Oral cancer (OC) is the most common form of head and neck cancer. Despite the high incidence and unfavourable patient outcomes, currently, there are no biomarkers for the early detection of OC. This study aims to discover, develop, and validate a novel saliva-based microRNA signature for early diagnosis and prediction of OC risk in oral potentially malignant disorders (OPMD).The Cancer Genome Atlas (TCGA) miRNA sequencing data and small RNA sequencing data of saliva samples were used to discover differentially expressed miRNAs. Identified miRNAs were validated in saliva samples of OC (n=50), OPMD (n=52), and controls(n=60) using quantitative real-time PCR. Eight differentially expressed miRNAs (miR-7-5p, miR-10b-5p, miR-182-5p, miR-215-5p,miR-431-5p, miR-486-3p, miR-3614-5p, and miR-4707-3p) were identified in the discovery phase and were validated. The efficiency of our eight-miRNA signature to discriminate OC and controls was:area under curve (AUC):0.954, sensitivity:86%, specificity:90%,positive predictive value (PPV):87.8%and negative predictive value (NPV):88.5%whereas between OC and OPMD was:AUC:0.911,sensitivity:90%, specificity:82.7%, PPV:74.2%and NPV:89.6%. We have developed a risk probability score to predict the presence or risk of OC in OPMD patients. We established a salivary miRNA signature that can aid in diagnosing and predicting OC,revolutionising the management of patients with OPMD. Together, our results shed new light on the management of OC by salivary miRNAs to the clinical utility of using miRNAs derived from saliva samples.

Effective communication among healthcare professionals in the intensive care unit (ICU) is a particular imperative,with accurate and efficient interdisciplinary communication being a critical prerequisite for high-quality *** and physicians are highly important parts of the healthcare system ***,identifying strategies that would improve communication between these two groups can provide evidence for practical improvement in the ICU,which will ultimately improve patient *** integrative literature review aimed to identify interventions that improve communication between nurses and physicians in *** databases (Medline,CINAHL,and Science Direct) were searched between September 2014 and June 2016 using 11 search terms,namely,nurse,doctor,physician,resident,clinician,ICU,intensive care unit,communication,teamwork,collaboration,and relationship.A manual search of the reference lists of found papers was also *** articles met the inclusion *** studies reported on the use of communication tools/checklists,team training,multidisciplinary structured work shift evaluation,and electronic situation-background-assessment -recommendation documentation templates to improve *** which intervention strategies are most effective remains unclear,this review suggests that these strategies improve communication to some *** studies should be rigorously designed and outcome measures should be specific and validated to capture and reflect the effects of effective communication.

The "five senses of success" in nursing students: Assessing first-year support engagement

Objectives: This study aimed to identify school and University support services available and accessed by nursing students transitioning into a university environment as many struggle to adjust to competing demands of personal commitments and expectations at ***: A mixed methods design was used,based on activity theory and Lizzio's Five Senses of Success frameworks as exploratory *** study was conducted amongst the first year cohort at one campus of the Bachelor of nursing (BN) program in queensland,*** initial baseline assessment of what University and school-based support services were on offer for students,and how the students interacted with the support services was *** was followed by a survey to identify awareness and access to support *** groups were then conducted to clarify the previous results and to determine engagement with these support ***: A randomly selected number of students (n =150) in the first-year 2014 cohort of the BN program (n =300) were included in this *** survey was completed by 54 students and three semistructured focus groups were *** analysis indicated that the support services in place were successful in reaching the majority of students and contributed to their sense of success at university,Specifically students identified that a whole cohort approach to support enhanced their transition to ***: Identifying lesser known services early in the first year will ensure that students are supported and encouraged to use all services,contributing to their sense of success at university.

Early mobilization implementation for critical ill patients:A cross-sectional multi-center survey about knowledge,attitudes,and perceptions of critical care nurses

重症监护病房医疗卫生人员间的有效沟通非常重要,准确高效的跨学科沟通是高质量服务的先决条件。医生和护士是医疗卫生人员的重要组成部分,确定医护沟通的促进策略可以为重症监护病房服务改进提供依据,进而保障患者安全和提高服务质量。该整合性综述旨在确定改善重症监护病房医护沟通的干预策略。2014年9月至2016年6月,使用11个检索词(nurse,doctor,physician,resident,clinician,ICU,intensive care unit,communication,teamwork,collaboration,relationship),检索3个数据库(Medline,CINAHL和Science-Direct)中关于重症监护病房医护沟通的文献,之后手动检索已有文献的参考文献列表。由2名研究员按照纳入和排除标准筛选文献,最终纳入11篇文章进行分析。这些研究报告了4种类别的干预策略:沟通工具/核查表、团队培训、多学科结构化交班评估和现状-背景-评估-建议(situation,background,assessment,recommendation,SBAR)记录模板。虽然该整合性综述不能说明哪种干预策略是最有效的,但结果显示这些策略在一定程度上都改善了医护沟通。目前还没有一个较客观的方法对沟通有效性进行评价,亟需构建科学化、标准化的评价指标体系,研究设计需要更加严谨。

Objective:This study aimed to explore the lived experiences of the disease journey and patients'care needs with idiopathic pulmonary fibrosis(IPF).Methods:Face-to-face semi-structured interviews were conducted with a purposive sampling of IPF patients admitted to the department of respiratory medicine in a tertiary hospital in *** data were analyzed using the thematic analysis *** the end,16 patients were ***:Four themes emerged from the qualitative data included the long and confusing journey to reach a diagnosis,living with the disease,understanding the disease and treatment and desire for continuity of care.A series of subthemes were also identified,including uncertainty of diagnosis,delaying the process,living with physical symptoms,living with emotional distress,loss of independence,uncertainty with the prognosis,questioning the cause of the disease,concerning the side effects of treatments,lacking continuity of care,and wanting a better quality ofhealthcare in community ***:Based on the findings,there is an urgent need to improve the care delivery to this vulnerable population in *** meet their health needs,it is of paramount importance to develop effective education programs for health professionals and IPF patients and improve care models of healthcare systems,especially in remote areas,to enhance care continuity in the communities.

目的呼吸防护在医疗卫生工作中至关重要,应使用适当的颗粒过滤呼吸器和等效的呼吸防护用品,以确保良好的佩戴气密性,最大限度地降低医务人员的空气传播感染风险。该文旨在对呼吸防护用品适合性检验和密合性检查有效性研究进行综述和分析。方法系统检索了2003年1月至2022年4月发表的文献,检索范围包括CINAHL、Cochrane图书馆、EMBASE、PubMed、Ovid数据库Medline子库和灰色文献。研究方案已在PROSPERO注册(CRD42020213968)。结果共检索出561篇文献,25篇被纳入该综述,包括22项量性研究和3篇指南文献。在评估呼吸防护用品的有效性方面,适合性检验是更有效且被广泛采用的可靠方法,但呼吸防护程序应该同时包括适合性检验和密合性检查,以最大限度地提高呼吸防护用品的有效性。新型冠状病毒疫情凸显了医务人员相关知识的缺乏,相关教育培训可有效提高呼吸防护用品使用的有效性。结论为确保医务工作者的安全,适合性测试和密合性检查均应采用。建议由专业人员实施全面的呼吸防护项目,包括教育培训、适合性测试和密合性检查,还需要探索适用于实际使用环境的适合性检验方法。

Development and Validation of a Questionnaire to Measure health Professionals’ Attitudes toward Identification of Female Victims of Domestic Violence

Back ground: Domestic violence against women is a major public health problem and violations of women’s human rights. health professionals could play an important role in screening for the victims. From the evidence to date, it is unclear whether health professionals do play an active role in identification of the victims. Objectives: To develop a reliable and valid instrument to measure health professionals’ attitude to identifying female victims of domestic violence. Methods: A primary questionnaire was constructed in accordance with established guidelines using the Theory of Planned Behaviour Ajzen (1975) to develop an instrument to measure health professionals’ attitudes in identifying female victim of DV. An expert panel was used to establish content validity. Focus groups amongst a group of health professionals (N = 5) of the target population were performed to confirm face validity. A pilot study (N = 30 nurses and doctors) was undertaken to elicit the feasibility and reliability of the questionnaire. The questionnaire was also administered a second time after one week to check the stability of the tests. Results: Feedbacks of the expert panel’s and group discussion confirmed that the questionnaire had the content and face validity. Cronbach’s alpha values for all the items were greater than 0.7. Strong correlations between the direct and indirect measures confirmed that the indirect measures were well constructed. High test-retest correlations confirmed that the measures were reliable in the sense of temporal stability. Significance: This tool has the potential to be used by researchers in expanding the knowledge base in this important area.
