Approximately 15%-19%of patients with severe acute respiratory syndrome coronavirus type 2(SARS-CoV-2)infections develop gastrointestinal *** pancreatitis(AP)has been reported in 0.1%of patients with coronavirus disease 2019(COVID-19).Biliary AP was most common(78.4%)before the COVID-19 pandemic;idiopathic AP is most common in patients with COVID-19(up to 57.1%).The number of emergency department presentations decreased by 23.3%during the pandemic and many governments made national recommendations to delay nonurgent endoscopic procedures,leading to decrements of 22%in combined esophagogastroduodenoscopy(EGD)and colonoscopy and 20%in EGD after the COVID-19 *** symptoms and signs of COVID-19-related AP are fever(63%),abdominal pain(58%),respiratory symptoms(40%),nausea and vomiting(39%),and headache(4%).Approximately 5-10%of patients develop necrotizing or hemorrhagic AP,and patients who required surgical intervention had a higher mortality *** to 2019,the rates of elective surgery decreased by 41.8%in 2020;including cholecystectomy(40.1%decrease)and pancreas(111.1%decrease).Surgical volumes also decreased by 18.7%in 2020;device-assisted laparoscopic and robot-assisted procedures reduced by 45.4%and 61.9%during the COVID-19 Lockdown in 2020.

BACKGROUND Primary aortoduodenal fistula is a rare cause of gastrointestinal(GI)bleeding consisting of abnormal channels between the aorta and GI tract without previous vascular intervention that results in massive intraluminal *** SUMMARY A 67-year-old man was hospitalized for coffee ground vomiting,tarry stools,and colic abdominal *** was repeatedly admitted for active GI bleeding and hypovolemic *** and spontaneously stopped bleeders were undetectable on multiple GI endoscopy,angiography,computed tomography angiography(CTA),capsule endoscopy,and ^(99)mTc-labeled red blood cell(RBC)*** patient received supportive treatment and was discharged without signs of ***,he was re-admitted for bleeder *** CTA after a bleed revealed a small aortic aneurysm at the renal level contacting the fourth portion of the duodenum.A ^(99)mTc-labeled RBC singlephoton emission CT(SPECT)/CT scan performed during bleeding symptoms revealed active bleeding at the duodenal *** to his clinical symptoms(intermittent massive GI bleeding with hypovolemic shock,dizziness,dark red stool,and bloody vomitus)and the abdominal CTA and ^(99)mTc-labeled RBC SPECT/CT results,we suspected a small aneurysm and an aortoduodenal *** duodenal excision and duodenojejunal anastomosis were performed.A 7-mm saccular aneurysm arising from the anterior wall of the abdominal aorta near the left renal artery was *** intravascular stenting of the abdominal aorta was performed and his symptoms *** Our findings suggest that ^(99)mTc-labeled RBC SPECT/CT scanning can aid the diagnosis of a rare cause of active GI bleeding.

AIM:To evaluate the effect of hands-on training of gastroenterology fellows in gastric polypectomy using an ex vivo ***:Eight gastroenterology fellows at mackay Memorial Hospital,Taipei were evaluated in gastricpolypectomy techniques using a pig stomach with artificial polyps created by a rubber band ligation *** performance of four second year(year-2)fellows who had undergone one year of clinical training was compared with that of four f irst year(year-1)fellows both before and after a 4-h workshop using the ex vivo *** workshop allowed for hands-on train-ing in the removal of multiple artif icial polyps and the placement of hemoclips at the excision *** included observation of technical skills,procedure time,and the fellows' conf idence ***:One week after the workshop,the year-1 fellows were re-evaluated and had significantly im-proved mean performance scores(from 17.9 ± 1.8 to 22.5 ± 0.7),conf idence scale(from 4.5 ± 1.0 to 7.8 ± 0.5)and procedure time(from 615.0 ± 57.4 s to 357.5 ± 85.0 s)compared with their baseline *** 4 h of training using the ex vivo simulator,the skills of the year-1 fellows were statistically similar to those of the year-2 ***:Use of this ex vivo simulator significantly improved the endoscopic gastric polypectomy skills of gastroenterology fellows who had not had previous clinical training in gastric polypectomy.

BACKGROUND Treatment efficacy for attention-deficit/hyperactivity disorder(ADHD)is reported to be poor,possibly due to heterogeneity of ADHD *** is known about poor treatment efficacy owing to ADHD *** To use generalized structural equation modeling(GSEM)to show how the heterogeneous nature of hyperactivity/impulsivity(H/I)symptoms in ADHD,irritable oppositional defiant disorder(ODD),and the presentation of aggression in children interferes with treatment responses in *** A total of 231 children and adolescents completed ADHD inattention and H/I *** scores from the Swanson,Nolan,and Pelham,version IV scale were *** child behavior checklist(CBCL)and parent’s satisfaction questionnaire were *** relationships were analyzed by *** GSEM revealed that the chance of ADHD remission was lower in children with a combination of H/I symptoms of ADHD,ODD symptoms,and childhood aggressive *** directly mediated ADHD symptom *** chance of reaching remission based on H/I symptoms of ADHD was reduced by 13.494%[=exp(2.602)]in children with comorbid ADHD and ODD[odds ratio(OR)=2.602,95%confidence interval(CI):1.832-3.373,P=0.000]after adjusting for the effects of other *** aggression mediated ODD symptom *** chance of reaching remission based on ODD symptoms was lowered by 11.000%[=1-exp(-0.117)]in children with more severe baseline symptoms of aggression based on the CBCL score at study entry[OR=-0.117,95%CI:(-0.190)-(-0.044),P=0.002].CONCLUSION Mediation through ODD symptoms and aggression may influence treatment effects in ADHD after adjusting for the effects of baseline ADHD symptom *** attention could be directed to the early recognition of risks leading to ineffective ADHD treatment,e.g.,symptoms of ODD and the presentation of aggressive or delinquent behaviors and thought problems in children with ADHD.

Objective:To investigate the effect of M3P(containing Deer antler,Cordyceps sinensis,Rhodiola rosea,and Panax ginseng);an herbal remedy with the function of tonifying Kidney(Shen)and invigorating Spleen(Pi),replenishing qi and nourishing blood;on fatigue alleviation,endurance capacity and ***:Swimming with weight-loading of 24 male ICR mice was used to evaluate the endurance capacity,and fatiguerelated plasma biomarkers were *** were randomly assigned to control or M3P treatment groups with 6 mice for each group and were orally administered with M3P everyday for 8 weeks at doses 0,10,33 or 100 mg/*** time to exhaustion was measured in a specialized water *** and kidney functions,body weight,and hematological profile were determined to evaluate the safety and toxicity after long-term M3P ***:M3P supplementation 100 mg/kg significantly increased swimming endurance time up to approximate 2.4 folds of controls(P0.05).The liver and kidney functions muscle damage biomarker creatine,body weight,and hemograms were not altered in M3P supplementation(P>0.05).Conclusion:M3P supplementation may improve swimming endurance accompanied by increasing hepatic glycogen content and serum cortisol level without major toxicity.

AIM: To investigate acute transient hepatocellular injury in patients with cholelithiasis and cholecystitis but no evidence of choledocholithiasis. METHODS: The medical records of patients with cholelithiasis who underwent cholecystectomy between July 2003 and June 2007 were retrospectively reviewed. Imaging studies to detect common bile duct (CBD) stones were performed in 186 patients, who constituted the study population. Biochemical liver tests before and after surgery, and with the presence or absence of CBD stones were analyzed. RESULTS: In 96 patients with cholelithiasis and cholecystitis without evidence of CBD stones, 49 (51.0%) had an alanine aminotransferase level elevated to 2-3 times the upper limit of normal, and 40 (41.2%) had an elevated aspartate aminotransferase level. Similar manifestations of hepatocellular injury were, as would be expected, even more obvious in the 90 patients with CBD stones. These markers of hepatocellular injury resolved almost completely within 2 wk to 1 mo after cholecystectomy. Compared to 59 patients with histologically less severe cholecystitis in the group undergoing urgent surgery (total 74 patients), the 15 patients with a gangrenous gallbladder had a higher mean level of total bilirubin (1.14 ± 1.27 mg/dL vs 2.66 ± 1.97 mg/dL, P < 0.001) and white cell count (9480 ± 4681/μL vs 12840 ± 5273/μL, P = 0.018).CONCLUSION: Acute hepatocellular injury in cholelithiasis and cholecystitis without choledocholithiasis is mild and transient. Hyperbilirubinemia and leukocytosis may predict severe inflammatory changes in the gallbladder.

Gastric polyposis is a rare disease. Not all polyps progress to cancer. Monoallelic mutation in Fanconi anemia(FA) genes, unlike biallelic gene mutations that causes typical FA phenotype, can increase risks of cancers in a sporadic manner. Aberrations in the FA pathway were reported in all molecular subtypes of gastric cancer. We studied a patient with synchronous gastric cancer from gastric polyposis by conducting a 13-year long-term follow up. Via pathway-driven massive parallel genomic sequencing, a germline mutation at FANCA D1359Y was identified. We identified several recurrent mutations in DNA methylation(TET1, V873I), the β-catenin pathway(CTNNB1, S45F) and RHO signaling pathway(PLEKHG5, R203C) by comparing the genetic events between benign and malignant gastric polyps. Furthermore, we revealed gastric polyposis susceptible genes and genetic events promoting malignant transformation using pathway-driven targeted gene sequencing.
