including acute kidney injury(AKI).[2]Removing circulating endotoxins and excess cytokines from the circulation restores chemotactic gradients,localizing the immune response to the principal site of infection and re-establishing a regulated immune response.[1,3]Therefore,using hemofilters with enhanced endotoxin and cytokine adsorptive properties may bring about additional therapeutic benefits compared with conventional continuous renal replacement therapy(CRRT)filters.
