Combinative approach of transzonular triamcinolonemoxifloxacin and perioperative drops to minimize postoperative complications of cataract surgery

AIM: To investigate the effectiveness of combinationtherapy with transzonular triamcinolone-moxifloxacin andconventional perioperative drops in reducing postoperativecomplications of cataract surgery. METHODS: Electronic medical records of cataractsurgery patients (single surgeon) were reviewed fromJanuary 2018 to September 2021. The rate of postoperativecomplications including prolonged and/or recurrentpostoperative inflammation, endophthalmitis, cystoid macularedema (CME), and intraocular pressure (IOP) was comparedbetween the patients receiving combinative therapy andpatients receiving drops only. RESULTS: Totally 596 patients and 1057 eyes(Combinative-Therapy group 493 and Drop-Only group 564)were included in this study. Using combination therapyreduced the relative risk of postoperative inflammationby 26.9% (16.6% Combinative-Therapy vs 22.7% Drop-Only, P=0.013). The incidence of endophthalmitis was 0in Combinative-Therapy group vs 0.5% in Drop-Only group(relative risk reduction 100%), although not statisticallysignificant (P=0.10). The incidence of severe IOP spikeswas not significantly different between Combinative-Therapy (2.4%) and Drop-Only (1.6%) groups (P=0.33).The relative risk of postoperative CME was 51.4% less inthree months follow up visit in Combinative-Therapy group,although not statistically significant (P=0.07). The visualoutcome 1-month postop. (best corrected visual acuity) wassignificantly better in Combinative-Therapy (logMAR 0.10)compared to Drop-Only (logMAR 0.14) groups (P=0.02) whilethe baseline visual acuity was not significantly different. CONCLUSION: The combinative approach oftranszonular triamcinolone-moxifloxacin plus perioperativeeyedrops is an effective method to minimize postoperativeinflammation, with better visual outcomes. It couldpotentially reduce the risk of postoperative endophthalmitisand CME (near-significant P-values;larger studies couldanalyze better considering low incidence).

This narrative review highlights routes of ocular drug delivery for age-related macular degeneration(AMD).AMD is the leading cause of irreversible blindness in industrialized countries and accounts for 8.7%of blindness *** AMD can be classified into two subtypes:late-stage dry AMD[known as geographic atrophy(GA)]and neovascular AMD(nAMD).GA is often bilateral and results from progressive and irreversible loss of photoreceptors and areas of the retinal pigment *** AMD is characterized by angiogenesis from the choroid to the normally avascular regions underneath the retinal pigment epithelium(RPE)or retina,a process known as choroidal neovascularization(CNV).Various targeted therapeutic options are currently available to reduce the progression rate and maintain vision in patients with *** delivery of anti-VEGF protein treatments to halt CNV is currently the gold-standard of care for *** and suprachoroidal delivery approaches are also being explored for gene and molecular *** in nanotechnology and biomaterials have also led to the development of microscopic drug delivery systems,including hydrogels,microparticles,nanoparticles,implants,and *** therapy and stem cell therapy has recently emerged as a potential candidate treatment modality for AMD and other retinal *** drug targets and modalities have stimulated exciting developments in ocular drug delivery with the promise of greater efficacy and durability of AMD treatment.

Falls represent a significant contribution to the morbidity and mortality of the elderly population. Because vitamin D is important in bone physiology, the use of vitamin D to restore deficient bone and ameliorate the effects of bone fractures due to falls has become a common practice in recent years. Following introduction of widespread use, reports began to emerge that vitamin D not only aided in repair of fall-induced bone fractures, but that it also reduced the occurrence of falls. Vitamin D now has become a routine intervention as a fall-prevention measure. Early analyses found evidence of prevention efficacy (reduced falls), but recent analyses are more equivocal. We retrospectively examined the records of 350 patients in a long-term care facility in which vitamin D administration and the number of falls were recorded as part of a comprehensive database of care. We found a dramatic rise in vitamin D use over the period covered (2006 – 2011) and a corresponding dramatic decrease in the number of falls. However, the number of falls continued to decline after 2008, despite a plateau in number of patients on vitamin D, particularly females. It appears that other factors contribute to the overall decline.

AIM: To investigate the diagnostic validity and therapeutic value of lumbar facet joint interventions in managing chronic low back ***: The review process applied systematic evidence-based assessment methodology of controlled trials of diagnostic validity and randomized controlled trials of therapeutic efficacy. Inclusion criteria encompassed all facet joint interventions performed in a controlled fashion. The pain relief of greater than 50% was the outcome measure for diagnostic accuracy assessment of the controlled studies with ability to perform previously painful movements, whereas, for randomized controlled therapeutic efficacy studies, the primary outcome was significant pain relief and the secondary outcome was a positive change in functional status. For the inclusion of the diagnostic controlled studies, all studies must have utilized either placebo controlled facet joint blocks or comparative local anesthetic blocks. In assessing therapeutic interventions, short-term and long-term reliefs were defined as either up to 6 mo or greater than 6 mo of relief. The literature search was extensive utilizing various types of electronic search media including Pub Med from 1966 onwards, Cochrane library, National Guideline Clearinghouse, ***, along with other sources includingprevious systematic reviews, non-indexed journals, and abstracts until March 2015. Each manuscript included in the assessment was assessed for methodologic quality or risk of bias assessment utilizing the Quality Appraisal of Reliability Studies checklist for diagnostic interventions, and Cochrane review criteria and the Interventional Pain Management Techniques- Quality Appraisal of Reliability and Risk of Bias Assessment tool for therapeutic interventions. Tropical cyclone （TC） Nargis （2008） made landfall in Myanmar on 02 May 2008, bringing a storm surge, major flooding, and resulting in a significant death toll. TC Nargis （2008） displayed abnormal features, including rare eastward motion in its late stage, rapid intensification before landing. Using reanalysis data and a numerical model, we investigated how a low-latitude westerly wind modulated TC Nargis’ （2008） track and provided favorable atmospheric conditions for its rapid intensification. More importantly, we found a possible counterbalance effect of flows from the two hemispheres on the TC track in the Bay of Bengal. Our analysis indicates that a strong westerly wind burst across the Bay of Bengal, resulting in TC Nargis’ （2008） eastward movement after its recurvature. This sudden enhancement of westerly wind was mainly due to the rapidly intensified mid-level cross-equatorial flow. Our results show that a high-pressure system in the Southern Hemisphere induced this strong, mid-level, cross-equatorial flow. During the rapid intensification period of TC Nargis （2008）, this strong and broad westerly wind also transported a large amount of water vapor to TC Nargis （2008）. Sufficient water vapor gave rise to continuously high and increased mid-level relative humidity, which was favorable to TC Nargis’ （2008） intensification. Condensation of water vapor increased the energy supply, which eventuated the intensification of TC Nargis （2008） to a category 4 on the Saffir-Simpson scale.

Interstate Comparison of Soil Remediation Standards among Six mid-atlantic States, USA

To address and help mitigate potential public health and ecological impacts associated with contaminated soil, most state environmental agencies have promulgated cleanup standards or action level criteria that are based broadly on US Environmental Protection Agency risk assessment methodologies. These standards or criteria often are assembled into easy-to-use look-up tables that allow responsible parties (RPs) to determine quickly the extent of remediation that could be required simply by comparing site investigation data to the listed cleanup goal or standard. This paper compares and contrasts soil remediation standards and criteria for 20 common soil pollutants taken from state environmental agency look-up tables for five middle atlantic States: New York, Connecticut, New Jersey, Delaware, Pennsylvania, and Maryland. We examine the differences between numeric remedial goals for these pollutants and propose a relative rank for each state based on the overall degree of soil cleanup standard or criterion stringency. In order to identify and rank the stringency of the residential cleanup goals or standards published by the six mid-atlantic States, a three-step process was used that included compiling in one data set, the numerical (mg/kg), residential or unrestricted use look-up values published by state for each of the 20 contaminants;organizing and grouping those values in numerical sequence into one of three categories ranging from lowest (Most Restrictive) to highest (Least Restrictive);and then ranking each state by the number of first place finishes in each stringency category: Most Restrictive, Moderately Restrictive, and Least Restrictive. The socioeconomic consequences of these ranks were examined relative to their effects on gross state product, unemployment, and health.
