Background &Aims: Our objective was to assess adherence to evidence-based guidelines by providers of the Department of Veterans Affairs nationwide. Methods: This was a cross-sectional study among veterans prescribed a nonsteroidal anti-inflammatory drug (NSAID) from January 1, 2002, to December 31, 2002. Prescrip tion data were linked to inpatient and outpatient medical records and death file s. The population was characterized as high risk based on the following: age 65 years or older, concurrent corticosteroid or anticoagulant use, history of pepti c ulcer, and high average daily dose of NSAIDs. Adherence was defined as the pre scription of a traditional NSAID with gastroprotection or a coxib in high-risk NSAID users. Univariate and multivariate analyses assessed the potential predict ors of adherence. Results: Three hundred three thousand seven hundred eighty-se ven met our definition of high risk. Most (97.3%) were male; 55.6%were white, 9.6%black, and 34.8%of other/unknown race. Age 65 years or older was the large st high-risk subset (87.1%). Overall, only 27.2%of highrisk veterans (n = 82, 766) were prescribed an adherent strategy. Among veterans with at least 2 risk f actors, adherence was 39.7%; among those with 3 risk factors, adherence was 41. 8%. Predictors of adherence included history of upper gastrointestinal events, anticoagulant use, rheumatologic disease, high Deyo comorbidity index score, use of low-dose salicylates, and concurrent corticosteroid use. Predictors of nona dherence included prescriptions≥90 days and high average daily dose of NSAIDs. Conclusions: Adherence to evidence-based guidelines for safe prescription of NS AIDs in the Department of Veterans Affairs is low (27.2%). The likelihood of ad herence is further decreased if veterans are prescribed NSAIDs for ≥90 days.

雪旺细胞(SCs)的可塑性潜能因其在神经损伤后和周围神经系统疾病中发挥作用而备受关注。有关免疫细胞和雪旺细胞之间相互作用的研究表明雪旺细胞参与炎症过程。然而,人类雪旺细胞的免疫能力主要是从神经病变中推断出来的,但神经损伤后雪旺细胞是否直接调节适应性免疫反应尚不清楚。本研究对人类修复相关雪旺细胞(hrSCs)的免疫调节能力进行综合分析,hrSCs概括了雪旺细胞对体外神经损伤的反应。我们使用有效的人类周围神经原代hrSCs培养模型,分析了转录组、分泌组、以及细胞表面与先天免疫和适应性免疫相关的通路和标志物相关的蛋白;进行了吞噬作用测定,并监测了同种异体共培养细胞中的T细胞亚群激活。本研究结果表明hrSC具有吞噬作用,这与MHCⅡ的高表达一致。此外,hrSC表达协同调节蛋白,如CD40、CD80、B7H3、CD58、CD86和HVEM,释放过多的趋化因子、基质重塑蛋白、促炎及抗炎细胞因子,并在IFNγ进行促炎刺激后上调T细胞对PD-L1分子的抑制。与单核细胞相反,单独的hrSC不足以触发同种异体CD4+和CD8+T细胞,但会限制外源激活的T细胞的数量和激活状态。本研究表明,hrSCs具有体外专职抗原呈递细胞的典型特征和功能,并表明这些细胞在神经再生过程中作为T细胞免疫负调节剂的新作用。

PURPOSE: To assess the role of systemic non-steroidal antiinflammatory drugs (NSAIDs) in the treatment of inflammatory cystoid macular edema (iCME). DESIGN: Pilot study. METHODS: We included 66 patients with iCME and treated them with naproxen (2 × 250 mg daily; n=28) or rofecoxib (1 × 25 mg daily; n=38). After 4 months of therapy, visual acuity and in-flammation activity were measured. We evaluated the grade of CME, retinal vasculitis, and papillary leakage with fluorescein angiography. RESULTS: No clear effect of NSAIDs on iCME, visual acuity, and intraocular inflammation was observed. A beneficial effect was noted in less than 8% of the affected eyes. The drop-out rate after 4 months of treatment was 52% , because of adverse effects or inefficacy of the treatment. Improvement of visual acuity was slightly better in patients who were treated with naproxen compared with rofecoxib (P ≤ .05). CONCLUSION: Systemic NSAIDs have a limited role (if any) in the treatment of iCME.

Cancer is one of the deadliest diseases in developing countries. In recent years, natural plant-based compounds have been used in the search for drugs to combat numerous diseases, including cancer. In this study, we evaluate the cytotoxic properties of paanfo tiben 1 and paanfo tiben 2, two traditional herbal formulations from Burkina Faso used in the treatment of cancer in Burkina Faso. To this end, the recipes were infused and freeze-dried. The dry extracts obtained were used to determine total phenolics and flavonoids content, assess antioxidant activity using the DPPH, ABTS and FRAP methods, evaluate anti-inflammatory properties by inhibiting 15-LOX, COX 1 and 2, and assess cytotoxic activity on HeLa cervical cancer and HePG2 liver cancer cell lines using the MTT test. The paanfo tiben 1 recipe showed the highest levels of total phenolics and flavonoids, as well as the best antioxidant activities, with IC50 values of 21.020 ± 0.6 µg/ml and 22.94 ± 0.57 µg/ml for DPPH and ABTS, and 165.15 mM EAA/mg dry extract for FRAP. It also exhibited the best cytotoxic activity with IC50 values of 112.02 ± 0.025 µg/ml on HeLa cells and 80.67 ± 6.08 µg/ml on HepG2 cells. On the other hand, paanfo tiben 2 exhibited the best anti-inflammatory activities through inhibition of 15-LOX and COX 1, with inhibition percentages at 100 µg/ml of 32.523% and 24.717 % respectively. These results could justify the traditional use of these two recipes by traditional health practitioners in the treatment of cancer sufferers in Burkina Faso.

关于多发性硬化(multiple sclerosis,MS)斑块的病理以及MS患者灰白质的弥散性改变,目前已有相当多的深入研究报道。但对于脱髓鞘斑块周围组织的改变,尤其对是脊髓MS斑块周围组织的病理研究,目前几乎没有报道。本研究即对进展性脊髓MS患者的脊髓脱髓鞘病灶周围组织(periplaque demyelinated lesions,PDLs)进行分子免疫病理方面的研究。纳入初发或复发进展型脊髓MS患者21例,尸检得到其颈髓组织,对其PDLs的神经病理特征进行分析。结果显示,PDLs存在大面积的不完全性脱髓鞘现象,而且存在促炎症和抗炎症的叠加分子反应。巨噬细胞和小胶质细胞聚集在PDLs,但对脱髓鞘碎片的吞噬作用很弱。下调PDLs的少突胶质细胞基因,胶质细胞相关的分子标志物,如水通道蛋白(aquaporin)-4、缝隙连接蛋白(connexin)-43及谷氨酸转运子(glutamate transporter)EAAT1的mRNA及蛋白表达水平均明显下调。本研究还显示,在进展型脊髓MS患者的脊髓组织中,伴随脱髓鞘斑块的形成,PDLs内的组织重塑早已开始并一直伴随着斑块的形成。笔者推测脊髓病灶PDLs内,组织重塑的过程一定伴随着星形胶质细胞功能的改变、较轻的炎症反应,共同促进了脱髓鞘的发生和进展。

为了解白芍提取物对罗非鱼(Oreochromis niloticus)肝损伤和炎症反应的保护作用,将罗非鱼随机分为5组,空白对照组(Ctrl组)、H2 O2组、白芍提取物组(0.5、1.0、5 g/kg),饲喂60 d后,用H2 O2处理24 h,在H2 O2处理前后,分别采集血液、肝、鳃和肌组织,并测定肝损伤指标、氧化应激参数和基因表达。结果显示:白芍提取物显著提高了血清、肝和鳃的抗氧化能力。氧化损伤实验中,白芍提取物不同程度地降低了谷丙转氨酶(GPT)、谷草转氨酶(GOT)和丙二醛(MDA)水平;提高了超氧化物歧化酶(SOD)、谷胱甘肽(GSH)和总抗氧化能力(T-AOC)水平;上调了肿瘤坏死因子α(TNF-α)、白细胞介素IL-1β和IL-8基因表达;下调了IL-10基因表达。以上结果表明,白芍提取物可提高罗非鱼抗氧化能力,抑制氧化应激引起的肝损伤和炎症反应。

Dermatomyositis,an idiopathic inflammatory myopathy,is characterized by distinctive skin manifestations,proximal muscle weakness,and multiple organ involvement and can be accompanied by *** provide a reference for dermatologists and clinicians in other relevant fields of clinical practice,experts from the Dermatology Branch of the China International Exchange and Promotion Association for Medical and Health Care and the National Clinical Research center for Dermatologic and Immunologic diseases developed this consensus on the diagnosis and treatment of adult dermatomyositis using Chinese and international literature and expert advice.
