AM: To clarify differences in antiviral effect of the drug in patients with different ALT levels, we examined the changes in HBV markers in patients with high or low ALT levels with or without lamivudine treatment. METHODS: Thirty-seven HBeAg-positive patients were studied. Ten patients with ALT levels higher than 200 IU/L (group 1) and 8 patients with ALT below 200 IU/L (group 2) were treated orally with 100 mg/d of lamivudine. As untreated control, 9 patients with ALT above 200 IU/L (group 3) and 10 patients with ALT below 200 IU/L (group 4) were examined. ALT level, HBeAg/HBeAb status, and HBV DNA level were examined monthly for 11.9±0.4 mo. RESULTS: The ALT level normalized in all 10 patients of group 1, 7/8 of group 2, 4/9 of group 3, and 1/10 of group 4 within 6 mo (groups 1 vs2, P= NS; groups 1 vs 3, P= 0.002; groups 1 vs4, P<0.0001). HBV DNA fell below the detection limit in all 10 patients of group 1, 7/8 of group 2, 0/9 of group 3, and 0/10 of group 4 within 6 mo (groups 1 vs 2, P - NS). HBeAg became seronegative in 7/10 patients of group 1, 1/8 of group 2, 3/9 of group 3, and 0/10 of group 4 within 12 mo (groups 1 vs2, P= 0.02; groups 1 vs 3, P= NS). CONCLUSION: Our data suggest that HBeAg-positive patients with higher ALT levels can be considered good candidates for lamivudine therapy, probably because lamivudine accelerates the natural seroconversion of HBeAg, accompanied by HBV DNA loss, in these patients.

The Kidney transplant is preferred option for treatment of chronic renal failure because with this modality treatment the life quality of patient is like normal also this modality is cost effective. The left allograft is choice for transplant because the short right renal vein predisposes this allograft for more complication. We introduce methods, which facilitate the anastomosing the short right allograft vein and probably reduce the vascular complication. Methods: In 20 right allograft kidneys after irrigation, with dissection between vein and artery near the hilum at the back table, placing the allograft kidney with inversion position and external iliac artery selection and first vein anastomosing and heparin injection transplantation is carried out and all the recipients are treated with three drugs (prednisolone, cyclosporine cellcept) and follow up is conducted. Results: In these cases, with at least six months follow up, flow of the urine and circulation of blood were normal. Conclusion: With regarding the results it may be concluded that with this technique it may reduces the vessel complication and facilitates the vessel anastomosing of the right allograft kidney.

BACKGROUND Radical surgical resection is regarded as the best treatment for hepatic hilar cholangiocarcinoma. However, 60%-70% of patients have lost the chance of surgery at the time of diagnosis. Simple biliary stent or drainage tube placement may fail in a short time due to tumor invasion or overgrowth, bile accumulation, or biofilm formation. Effective palliative treatments to extend the effective drainage time are of great significance for improving the quality of life of patients and changing the prognosis of patients. AIM To investigate the clinical efficacy of gemcitabine and cisplatin-based transcatheter arterial chemoembolization (TACE) combined with radiotherapy in hilar *** A retrospective analysis was conducted on patients clinically diagnosed with hilar cholangiocarcinoma from June 2014 to January 2017 at the Liaoning Provincial Cancer Hospital. Patients were evaluated by specialists, and those who were not suitable for surgery or unwilling to undergo surgery and met the inclusion criteria were included in the study. There were a total of 72 patients (34 males and 38 females) with an average age of 59.9 years (range, 40-72 years). According to percutaneous transhepatic biliary angiography and the patients’ wishes, stent implantation or biliary drainage tube implantation was used to relieve biliary obstruction. The patients were divided into either a control group or a combined treatment group according to their follow-up treatment. The control group consisted of a total of 35 patients who received simple biliary drainage tube placement and biliary stent implantation (7 patients with bilateral stents and 6 with a unilateral stent) and 22 patients receiving biliary drainage tube placement alone. Artificial Intelligence is revolutionising our communication practices and the ways in which we interact with each other. This revolution does not only impact how we communicate, but it affects the nature of the partners with whom we communicate. Online discussion platforms now allow humans to communicate with artificial agents in the form of socialbots. Such agents have the potential to moderate online discussions and even manipulate and alter public opinions. In this paper, we propose to study this phenomenon using a constructed large-scale agent platform. At the heart of the platform lies an artificial agent that can moderate online discussions using argumentative messages. We investigate the influence of the agent on the evolution of an online debate involving human participants. The agent will dynamically react to their messages by moderating, supporting, or attacking their stances. We conducted two experiments to evaluate the platform while looking at the effects of the conversational agent. The first experiment is a large-scale discussion with 1076 citizens from Afghanistan discussing urban policy-making in the city of Kabul. The goal of the experiment was to increase the citizen involvement in implementing Sustainable Development Goals. The second experiment is a small-scale debate between a group of 16 students about globalisation and taxation in Myanmar. In the first experiment, we found that the agent improved the responsiveness of the participants and increased the number of identified ideas and issues. In the second experiment, we found that the agent polarised the debate by reinforcing the initial stances of the participant.
