Level of Adherence to Cytotoxic Drugs by Breast Cancer Patients’ in lagos state university teaching hospital

Background: Breast cancer is one of the most common malignant diseases in women and adjuvant combination chemotherapy has been shown to reduce mortality from this disease. Adherence to medical treatment is a multifaceted issue that can substantially alter the outcomes of therapy. Patient non-adherence to chemotherapy is the ultimate barrier to the treatment effectiveness. Objective: This study was carried out to determine the relationship between cancer chemotherapy adherence and breast cancer staging, patient’s perception of cancer care and patient’s socio-demographic characteristics. Material and method: This was a cross sectional study selection of respondents and was based on simple random sampling technique, 184 patients were interviewed and data was collected using a semi-structured questionnaire to obtain socio-demographic data, adherence data, and facility-related information. Results: There was a significant association between marital status and non-adherence (P?= 0.013). Both separated and single subjects had higher proportion of non-adherence compared with married subjects. Analysis of perception of chemotherapy care revealed a significant association between the satisfaction score and non-adherence, with non-adherent patients showing higher scores or being less satisfied. The quality of service (P?= 0.0052);rating of needs been met (P?= 0.0079);rating on whether the services helped the subject (P?= 0.0405);rating on the general satisfaction of the services provided (P?= 0.0115);and rating on whether subject would seek help again (P?= 0.0320) all had a significant association with non-adherence. Conclusion: The awareness of oncologist and patient of the problem of non-adherence and communication regarding the importance of adherence to therapy may improve health outcomes.

Placental Malaria and Pre-Eclampsia from the lagos state university teaching hospital, Ikeja, lagos Nigeria

Objective: To determine the relationship between placental malaria infection and pre-eclampsia in a holo-endemic zone. Design: Prospective case-control study. Materials and Methods: One hundred and twenty seven (127) pregnant women with a diagnosis of pre-eclampsia in labour or having caesarean section served as cases while controls were 127 normotensive parturient women. They were recruited from the maternity unit of Ifako Ijaiye and Isolo General hospitals, lagos that served as secondary care centers. At delivery, either spontaneous vaginal delivery or by caesarean section, a 2.0 cm × 2.0 cm placenta tissue was cut with scalpel and fixed in 10% formaldehyde in a specimen bottle and sent to the pathologist. Following this, 2.5 mls of Cord blood and 2.5 mls of the maternal venous blood were taken into separate EDTA bottles properly labeled at delivery, samples were sent to the haematology laboratory immediately for peripheral thick film smear for malaria parasite. Results were obtained from the laboratory and together with data from the case files, they were entered into SPSS version 16 for analysis. Independent Student t-test was used for significance for continuous variables while Chi-square was used for qualitative data. The significance was set at 0.05. Results: There were statistically significant differences between the cases and controls regarding the maternal age, number of pregnancies, p p > 0.05. There was statistically significant difference between the two groups regarding the diagnosis of placental malaria as well as past history of alcohol intake and occupation, p Conclusion: Placental malaria infection was more common in patients with preeclampsia than their matched normotensive patients in our environment. At the same time, chronic malaria was found to be an independent risk factor for preeclampsia. More concerted efforts by all stake holders should be geared towards primary prevention together with early diagnosis and treatment of malaria especially

Characterization of Depleted Hydrocarbon Reservoir AA-01 of KOKA Field in the Niger Delta Basin for Sustainable Sub-Sea Carbon Dioxide Storage

This study characterized the AA-01 depleted hydrocarbon reservoir in the KOKA field, Niger Delta, using a multidimensional approach. This investigation involved data validation analysis, evaluation of site suitability for CO_(2) storage, and compositional simulation of hydrocarbon components. The primary objective was to determine the initial components and behavior of the hydrocarbon system required to optimize the injection of CO_(2) and accompanying impurities, establishing a robust basis for subsequent sequestration efforts in the six wells in the depleted KOKA AA-01 reservoir. The process, simulated using industry software such as ECLIPSE, PVTi, SCAL, and Petrel, included a compositional fluid analysis to confirm the pressure volume temperature(PVT) hydrocarbon phases and components. This involved performing a material balance on the quality of the measured data and matching the initial reservoir pressure with the supplied data source. The compositional PVT analysis adopted the Peng–Robinson equation of state to model fluid flow in porous media and estimate the necessary number of phases and components to describe the system accurately. Results from this investigation indicate that the KOKA AA-01 reservoir is suitable for CO_(2)sequestration. This conclusion is based on the reservoir's good quality, evidenced by an average porosity of 0.21 and permeability of 1 111.0 mD, a measured lithological depth of 9 300 ft, and characteristic reservoir – seal properties correlated from well logs. The study confirmed that volumetric behavior predictions are directly linked to compositional behavior predictions, which are essential during reservoir initialization and data quality checks. Additionally, it highlighted that a safe design for CO_(2) storage relies on accurately representing multiphase behaviour across wide-ranging pressure–temperature–composition conditions.

Impact of Relative Dose Intensity (RDI) on Survival in Non-Metastatic Breast Cancer: Nigerian Experience

Background: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic breast cancer and to examine the relationship between received dose intensity (RDI) and survival in patients with breast cancer Nigeria. Methods: Our study was a retrospective analysis of patients with breast cancer recruited from 2012 and 2015. A total of 204 patients were initially entered into the study, 102 were lost to follow-up leaving 102 patients who were suitable for the survival analysis. Survival time was calculated from 106 days, the scheduled end of chemotherapy. Results: The total average RDI for patients was 74%. Over the 204 patients that were reviewed, 144 (70.6%) had some reduction of RDI. This subgroup had an average RDI of 63%. On average, 79% of the intended dose of chemotherapy was given. The time to completion of chemotherapy was 1.33 times that specified by the protocol. Dose delays an overall reduction was mainly attributed to intolerability and financial constraints. Survival by RDI showed a significant decrease in survival rate for patients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, P = 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, P = 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, P = 0.003) compared with patients who received an RDI of 100%. Although associated with poorer prognosis, there were no significant changes in the survival rate for patients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, P = 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, P = 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, P = 0.405) compared with patients who received an RDI of 100%. Conclusion: This study provides evidence that decreased RDI of <70% in non-metastatic breast cancer patients is strongly associated with decreased overall survival.

Activated Protein C Resistance in Patients with Pre-Eclampsia in lagos, Nigeria

Background: Preeclampsia is reported to complicate 2% - 8% of pregnancies globally and is an important cause of maternal and perinatal morbidity and mortality. The aetiology and pathogenesis are still poorly understood and substantial improvement has not been made in the prediction, prevention and treatment of the disease. Objective: To compare the frequency of activated protein C resistance (APC-R) in patients with pre-eclampsia to that of normotensive pregnant women and to determine the correlation between activated protein ratio (APC-ratio) and the severity of pre-eclampsia. Methodology: A cross-sectional study was carried out in 100 pre-eclamptic patients and 100 normotensive pregnant controls. The APC-ratio was determined using the modified activated partial thromboplastin time. Study participants with APC-ratio of less than 2.0 were defined as having APC-R. Data was analyzed using SPSS version 22.0. Results: Mean APC-ratio was significantly lower in pre-eclamptics (2.89 ± 1.70) compared to normotensive pregnant women (3.57 ± 1.06) (p = 0.0008) and the levels were also higher in mild (2.95 ± 1.15) compared to severe pre-eclamptics (2.62 ± 1.14). The frequency of APC-R was 26% among women with pre-eclampsia compared to 4% among normotensive controls (p = 0.000). Among 100 pre-eclamptic women 7 (21.2%) out of 33 with mild pre–eclampsia had APC-R, while 19 (28.4%) out of 67 with severe pre-eclampsia had APC-R. APC-ratio had a significant negative correlation with mean arterial blood pressure (r = −0.324;p = 0.000) and proteinuria (r = −0.379;p = 0.000) among study participants. Conclusion: The frequency of activated protein c resistance is significantly higher in pre-eclamptics compared to normotensive pregnant women and this is more pronounced in those with severe pre-eclampsia compared with those with mild disease. APC-R may therefore be used as a marker of severity in the disease.

Prevalence, Awareness and Risk Factors for Hypertension in Adults Attending a Tertiary hospital in South-East Nigeria

Hypertension, a non-communicable disease, is considered a major public health challenge because of its widespread prevalence globally coupled with its huge morbidity and mortality burden, which is largely preventable if early detection and prompt initiation of management are done. Hypertension prevalence is increasing especially in the developing world, despite this, its awareness among the general population is low. This study aimed at determining the prevalence of hypertension among adult attendees of the General Outpatient Clinic of the Federal university teaching hospital (FUTH), Owerri, with an assessment of the proportion of hypertensives who were aware of their hypertensive status, and identifying risk factors of hypertension in the study participants. A cross-sectional analytical study was conducted between October and November 2022 at the General Outpatient Clinic of the FUTH, Owerri. A total of 257 consenting and eligible adult patients made up of 135 males and 122 females, aged 18 years and above, were selected by systematic random sampling method. The overall prevalence of hypertension was 34.6%. The prevalence was higher in females than in males (37.7% vs 31.9%, P = 0.325). Among the hypertensive subjects 56.2% had awareness of their hypertensive status. Following a multiple regression analysis, hypertension was independently associated with age, family history of hypertension, occupation (retirees, traders, farmers and the unemployed), and marital status (being widowed). Hypertension is prevalent in our environment;the prevalence rate from this study is higher than in most studies in our environment, suggesting possibly, a rising burden. The results from the study underscore the need for increased and sustained advocacy for implementation of policies and programs directed at increased detection and management of hypertension in the different population groups such as annual wellness check for employees in the formal sector, largescale dietary and lifes

Post Episiotomy Morbidity among Parturient in Alex Ekwueme Federal university teaching hospital Abakaliki, Ebonyi state

Background: Episiotomy is a very common obstetric surgery, and it could be associated with serious complications. However, these complications largely are not noticed due to the shift of attention from mother to baby, after a successful delivery. Objective: To identify the morbidities associated with episiotomies and factors associated with such morbidities. Method: This was a questionnaire based cross sectional descriptive study among women who attended the Obstetric Department of Alex Ekwueme Federal university teaching hospital Abakaliki Ebonyi state, between 1st July and 31st Nov, 2023. The study population consisted of parturients who had episiotomies in their previous confinements, attending either the antenatal clinic or the postnatal clinic at Alex Ekwueme Federal university teaching hospital Abakaliki Ebonyi state. Results: Macrosomia was the most common indication of episiotomy at a rate of 31.6%. The majority of episiotomies were performed on the parturients in their first confinement at a rate of 64.5% and the majority of repairs were performed by the Registrar at a rate of 65.0%. Informed consent was obtained from 45.79% of parturients while 54.21% were not informed before the episiotomy was administered. Only 55.3% of the parturient received analgesia before episiotomy was administered. The majority of the parturients had their episiotomy repaired between 10 - 15 minutes at a rate of 40.3%. Postoperative pain at the rate of 44.5% was the most common complication and dyspareunia as a form of sexual complication was the commonest at a rate of 31.3%. Conclusion: The parturient needs to be properly counselled before administration of episiotomy and adequate analgesia should be given, as episiotomy is a surgical procedure. Proper training of health workers on both the technique of administering and repairing episiotomy is important. Restrictive use of routine episiotomy in primigravidae is advised to reduce the rate of episiotomy.

The utilization of sequence stratigraphic concepts in identifying sands and their spatial continuity in distinct gross depositional settings is key,especially in frontier settings where data paucity is a common *** the Baka field,onshore Niger Delta,detailed reservoir correlation guided by sequence stratigraphic framework analysis showed the distribution of sand and shale units constituting reservoirseal pairs(RSP)correlatable across the *** the 3rd-order packages,it is observed that the lowstand systems tract(LST)and highstand systems tract(HST)contain more RSPs and thicker 4th-and 5th-order sands than the transgressive systems tract(TST).In terms of bathymetry,it is noted that irrespective of systems tracts,the RSP Index(RI)decreases from the proximal shallow/inner shelf settings to the more distal outer shelf *** all three systems tracts,intervals interpreted as lowstand prograding complexes contain the best developed sands and highest *** development within the LSTs has been controlled by a pronounced growth fault regime accompanied by high subsidence and sedimentation *** is linked to the basinward migration of the sands during prolonged sea-level fall,creating significant accommodation space for sand *** the other hand,the TSTs known to mark periods of progressive sea-level rise and landward migration of sandy facies,show thinner sands enclosed in much thicker,laterally extensive,and better-preserved deeper marine *** seismic sections indicate intense growth faulting and channelization that influenced the syn-and postdepositional development of the sand packages across the *** initial timing of deformation of subregional faults in this area coincides with periods of abrupt falls in sea *** approach could be useful for predicting sand-prone areas in frontier fields as well as possible reservoir-seal parameters required for some aspects of petroleum system analysis and quick-look volum

Background: An abnormal vaginal discharge is a common complaint among women of reproductive age, and it can indicate serious conditions like pelvic inflammatory disease and cervical cancer. This study aimed to assess the predictors of abnormal vaginal discharge in women of reproductive age group in Imo state, Southeast Nigeria. Methods: A cross-sectional study was conducted among 368 women of reproductive age group attending the clinic at Federal university teaching hospital Owerri, in Imo state, Nigeria. Respondents were recruited using a systematic sampling technique. Data were collected using a pre-tested interviewer-administered questionnaire. Multivariable analysis was performed to determine predictors of abnormal vaginal discharge. Statistical significance was set at p Results: The mean age of the respondents was 30 ± 4.5 years. Predictors of abnormal vaginal discharge were: age 36 - 45 years (OR: 4.5;95% C.I: 1.023 - 8.967, p = 0.041), being a student (OR: 2.4: 95% C.I: 1.496 - 7.336, p = 0.003), use of oral contraceptives (OR: 3.4;95% C.I: 1.068 - 6.932, p = 0.010), use of water cistern (OR: 4.7;C.I: 1.654 - 5.210, p = 0.028) anal hygiene practices (OR: 2.7;95% C.I: 1.142 - 4.809, p Conclusion: These findings suggest that targeted sexual and reproductive health interventions should be provided to reduce the risk of abnormal vaginal discharge in women of reproductive age group.
