Objective： Since appropriate and time- table methods in trauma care have an important impact on patients＇ outcome, we evaluated the effect of Advanced Trauma Life Support （ATLS） program on medical interns＇ performance in simulated trauma patient management. Methods： A descriptive and analytical study before and after the training was conducted on 24 randomly se- lected undergraduate medical interns from Imam Reza Hospital in Mashhad, Iran. On the first day, we assessed in- terns＇ clinical knowledge and their practical skill performance in confronting simulated trauma patients. After 2 days of ATLS training, we performed the same study and evaluated their score again on the fourth day. The two findings, pre- and post- ATLS periods, were compared through SPSS version 15.0 software. P values less than 0.05 were considered statistically significant. Results： Our findings showed that interns＇ ability in all the three tasks improved after the training course. On the fourth day after training, there was a statistically significant increase in interns＇ clinical knowledge of ATLS procedures, the sequence of procedures and skill performance in trauma situations （P〈0.001, P=0.016 and P=0.01 respectively）. Conclusion： ATLS course has an important role in increasing clinical knowledge and practical skill performance of trauma care in medical interns.

Enzyme pattern of biliary colic: A counterintuitive picture

AIM To evaluate the diagnostic value of serial biochemical blood tests in the diagnosis of biliary *** Files were reviewed of 1039 patients who were admitted to the Share'e Zedek medical Center emergency department between the years 2012-2013, and received the coding of acute biliary disease. Of these, the first 100 cases were selected that met the following criteria:(1) a diagnosis of biliary colic or symptomatic cholelithiasis;(2) at least two biochemical blood tests performed; and(3) 18 years of age or older. Patients with other acute biliary diseases were excluded. The biochemical profile of the patients was analyzed as were their clinical and radiological *** Three-quarters of the patients were women, whose average age of 37 years was younger than the averageof the men, at 50 years. According to their histories, 47% of the patients had previously known cholelithiasis. Pain in either the right upper quadrant or the epigastrium was the presenting symptom in 93% cases. The greatest change in serum biochemical results was seen during the first day of the patients' admissions. Alanine aminotransferase(ALT) showed the highest initial rise above the reference range, followed by aspartate aminotransferase(AST), gamma-glutamyl transferase(GGT), bilirubin and alkaline phosphatase(ALKP)- all these increases were statistically significant(P < 0.05). AST showed the sharpest decline followed by bilirubin and ALT. GGT and ALKP did not fall. A sharp rise and fall in liver enzymes, especially during the first day, most prominently in AST and ALT, was seen in 70% percent of cases. In 65% of cases trans-abdominal sonography did not give diagnostic *** Serial serum liver enzyme measurements are helpful in the initial diagnosis of acute biliary colic.

Awareness,practice and views about integrating AYUSH in allopathic curriculum of allopathic doctors and interns in a tertiary care teaching hospital in New Delhi,India

Objective： To assess the knowledge, attitude, perception and practice of Ayurveda, yoga, Unani, Siddha, homeopathy and naturopathy （AYUSH） among allopathic doctors and interns and determine their opin- ion regarding integration of AYUSH education within the allopathic curriculum. Methods： This prospective, cross sectional study was conducted in a tertiary care teaching Hospital, New Delhi, India, from March 2016 to March 2017. A survey of 500 aUopathic doctors and 150 interns was performed using a structured questionnaire. Results： A majority of doctors （95.2%） were aware of the term AYUSH and had knowledge of yoga （70.6%）, homeopathy （55.6%）, Ayurveda （50.3%）, Siddha （47.8%）, naturopathy （32.9%） and Unani （28.9%）. Most of the participants （63.0%） felt that therapies under AYUSH are effective. A majority （84.0%） of the doctors were of the view that more research is required in the field of AYUSH. While homeopathy （44.0%） was the most common system used by doctors for themselves, yoga was the therapy most commonly （60.0%） recommended to patients. Nearly half of the doctors （46.7%） recommended AYUSH to their patients, mainly for chronic illness （74.0%）. A majority （77.5%） of the respondents agreed that students of allopathic med- icine should be familiarized with AYUSH. The modes of incorporation of AYUSH into the medical curriculum most commonly suggested by respondents were optional courses （54.5%） and introductory lectures （42.3%）. Conclusion： Doctors and interns ofallopathic medicine are aware of AYUSH systems. They use it for them- selves and recommend it to patients. They believe that familiarization of Bachelor of Medicine and Bachelor of Surgery students with AYUSH therapies and their integration with allopathic medicine may help in improving patient care.

Knowledge,attitude and anxiety pertaining to basic life support and medical emergencies among dental interns in Mangalore City,India

BACKGROUND: This cross-sectional study aimed to assess the knowledge, attitude and anxiety pertaining to basic life support(BLS) and medical emergencies among interns in dental colleges of Mangalore city, Karnataka, ***: The study subjects comprised of interns who volunteered from the four dental colleges. The knowledge and attitude of interns were assessed using a 30-item questionnaire prepared based on the Basic Life Support Manual from American Heart Association and the anxiety of interns pertaining to BLS and medical emergencies were assessed using a State-Trait Anxiety Inventory(STAI) Questionnaire. Chi-square test was performed on SPSS 21.0(IBM Statistics, 2012) to determine statistically signifi cant differences(Pinterns, 39.89% had below average knowledge. A total of 123(67.21%) reported unavailability of professional training. The majority(180, 98.36%) felt the urgent need of training in basic life support procedures. Assessment of stress showed a total of 27.1% participants to be above highstress level. Comparison of assessed knowledge and stress was found to be insignifi cant(P=0.983).CONCLUSION: There was an evident lack of knowledge pertaining to the management of medical emergencies among the interns. As oral health care providers moving out to the society, a focus should be placed on the training of dental interns with respect to Basic Life Support procedures.

The emergence of novel severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)has caused the global outbreak and major public health *** the outbreak human-to-human transmission was confirmed with or without symptoms of upper and lower respiratory tract *** to date,there has been evidence that COVID-19 is beyond that of a typical pulmonary disease and revealing pathomechanics of COVID-19-associated acute respiratory distress syndrome(CARDS),which include severe inflammation and pulmonary edema leading to impaired alveolar homeostasis,and resulting in an alteration of lung physiology,lung fibrosis,inflammation of endothelium,vascular thrombosis,as well as exaggerated immune re*** this pathophysiology,the use of quercetin as phytotherapeutic may merit in the management of COVID-19 *** this review,the authors wish to elaborate on the molecular effect of quercetin on SARS-CoV-2 by giving a detailed mechanism of quercetin against the binding of the S-protein of the virus to angiotensin-converting enzyme 2(ACE2)receptors,the main protease(M^(pro))or 3C-like protease(3CL^(pro)),papain-like protease(PL^(pro)),and RNA-dependent RNA polymerase(RdRP).Recent clinical evidence supporting the use of quercetin in COVID-19 management is also discussed in this paper.

Disorders of sex development(DSD)are a group of rare complex clinical syndromes with multiple *** the various causes of DSD is quite difficult in clinical practice,even for senior general physicians because of the similar and atypical clinical manifestations of these *** addition,DSD are difficult to diagnose because most primary doctors receive insufficient training for *** diagnoses and misdiagnoses are common for patients with DSD and lead to poor treatment and *** the basis of the principles and algorithms of dynamic uncertain causality graph(DUCG),a diagnosis model for DSD was jointly constructed by experts on DSD and engineers of artificial intelligence.“Chaining”inference algorithm and weighted logic operation mechanism were applied to guarantee the accuracy and efficiency of diagnostic reasoning under incomplete situations and uncertain *** was performed using 153 selected clinical cases involving nine common DSD-related diseases and three causes other than DSD as the differential *** model had an accuracy of 94.1%,which was significantly higher than that of interns and third-year *** conclusion,the DUCG model has broad application prospects as a computer-aided diagnostic tool for DSDrelated diseases.

Comparison of two reader modes of computer-aided diagnosis in lung nodules on low-dose chest CT scan

Low-dose computerized tomography(LDCT)scanning is of great significance for monitoring and management of pulmonary nodules on chest computerized tomography(CT).Nevertheless,the malignant potential of these nodules is often difficult to detect,especially for some smaller pulmonary nodules on LDCT *** advances using the state-of-art computer-aided detection(CAD)system have attempted to address this problem by identifying small nodules that can be easily missed during clinical *** is used in two reading modes:Concurrent-reader(CR)mode or second-reader(SR)*** this study,we prospectively evaluated the efficiency of a CAD system's SR and CR modes in detecting pulmonary nodules on *** found that the SR mode improves pulmonary nodule detection regardless of the dose and ex-perience level,especially for interns in the low-dose *** CR mode maintains the sensi-tivity of SR mode while significantly decreasing reading times.

Efficacy of Aescuven forte on male infertility with varicocele

Objective:To explore the effectiveness of Aescuven forte in treatment of male infertility with ***:One hundred and forty-three male patients with infertility with vsricocele were randomly divided into medication group,control group and surgery *** them,seventy-four patients in medication group took Aescuven forte(0.3g,2/day)for 2-3 months combined with other medicine availing for semen *** was analyzed and the diameter of vena spermatica interns was measured with ultrasound before and after the ***-one patients in control group only took the same medicine availing for semen ***-eight patients in surgery group accepted varicocelectomy and took the same medicine availing for semen quality after the ***:After the operation or medication,the improving rates for the sperm density in medication group,con-trol group and surgery group were 53.2%,39.0%,and 67.9% *** significant difference existed be-tween control group and surgery group(P<0.05).The improving rates for the sperm motility in the three groups were 51.6%,46.3%,and 71.4% *** significant difference existed between control group and surgery group(P<0.05).After Aescuven forte treatment,the effective retracting rates for the 108 sides varicocele in 74 patients in medication group reached 43.5%.The effective retracting rates for varicoeele in mild,moderate and se-vere varicocele group defined by the diameter of the vessel were 39.2%,60.7%,and 16.7% *** im-proving rate in moderate varicocele group was much higher than that in the other two group.(P<0.05)Conclusion:Aescuven forte combined with other medicine can improve semen quality in male infertility with *** can retract the vein to some degree in some patients with varicocele,especially for the patients with middle degree *** could be a therapeutic choice for male infertility with varicocele.
