To scan or not to scan:Use of transient elastography in an integrated health system

BACKGROUND Non-invasive tests,such as Fibrosis-4 index and transient elastography(com-monly FibroScan),are utilized in clinical pathways to risk stratify and diagnose non-alcoholic fatty liver disease(NAFLD).In 2018,a clinical decision support tool(CDST)was implemented to guide primary care providers(PCPs)on use of FibroScan for *** To analyze how this CDST impacted health care utilization and patient *** We performed a retrospective review of adults who had FibroScan for NAFLD indication from January 2015 to December 2017(pre-CDST)or January 2018 to December 2020(post-CDST).Outcomes included FibroScan result,laboratory tests,imaging studies,specialty referral,patient morbidity and *** We identified 958 patients who had FibroScan,115 before and 843 after the CDST was *** percentage of FibroScans ordered by PCPs increased from 33%to 67.1%.The percentage of patients diagnosed with early F1 fibrosis,on a scale from F0 to F4,increased from 7.8%to 14.2%.Those diagnosed with ad-vanced F4 fibrosis decreased from 28.7%to 16.5%.There were fewer laboratory tests,imaging studies and biopsy after the CDST was *** there were more specialty referrals placed after the CDST was implemented,multivariate analysis revealed that healthcare utilization aligned with fibrosis score,whereby patients with more advanced disease had more *** few patients were hospitalized or *** This CDST empowered PCPs to diagnose and manage patients with NAFLD with appropriate allocation of care towards patients with more advanced disease.

According to a recently advanced unified theory of low frequency fluctuation-dissipationproperties of condensed matter, we have extended the classical Markovian diffusion transportto non-Markovian diffusion transport. The theory involves a single parameter n, the infrareddivergence exponent. Synchronous and Asynchronous Blastomere Cleavage at Cryopreservation: Effect on Subsequent Embryo Survival, Pregnancy and Live Birth Rates

Capsule: Although embryos with synchronous blastomere cleavage showed higher post-thaw survival rates, pregnancy rates did not differ. Thus, embryos with all cleavage patterns may be safely cryopreserved. Objective: To compare post-thaw embryo survival, pregnancy and live birth rates of embryos with synchronous vs asynchronous blastomere cleavage in frozen embryo transfer (FET) cycles. Design: Retrospective study. Setting: University-affiliated IVF unit. Patients: One thousand and sixty FET cycles performed from 2004-2006. Interventions: Cycles were divided into 3 groups: 1: cycles in which only embryos with synchronous blastomere cleavage were frozen;2: cycles in which only embryos with asynchronous blastomere cleavage were frozen;3: cycles in which both embryos with synchronous and asynchronous blastomere cleavage were frozen. Clinical and laboratory data were recorded and analyzed. Main Outcome Measures: Post-thaw embryo survival, morphologic grading, pregnancy and live birth rates. Results: A total of 1863 embryos were analyzed. Synchronous embryos had higher blastomere survival rates and morphological grading at thawing. Pregnancy and birth rates did not differ among groups. In a multivariant logistic regression analysis, a number of transferred embryos and embryo morphological grading at thawing were the only parameters that affected pregnancy and live birth rates. Conclusions: Embryos with both synchronous and asynchronous blastomere cleavage can be selected by classical embryo grading and safely cryopreserved.

Multi-parameter Triebel-Lizorkin and Besov Spaces Associated with Flag Singular Integrals

Though the theory of one-parameter Triebel-Lizorkin and Besov spaces has been very well developed in the past decades, the multi-parameter counterpart of such a theory is still absent. The main purpose of this paper is to develop a theory of multi-parameter Triebel-Lizorkin and Besov spaces using the discrete Littlewood-Paley-stein analysis in the setting of implicit multi-parameter structure. It is motivated by the recent work of Han and Lu in which they established a satisfactory theory of multi-parameter Littlewood-Paley-stein analysis and Hardy spaces associated with the flag singular integral operators studied by Muller-Ricci-stein and Nagel-Ricci-stein. We also prove the boundedness of flag singular integral operators on Triebel-Lizorkin space and Besov space. Our methods here can be applied to develop easily the theory of multi-parameter Triebel-Lizorkin and Besov spaces in the pure product setting.

Long aftershock sequences in North China and Central US: implications for hazard assessment in mid-continents

Because seismic activity within mid-continents is usually much lower than that along plate boundary zones, even small earthquakes can cause widespread con- cerns, especially when these events occur in the source regions of previous large earthquakes. However, these small earthquakes may be just aftershocks that continue for decades or even longer. The recent seismicity in the Tangshan region in North China is likely aftershocks of the 1976 Great Tangshan earthquake. The current earthquake sequence in the New Madrid seismic zone in central United States, which includes a cluster of M- 7.0 events in 1811-1812 and a number of similar events in the past millennium, is believed to result from recent fault reactivation that releases pre-stored strain energy in the crust. If so, this earthquake sequence is similar to aftershocks in that the rates of energy release should decay with time and the sequence of earthquakes will eventually end. We use simple physical analysis and numerical simulations to show that the current sequence of large earthquakes in the New Madrid fault zone is likely ending or has ended. Recognizing that mid-continental earthquakes have long aftershock sequences and complex spatiotemporal occur- rences are critical to improve hazard assessments.
