AIM:To compare clinical success and complications of uncovered self-expanding metal stents(SEMS)vs covered SEMS(cSEMS)in obstruction of the small ***:Technical success,complications and outcome of endoscopic SEMS or cSEMS placement in tumor related obstruction of the duodenum or jejunum were retrospectively *** primary end points were rates of stent migration and *** end points were the effect of concomitant biliary drainage on migration rate and overall *** data was analyzed according to the Strengthening the Reporting of Observational Studies in Epidemiology ***:Thirty-two SEMS were implanted in 20 *** all patients,endoscopic stent implantation was *** migration was observed in 9 of16 cSEMS(56%)in comparison to 0/16 SEMS(0%)implantations(P=0.002).Stent overgrowth did not significantly differ between the two stent types(SEMS:3/16,19%;cSEMS:2/16,13%).One cSEMS dislodged and had to be recovered from the jejunum by way of *** until migration between SEMS and cSEMS in patients with and without concomitant biliary stents did not significantly differ(HR=1.530,95%CI0.731-6.306;P=0.556).The mean follow-up was 57±71 d(range:1-275 d).CONCLUSION:SEMS and cSEMS placement is safe in small bowel tumor ***,cSEMS is accompanied with a high rate of migration in comparison to uncovered SEMS.

Postoperative adjuvant antiviral therapy for hepatitis B/C virus-related hepatocellular carcinoma:A meta-analysis

Current improvements in CT technology include fast rotation times, tubes with external cooling and flying focal spots, more detector slices and thinner collimation. These developments yield a higher spatial and temporal resolution, which are prerequisites for state of the art cardiac imaging. The studies performed.with the new technology indicate an improved sensitivity, specificity and a high negative predictive value for the assessment of coronary artery disease. Also, the depiction of coronary plaque morphology, myocardial infarctions, scars or masses and a functional evaluation have become clinical routine.

获得性免疫缺陷综合征(艾滋病,AIDS)是从人传播给人的病毒性(HIV)疾病,感染的危险性与病原体的数量有关,在感染的第一周和严重艾滋病的晚期病原体数最高。带有大量病原体的感染性血液具有高度危险性(>90％)。精子在感染性来源中占第二位,性接触是全世界最主要的传播途径。同性恋男性是最受危害的人群,但是,在全球的许多地区异性间的传播正在增长。静脉药瘾可因共用注射用具传播疾病。血液和血液制品在一些严格管理的国家已经不是疾病传播的主要来源,只是在极其例外的情况下才是感染的原因,医疗机构如果在护理上严格遵守消毒措施,传播疾病的危险性很小。昆虫并无感染性。人与人日常生活接触实际上不存在感染危险性,共用衣物、茶杯、在同一游泳池游泳或浴室蒸气浴和接吻也无危险性小心的应用阴茎套可以预防HIV感染。

Chronic hepatitis B virus (HBV) infection is characterized by a weak immune response to HBV. Regulatory T cells (Treg) can suppress the function of effector T cells and may thus be key players in this impaired immune response. Changes in the functionality or number of Treg could explain the decreased antiviral response in chronic HBV patients. To investigate the role of Treg in chronic HBV infection, we compared the proportional frequency and functionality of Treg in peripheral blood of 50 chronic HBV patients, 23 healthy controls, and 9 individuals with a resolved HBV infection. A higher percentage of Treg, defined as CD4, CD25, CD45RO, and cytotoxic T-lymphocyte-associated antigen 4-positive cells, was detected within the population of CD4+cells in peripheral blood of chronic HBV patients compared with healthy controls and individuals with a resolved HBV infection. Accordingly, chronic HBV patients displayed a higher FoxP3 messenger RNA level than healthy controls. Depletion of CD25+cells from peripheral blood mononuclear cells (PBMC) of chronic HBV patients resulted in an enhanced proliferation after stimulation with HBV core antigen. Reconstitution of these depleted PBMC with CD4+CD25+Treg resulted in a dose-dependent reduction of both HBV-specific proliferation and interferon γproduction. In conclusion, chronic HBV patients harbor an increased percentage of Treg in peripheral blood compared with controls. Treg have an immunosuppressive effect on HBV-specific T helper cells. The presence of HBV-specific Treg could contribute to an inadequate immune response against the virus, leading to chronic infection.
