Treatment of distal femur fractures in a regional Australian hospital

AIM:To review our outcomes and compare the results of the Less Invasive Stabilization System(LISS)to other implants for distal femur fracture management at a regional Australian ***:The LISS is a novel implant for the management of distal femur *** is,however,technically demanding and treatment results have not yet been assessed outside tertiary ***-seven patients with 28 distal femur fractures who had been managed surgically at the mackay Base Hospital from January 2004 to December 2010 were retrospectively enrolled and assessed clinically and *** were union,pain,Lysholm score,knee range of motion,and complication ***:Twenty fractures were managed with the LISS and eight fractures were managed with alternative *** of the surgical techniques re-vealed that 11 fractures managed with the LISS were performed according to the recommended principles(LISS-R)and 9 were not(LISS-N).Union occurred in67.9%of fractures overall:9/11(82%)in the LISS-R group vs 5/9(56%)in the LISS-N group and 5/8(62.5%)in the alternative implant *** was no statistically significant difference between pain,Lysholm score,and complication rates between the ***,there was a trend towards the LISS-R group having superior outcomes which were clinically *** was a statistically significant greater range of median knee flexion in the LISS-R group with compared to the LISS-N group(P=0.0143)and compared with the alternative implant group(P=0.0454).CONCLUSION:The trends towards the benefits of the LISS procedure when correctly applied would suggest that not only should the LISS procedure be performed for distal femur fractures,but the correct principle of insertion is important in improving the patient’s outcome.

Is p53 gene mutation an indicatior of the biological behaviors of recurrence of hepatocellular carcinoma?

AIM: To evaluate mutant p53 gene in primary hepatocellular carcinoma and to investigate the correlation between it and the recurrence of hepatocellular ***: Mutations of p53 gene were examined using antihuman p53 monoclonal antibody and immunohistochemical staining in 79 resected hepatocellular carcinomas. The correlations among variables of p53 positivity and invasiveness, disease free interval and survival were *** addition, in those who developed recurrence, the correlation among p53 positivity, clinical features and postrecurrence survival were also ***: Of these 79 cases, 64 (81%) had p53 *** patients with mutant p53 positivityhad significantly more tumor recurrence (76.6 % vs 40.0 %, P=0.0107).However, the COX proportional hazards model showed that p53 overexpression had only weak correlations with recurrence free interval and survival time (P=0.088 and 0.081), which was probably related to the short duration of follow-up. The invasiveness variables may be predictors of HCC recurrence. On univariate analysis, more patients with mutant p53 positivity had vascular permeation [78.1vs 40.0 %, P=0.0088, O.R. (odds ratio) =5.3], grade Ⅱ-ⅣV differentiation (98.4 vs 80.0 %, P=0.0203, O.R. =15.7), no complete capsule (82.8 vs 53.3 %, P=0.0346, O.R. =4.2)and daughter nodules (60.9 vs. 33.3 %, P=0.0527, O.R.=3.1) than patients with negative p53 staining. Onmultivariate analysis, only vascular permeation and grade of differentiation remained significant (P=0.042 and 0.012).There was no statistically significant correlation betweenthe status of p53 in the primary lesion and the clinical features of recurrent hepatocellular carcinomas examined,including extrahepatic metastasis (P=0.1103) and the number of recurrent tumors (P= 1.000) except for diseaseover more than one segment in the extent of recurrent tumors (P=0.0043). AIM: We studied the effect of colchicine combined with radiation on the survival of human hepatocellular carcinoma (HCC) HA22T/VGH ***: Twenty-four hours after treatment with 0-8 ng/mL colchicine, HA22T/VGH cells were irradiated at various doses (0, 1, 2, 4, and 8 Gy). Colony assay was performed to assess the surviving cell fraction. Survival curves were fitted by using a linear-quadratic model to estimate the sensitizer enhancement ratio (SER). Flow cytometry was used for cell cycle ***: Colchicine at lower concentrations (1 and 2 ng/mL) had obvious synergy with radiation to inhibit HCC cell growth, whereas higher concentrations (4 and 8 ng/mL) had only additive effect to radiation. Pretreatment with 1 and 2 ng/mL colchicine for 24-h enhanced cell killing by radiation with SERs of 1.21 and 1.53, respectively.G2/M arrest was only observed with higher colchicine doses (8 and 16 ng/mL) after 24-h treatment; this effect was neither seen with lower doses (1, 2, and 4 ng/mL)nor with any dose after only 1 h of ***: Our results suggest that colchicine has potential as an adjunct to radiotherapy for HCC *** doses of colchicine possess radiosensitizing effects via some mechanism other than G2/M arrest. Further study is necessary to elucidate the mechanism.

Computer Tomography and Ultrasonography Image Registration Based on the Cooperation of GPU and CPU

Image registration is wildly used in the biomedical image, but there are too many textures and noises in the biomedical image to get a precise image registration. In order to get the excellent registration performance, it needs more complex image processing, and it will spend expensive computation cost. For the real time issue, this paper proposes edge gradient direction image registration applied to Computer Tomography(CT) image and Ultrasonography (US) image based on the cooperation of Graphic Processor Unit (GPU) and Central Processor Unit (CPU). GPU can significantly reduce the computation time. First, the CT image slice is extracted from the CT volume by the region growing and the interpolation algorithm. Secondly, the image pre-processing is employed to reduce the image noises and enhance the image features. There are two kinds of the image pre-processing algorithms invoked in this paper: 1) median filtering and 2) anisotropic diffusion. Last but not least, the image edge gradient information is obtained by Canny operator, and the similarity measurement based on gradient direction is employed to evaluate the similarity between the CT and the US images. The experimental results show that the proposed architecture can distinctively improve the efficiency and are more suitably applied to the real world.

Clinical features of human intestinal capillariasis in Taiwan

Human intestinal capillariasis is a rare parasitosis that was first recognized in the Philippines in the 1960 s. Parasitosis is a life threatening disease and has been reported from Thailand, Japan, South of Taiwan (Kaoh-Siung), Korea,Iran Egypt, Italy and Spain. Its clinical symptoms are characterized by chronic diarrhea, abdominal pain,borborygmus, marked weight loss, protein and electrolyte loss and cachexia. Capillariasis may be fatal if early treatment is not given. We reported 14 cases living in rural areas of Taiwan. Three cases had histories of travelling to Thailand. They might have been infected in Thailand while stayed there. Two cases had the diet of raw freshwater fish before. Three cases received emergency laparotomy due to peritonitis and two cases were found of enteritis cystica profunda. According to the route of transmission,freshwater and brackish-water fish may act as the intermediate host of the parasite. The most simple and convenient method of diagnosing capillariasis is stool examination. Two cases were diagnosed by histology. Mebendazole or albendezole 200 mg orally twice a day for 20-30 d is the treatment ef choice. All the patients were cured, and relapses were not obsetbed within 12 mo,

The Saccharum genus comprises species with large and variable chromosome numbers, leading to challenges in genomic studies and breeding improvement. Cytogenetics, including classical and molecular approaches, has played a central role in deciphering the genome structure, classification, and evolution of the genus Saccharum. The application of fluorescence in situ hybridization using oligonucleotide probes significantly improved our understanding of the complex genomes of Saccharum species. This paper reviews the application and progress of cytogenetic techniques in Saccharum. Future applications of cytogenetics are discussed, as they could benefit both genomic studies and breeding of sugarcane as well as other plants with complex genomes.

Predictors of the outcomes of acute-on-chronic hepatitis B liver failure

AIM:To identify the risk factors in predicting the outcome of acute-on-chronic hepatitis B liver failure ***:We retrospectively divided 113 patients with acute-on-chronic liver failure-hepatitis B virus(ACLF-HBV) and without concurrent hepatitis C or D virus infection and hepatocellular carcinoma into two groups according to their outcomes after anti-HBV *** demographic,clinical,and biochemical data on the day of diagnosis and after the first week of treatment were analyzed using the Mann-Whitney U test,Fisher's exact test,and a multiple logistic regression ***:The study included 113 patients(87 men and 26 women) with a mean age of 49.84 *** patients survived,and 61 patients *** failure(85.2%),sepsis(34.4%),and multiple organ failure(39.3%) were the main causes of *** analyses showed that Acute Physiology and Chronic Health Evaluation(APACHE) Ⅱ scores ≥ 12 [odds ratio(OR) = 7.160,95% CI:2.834-18.092,P < 0.001] and positive blood culture(OR = 13.520,95% CI:2.740-66.721,P = 0.001) on the day of diagnosis and model for end-stage liver disease(MELD) scores ≥ 28(OR = 8.182,95% CI:1.884-35.527,P = 0.005) after the first week of treatment were independent predictors of ***:APACHE Ⅱ scores on the day of diagnosis and MELD scores after the first week of anti-HBV therapy are feasible predictors of outcome in ACLFHBV patients.

In the last two decades,significant research has been conducted in the field of automated extraction of rock mass discontinuity characteristics from three-dimensional(3D)*** provides several methodologies for acquiring discontinuity measurements from 3D models,such as point clouds generated using laser scanning or ***,even with numerous automated and semiautomated methods presented in the literature,there is not one single method that can automatically characterize discontinuities accurately in a minimum of *** this paper,we critically review all the existing methods proposed in the literature for the extraction of discontinuity characteristics such as joint sets and orientations,persistence,joint spacing,roughness and block size using point clouds,digital elevation maps,or *** a result of this review,we identify the strengths and drawbacks of each method used for extracting those *** found that the approaches based on voxels and region growing are superior in extracting joint planes from 3D point *** tensor voting with trace growth algorithm is a robust method for measuring joint trace length from 3D *** is estimated by calculating the perpendicular distance between joint *** independent roughness indices are presented to quantify roughness from 3D surface models,but there is a need to incorporate these indices into automated *** is a lack of efficient algorithms for direct computation of block size from 3D rock mass surface models.

Etoposide sensitizes CT26 colorectal adenocarcinoma to radiation therapy in BALB/c mice

AIM: To investigate the combined effect of etoposide and radiation on CT26 colorectal adenocarcinoma implanted into BALB/c ***: We evaluated the radiosensitizing effect of etoposide on CT26 colorectal adenocarcinoma in a syngeneic animal model. BALB/c mice were subcutaneously implanted with CT26 cells and divided into four groups:control (intra-peritoneal salinex2) group, etoposide (5 mg/kgintra-peritoneallyx2) group, radiation therapy (RT 5 Gyx2fractions) group, and combination therapy with etoposide (5 mg/kg intra-peritoneally 1 h before radiation) ***: Tumor growth was significantly inhibited by RT and combination therapy. The effect of combination therapy was better than that of RT. No significant changes were noted in body weight, plasma alanine aminotransferase,or creatinine in any group. The leukocyte count significantly but transiently decreased in the RT and combination therapy groups, but not in the etoposide and control groups. There was no skin change or hair loss in the RT and combination therapy ***: Etoposide can sensitize CT26 colorectal adenocarcinoma in BALB/c mice to RT without significant toxicity.
