Dendritic cell vaccination in glioblastoma after fluorescenceguided resection

AIM: To assess whether the addition of a customized, active immunotherapy to standard of care including fluorescence-guided surgery, may provide hints of an improved survival for patients with poor-prognosis, incurable glioblastoma multiform. METHODS: Preliminary to our ongoing, phase-Ⅱ clinical trial, we conducted a small pilot study enrolling five consecutive patients with resectable glioblastoma. In terms of Recursive Partitioning Analysis, four patientswere class Ⅴ and one was class Ⅳ. In all five cases, fluorescence-guided surgery was employed, followed by rapid steroid discontinuation. Patients were then treated with a combination of standard radio-chemotherapy with temozolomide and tumor lysate-pulsed, mature dendritic cell-based ***: Though all five patients ultimately progressed, with any further treatment left to the sole decision of the treating oncologist, active immunotherapy was very well tolerated and induced specific immune responses in all three patients for whom enough material was available for such an assessment. Median progression-free survival was 16.1 mo. Even more important, median and mean overall survival were 27 mo and 26 mo, respectively. Three patients have died with an overall survival of 9 mo, 27 mo and 27.4 mo, while the other two are still alive at 32 mo and 36 mo, the former receiving treatment with bevacizumab, while the latter has now been off therapy for 12 mo. Four of five patients were alive at two ***: Active immunotherapy with tumor lysate-pulsed, autologous dendritic cells is feasible, safe, well tolerated and biologically efficacious. A phase-Ⅱ study is ongoing to possibly improve further on our very encouraging clinical results.

Dear Editor,Induction chemotherapy has been utilized for decades in locally advanced head and neck squamous cell carcinoma(HNSCC).The docetaxel,cisplatin,5-fluorouracil(TPF)regimen is the most recommended induction chemotherapy regimen forHNSCC and oral squamous cell carcinoma(OSCC)[1].However,our initial phase Ⅲ trial failed to demonstrate a significant survival benefit of TPF induction chemotherapy in patients with locally advanced OSCC[2].

Monitoring of Tumor Response to Neoadjuvant Radio-Chemotherapy of Esophageal Carcinoma by F-18-FDG-PET

Introduction: For clinical assessment of neoadjuvant radiochemotherapy of esophageal cancer reliable in-vivo methods are necessary. Therefore, the capabilities of F-18-Fluorodesoxyglucose-PET in comparison to histomorphological grading of tumor regression were studied. Methods: In 33 patients with locally advanced esophageal carcinoma (uT3, uN0-1, cM0) F-18-FDG-PET was performed before and 2 weeks after radiochemotherapy. All tumors were resected by transthoracic en-bloc esophagectomy 3–4 weeks after induction therapy. A subgroup of 11 patients underwent weekly PET scan during neoadjuvant therapy. PET was performed in a dedicated scanner 1.3 h after administration of 370 MBq F-18-FDG. Data analysis based on maximum SUV data derived from individual regions of interest in pre- and posttherapeutic images. PET data were compared to histomorphological grading parameters for tumor regression whithin the resected tissues. Results: The comparison of histopathological tumor regression after neoadjuvant therapy and PET SUV di?erences showed a signi?cant χ2 P -value of 0.006. There was a signi?cant decrease 五笔字型计算机汉字输入技术 of the SUV data from 9.1±3.5 to 4.3±1.9 (P <0.0001). In therapy responders SUV was diminished by 59 % and in non-responders by 34 %. Longitudinal SUV measurement during neoadjuvant therapy showed a strong SUV decrease already after one and two weeks (P =0.021 and 0.003). Conclusion: The recent data of the FDG-PET follow-up after neoadjuvant therapy show that PET is able to predict therapy response. Longitudinal PET data advocate that it may be possible to recognize response also very early during radiochemotherapy.

Comparative Analysis of MRI Diagnosis and Multislice CT in the Diagnosis of Soft Tissue Tumors in Limbs

The purpose of this study was to compare and analyze the diagnostic applications of multisliecs spiral computed tomography(MSCT)and diffusively-weighted magnetic resonance imaging(MRI)in soft tissue tumors of extremities.A total of 104 patients with primary soft tissue tumors of limbs were selected for MSCT and MRI *** values of various tumor types were compared during CT *** MRI detection,the diffusion sensitivity factor(b)of diffuse-weighted MRI was 0.500 s/mm2 to avoid as much as possible the bleeding,necrosis,scar and calcification heterogeneity components during *** maximum interest point was selected to observe the apparent diffusion coefficient(ADC)between benign and malignant tumors and muscle *** showed that among 104 patients,36 of patients were malignant and 68 of patients were *** examination was consistent with pathology in 45 cases and MRI in 87 *** addition,the 59 of patients in MSCT examination and the 17 of patients in MRI were qualitative error and *** tissue tumors on MSCT showed a low-density mass,and lipoma and cyst were *** MRI examination,T1WI and T2WI were significantly different in different types of tumors,while ADC values of malignant tumors were significantly lower than those of benign tumors and muscle tissues(P0.05).Therefore,MSCT can clearly show the soft tissue tumor lesions of the limbs and identify the relationship between them and the surrounding ***,MSCT cannot make accurate *** diffusivity weighted imaging can better differentiate benign from malignant and infer the histological origin of *** detection level of MRI was significantly higher than that of CT,which was more consistent with ***,in the preoperative diagnosis of soft tissue tumors in limbs,MRI diffused-weighted imaging should be the first choi

BACKGROUND Intraductal papillary mucinous neoplasms(IPMNs)are non-invasive pancreatic precursor lesions that can potentially develop into invasive pancreatic ductal ***,the International Consensus Guidelines(ICG)for IPMNs provides the basis for evaluating suspected IPMNs on computed tomography(CT)*** using the ICG,it remains challenging to accurately predict whether IPMNs harbor high grade or invasive disease which would warrant surgical resection.A supplementary quantitative radiological tool,radiomics,may improve diagnostic accuracy of radiological evaluation of *** hypothesized that using CT whole lesion radiomics features in conjunction with the ICG could improve the diagnostic accuracy of predicting IPMN *** To evaluate whole lesion CT radiomic analysis of IPMNs for predicting malignant histology compared to International Consensus *** Fifty-one subjects who had pancreatic surgical resection at our institution with histology demonstrating IPMN and available preoperative CT imaging were included in this retrospective *** lesion semi-automated segmentation was performed on each preoperative CT using Healthmyne software(Healthmyne,Madison,WI).Thirty-nine relevant radiomic features were extracted from each lesion on each available contrast *** analysis of the 39 radiomics features was performed for each contrast phase and values were compared between malignant and benign IPMN groups using logistic *** quantitative and qualitative CT measurements were also compared between groups,viaχ2(categorical)and Mann Whitney U(continuous)*** Twenty-nine subjects(15 males,age 71±9 years)with high grade or invasive tumor histology comprised the"malignant"cohort,while 22 subjects(11 males,age 70±7 years)with low grade tumor histology were included in the"benign"*** analysis showed 18/39 precontrast,19/39 arterial phase,and 21/39 venous phase features

Clinical pathology analysis of esophageal sarcomatoid carcinoma

Objective： The purpose of this study was to study the clinical, imaging characters and pathological characteristics of esophageal sarcomatoid carcinoma. Methods： We reviewed 23 cases of esophageal sarcomatoid carcinoma from Janu ary 2006 to December 2013 in four hospitals. The data of patients who were esophageal sarcomatoid carcinoma operated were retrospectively analyzed. All cases had completed upper gastrointestinal barium images materials and 14 of these cases had completed CT images materials. Upper gastrointestinal barium images and CT imaging features include tumor location, size, shape, and strengthen, etc. The biological parameters of lesions including the express of cytokeratin AE1/AE3, 34E12,, Vimentin, desmin, Actin, S100 and Ki67 detected by immunhistochemical UltraSensitiveTM SP method （n = 23）, and the patients＇ data of contrastographic picture （n = 23）, imaging characters of CT scan （n = 14）, and their relationship were studied. Results： Upper gastrointestinal barium images, CT imaging and gastrointestinal fiberscopy revealed Iobulated intraluminal filling defect 0.4 cm to 5.7 cm x 3.5 cmx 1.3 cm （mean = 3. 7 cm） in the mid （n = 14）, lower （n = 7） and upper （n = 2） intrathoracic esophagus. Among 23 cases of esophageal sarcomatoid carcinoma, 19 patients were of mushroom type, 2 patients was of ulcer type, and 2 patients were of medulla type; 19 patients were pedunculated, and 4 patients were no pedunculated （2 patients was of ulcer type）. The tumor surface was relatively smooth and esophageal compliance was maintained. The pathological changes of esophagus such as lightly locked, rigid wall nomanifest partly, esophageal lumens expand partly, major filling sublobe defect could be shown through contrast medium. Normal esophagus was no unpack obviously over pathological changes. Enhanced computed tomography showed tumors in the intrathoracic esophagus and 8 lymph nodes metastases in 3 cases. In search of the medulloblast： neural stem cells and embryonal brain tumors

Medulloblastomas have a cellular and molecular phenotype similar in many ways to that of neural stem cells。Indeed, it has long been believed that a medulloblastoma can arise from transformed neural stem ceils。Recent analyses of murine transgenic lines has confirmed that ceils of the external germinal layer (EGL) can be transformed into a medulloblastoma, generally in association with activation of the Hedgehog signaling pathway。Stem or progenitor cell populations outside the EGL, however, are also likely the cells of origin for a subset of medulloblastomas。
