护士主导的跨学科中心静脉导管维护管理模式的建立及应用

目的建立护士主导的跨学科中心静脉导管维护管理团队(vascular access specialist team,VAST)模式,并评价其应用效果。方法为提高中心静脉导管维护质量,2021年,在某医院实施了护土主导的VAST管理模式。管理团队由1名资深静脉治疗专业护士主导,联合超声/放射技师、进行中心静脉穿刺的医生、静脉通路维护护士以及行政管理人员等组成。管理模式主要包括完善以护士为主要对象的在岗培训、建立跨学科合作中心静脉穿刺“绿色通道”、推行护土主导的规范静脉治疗查房、指导中心静脉治疗患者进行标准化的自我护理。在管理模式实施前(2020年7月至2021年4月)及实施后(2021年5月至2022年5月)分别调查首次置管成功率、患者的自我护理能力、导管维护依从性及患者综合满意度情况,并进行比较。结果实施护士主导的跨学科VAST管理模式后,中心静脉导管首次置管成功率从92.9%(209/225)提升至100%(225/225);中心静脉导管置入患者自我护理能力总分由实施前(74.7518.4)提升至(99.10±23.65);患者导管维护依从性从93.3%(210/225)提升至99.6%(224/225);患者对导管维护的满意度也优于实施前(P<0.05)。结论护士主导的VAST能有效提高中心静脉置管临床管理质量。

It's a hot issue to improve the accuracy of network coordinate systems(NCS). By analyzing the error source of NCSes, it's emphasized that guaranteeing the low dimensionality of embedding datasets is the principle on the designing of NCSes. Based on this observation, a PCA based Internet delay space dividing algorithm(IDSD-PCA) is proposed. Beginning at choosing an optimized sub dataset randomly, IDSD-PCA partitions the Internet delay dataset into several sub datasets with low dimensionality by iterations. After the partition, the whole delay dataset and sub datasets are embedded into a hierarchical NCS. The experimental results show that with this framework both the metric space based models and the non-metric space based models can predict the end-to-end delays more accurately.

Battlefield environment simulation process is an important part of battlefield environment information support, which needs to be built around the task process. At present, the interoperability between battlefield environment simulation system and command and control system is still imperfect, and the traditional simulation data model cannot meet war fighters’ high-efficient and accurate understanding and analysis on battlefield environment’s information. Therefore, a kind of task-orientated battlefield environment simulation process model needs to be construed to effectively analyze the key information demands of the command and control system. The structured characteristics of tasks and simulation process are analyzed, and the simulation process concept model is constructed with the method of object-orientated. The data model and formal syntax of GeoBML are analyzed, and the logical model of simulation process is constructed with formal language. The object data structure of simulation process is defined and the object model of simulation process which maps tasks is constructed. In the end, the battlefield environment simulation platform modules are designed and applied based on this model, verifying that the model can effectively express the real-time dynamic correlation between battlefield environment simulation data and operational tasks.

Effects of cannabinoid receptor type 2 on endogenous myocardial regeneration by activating cardiac progenitor cells in mouse infarcted heart

Cannabinoid receptor type 2(CB2)activation is recently reported to promote proliferation of some types of resident stem cells(e.g.,hematopoietic stem/progenitor cell or neural progenitor cell).Resident cardiac progenitor cell(CPC)activation and proliferation are crucial for endogenous cardiac regeneration and cardiac repair after myocardial infarction(MI).This study aims to explore the role and possible mechanisms of CB2receptor activation in enhancing myocardial *** results revealed that CB2receptor agonist AM1241 can significantly increase CPCs by c-kit and Runx1 staining in ischemic myocardium as well as improve cardiomyocyte ***1241 also decreased serum levels of MDA,TNF-αand IL-6 after *** addition,AM1241 can ameliorate left ventricular ejection fraction and fractional shortening,and reduce ***,AM1241 treatment markedly increased p-Akt and HO-1 expression,and promoted Nrf-2 nuclear ***,PI3K inhibitor wortmannin eliminated these cardioprotective roles of *** conclusion,AM1241 could induce myocardial regeneration and improve cardiac function,which might be associated with PI3K/Akt/Nrf2 signaling pathway *** findings may provide a promising strategy for cardiac endogenous regeneration after MI.

Ascent schedules,acute altitude illness,and altitude acclimatization:Observations on the Yushu Earthquake

During the Yushu Earthquake on April 14,2010,a large number of rescuers from sea level or lowlands ascended to the quake areas very rapidly or rapidly less than 24 h. However,Yushu Earthquake is the highest quake in the world at altitudes between 3 750 m and 4 878 m where is a serious hypoxic environment. A high incidence of acute altitude illness was found in the unacclimatized rescuers;the mountain rescue operation changed as "rescue the rescuers". Lesson from the Yushu Earthquake is that the occurrence of acute altitude illness may be closely related to the ascent schedules. This prompted us to study the relationship between ascent rate and the incidence and severity of acute altitude illness;five different groups were compared. The first group was 42 sea level male young soldiers who ascended to quake area very rapidly within 8 h at 4 000 m;the second group was 48 sea level male young soldiers who ascended to 4 000 m rapidly less than 18 h;the third group was 66 acclimatized medical workers from 2 261 m who ascended to 4 000 m rapidly within 12 h;the fourth group was 56 Tibetan medical workers from 2 800 m who ascended to 4 000 m rapidly within 8 h;the fifth group was 50 male sea level workers who ascended to 4 000 m gradually over a period of 4 d. The results showed that the sea level rescuers ascended to 4 000 m very rapidly or rapidly had the highest incidence of acute mountain sickness (AMS) with the greatest AMS scores and the lowest arterial oxygen saturation (SaO2);the sea level workers ascended to 4 000 m gradually had moderate incidence of AMS with moderate AMS scores and SaO2 values;whereas the acclimatized and adapted rescuers had the lowest incidence of AMS,lowest AMS scores and higher SaO2;especially none AMS occurred in Tibetan rescuers. AMS score is inversely related to the ascent rate (r=-0.24,p< 0.001). Additionally,acute altitude illness is significantly influenced by altitude acclimatization. The characteristics of long-distance dust transport from Asia to the United States over the Pacific Ocean are statistically analyzed using OMI AI （Ozone Monitoring Instrument Aerosol Index） data during 2005-007. The results show that there is a high correla- tion of At （the correlation coefficients are as high as 0.83 in March, 0.62 in April and 054 in May） between the Mongolia Gobi Desert and the west coast region of the United States （40°-50°N, 120°- 130°W）, indicating a strong source-receptor area relationship. Through calculating the lag correlation coefficient of the dust propagation time, we determined that the dust propagation lime fi＇om the Mongolia Gobi Desert to the west coast of the United States is about 6-7 days at the 99% confidence level.

A successful mountain rescue operation in Yushu Earthquake

On April 14, 2010, an earthquake reaching 7.1 Richter scale struck Jiegu Town of Yushu. More than 2 698 people were confirmed dead, and 12 135 were injured, of which 1 434 were severely injured. Rescue operation was carried out soon after the disaster; however, the rescue teams face great challenges of altitude hypoxia, freezing temperature and very bad weather. Thus, 1 434 severe injuries were rapidly transported airlifted to hospitals in Xining and neighboring provinces for effective treatment. The extremity trauma （49.9 % ） was the most common patteru of injuries. Asphyxia （40.8 % ） was by far the most important reason for death. A high incidence of acute altitude illness in the lowland rescuers was a special medical problem during the highest earthquake in Yushu. We have learned more lessons from Yushu Earthquake.
