Salmonella Berta myocarditis:Case report and systematic review of non-typhoid Salmonella myocarditis

AIM: To study trends in the epidemiology,clinical presentation,microbiology and prognosis of nontyphoid Salmonella(NTS) ***: We performed a systematic literature search for all reported NTS cases. The search yielded 838 publications. A total of 21 papers were deemed eligible. No language restrictions were enforced. Articles that were not written in English were translated. Pre-specified data such as clinical presentation,electrocardiogram(ECG) changes,transthoracic echocardiographic findings,cardiac magnetic resonance findings,microbiology cultures,Salmonella species,inflammatory markers(erythrocyte sedimentation rate and C-reactive protein),cardiac biomarkers and severity of illness were collected using data extraction sheets. Cases were classified by age into 2 groups; pediatric cases(defined as statistics(Windows,Version 20.0. Armonk,NY: IBM Corp.) for demographic characteristics,presenting symptoms,microbiology,diagnostic methods,treatment modalities and ***: From the selected articles,we identified a total of 24 individual cases with verifiable data. There were 20 males with a male to female ratio of 5:1. The mean age at presentation was 30.8 years(range 1 mo-67 years),16% of cases were children aged < 18 years. Most patients presented with chest pain,fever,and abdominal pain. The most common ECG finding was ST elevation. Cardiac biomarkers were elevated in around 70% of cases. Salmonella Enteritidis was the most common NTS isolated. Definitive diagnosis was established by blood and stool cultures in most of the cases. The pediatric and adults cases had similar incidence of bacteremia(40% vs 36.8%) while the pediatric group had more stool cultures positive compared to the adult group(100% vs 63.1%). The formation of a bezoar is a relatively infrequentdisorder that affects the gastrointestinal *** are mainly classified into four types dependingon the material constituting the indigestible mass of thebezoar phytobezoars, trichobezoars, pharmacobezoars,and lactobezoars. Gastric bezoars often cause ulcerativelesions in the stomach and subsequent bleeding,whereas small intestinal bezoars present with smallbowel obstruction and ileus. A number of articles haveemphasized the usefulness of Coca-Cola？ administrationfor the dissolution of phytobezoars. However, persimmonphytobezoars may be resistant to such dissolutiontreatment because of their harder consistency comparedto other types of phytobezoars. Better understanding ofthe etiology and epidemiology of each type of bezoarwill facilitate prompt diagnosis and *** we provide an overview of the prevalence, classification,predisposing factors, and manifestations ofbezoars. Diagnosis and management strategies arealso discussed, reviewing mainly our own case *** progress in basic research regarding persimmonphytobezoars is also briefly reviewed.

台湾从2007年5月启动了耐多药结核病项目。在台湾东部,自2007年5月到2009年2月共纳入了77例耐多药结核病(MDR-TB)患者。获取了其中的73例(93%)患者的分离株以应用间隔区寡核苷酸基因分型(spoligotyping)和MIRU-VNTR(分枝杆菌分散重复单位-可变数目的串联重复序列)进行基因分型。spoligotyping的分析结果表明,北京基因型是主要的流行基因型(n=48,66%)。在73株菌株中,28(38.4%)株有独特的基因型,45(61.6%)株菌株成簇。在45位基因型成簇的患者中,有21例患者发现存在流行病学联系。在台湾东部MDR结核病患者的菌株成簇比例高。

Cardiovascular disease(CVD) is an important cause of mortality and morbidity in *** statistics and morbidity surveys indicate substantial regional variations in CVD prevalence and mortality *** from the Registrar General of India reported greater ageadjusted cardiovascular mortality in southern and eastern states of the *** heart disease(CHD) mortality is greater in south India while stroke is more common in the eastern Indian *** prevalence is higher in urban Indian populations while stroke mortality is similar in urban and rural ***-control studies in India have identified that the common major risk factors account for more than 90% of incident myocardial infarctions and *** case-control INTERHEART and INTERSTROKE studies reported that hypertension,lipid abnormalities,smoking,obesity,diabetes,sedentary lifestyle,low fruit and vegetable intake,and psychosocial stress are as important in India as in other populations of the *** studies have reported that there are substantial regional variations in risk factors in *** a macro-level these regional variations in risk factors explain some of the regional differences in CVD ***,there is need to study the prevalence of multiple cardiovascular risk factors in different regions of India and to correlate them with variations in CVD mortality using a uniform *** is also a need to determine the 'causes of the causes' or fundamental determinants of these risk *** India Heart Watch study has been designed to study socioeconomic,anthropometric and biochemical risk factors in urban populations in different regions of the country in order to identify regional differences.
