Pulmonary fibrosis, a progressive chronic disease with a high mortality rate, has limited treatment options. Currently, lung transplantation remains the only effective treatment. Here we report that a small RNA, HJT-sRNA-m7, from a Chinese herbal medicine Hong Jing Tian(HJT, RHODIOHAE CRENULATAE RADIX ET RHIZOMA, Rhodiola crenulata) can effectively reduce the expressions of fibrotic hallmark genes and proteins both in alveolar in vitro and in mouse lung tissues in vivo. We also discovered over one hundred oil-soluble chemicals from HJT decoctions, most of which are found in lipid extracts from other Chinese herbals decoctions, including Pu Gong Ying(PGY, TARAXACI HERBA, Taraxacum mongolicum), Chuan Xin Lian(CXL, changed to "ANDROGRAPHIS HERBA, Andrographis paniculata"), and Jin Yin Hua(JYH, lonicera japonica or Honeysuckle). We identified the active component in these decoctions as two forms of phosphocholines, PC(18:0/18:2) and PC(16:0/18:2). These PCs potentially could form liposomes with small RNAs to enter human alveolar and gastric cells. Our experimental results suggest an unprecendent lipid complex route through which botanic small RNA can enter human *** results provide an innovative treatment strategy for oral delivery of siRNAs as therapeutic medication.

BACKGROUND Cholangiocarcinoma is a highly lethal disease that had been underestimated in the past two decades. Many risk factors are well documented for in cholangiocarcinoma, but the impacts of advanced biliary interventions, like endoscopic sphincterotomy(ES), endoscopic papillary balloon dilatation(EPBD),and cholecystectomy, are inconsistent in the previous *** To clarify the risks of cholangiocarcinoma after ES/EPBD, cholecystectomy or no intervention for cholelithiasis using the National Health Insurance Research Database(NHIRD).METHODS From data of NHIRD 2004-2011 in Taiwan, we selected 7938 cholelithiasis cases as well as 23814 control group cases(matched by sex and age in a 1:3 ratio). We compared the previous risk factors of cholangiocarcinoma and cholangiocarcinoma rate in the cholelithiasis and control groups. The incidences of total and subsequent cholangiocarcinoma were calculated in ES/EPBD patients, cholecystectomy patients, cholelithiasis patients without intervention,and groups from the normal *** In total, 537 cases underwent ES/EPBD, 1743 cases underwent cholecystectomy,and 5658 cholelithiasis cases had no intervention. Eleven(2.05%), 37(0.65%), and7(0.40%) subsequent cholangiocarcinoma cases were diagnosed in the ES/EPBD,no intervention, and cholecystectomy groups, respectively, and the odds ratio for subsequent cholangiocarcinoma was 3.13 in the ES/EPBD group and 0.61 in the cholecystectomy group when compared with the no intervention *** In conclusion, symptomatic cholelithiasis patients who undergo cholecystectomy can reduce the incidence of subsequent cholangiocarcinoma, while cholelithiasis patients who undergo ES/EPBD are at a great risk of subsequent cholangiocarcinoma according to our findings.

Treatment and prognosis of hepatic epithelioid hemangioendothelioma based on SEER data analysis from 1973 to 2014

Background: Hepatic epithelioid hemangioendothelioma (HEH) is a rare tumor of vascular origin with an unknown etiology, a low incidence, and a variable natural course. We evaluated the management and prognosis of HEH from the Surveillance, Epidemiology and End Results (SEER) program and changes in treatment modalities of HEH over 30 years. Methods: From 1973 to 2014 in the SEER database, we selected patients diagnosed with HEH. We analyzed the clinical characteristics, patterns of management, and clinical outcomes of patients with HEH. Results: We identi ed 79 patients with HEH (median age: 54.0 years;male to female ratio: 1:2.6). The initial extent of disease was local in 22 (27.8%) patients, regional metastasis in 22 (27.8%), distant metas-tasis in 31 (39.2%) and unknown in 4 (5.1%). The median size of primary tumor was 3.85 cm (interquartile range, 2.50 7.93 cm). Among 74 patients with available management data, the most common manage-ment was no treatment (29/74, 39.2%), followed by chemotherapy only (22/74, 29.7%), liver resection-based (13/74, 17.6%), and transplantation-based therapy (6/74, 8.1%). The 5-year cancer-speci c survival rate was 57.8%. Patients who underwent surgical treatment had signi cantly higher survival than those who underwent non-surgical treatment (5-year survival;88% vs. 49%, P=0.019). Multivariate analysis revealed that surgical therapy was the only independent prognostic factor for survival (hazard ratio: 0.20, P=0.040). Conclusions: Resection or liver transplantation is worth considering for treatment of patients with HEH.

Objective: To investigate protective effects of Spilanthes acmella(S,acmella) Murr,extracts against pesticide-induced neuronal cells death and to elucidate the underlying molecular mechanism in dopaminergic(SH-SY5Y) cells lines,Methods: Cell viability of SH-SY5 Y cells was studied by treating the cells with various concentration of pirimicarb for 24 hr,Neuroprotective effect of S,acmella Murr,extracts was investigated by adding the plant extracts to the medium for 24 hr prior to the incubation with 100 μM H_2O_2 or with pirimicarb for 24 hr,Control-untreated cells were incubated with the culture medium,Cell viability was measured by MTT assay,calpain and calpastatin expressions were analyzed by Western blotting and immunocytochemistry,Results: Pretreatment of SH-SY5 Y cells with S,acmella Murr,extracts(1 μg/m L) for 24 hr significantly increased the dopaminergic neurons in pirimicarb-induced neurotoxicity,In addition,pretreatment with the S,acmella Murr,extracts led to decreased calpain but increased calpastatin protein levels,Conclusion: S,acmella Murr,extracts exerted neuroprotective effect,via an alteration of calcium homeostasis,against pirimicarb induced neurotoxicity,The S,acmella Murr,might be a potential natural candidate with neuroprotective activity.

BACKGROUND This was an observational, descriptive, and retrospective study from 2011 to 2020 from the Department of informatics of the Brazilian Healthcare System *** To describe the intestinal complications(IC) of patients with ulcerative colitis(UC) who started conventional therapies in Brazil’s public Healthcare *** Patients ≥ 18 years of age who had at least one claim related to UC 10th revision of the International Statistical Classification of Diseases and Related Health Problems(ICD-10) code and at least 2 claims for conventional therapies were included. IC was defined as at least one claim of: UC-related hospitalization, procedures code for rectum or intestinal surgeries, and/or associated disease defined by ICD-10 codes(malignant neoplasia of colon, stenosis, hemorrhage, ulcer and other rectum or anus disease, megacolon, functional diarrhea volvulus, intussusception and erythema nodosum). Descriptive statistics, annual incidence, and incidence rate(IR) [per 100 patient-years(PY)] over the available follow-up period were *** In total, 41229 UC patients were included(median age, 48 years;65% women) and the median(interquartile range) follow-up period was 3.3(1.8-5.3) years. Conventional therapy used during follow-up period included: mesalazine(87%), sulfasalazine(15%), azathioprine(16%) or methotrexate(1%) with a median duration of 1.9(0.8-4.0) years. Overall IR of IC was 3.2 cases per 100 PY. Among the IC claims, 54% were related to associated diseases, 20% to procedures and 26% to hospitalizations. The overall annual incidence of IC was 2.9%, 2.6% and 2.5% in the first, second and third year after the first claim for therapy(index date), respectively. BackgroundDisagreement exists on the association between changes in blood pressure and cognitive impairment. We aimed to ex-amine whether 4-year changes in systolic and diastolic blood pressure （SBP and DBP） are associated with cognitive status in a representative sample of older men and *** of longitudinal data from 854 participants of a population-based German sample （aged 60-87 years） was performed with standard cognitive screening and blood pressure measurements. Effects of changes in SBP and DBP （10 mmHg and 5 mmHg respectively as unit of regression effect measure） on cognitive status were evaluated using non-parametric and lin-ear regression *** clear associations were seen between changes in SBP or in DBP and cognitive scores. Small effects were found after stratification for sex and hypertension awareness. Specifically, larger decreases in SBP were associated with higher cogni-tive scores in those men aware of their hypertension （10 mmHg decrease in SBP,b =-0.26, 95% CI：-0.51 to-0.02） and men with con-trolled hypertension （10 mmHg decrease in SBP,b =-0.44, 95% CI：-0.92 to-0.03）. Additionally larger increases in DBP were associated with higher cognitive scores in men with controlled hypertension （5 mmHg increase in DBP,b = 0.67, 95% CI： 0.19-1.15）. For women aware of their hypertension, larger decreases in DBP were associated with higher cognitive scores （5 mmHg decrease in DBP,b =-0.26; 95%CI：-0.51 to-0.01）.ConclusionsChanges in blood pressure were only weakly associated with cognitive status. Specifically, decreases in SBP were associated with higher cognitive scores in men aware of their hypertension and especially those that were medically controlled.

The network-based intrusion detection has become common to evaluate machine learning algorithms. Although the KDD Cup’99 Dataset has class imbalance over different intrusion classes, still it plays a significant role to evaluate machine learning algorithms. In this work, we utilize the singular valued decomposition technique for feature dimension reduction. We further reconstruct the features form reduced features and the selected eigenvectors. The reconstruction loss is used to decide the intrusion class for a given network feature. The intrusion class having the smallest reconstruction loss is accepted as the intrusion class in the network for that sample. The proposed system yield 97.90% accuracy on KDD Cup’99 dataset for the stated task. We have also analyzed the system with individual intrusion categories separately. This analysis suggests having a system with the ensemble of multiple classifiers;therefore we also created a random forest classifier. The random forest classifier performs significantly better than the SVD based system. The random forest classifier achieves 99.99% accuracy for intrusion detection on the same training and testing data set.

We present version 1.2.0 of DASHMM,a general library implementing hierarchical multipole methods using the asynchronous multi-tasking HPX-5 runtime *** with the previous release[10],this new version:(1)enables execution in both shared and distributed memory architectures;(2)extends DASHMM’s infrastructure to support advanced multipole methods[18];and(3)provides built-in implementations of both the Yukawa[15]potential and Helmholtz[16]potential in the low frequency *** additions have not impacted the user interface,which remains simple and extensible.
