Effects of Diabetes Education on Emotional Distress in Patients with Type 2 Diabetes—An Experimental Study

Objective: It was to evaluate the effect of diabetes education on emotional distress in type 2 diabetes patients treated with oral medications. Methods: The experimental study took place in Albania and overall, 200 type 2 diabetes patients were enrolled (in both groups, intervention, and control) treated with oral medications, having levels of Glycated hemoglobin HbA1c > 6.5% as well the absence of associated diseases such as dementia and psychiatric disorders. Patients were randomly selected from the medical registry of family physicians in the Tirana region. Patients were screened for the emotional distress before and after the intervention with the self-administered questionnaire Problem Areas in Diabetes PAID 5. In addition, the levels of HbA1c in % were evaluated before and after intervention in both groups. Only intervention group underwent four diabetes education sessions offered by trained nursing staff while the control group continued the previous regime. The questionnaire reliability analysis was estimated by the Cronbach alpha coefficient. To compare the groups the t-test was used and the value of p Results: Mean age of patients in intervention and control group was respectively 54.03 ±9.57 and 55.82 ± 7.86. Before and after health education PAID 5 scores for the intervention group were respectively 11.3 vs. 8.75 while for the control group 11.9 vs. 11.35, p = 0.018. Levels of HbA1c% before and after education for the intervention group were 7.02 vs. 6.2 while for the control group 6.9 vs. 6.8, p = 0.001. Positive and significant correlation (r = 0.321, p = 0.001) was between level of emotional distress and the age of the patients. Conclusions: The study found that besides better control of diabetes, additional education of diabetic patients seemed to significantly improve the level of emotional distress due to diabetes in diabetic patients.

Zusammenfassung: Evaluierung der kombinierten Therapie bestehend aus transarterieller Chemoembolisation (TACE) und lokaler Immunotherapie des hepatozellulren Karzinoms (HCC) am Kleintiermodell. Bei mnnlichen ACI-Ratten (n=30) erfolgte nach subkapsulrer Implantation eines soliden Morris Hepatoms (2 mm3) in den linken Leberlappen (Tag 0) die magnettomographische Tumorvolumenbestimmung (V1: Tag 13). Nach retrograder Katheterisierung der Arteria gastroduodenalis wurden die Tiere entsprechend den folgenden Therapieschemata behandelt (Tag 14): (A) TACE (0,1 mg Mitomycin C+0,1 ml Lipiodol)+( TNF (7,5μg)+IL-2 (5×103IU) (n=10); (B) TACE+OK-432 (0,05 mg) +IL-2 (5×103 IU) (n=10); (C) TACE allein (Kontrollgruppe, n=10). Zu Beurteilung des Therapieerfolges der unterschiedlichen Behandlungsprotokolle wurde das Tumorvolumen (V2) erneut magnettomographisch ermittelt (Tag 28). Das durchschnittliche Tumorvolumenverhltnis (V2/V1) betrug 7,30, 6,53 und 9,14 in Gruppe A, B und C. Im Vergleich zur Kontrollgruppe zeigten die Therapiegruppen A (P<0,05) und B (P<0,01) ein statistisch signifikant geringeres Tumorvolumenverhltnis (V2/V1). Background: Systemic inflammation has long been recognized as a precipitator of acute congestive heart failure(CHF). The impact of inflammation on prognosis in acute CHF, however, is unknown. Methods: This study evaluated the prognostic role of inflammation among 214 consecutive patients presenting with acute CHF to the emergency department. Patients were stratified according to C-reactive protein(CRP) levels determined on admission. The primary end point was all-cause mortality during 24-month follow-up. Results: The median CRP level was 13.0 mg/L, with an intertertile range of 6.0 to 25.0 mg/L. Initial and long-term outcomes were significantly different to the detriment of patients with higher CRP levels. Patients in the highest CRP tertile significantly more often required admission to the intensive care unit(33%vs 14%in patients in the first tertile, P=.028) and died inhospital(15%vs 2%in patients in the first tertile, P=.027). Cumulative 24-month mortality rates were 33.5%in the first, 42.4%in the second, and 53.6%in the third tertile(P=.0265 by log-rank test). After multivariate adjustment, CRP remained an independent predictor of death(hazard ratio 1.4, 95%CI 1.1-1.8 for each step up in tertile, P=.044). Conclusions: Inflammation is a significant and independent predictor of long-term mortality in patients with acute CHF.

A Schematic Simulation for Health Economic Feasibility Studies: Results from a Model for Cardiac Remote Monitoring

The market for active implants and biosensors is of high economic and medical interest. As health economic considerations get into focus in terms of business planning and reimbursement, valid and flexible economic feasibility studies get more important. Unfortunately, literature mostly provides only single economic views on specific aspects like cost savings from reduced rehabilitation in a special patient cohort. To make planning and technology value negotiation more effective and more valid, a methodology to collect relevant data from different studies and normalize it to a common set of parameters was developed for the field of cardiac monitoring in a mixed example population with an approach of simple external weight, ECG and blood-pressure measurement or implanted devices for cardiac monitoring. The target entities taken into account by the simulation model were the impacts on heart attack, stroke, heart failure and the process of implant monitoring. Simulation took place at an example population of 500 patients with specific morbidity criteria. The health economic value was calculated over a period of three years and was split into a technology effectiveness measurement in Quality-adjusted-Lifetime-Years (QALYs) and a “cost- saving-part”. QALYs were chosen as technology effectiveness parameter for a combined and weighted mortality- and morbidity-reduction. Allocating 24.000 Euro to a saved QALY, 42% of the cost would be allocated to QALYs meaning money being spent for gained lifetime-years. The remaining 58% would be the different real cost savings: a per patient gross saving of 3.308 € per year would result for that part (21% on heart attack, 3% on stroke, 68% on heart failure and 8% on implant monitoring). Up-to-date studies do not provide a simple mechanism to allow custom-tailored health economic feasibility study results in terms of other specific population mixes or outcome parameters. Ultrasound findings in autoimmune hepatitis(AIH) have not been reported systematically so far. The use of reliable and accurate noninvasive methods for determining fibrosis stage is important in evaluation of treatment efficacy and fibrosis regression in AIH. Imaging plays an important role in detection of complications and ruling out other possible causes of chronic liver diseases. Ultrasound elastography cutoff values in AIH patients are not the same as those in patients with chronic viral hepatitis or non-alcoholic fatty liver disease. AIH is characterized by wide fluctuations in inflammatory activity. Here we report on current knowledge of ultrasound findings in AIH.

AIM： To investigate the impact of dietary copper given at different time points on the onset of fulminant hepatitis. METHODS： The Long-Evans cinnamon （LEC） rat mod- el of Wilson＇s disease （WD） was used to study the im- pact of high dietary copper （hCu） on the induction of fulminant hepatitis at early or late time points of life. High Cu diet was started in rat pups or in adults （month 5） for three months. Animals that received reduced di- etary copper （rCu） throughout their lifetime served as a control. Hepatitis-associated serum markers （alanine aminotransferase, aspartate transaminase, bilirubin） were analyzed in animal groups receiving hCu or rCu. Liver copper content and liver histology were revealed at sacrifice. A set of 5 marker genes previously found to be affected in injured liver and which are related to angiogenesis （Vegfa）, fat metabolism （Srebf1）, ex- tracellular matrix （Timp1）, oxidative stress （Hmox1）, and the cell cycle （Cdknla） were analyzed by real-time polymerase chain reaction. RESULTS： Regardless of the time point when hCu was started, LEC rats （35/36） developed fulminant hepati- tis and died. Animals receiving rCu （36/36） remained healthy, did not develop hepatitis, and survived long term without symptoms of overt disease, although liver copper accumulated in adult animals （477 ± 75 μg/g）. With regard to start of hCu, onset of fulminant hepatitis was significantly （P 〈 0.001） earlier in adults （35±9 d） that showed pre-accumulation of liver copper as com- pared to the pup group （77±15 d）. Hepatitis-associ- ated serum markers, liver copper and liver histology, as well as gene expression, were affected in LEC rats re- ceiving hCu. However, except for early and rapid onset of hepatitis, biochemical and molecular markers were similar at the early and late time points of disease. Ursodeoxycholic acid (UDCA) has beneficial effects in cholestatic liver diseas es. Inprimary sclerosing cholangitis (PSC), there is evidence that high doses ( ±20 mg/kg) of UDCA may be more effective than average doses. Biliary enrichment of UDCA at such high doses may represent the decisive factor for its beneficial effect. Up to now it is not clear how highdose UDCA correlates with its biliary enrichment and whether bacterial degradation of large amounts of UDCA may lead to an increased bacterial formation of more toxic hydrophobic bile acids. We det ermined the biliary bile acid composition in 56 patients with PSC including 30 p atients with repeat bile samples treated with various doses of UDCA. At a UDCA d ose of 10-13 mg/kg/d (n = 18) biliary UDCA represented 43.1%+0.3%(mean +SD) of total bile acids; at a UDCA dose of 14-17 mg/kg (n = 14), its biliary conte nt increased to 46.9%+0.3%, at 18-21 mg/kg (n = 34) to 55.9%+0.2%, at 22 -25 mg/kg (n = 12) to 58.6%+2.3%, and at 26-32 mg/kg (n = 8) to 57.7%+0.4 %. During UDCA treatment, the biliary content of all other bile acids was uncha nged or decreased. In conclusion, biliary enrichment of UDCA increases with incr easing dose and reaches a plateau at 22-25 mg/kg. There was no increase of toxi c hydrophobic bile acids. If biliary enrichment of UDCA represents the decisive factor for its clinical effect, it seems likely that UDCA doses of up to 22-25 mg/kg may be more effective than lower doses.
