Role of nuclear cardiology for guiding device therapy in patients with heart failure

Heart failure is a dynamic condition with high morbidity and mortality and its prognosis should be reassessed frequently, particularly in patients for whom critical treatment decisions may depend on the results of prognostication. In patients with heart failure, nuclear cardiology techniques are useful to establish the etiol-ogy and the severity of the disease, while fewer studies have explored the potential capability of nuclear cardi-ology to guide cardiac resynchronization therapy （CRT） and to select patients for implantable cardioverter defbrillators （ICD）. Left ventricular synchrony may be assessed by radionuclide angiography or gated single-photon emission computed tomography myocardial perfusion scintigraphy. These modalities have shown promise as predictors of CRT outcome using phase analysis. Combined assessment of myocardial viability and left ventricular dyssynchrony is feasible using posi-tron emission tomography and could improve conven-tional response prediction criteria for CRT. Preliminary data also exists on integrated positron emission tomog-raphy/computed tomography approach for assessing myocardial viability, identifying the location of biven-tricular pacemaker leads, and obtaining left ventricular functional data, including contractile phase analysis. Fi-nally, cardiac imaging with autonomic radiotracers may be useful in predicting CRT response and for identifying patients at risk for sudden cardiac death, therefore po-tentially offering a way to select patients for both CRT and ICD therapy. Prospective trials where imaging is combined with image-test driven therapy are needed to better defne the role of nuclear cardiology for guiding device therapy in patients with heart failure.

carbonylation and Loss-of-Function Analyses of SBPase Reveal Its metabolic Interface Role in Oxidative Stress, carbon Assimilation, and Multiple Aspects of Growth and Development in Arabidopsis

Sedoheptulose-l,7-bisphosphatase （SBPase） is a Calvin cycle enzyme and functions in photosynthetic carbon fixation. We found that SBPase was rapidly carbonylated in response to methyl viologen （MV） treatments in detached leaves of Arabidopsis plants. In vitro activity analysis of the purified recombinant SBPase showed that SBPase was car- bonylated by hydroxyl radicals, which led to enzyme inactivation in an H202 dose-dependent manner. To determine the conformity with carbonylation-caused loss in enzymatic activity in response to stresses, we isolated a loss-of-function mutant sbp, which is deficient in SBPase-dependent carbon assimilation and starch biosynthesis, sbp mutant exhibited a severe growth retardation phenotype, especially for the developmental defects in leaves and flowers where SBPASE is highly expressed. The mutation of SBPASE caused growth retardation mainly through inhibition of cell division and ex- pansion, which can be partially rescued by exogenous application of sucrose. Our findings demonstrate that ROS-induced oxidative damage to SBPase affects growth, development, and chloroplast biogenesis in Arabidopsis through inhibiting carbon assimilation efficiency. The data presented here provide a case study that such inactivation of SBPase caused by carbonyl modification may be a kind of adaptation for plants to restrict the operation of the reductive pentose phosphate pathway under stress conditions.

Enhancing the Efficacy of Anti-tumor Immunotherapy by Targeting Tcell Metabolism

Tumor immunotherapy has become the fourth major type of cancer therapy, being used alone or in combination with surgery, radiotherapy or chemotherapy. In recent years, immunotherapies, especially immune checkpoint blockade (ICB) therapy and chimeric antigen receptor T-Cell (car-T), have achieved surprising curative effects in both preclinical studies and clinical practice. However, with the expansion of clinical cases and tumor types treated, the limitations of immunotherapy have gradually emerged. For example, the clinical positive response rate of ICB therapy is only 20%-30%, and is ineffective against or may even promote the progression and metastasis of certain types of tumors. car-T cells therapy is very effective against hematological tumors, but its application for treating solid tumors has encountered a bottleneck. Therefore, combination therapeutic strategies have emerged to overcome the drawbacks associated with the different treatments. At present, research on immunotherapy combined with radiotherapy, chemotherapy and targeted therapy is booming. Investigations about the metabolism of tumors and immune cells have become one of the hotspots in recent years. Regulating the metabolism of effector T cells in the tumor microenvironment represents an effective way to improve immunotherapy, resulting in the restoration or enhancement of the ability of effector T cells to produce an anti-tumor immune response. In this review, we discuss recent progress in this field, with an emphasis on the metabolic characteristics of tumor and immune cells, especially T cells in the tumor microenvironment. We also provide a snapshot of how T cell metabolic reprogramming can be regulated to restore or enhance the efficacy of tumor immunotherapy, as well as the challenges and solutions associated with this metabolic reprogramming.

Background: Glucocorticoids have adverse systemic effects, including obesity, hypertension, and hyperglycemic, that may predispose to cardiovascular disease. The effect of glucocorticoid use on cardiovascular disease has not been quantifi ed. Objective: To test the hypothesis that users of exogenous glucocorticoids ha ve an increased risk for cardiovascular disease. Design: A cohort study using a record linkage database. Setting: Tayside, Scotland, United Kingdom. Patients: 6 8 781 glucocorticoid users and 82 202 nonusers without previous hospitalization for cardiovascular disease who were studied between 1993 and 1996. Measurements: The average daily dose of glucocorticoid exposure during follow-up was categor ized as low(inhaled, nasal, and topical only), medium(oral, rectal, or parentera l cardiovascular outcome. Results: 4383 cardiovascular events occurred in 257 487 personyears of follow-up for a rate of 17.0(95%CI, 16.5 to 17.5) per 1000 pers on-years in the comparator group, and 5068 events occurredin 212 287 person-ye ars for a rate of 23.9(CI, 23.2 to 24.5) per 1000 person-years in the group exp osed to glucocorticoids(22.1, 27.2, and 76.5 in low, medium, and high groups, re spectively). The absolute risk difference was 6.9(CI, 6.0 to 7.7) per 1000 perso nyears(5.1, 10.1, and 59.4, respectively). After adjustment for known covariates , the relative risk for a cardiovascular event in patients receiving high-dose glucocorticoids was 2.56(CI, 2.18 to 2.99). Limitations: Because the data were o bservational, residual confounding cannot be excluded. Conclusion: Treatment wit h high-dose glucocorticoids seemed to be associated with increased risk for car diovascular disease.

Plasma membrane Ca2+pumps(PMCA)play a major role in Ca2+homeostasis and signaling by extruding cellular Ca2+with high *** isoforms are encoded by four genes which are expressed differentially in various cell types in normal and disease ***, PMCA isoform selective inhibitors would aid in delineating their role in physiology and *** are testing the hypothesis that extracellular domains of PMCA can be used as allosteric targets to obtain a novel class of PMCA-specific inhibitors termed caloxins. This review presents the concepts behind the invention of caloxins and our progress in this area.A section is also devoted to the applications of caloxins in literature. We anticipate that isoform-selective caloxins will aid in understanding PMCA physiology in health and disease. With strategies to develop therapeutics from bioactive peptides,caloxins may become clinically useful in car diovascular diseases,neurological disorders,retinopathy,cancer and contraception.
