There was remarkable progress in the understanding of the role genetic risk factors in chronic pancreatitis. These factors seem to be much more important than thought in the past. The rare autosomal-dominant mutations N29I and R122H of PRSS1 (cationic trypsinogen) as well as the variant N34S of SPINK1 (pancreatic secretory trypsin inhibitor) are associated to a disease onset in childhood or youth. Compared to chronic alcoholic pancreatitis the progression is slow so that for a long time only signs of acute-recurrent pancreatitis are found. Only at later time points (more than 10-15 years) there is evidence for chronic pancreatitis in the majority of patients. Acute recurrent pancreatitis may therefore be regarded as a transition state until definite signs of chronic pancreatitis are detectable.

Hepatocellular carcinoma(HCC) is among the most common cancer diseases *** hypervascularisation is an essential step for HCC tumorigenesis and can be targeted by transarterial chemoembolization(TACE).This interventional method is the standard treatment for patients with intermediate stage HCC,but is also applied as "bridging" therapy for patients awaiting liver transplantation in many centers *** the devascularization effect induced by TACE is transient,consequently resulting in repeated cycles of TACE every 4-8 *** documented survival benefits,TACE can also induce the up-regulation of proangiogenic and growth factors,which might contribute to accelerated progression in patients with incomplete *** 2007,sorafenib,a multi-tyrosine kinase and angiogenesis inhibitor,was approved as the first systemic treatment for advanced stage *** active targeted compounds,either inhibitors of angiogenesis and/or growth factors,are currently being investigated in numerous clinical *** overcome revascularisation or tumor progression under TACE treatment it seems therefore attractive to combine TACE with systemic targeted agents,which might theoretically block the effects of proangiogenic and growth *** the last 12 mo,several retrospec-tive or prospective cohort studies combining TACE and sorafenib have been ***,robust results of the efficacy and tolerability of such combination strategies as proven by randomized,controlled trials are awaited in the next two years.

Patienten beiderlei Geschlechts mit portaler Hypertension bei Leberzirrhose wurden nach erfolgloser endoskopischer Behandlung einer akuten sophagusvarizenblutung operiert. 134 Flle waren als posthepatitische, 62 als schistosomatische und 25 als bilire Leberzirrhosen diagnostiziert. Das wichtigste bei unserem Operationsverfahren ist: Darstellung der V. coronaria ventriculi in der Mitte der kleinen Kurvatur und Kanlierung der beiden Venensegmente. Nach Setzen von Klemmen 5 cm pr- und 5 cm postkardial und in der Korpusmitte Sklerosierung des kranialen V. coronaria-Segmentes mit α-Octyl-Cyanoacrylat und Venenligatur; Messung des portalvensen Druckes im kaudalen Segment. Die selektive Sklerosierung der V. coronaria ventriculi (SSVCV) wurde bei 18 Fllen als prophylaktischer, bei 106 Fllen als notfallmβiger und bei 98 Fllen als elektiver Eingriff durchgefhrt, und bei allen Patienten konnte die Varizenblutung gestillt werden. 219 Patienten berlebten , 3 Patienten verstarben an Leberversagen 24 Stunden postoperativ. Bei 189 operierten Patienten der Gruppe zeigte sich bei der Durchleuchtung, Rntgenkontrastfllung und Endoskopie, daβ die sophagusvarizen 3-6 Monate nach der SSVCV eine offensichtliche Verbesserung aufwiesen. Die postoperative Untersuchung nach 1 bis 2 Jahren zeigte, daβ die sophagusvarizen nicht mehr ezistierten. Bei 182 operierten Patienten hat sich beim Follow-up im Zeitraum von 6 1/2 Jahren bis 7 1/2 Jahren ergeben, daβ nach der Operation keine Blutung mehr aufgetreten ist.

Psychiatric side effects of pegylated interferon alfa-2b as compared to conventional interferon alfa-2b in patients with chronic hepatitis C

AIM: To assess systematically the spectrum and extent of depressive symptoms comparing patient groups receiving peginterferon or conventional ***: Ninety-eight patients with chronic hepatitis C and interferon-based therapy (+ribavirin) were consecutively enrolled in a longitudinal study. Patients were treated with conventional interferon alfa-2b (48/98patients; 5 MIU interferon alfa-2b thrice weekly) or peginterferon alfa-2b (50/98 patients; 80-150 μgpeginterferon alfa-2b) in combination with weight-adapted ribavirin (800-1 200 mg/d). Repeated psychometric testing was performed before, three times during and once after antiviral therapy: Depression was evaluated by the Hospital Anxiety and Depression Scale (HADS), anger/hostility by the Symptom Checklist-90 Items Revised (SCL-90-R).RESULTS: Therapy with pegylated interferon alfa-2bproduces comparable scores for depression (ANOVA:P = 0.875) as compared to conventional *** of depression scores were even higher and cases of clinically relevant depression were frequent during therapy with peginterferon. Scores for anger/hostility were comparable for both therapy ***: Our findings suggest that the extent and frequency of depressive symptoms in total are not reduced by peginterferon. Monitoring and management of neuropsychiatric toxicity especially depression have to be considered as much as in antiviral therapy with unmodified interferon.

Nach der Transplantation stellt die Messung des Leber-PO2-Wertes einen Parameter dar. der die PNF fruhzeitig erkennen kann. Die durchgefuhrte Studie erbrachte den Nachweis, daβ die Messung des Leber-PO2 eine sofortige Beurteilung und Verlaufskontrolle der Durchblutungsstorung nach Lebertransplantation ermoglicht. Somit war die 'Rapid-Perfusion'-Technik der 'In-Situ'-Technik uberlegen. Bei der 'In-Situ'-Gruppe erfolgten in durchblutetem Zustand, bei den Tieren der 'Rapid-Perfusion'-Gruppe in blutleerem, perfundiertem Zustand. Zur Perfusion erfolgte die Implantation der groβlumigen Katheter in die Aorta und in die Vena portae. Anschlieβend folgte die Perfusion mit'UW'-Losung. Die Messung des Sauerstoffpartialdruckes im Gewebe zur indirekten Bestimmung der Mikrozirkulation beruht auf den polarographischen Prinzipien. Das Meβsystem beruht auf einer kommerziell erhaltlichen Mikrooxygenelektrode. Messungen der Leberoxygenation erfolgten nach Baucheroffnung in zwei Gruppen. Die Wertigkeit der Durchblutungsmessung fur eine Einschatzung des Funktionszustandes der transplantierten Leber hat uns bewogen, die Bestimmung des PO2-Druckes als ein zusatzliches Diagnostikum in die klinische Lebertransplantation einzuziehen.

Durch die systematische Untersuchung und grundliche Studie uber eine Leiche von der Zeit der Streitenden Reiche aus dem Grab Nr. 1 am GuoJia-Gang in der Stadt Jingmen der Provinz Hubei wurde festgestellt , daβdiese Leiche vor mehr als 2300 Jahren, namlich in dem mittleren Zeitabschnitt der Streitenden Reiche Chinas , in einem Holzsarg beerdigt wurde und die alteste unter allen bisher in China gefundenen alten Sargleichen ist. Sie gehort grundsatzlich auch zu dem Typ der jeweils 1972 in Changsha und 1975 in Jiangling gefundenen alten Leichen der Westlichen Han-Dynastie. Die kollagenen Bindegewebsfasern aus allen Korperteilen dieser Leiche zeigten eine sehr gut erhaltene Ultrastruktur mit ganz deutlichen, ordentlichen, periodischen Querstreifungen. Zwischen den Kollagenfibrillen war interessanterweise wie bei den beiden Leichen der Westlichen HanDynastie auch eine Menge von Bakteriensporen vorhanden. Auβerdem wurde noch eine groβe Menge von Clonorchis sinensis-und Trichuris trichiura-Eier in dem Darminhalt der alten Leiche gefunden. Uber den Erhaltungszustand und die Faktoren der guten Erhaltung der Leiche wurde diskutiert. Unserer Ansicht nach sei die Erhaltung der Leiche ein Resultat aus einem kombiniertem Effekt von internen und externen Faktoren der Leiche.

Summary: The purpose of this study was to evaluate relative regional blood volume (rCBV) in meningioma and compare the utility of reconstruction using both gradient-echo sequence and echo-planar-imaging (EPI) *** patients with meningiomas were studied on a Siemens 1.5-T scanner. During the gradient-echo sequence (n=12) and EPI sequence (n=6), a bolus ^(0.2 mmol/kg) of Gd-DTPA was injected mechanically with a flow rate of 5 ml/second. Image processing of dynamic data was performed on a pixel-by-pixel basis. The ratio of tumor rCBV/gray matter rCBV was 3.01±1.18 (3.07 ± 1.39 in gradient sequence and 2.84 ± 0.94 in EPI sequence). The ratio of gray matter/ white matter as the reference tissue had a mean of 2.79 ± 0.76 using the FLASH sequence, and a mean of 3.04 ± 1.31 using EPI. These differences were not statistically signifcant (P>0.5, t-test). According to the ratio of tumor rCBV/gray matter rCBV, a mean value, 14.5 ml/100 g, of rCBV in meningiomas was calculated. Compared with gray matter, increased inhomogeneous rCBV was observed in meningioma. Based on the two different sequences, no bias can be observed in our rCBV reconstruction.

AIM： To study the efficacy, safety, and feasibility of a granulocyte adsorptive type apheresis system for the treatment of patients with chronically active ulcerative colitis despite standard therapy. METHODS： An open label multicenter study was carried out in 39 patients with active ulcerative colitis （CAI 6-8） despite continuous use of steroids （a minimum total dose of 400 mg prednisone within the last 4 wk）. Patients received a total of five aphereses using a granulocyte adsorptive technique （Adacolumn, Otsuka Pharmaceutical Europe, UK）. Assessments at wk 6 and during follow-up until 4 mo comprised clinical （CAI） and endoscopic （EI） activity index, histology, quality of life （IBDQ）, and laboratory tests. RESULTS： Thirty-five out of thirty-nine patients were qualified for intent-to-treat analysis. After the apheresis treatment at wk 6, 13/35 （37.1%） patients achieved clinical remission and 10/35 （28.6%） patients had endoscopic remission （CAI〈4, EI〈4）. Quality of life （IBDQ） increased significantly （24 points, P〈0.01） at wk 6. Apheresis could be performed in all but one patient. Aphereses were well tolerated, only one patient experienced anemia. CONCLUSION： In patients with steroid refractory ulcerative colitis, five aphereses with a granulocyte/ monocyte depleting filter show potential short-term efficacy. Tolerability and technical feasibility of the procedure are excellent.
