A Prospective Survey Comparing Patients’ Experience of a Nurse-Led Clinic and Consultant-Led Clinic for Surveillance of Patients with Dilatation of the Intrathoracic Aorta

Background: Increasing workload in consultant-led clinics often means patients to wait a long time for clinic appointments. To address this, there is an increasing trend in developing nurse-led clinics across many specialities in the National Health Service. This study aims to assess whether the implementation of a nurse-led clinic in thoracic aortic surgery will optimise the utilisation of health care services and improve overall patient satisfaction. Methods: 80 follow-up patients were asked to complete a questionnaire following their appointment in an aortic clinic, which was led either by a consultant (n = 40) or an aortic specialist nurse (n = 40). All patients seen by a nurse in the clinic were assessed by a consultant surgeon prior to the clinic for suitability. No new patients were seen by a nurse. Any patient with an aortic dimension of 5 cm or greater was seen by the consultant. If there were any complicated clinical features, the patient was seen in the consultant-led clinic. Patients were asked questions about their time spent with the respective health care professionals across 12 categories (punctuality, preparedness, understanding of concerns, clarity of speech, listening, respect, explaining, letting you talk, putting you at ease, emotional support, advice and advice for next follow-up). Patients rated each category using an ordinal scale from 0 - 10. Results: Patient scores were greater in nurse-led clinics compared to consultant-led clinics across a number of categories although only punctuality reached significance (mean 9.2 vs. 6.8, p 0.05). Conclusion: Patients were highly satisfied with the nurse-led clinic across all categories, with greater satisfaction for punctuality. These findings suggest that a nurse-led clinic can be implemented for the management of carefully selected thoracic aortic surgery patient without reduction in patient satisfaction.

This article proves existence results for singular problem ( - 1)n-px(n)(t) = f(t,x(t),…,x(n-1)(t)), for 0 < t < l,x(i)(0) = 0,i = 1,2.…,p - l,x(i)(1) = 0,i = p,p + 1,…, n - 1. Here the positive Carathedory function f may be singular at the zero value of all its phase variables. The interesting point is that the degrees of some variables in the nonlinear term f(t,x0,x1,…,xn-1) are allowable to be greater than 1. Proofs are based on the Leray-Schauder degree theory and Vitali's convergence theorem. The emphasis in this article is that f depends on all higher-order derivatives. Examples are given to illustrate the main results of this article.

A Leverrier-like algorithm is presented which allows the computation of the transfer function of a linear regular system from its m-D state-space description,without inverting a multivariable polynomial matrix. This algorithm is an extension of the classic Leverrier's algorithm for 1-D system and it reduces the computational ***-D Cayley-Hamilton theorem is also shown by the algorithm.

Background: Phylogeographical patterns and population dynamics are usually interpreted by environmental disturbances and geographic barriers of the past. However, sister species may exhibit disparate patterns of genetic structures and population dynamics due to their habitat preference and altitude segregation. In this study, we tested how species-specific altitude habitat affected phylogeographical patterns in two sister snowcock species, Tibetan(Tetraogallus tibetanus) and Himalayan Snowcocks(T. himalayensis).Methods: A panel of seven microsatellite loci and a fragment of Mitochondrial DNA Control Region were used to investigate genetic structures and population dynamics in hope of revealing the underlying evolutionary processes through the identification of possible past demographic ***: Our results suggest that T. himalayensis showed a significant phylogeographical signal in mtDNA(FST microsatellite(F the glacial-interglacial cycles in the = 0.66, p specialities were reflected in the genetic structure patterns and population dynamics of these two species. Marine geographic information system （MGIS） has great ability to deal with the spatio-temporal problems and has potential superiority when it is applied to oceanography. Using the feature extraction of oceanic phenomena as a case study, the functions of the MGIS are analyzed, and thus the position of MGIS in the oceanography is defined. Comparing the requirement of MGIS with that of the traditional GIS which has been developed in the terrestrial applications in the past four decades, the frame for the functions of MGIS is constructed. According to the established MGIS, some key technologies are discussed in detail with emphasis on the specialities which can distinguish the MGIS from the traditional GIS.

A Note on Vulnerability in Graphs of Diameter Five （ Ⅱ ）

The line persistence of a graph G, Pt （ G ） is the minimum number of lines which must be removed to increase the diameter of G. In Ref. [7] （J. Shanghai Univ., 2003,7（4）：352-357）, we gave a characterization of graphs of diameter five with ρ1 （ G ）≥2. In this paper we will show that each of the 8 special graphs Xi （ i = 1,2,3,4,5,6,7,8） listed in condition （2） of Theorem 1 in Ref. [7] can not be deleted. Therefore the results we obtained in Ref. [7] can not in general be improved.

Improved gradient iterative algorithms for solving Lyapunov matrix equations

In this paper, an improved gradient iterative （GI） algorithm for solving the Lyapunov matrix equations is studied. Convergence of the improved method for any initial value is proved with some conditions. Compared with the GI algorithm, the improved algorithm reduces computational cost and storage. Finally, the algorithm is tested with GI several numerical examples.
