Kinetics of testosterone recovery in clinically localized prostate cancer patients *** radical prostatectomy and subsequent short-term adjuvant androgen deprivation therapy

deprivation therapy （ADT） is a standard treatment for metastatic, recurrent and locally advanced prostate cancer （PCa）. The aim of this study is to investigate the timing and extent of testosterone recovery in clinically localized PCa patients treated with radical prostatectomy （RP） and subsequent short-term adjuvant ADT. A total of 95 localized PCa patients underwent RP and 9-month adjuvant ADT were included in this prospective study. Serum testosterone level was measured before adjuvant ADT, at ADT cessation, and at 1, 3, 6, 9 and 12 months after cessation of ADT. A Cox proportional hazards model was used to assess variables associated with the ti me of testosterone normalization. The results showed that median patient age was 67 years and median testosterone level before adjuvant ADT was 361 （230-905） ng d1-1. All patients finished 9-month adjuvant ADT and achieved castrate testosterone level. At 3 months after ADT cessation, testosterone recovered to supracastrate level in 97.9% patients and to normal level in 36.9% patients. The percentage of patients who recovered to normal testosterone level increased to 66.3%, 86.3% and 92.6% at 6, 9 and 12 months, respectively. Cox regression model found that higher baseline testosterone level （ 300 ng dl- 1） was the only variable associated with a shorter time to testosterone normalization （hazard ratio： 1.98; P -- 0.012）. In conclusion, in most patients, testosterone recovered to supracastrate level at 3 months and to normal level at 12 months after 9-month adjuvant ADT cessation. Patients with higher baseline testosterone level need shorter time of testosterone normalization.

High IL-23^(+)cells infiltration correlates with worse clinical outcomes and abiraterone effectiveness in patients with prostate cancer

Individualized treatment of prostate cancer depends on an accurate stratification of patients who are sensitive to various ***-23(IL-23)was reported to play a significant role in prostate ***,we aimed to explore the clinical value of IL-23-secreting(IL-23^(+))cells in prostate cancer *** evaluated interleukin-23A(IL-23A)expression in The cancer Genome Atlas database and retrospectively enrolled 179 treatment-naive metastatic prostate cancer patients diagnosed in our institute between June 2012 and December ***-23^(+)cells were stained and evaluated via ***,survival and multivariate Cox regression analyses were conducted to explore the prognostic value of IL-23^(+)*** found that IL-23A expression correlated with disease progression,while IL-23^(+)cells were clearly stained within prostate cancer *** with higher Gleason scores and multiple metastatic lesions tended to have more IL-23^(+)cell *** analyses showed that patients with higher levels of IL-23^(+)cells had significantly worse overall survival(hazard ratio[HR]=2.996,95%confidence interval[95%CI]:1.812–4.955;P=0.001)and a higher risk of developing castration resistance(HR=2.725,95%CI:1.865–3.981;P=0.001).Moreover,subgroup analyses showed that when patients progressed to a castration-resistant status,the prognostic value of IL-23^(+)cells was observed only in patients treated with abiraterone instead of ***,we showed that high IL-23^(+)cell infiltration is an independent prognosticator in patients with metastatic prostate ***-23^(+)cell infiltration may correlate with abiraterone effectiveness in castration-resistant prostate cancer patients.

Clinical activity of abiraterone plus prednisone in docetaxel-naive and docetaxel-resistant Chinese patients with metastatic castration-resistant prostate cancer

This study investigated the clinical activity of abiraterone plus prednisone in docetaxel-naive and docetaxel-resistant Chinese patients with metastatic castratio n-resista nt prostate can cer(mCRPC).A total of 146 patie nts with docetaxel-naive group(103 cases)and docetaxel-resista nt group(43 cases)were en rolled from the Sha nghai cancer center(Sha nghai,Chin a)in this retrospective cohort *** efficacy endpoints were prostate-specific antigen response rate,prostate-specific antigen progress!on-free survival,clinical/radiographic progression-free survival,and overall survival in response to abiraterone plus *** higher prostate-specific antigen response rate was found in docetaxel-naive group(54.4%,56/103)compared to docetaxel-resistant group(34.9%,15/43)(P=0.047).In addition,significantly higher median prostate-specific antigen progress!on-free survival(14.0 vs 7.7 months,P=0.005),clinical or radiographic progression-free survival(17.0 vs 12.5 months,P=0.003),and overall survival(27.0 vs 18.0 mon ths,P=0.016)were found in docetaxel-naTve group compared to docetaxel-resistant group,*** univariate and multivariate analyses indicated that lower albumin and visceral metastases were independent significant predictors for shorter overall *** sum up,our data suggested that abiraterone plus prednisone was efficient in both docetaxel-naTve and docetaxel-resistant Chinese ***,higher PSA response rate and longer overall survival were observed in the docetaxel-naTve group,which suggested that abiraterone was more effective for docetaxel-naive patients than for docetaxel failures.

Prognostic factors in Chinese patients with metastatic castration-resistant prostate cancer treated with docetaxel-based chemotherapy

This study aims to evaluate the potential value of patient characteristics in predicting overall survival （OS） in patients with metastatic castration-resistant prostate cancer （mCRPC） treated with docetaxel-based thermotherapy. A total of 115 patients with mCRPC undergoing a docetaxel q3w regimen were enrolled in this study. A survival analysis was performed using the Kaplan-Meier method. Cox proportional hazards models were used to evaluate the prognostic value of all covariates for OS. OS was also analysed after stratifying patients according to the results of multivariate analysis. The median OS for the entire cohort was 17.0 months. The multivariate analysis showed that the prostate-specific antigen doubling time （PSADT）, baseline haemoglobin （Hb） concentration, alkaline phosphatase （ALP） concentration, cycles of chemotherapy and time to castration resistance were independent prognostic factors of OS. According to the presence of PSADT 〈46.3 days and baseline ALP/〉 110 IU 1-1, all patients were divided into three risk groups： low-risk group （no risk factors）, intermediate-risk group （one risk factor） and high-risk group （two risk factors）. Median OSs for patients in low-, intermediate- and high-risk groups were 28.0 months （95% Ch 23.8-32.2）, 21.0 months （95% Ch 18.9-23.1） and 11.0 months （95% Ch 7.6-14.4）, respectively （P〈O.O01）. In conclusion, PSADT, baseline Hb concentration, ALP concentration, cycles of chemotherapy and time to castration resistance were independent prognostic factors of OS in Chinese patients with mCRPC treated with docetaxel. PSADT combined with the baseline ALP concentration could be a useful risk stratification parameter for evaluating survival outcomes.

In recent years,it has come a long way in the diagnosis,treatment,and follow-up of prostate *** this,it was argued that definitive treatments could cause overtreatment,particularly in the very low,low,and favorable risk *** alternative treatment and follow-up methods are being considered for this group of patients,active surveillance is seen as a good alternative for patients with very low and low-risk groups in this ***,it has become necessary to find other alternatives for patients in the favorable risk group or patients who cannot adopt active *** the light of technological developments,the concept of focal therapy was introduced with the intensification of research to treat only the lesioned area instead of treating the entire organ for prostate lesions though there are not many publications about many of them *** to the initial results,it was understood that the results could be good if the appropriate focal therapy technique was applied to the appropriate ***,focal therapies have begun to find their"middle ground"place between definitive therapies and active follow-up.

Gambogic Acid Induces Cell Apoptosis and Inhibits MAPK Pathway in PTEN^(-/-)/p53^(-/-) prostate cancer Cells In Vitro and Ex Vivo

Objective： To investigate the effect of gambogic acid（GA） on the growth and cell death of castrate resistant prostate cancer（PC） with phosphate and tension homology（PTEN） and p53 genes deleted in vitro and ex vivo, and elucidate the underlying possible molecular mechanisms. Methods： PTEN^（-/-）/p53^（-/-）PC cells and Los Angeles prostate cancer-4（LAPC-4） cells were treated with GA for 24 h and 48 h, then cell viability was determined by cell proliferation assay. PTEN^（-/-）/p53^（-/-）PC cells organoids number was calculated under GA treatment for 1 week. In addition, cell titer glo assay was performed to analyze 3 dimensional cell viability of patients derived xenografts（PDX） 170.2 organoids. Flow cytometry was used to detect apoptotic cells treated with GA. And confocal image was performed to detect the apoptotic mitochondrial morphological changes. Apoptotic cell death related protein levels were measured through Western blot（WB） in GA treated cells and organoids. The expression levels of mitogen-activated protein kinases（MAPKs） pathway related ribonucleic acid（RNAs） and proteins were analyzed by reverse transcription polymerase chain reaction（RT-PCR） and WB, respectively. Results： The treatment of GA significantly reduced cell viability of PTEN^（-/-）/p53^（-/-）PC cells and LAPC-4 in a time-and concentration-dependent manner. In organoids, GA showed strong inhibition towards organoids＇ numbers and diameters and continuously led to a complete organoids inhibition with GA 150 nmol/L. Ex vivo results validated that GA 1 μmol/L inhibited 44.6% PDX170.2 organoids growth. As for mechanism, flow cytometry detected continuously increased apoptotic portion under GA treatment from 1.98% to 11.78%（6 h） and 29.94%（8 h, P〈0.05）. In addition, mitochondrial fragmentation emerged in GA treated cells indicated the mitochondrial apoptotic pathway might be involved. cancer risk in relatives of BRCA1/2 pathogenic variant carriers in a large series of unselected patients with breast cancer

Interactions between life expectancy and the incidence and mortality rates of cancer in China: a population-based cluster analysis

Evaluation of 99mTc-labeled PSMA-SPECT/CT imaging in prostate cancer patients who have undergone biochemical relapse

Using conventional imaging modalities, it is difficult to detect recurrent lesions in prostate cancer patients who have undergone biochemical relapse, especially in patients with low prostate-specific antigen （PSA） levels. We retrospectively reviewed the files of fifty patients with histopathologically confirmed prostate cancer who underwent 99mTc-labeled prostate-specific membrane antigen （PSMA） single-photon emission computed tomography （SPECT）/computed tomography （CT）, magnetic resonance imaging （MRI）, and bone scan within a 30-day period. PSMA-SPECT/CT indicated metastatic lesions in 39 patients and had a higher detection rate （78.0%） than bone scan （34.0%） or MRI （40.0%）. The diagnostic efficiency of PSMA-SPECT/CT imaging for bone and lymph node metastases （50.0% and 42.0%） was better than bone scan （34.0% and 0.0%） or MRI （24.0% and 20.0%）. PSMA-SPECT/CT provided a higher detection rate at serum PSA levels of 〈1 ng ml-1, 1-4 ng ml-1, 4-10 ng ml-1, and 〉10 ng ml-1. No correlation was found between Gleason score, PSA level, and the tracer tumor/background ratio of metastatic lesions. With the aid of PSMA-SPECT/CT imaging, the therapeutic strategy was changed for 31 patients, and this may have enhanced their clinical outcome. In conclusion, PSMA-SPECT/CT imaging could detect more metastatic lesions and achieve a higher detection rate than conventional imaging modalities at different serum PSA levels in prostate cancer patients who had undergone biochemical relapse.
