Effects of adrenomedullin gene overexpression on biological behavior of hepatic stellate cells

AIM: To investigate the effects of adrenomedullin (AM) gene overexpression on the biological characteristics of human hepatic stellate cells (hHSCs) by stable ***: hHSCs which express low basal levels of AM were stably transfected with an expression construct containing rat AM gene or with an empty expression vector. Expression of AM in hHSCs was determined by reverse transcription (RT)-polymerase chain reaction (PCR) and radioimmunoassay (RIA). Cell proliferation was evaluated by 5-bromo-2'-deoxyuridine (BrdU) incorporation and immunocytochemistry. RT-PCR and Western blot were used to test the expression of procollagen types Ⅰ and Ⅲ. Protein expressions of interstitial collagenase (MMP-1), gelatinase (MMP-2) and tissue inhibitors of matrix metalloproteinases-2 (TIMP-2) were assessed by Western ***: Two cell clones (A-2, A-8) transfected withthe AM gene expressed higher levels of AM mRNA (nontransfected group: 0.86±0.11, empty vector group: 1.01±0.11, A-2 clone group: 1.44±0.08 and A-8 clone group: 1.36±0.05) and protein (12.31±0.17, 12.35±0.12,12.56±0.06 and 12.62±0.07) (P＜0.05). AM geneoverexpression had inhibitory effects on cell proliferation of hHSCs (29.6%, 30.9%, 18.9% and 21.8%, respectively. P＜0.05) and expression of procollagen type Ⅰ (0.58±0.1,0.48±0.11, 0.3±0.06 and 0.31±0.07 at mRNA level)(0.27±0.07, 0.3±0.06, 0.14±0.05 and 0.13±0.05 at protein level) (P＜0.05) and procollagen type Ⅲ (0.17±0.04, 0.15±0.03, 0.1±0.02 and 0.09±0.02 at mRNA level) (0.22±0.04, 0.2±0.03, 0.11±0.04 and 0.13±0.03 at protein level) (P＜0.05). Compared with cells non-transfected (TIMp2: 2.77±0.03, MMP-2: 0.5±0.04, MMP-1: 0.49±0.07) and transfected with empty vector (TIMP2: 2.79±0.04,MMP-2: 0.48±0.03, MMP-1: 0.45±0.09), these two clones had lower expression levels of TIMP2(A-2 clone group: 2.7±0.02 and A-8 clone group: 2.71±0.02) (P＜0.05) and MMP-2(A-2 clone group: 0.15±0.05 and A-8 clone group: 0.13±0.04) (P＜0.05) but displayed a higher expression level of

Guidelines for cross-sex hormone treatment of transsexual people have been developed, but no attention has been paid to the specifics of ethnic groups. South East （SE） Asian male-to-female （MtoF） transsexual people may be able to transition to the female sex with lower doses of estrogens/progestins than Caucasians thus reducing health risks. Female-to-male （FtoM） may virilize less profoundly with standard doses of androgens, but this is probably sufficient to pass acceptably as men in view of the less pronounced sex differences in physique in Asians compared with Caucasians. It is timely that studies in Asians are conducted to get a better insight into their specific needs and risks of cross-sex hormone treatment.

Retrospective analysis of old-age colitis in the Dutch inflammatory bowel disease population

AIM： To describe the characteristics of Dutch patients with *** bowel disease （IBD） first diagnosed above 60 years of age-a disease also known as old-age colitis （OAC） and to highlight a condition that has a similar appearance to IBD, namely segmen- tal colitis associated with diverticular disease （SCAD）. METHODS： A retrospective longitudinal survey of patient demographic and clinical characteristics, disease characteristics, diagnostic methods, management and course of disease was performed. The median follow-up period was 10 years. RESULTS： Of a total of 1100 IBD patients attending the Department of Gastroenterology, 59 （50） [median age 82 years （range 64-101）; 25 male （42%）] were identified. These patients were diagnosed with ulcerative colitis （n = 37, 61%）, Crohn＇s disease （n = 14, 24%）, and indeterminate colitis （n = 8, 15%）. Remission was induced in 40 （68%） patients within a median interval of 6 mo （range 1-21） and immunosuppressive therapy was well tolerated. Histological evaluation based on many biopsy samples and the course of the disease led to other diagnosis, namely SCAD instead of IBD in five （8%） patients. CONCLUSION： OAC is not an infrequent problem for the gastroenterologist, and should be considered in the evaluation of older patients with clinical features suggestive of IBD. Extra awareness and extensive biopsy sampling are required in order to avoid an erroneous diagnosis purely based on histological mimicry of changes seen in SCAD, when diagnosing IBD in the presence of diverticulosis coli.

Suppressing autoimmunity by TGF-β： not just through Treg cells

It regulates manyaspects of T-cell biology including thymocyte development, T cell differentiation and effector T cell function.1 Previous work has shown that the loss of TGF-β signaling in the T-cell lineage leads to severe lymphopro- liferafive autoimmunity.2.3 Until now, it has been unclear to what extent the autoimmunity observed in those mice is driven by TGF- unresponsive conventional T cells, as TGF- is also crucial for the maintenance and function of regulatory T cells （Treg）. A recent study pub- lished in Nature Immunology revealed a novel role of TGF-β signaling in non-regulatory naive T cells. It was found that TGF-β controls lym- phopenia-driven T-ceU proliferation in neonatal mice, thereby preventing the development of autoimmtmity.

Anaerobic bacteria and intrahepatic stones: detections of Clostridium sp. and Bacteroides fragilis

To detect anaerobic bacteria Clostridium sp . and Bacteroides fragilis in intrahepatic stones by molecular genetic method Methods DNA was extracted from 59 stone samples and subjected to polymerase chain reaction (PCR) amplification targeting the 16S rRNA gene of Clostridium sp . and the glutamine synthetase gene of Bacteroides fragilis Single-strand conformational polymorphism (SSCP) analysis was performed to identify the Clostridium sp Results 16S rRNA gene sequences for Clostridium sp. were identified in 49 stones (83%, 49/59) The two most common groups were detected in 19 (41%) and 17 (37%) of the 46 samples using SSPC analysis, and 25/59 (42%) stones were tested positive for Bacteroides fragilis Conclusions Anaerobes such as Clostridium sp and Bacteroides fragilis present in intrahepatic stones and may play a role in stone formation PCR is a useful technique to detect fastidious pathogens, which are difficult to culture SSCP of PCR products is a rapid method in differentiating bacterial species

Further analysis of PREVAIL： enzalutamide use in chemotherapy-naive men with metastatic castration- resistant prostate cancer

PREVAIL was a phase III multinational, double-blind, placebo-controlled trial that enrolled chemotherapy-naive men with metastatic castration-resistant prostate cancer （mCRPC）, which showed remarkable improvement in co-primary endpoints with an overall 81% reduction in the risk of radiographic progression, as well as 29% reduction in the risk of death in favor of the enzalutamide arm over placebo. All secondary endpoints including time to subsequent chemotherapy initiation and prostate specific antigen （PSA） progression were in favor of the enzalutamide arm. The results of PREVAIL shows the utility of enzalutamide that would likely soon expand the indication to asymptomatic or minimally symptomatic men with mCRPC not previously treated with chemotherapy.

Prostate cancer （PC） is the second most diagnosed visceral malignancy in men worldwide, with over 900 000 new diagnoses each year.1 Approximately 50% of patients treated in industrialized nations will receive androgen deprivation therapy （ADT） at some point in their lifetimes.2 The use of ADT as a treatment approach is likely to increase as new drug developments have focused on intensifying the effect of reducing androgen receptor activation.

Helicobacter pylori and gastric mucin expression: A systematic review and meta-analysis

AIM: To investigate the relationship between Helicobacter pylori(H. pylori) and mucin expression in gastric ***: English Medical literature searches were conducted for gastric mucin expression in H. pylori infected people vs uninfected people. Searches wereperformed up to December 31 th 2014,using MEDLINE,Pub Med,EMBASE,Scopus,and CENTRAL. Studies comparing mucin expression in the gastric mucosa in patients positive and negative for H. pylori infection,were included. Meta-analysis was performed by using Comprehensive meta-analysis software(Version 3,Biostat Inc.,Englewood,NJ,United States). Pooled odds ratios(ORs) and 95% confidence intervals(CIs) were calculated compared mucin expression in individual studies by using the random effects model. Heterogeneity between studies was evaluated using the Cochran Q-test,and it was considered to be present if the Q-test P value was less than 0.10. I2 statistic was used to measure the proportion of inconsistency in individual studies,with I2 > 50% representing substantial heterogeneity. We also calculated a potential publication ***: Eleven studies,which represent 53 substudies of 15 different kinds of mucin expression,were selected according to the inclusion criteria. Every kind of mucin has been considered as one study. When a specific mucin has been studied in more than one paper,we combined the results in a nested metaanalysis of this particular mucin: MUC2,MUC6,STn,Paradoxical con A,Tn,T,Type 1 chain mucin,Le A,SLe A,Le B,AB-PAS,MUC1,and MUC5 AC. The odds ratio of mucin expression in random analysis was 2.33,95%CI: 1.230-4.411,P = 0.009,higher expression in H. pylori infected patients. Odds ratio for mucin expression in H. pylori positive patients was higher for MUC6(9.244,95%CI: 1.567-54.515,P = 0.014),and significantly lower for MUC5AC(0.447,95%CI: 0.211-0.949,P = 0.036). Thus,H. pylori infection may increase MUC6 expression and decrease MUC5 AC expression by 924% and 52%,***: H.

Behavioral consequences of children with sleep-disordered breathing after adenotonsillectomy

Background Adenotonsillectomy (AT) has been an effective treatment for sleep-disordered breathing (SDB) in children, and several studies described the risk of postoperative weight gain and obesity in children treated with AT. The present study aimed to evaluate behavioral improvements in children with SDB one year after adenotonsillectomy and to investigate an influence of postoperative weight gain on behaviors. Methods The study included 170 children aged 5–11 years who underwent adenotonsillectomy for SDB and 150 controls. Body mass index percentile was obtained for age and gender, and parental sleep-related breathing disorder (SRBD) question-naire was used to assess the severity of SDB. Psychological assessment was performed pre- and post-adenotonsillectomy using standardized questionnaires including strength and difficulties questionnaire, children's depression inventory and screen for child anxiety-related emotional disorder. Results The mean age of 170 patients was 7.7 ± 1.5 years with 73 (42.9%) girls and 97 (57.1%) boys. The mean follow-up period were 15.4 ± 2.7 months. The patients had shown significant improvements in SDB scores as well as in questionnaire-based behavioral problems after adenotonsillectomy. The odds of a child being overweight were significantly increased after adenotonsillectomy. Less improvements in hyperactivity and conduct problems were observed in the patients with older ages, higher SRBD scores, and overweight/obesity at 1-year follow-up after adenotonsillectomy. Conclusion These data suggest that abnormal behavioral outcomes should be evaluated postoperatively, which potentially could be reduced with the early adenotonsillectomy and adequate postoperative weight control.
