Risk factors and care of early surgical site infection after primary posterior lumbar interbody fusion

Objectives:To explore the risk factors and nursing measures of early surgical site infection(SSI)after posterior lumbar interbody fusion(PLIF).Methods:A total of 468 patients who received PLIF in our hospital from January 2017 to June 2020 were enrolled into this *** to the occurrence of early SSI,the patients were divided into two groups,and the general data were analyzed by univariate *** logistic regression analysis was conducted with the dichotomous variable of whether early SSI occurred and other factors as independent variables to identify the risk factors of early SSI and put forward targeted prevention and nursing ***:Among 468 patients with PLIF,18 patients developed early SSI(3.85%).The proportion of female,age,diabetes mellitus and urinary tract infection(UTI),operation segment,operation time,post-operative drainage volume,and drainage time were significantly higher than those in the uninfected group,with statistical significance(P0.05).Logistic regression analysis showed that preoperative diabetes mellitus(OR=2.109,P=0.012)/UTI(OR=1.526,P=0.035),prolonged drainage time(OR=1.639,P=0.029)were risk factors for early ***(OR=0.736,P=0.027)and albumin level(OR=0.526,P=0.004)were protective factors in reducing early ***:Women,preoperative diabetes/UTI,hypoproteinemia,and prolonged drainage time are risk factors for early SSI after *** effective preventive measures should be taken in combination with targeted nursing intervention to reduce the risk of early SSI.

Spectral computed tomography in advanced gastric cancer: Can iodine concentration non-invasively assess angiogenesis?

AIM To investigate the correlation of iodine concentration(IC) generated by spectral computed tomography(CT) with micro-vessel density(MVD) and vascular endothelial growth factor(VEGF) expression in patients with advanced gastric carcinoma(GC).METHODS Thirty-four advanced GC patients underwent abdominal enhanced CT in the gemstone spectral imaging mode. The IC of the primary lesion in the arterial phase(AP) and venous phase(VP) were measured, and were then normalized against that in the aorta to provide the normalized IC(nI C). MVD and VEGF were detected by immunohistochemical assays, using CD34 and VEGF-A antibodies, respectively. Correlations of nI C with MVD, VEGF, and clinical-pathological features were *** Both nI Cs correlated linearly with MVD and were higher in the primary lesion site than in the normal control site, but were not correlated with VEGF expression. After stratification by clinical-pathological subtypes, nI C-AP showed a statistically significant correlation with MVD, particularly in the group with tumors at stage T4, without nodular involvement, of a mixed Lauren type, where the tumor was located at the antrum site, and occurred in female individuals. nI C-VP showed a positive correlation with MVD in the group with the tumor at stage T4 and above, had nodular involvement, was poorly differentiated, was located at the pylorus site, of a mixed and diffused Lauren subtype, and occurred in male individuals. nI C-AP and nI C-VP showed significant differences in terms of histological differentiation and Lauren *** The IC detected by spectral CT correlated with the MVD. n IC-AP and n IC-VP can reflect angiogenesis in different pathological subgroups of advanced GC.

Introduction:This study examines the seasonal and genetic characteristics of human metapneumovirus(HMPV)in Henan from 2017 to ***:Samples from patients with acute respiratory infection(ARI)testing positive for HMPV were subjected to real-time reverse transcription polymerase chain reaction The G gene was amplified and sequenced from these samples for epidemiological and phylogenetic ***:We enrolled 2,707 ARI patients from October 2017 to March 2023,finding an HMPV positivity rate of 6.17%(167/2,707).Children under five exhibited the highest infection rate at 7.78%(138/1,774).The 2018 and 2019 HMPV outbreaks predominantly occurred in spring(March to May),with peak positivity rates of 31.11%in May 2018 and 19.57%in May 2019.A notable increase occurred in November 2020,when positivity reached a historic high of 42.11%,continuing until January *** February 2021 through March 2023,no significant seasonal peaks were observed,with rates ranging from 0%to 8.70%.Out of 81 G gene sequences analyzed,46.91%(38/81)were identified as subtype A(A2c:45.67%,37/81;A2b:1.23%,1/81)and 53.09%(43/81)as subtype B(B1:9.88%,8/81;B2:43.21%,35/81).Notably,an AAABBA switch pattern was observed in HMPV *** dominant strains were A2c111nt-dup in subtype A and B2 in subtype ***:Six years of surveillance in Henan Province has detailed the seasonal and genetic dynamics of HMPV,contributing valuable insights for the control and prevention of HMPV infections in *** findings support the development of targeted HMPV vaccines and immunization strategies.

Effect of autocontrol micromotion intramedullary interlocking nail on fracture healing: an experimental study

Objective： To investigate the effect of autocontrol micromotion locking nail （AMLN） on experimental fracture healing and its mechanism. Methods： 16 goats undergoing both sides of transverse osteotomy of the femoral shafts were fixed intramedullary with AMLN and Gross-Kempf （GK） nail, respectively. The follow-up time was 7, 14, 28 and 56 days. Rocntgenographic, biomechanieal, histological, scanning electromicroscopic and biochemical analyses were done. Results. （1） The strength of anticompression, antiflexion and antitorsion in the fractural end in the AMLN-fixed group was higher than that of GK nail-fixed group; whereas, the rate of stress shelter in the fractured end decreased significantly （ P〈0.01 ）. （ 2 ） The content of the total collagen, insoluble collagen, calcium and phosphate in the AMLN-fixed group was higher than that in the GK nail-fixed group （ P〈0.05 ）. （ 3 ） Histological observation and quantitative analysis of calluses revealed that AMLN could promote the growth of bridge calluses and periosteum calluses. Hence the facture healing and remolding process achieved early, which was much better than traditional GK nail fixation. （P〈0.05）. （4） 7-14 days postoperation, the calluses of AMLN-fixed group was flourish and camellarly arranged and the collagen fibril formed constantly in the absorption lacuna of bone trabecula. 28-56 days postoperation, the collagen fibril was flourish around the absorption lacuna and was parallel to the bone＇s longitudinal axis. Active bony absorption and formation were seen, so was remolding and rebuilding. Haversian system was intact and the bony structural net was very tenacious because of the deposition of calcium salt. None of the above findings was observed in the GK nailfixed group. Conclusions： The design of AMLN accords well with the plastic fixation theory. As the geometry ametabolic system constituted by the intramedullary fixation instruments and the proximal and distal end of the fracture is very firm and stable

Therapeutic effect and mechanism of breviscapine on cisplatin-induced nephrotoxicity in mice

Objective:To observe the protective effect of breviscapineon mice with cisplatin-induced ***:Mice were given a single injection of cisplalin(8 mg/kg,up.);then,breviscapine was given to mice at 25 mg/kg and 50 mg/kg doses,respectively,once a day for seven *** tissue structure was observed after animals were *** urea nitrogen(BUN),serum creatinine(Scr),lipid peroxide(MDA) and superoxide dismutase(SOD) serum levels were detected;and MDA,glutathione peroxidase,and SOD levels in the renal cortex were ***:Compared with the blank control group(BCG),the kidney pathological damage of mice in the model control group(MCG) was more *** applying different doses of breviscapine,different degrees of renal injury improvement *** with the BCG,the serum levels of Scr and BUN in the MCG increased to(89.92±6.78) μmoL/L and(15.32±4.53) mmoL/*** differences were statistical significant(P<0.01).Compared with the MCG,the serum levels of Scr and BUN in the Bre low-dose groups and Bre high-dose groups decreased significantly(P<0.05).Compared with the BCG,the MDA levels in serum and in the renal cortex in the MCG significantly increased,while the SOD levels significantly *** the differences were statistically significant(P<0.01).In the Bre low-dose groups and Bre high-dose groups,MDA levels in serum and in the renal cortex significantly decreased,while SOD and glutathione peroxidase levels in the renal cortex significantly increased,compared with the MCG;and the differences were statistically significant(P<0.05).Conclusions:Breviscapine can reduce cisplatin induced renal toxicity in mice and it's possible through inhibition of renal tubule cell lipid peroxidation and reduces the nephrotoxicity of cisplatin.
