A 17-year-old adolescent with non-operated double inlet left-ventricle and severe stenosed parachute mitral valve is reported. He was admitted with repetitive syncope related to intermittent atrial fibrillation. Life-threatening syncope combined with pre- and post-capillary pulmonary hypertension together with his single ventricle pathophysiology led to the decision for left atrial decompression by percutaneous static atrial septum ballooning after transseptal needle perforation. Aiming to create a restrictive atrial septum defect, unloading of the left atrium without disturbing the balanced hemodynamics was directed for a long-term palliation or as a basis for a further surgical follow-up approach.

Mesenchymal Stem Cells Attenuate Vascular Remodeling in Monocrotaline-induced Pulmonary Hypertension Rats

Intravenous and intratracheal implantation of mesenchymal stem cells (MSCs) may offer ameliorating effects on pulmonary hypertension (PH) induced by monocrotaline (MCT) in rats. The aim of this study was to examine the anti-remodeling effect of intravenous MSCs (VMSCs) and intratracheal MSCs (TMSCs) in rats with PH, and the underlying mechanisms. MSCs were isolated from rat bone marrow and cultured. PH was induced in rats by intraperitoneal injection of MCT. One week after MCT administration, the rats were divided into 3 groups in terms of different treatments: VMSCs group (intravenous injection of MSCs), TMSCs group (intratracheal injection of MSCs), PH group (no treatment given). Those receiving saline instead of MCT served as negative control (control group). Pulmonary arterial structure was pathologically observed, pulmonary arterial dynamics measured, and remodeling-associated cytokines Smad2 and Smad3 detected in the lungs, three weeks after MCT injection. The results showed that PH group versus control group had higher pulmonary arterial pressure (PAP) and wall thickness index (WTI) 21 days after MCT treatment. The expression of phosphorylated (p)-Smad2 and the ratio of p-Smad2/Smad2 were much higher in PH group than in control group. Fluorescence-labeled MSCs were extensively distributed in rats’ lungs in VMSCs and TMSCs groups 3 and 14 days after transplantation, but not found in the media of the pulmonary artery. WTI and PAP were significantly lower in both VMSCs and TMSCs groups than in PH group three weeks after MCT injection. The p-Smad2 expression and the ratio of p-Smad2/Smad2 were obviously reduced in VMSCs and TMSCs groups as compared with those in PH group. In conclusion, both intravenous and intratracheal transplantation of MSCs can attenuate PAP and pulmonary artery remodeling in MCT-induced PH rats, which may be associated with the early suppression of Smad2 phosphorylation via paracrine pathways.

Long-term outcome of percutaneous or surgical revascularization with and without prior stroke in patients with threevessel disease

Relaxin Inhibit Cardiac Fibrosis Induced by Phorbol 12-myristate 13-acetate

Relaxin is known to inhibit cardiac fibrosis. However, it is unclear whether relaxin could regulate the effects of Phorbol 12-myristate 13-acetate （PMA, PKC activator） on cardiac fibrosis. So the influence of relaxin on the cell proliferation and collagen expression induced by PMA in cultured cardiac fibroblasts was studied. It showed that PMA significantly increased cardiac fibroblasts proliferation, Type I pro-collagen protein expression, Type I pro-collagen mRNA expression, and rhRLX absolutely significantly decreased PMA induced effects on cardiac fibroblasts proliferation and Type I pro-collagen expressions, indicating that relaxin could inhibit cardiac fibrosis induced by PMA.

Catheter ablation versus medical therapy for atrial fibrillation with prior stroke history:a prospective propensity score-matched cohort study

His-Purkinje conduction system pacing combined with atrioventricular node ablation improves quality of life in older patients with persistent atrial fibrillation refractory to multiple ablation procedures

BACKGROUND Recurrence of atrial fibrillation(AF)is common in patients with persistent AF even after multiple ablation ***-Purkinje conduction system pacing(HPCSP)combined with atrioventricular node ablation(AVNA)is effective in managing patients with AF and heart *** study aimed to determine whether HPCSP combined with AVNA can improve quality of life and alleviate symptoms in older patients with symptomatic persistent AF refractory to multiple ablation procedures,as well as evaluate the feasibility and safety of this *** Older patients(≥65 years)with symptomatic persistent AF refractory to at least two ablation procedures were treated with combined HPCSP and *** success rates and complications were *** parameters,European heart Rhythm Association(EHRA)scores,and Atrial Fibrillation Effect on Quality-of-Life(AFEQT)scores obtained perioperatively were compared with those recorded at the 6-month follow-up *** Thirty-one patients were enrolled;of those,only thirty patients were eventually treated with AVNA because one patient developed a complete atrioventricular block following the withdrawal of the His bundle pacing *** success rates were 100%for HPCSP(22 cases with His bundle pacing,and 9 cases with left bundle branch pacing)and 93.3%(28/30)for AVNA,*** the 6-month follow-up examination,EHRA scores improved significantly(3.00±0.73 vs.2.44±0.63,P=0.014)and AFEQT scores increased markedly(49.6±20.6 vs.70.9±14.0,P=0.001).No severe complications *** When used in older patients with symptomatic persistent AF refractory to multiple ablation procedures,HPCSP combined with AVNA significantly alleviated symptoms and improved quality of life during short-term *** therapy was proved to be safe and effective in this patient population.

Effect of advanced age on plasma homocysteine levels and its association with ischemic stroke in non-valvular atrial fibrillation

Background Elevated homocysteine （Hcy） has been reported to be associated with cardiovascular events in atrial fibrillation （AF） pa- tients, while the age-related expression pattern of plasma Hcy in AF remains unknown. The study was aimed to investigate the effect of ad- vanced age on plasma Hcy levels and its association with ischemic stroke in non-valvular AF patients. Methods A total of 2562 consecu- tive patients with non-valvular AF and 535 controls were enrolled and divided into six age groups. Plasma Hcy levels were analyzed among different age groups, and the effect of advanced age on Hcy was investigated. Results Plasma Hcy levels did not show any difference among groups aged below 65 years, while it increased sharply in patients aged 65-74 years and aged over 75 years （15.7 ±4.6 μmol/L, 17.1 ±4.9 μmol/L, both P 〈 0.01 compared with the first four age groups）. Hcy was much higher in AF patients than in controls at the same age group （all P 〈 0.05）. The proportion of patients with hyperhomocysteinemia increased gradually with age from 32.3%, 29.2%, 31.2%, 32.4%, 45.9%, to 51.4% in six age groups. The concentration of Hcy in AF patients with ischemic stroke increased progressively with age, and was higher than those without stroke at the same age. Logistic regression analysis demonstrated that age 65-74 years [odds ratios （OR）： 1.742, 95% confidence interval （CI）： 1.223-2.482, P = 0.002] and age ≥ 75 years （OR： 2.637, 95% CI： 1.605-4.335, P 〈 0.001） were significantly independent predictors of elevated plasma Hcy levels. Conclusions Advanced age was significantly associated with elevated Hcy levels, which may provide a possible explanation for the progressive increase in ischemic stroke especially in elderly AF patients.
