Intracellular Ca2+homeostasis is essential for vascular function and blood pressure *** of their unique roles in regulating intracellular Ca2+concentration and vascular function,a novel class of non-selective cation channels,called transient receptor potential(TRP)channels,have emerged at the frontier of hypertension *** on their role in vasculature function regulation,TRP channels can be divided into two functional subtypes:one that participates in vasoconstriction and one that participates in vasodilatation.A functional imbalance of these two subtypes of TRP channels may disturb intracellular calcium([Ca2+]i)homeostasis,and the consequent vascular dysfunction may contribute to the development of *** potential of these TRP channels as novel pharmacological targets for the treatment of human hypertension is of great interest.

The “2018 Chinese Guidelines for Prevention and Treatment of hypertension”[1](hereinafter referred to as the 2018 Guidelines) published in this issue was revised by the Guidelines Revision Committee which is initiated and organized by the Chinese hypertension *** on the “2010 Chinese Guidelines for the Management of hypertension” and recent evidence from domestic and international research in the field of hypertension,considering national conditions and the current situation of hypertension management in China,and reference to the latest European and American hypertension guidelines,the 2018 Guidelines make comprehensive and in-depth elaborations and recommendations of the diagnosis and assessment of hypertension,strategies and goals of prevention and treatment of hypertension and pathway of intervention,*** 2018 Guidelines not only provide Chinese medical personnel with guidance and reference for the clinical practice and population management of hypertension,but also serve as the latest textbook on updating concepts and knowledge in the field of hypertension.

AIM： To study the blood-brain barrier integrity in prehepatic portal hypertensive rats induced by partial portal vein ligation, at 14 and 40 dafer ligation when portal pressure is spontaneously normalized. METHODS： Adult male Wistar rats were divided into four groups： Group Ⅰ： Sham14d, sham operated; Group Ⅱ： PHil, portal vein stenosis, （both groups were used 14 days after surgery）; Group Ⅲ： Sham4od, Sham operated and Group Ⅳ： PH4od Portal vein stenosis （Groups Ⅱ and Ⅳ used 40 d afer surgery）. Plasma ammonia, plasma and cerebrospinal fluid protein and liver enzymes concentrations were determined. Trypan and Evans blue dyes, systemically injected, were investigated in hippocampus to study blood-brain barrier integrity. Portal pressure was periodically recorded. RESULTS： Forty days afer stricture, portal pressure was normalized, plasma ammonia was moderately high, and both dyes were absent in central nervous system parenchyma. All other parameters were reestablished. When portal pressure was normalized and ammonia level was lowered, but not normal, the altered integrity of blood-brain barrier becomes reestablished. CONCLUSION： The impairment of blood-brain barrier and subsequent normalization could be a mechanism involved in hepatic encephalopathy reversibility. Hemodynamic changes and ammonia could trigger blood-brain barrier alterations and its reestablishment.

Background Homocysteine(Hcy)is a risk factor for hypertension,although the mechanisms are poorly *** We first explored the relationship between Hcy levels and blood pressure(BP)by analyzing the clinical data of primary hypertensive patients admitted to our ***,we explored a rat model to study the effect of Hcy on blood pressure and the role of *** hyperhomocysteinemia(HHcy)rat model was induced to explore the effect of Hcy on blood pressure and the possible *** carried out tissue histology,extraction and examination of RNA and ***,we conducted cell experiments to determine a likely mechanism through renin-angiotensin-aldosterone system(RAAS)and extracellular signal-regulated kinase 1/2(ERK1/2)signaling *** In primary hypertensive inpatients with HHcy,blood pressure was significantly higher as compared with inpatient counterparts lacking *** the rat model,blood pressure of the Wistar rats was significantly increased with increases in serum Hcy levels and decreased after folate *** converting enzyme 1(ACE1)expression in the Wistar Hcy group was enhanced comparing to controls,but was decreased in the Wistar folate *** II receptor type 1(AGTR1)levels in the kidney tissue increased in the Wistar folate *** serum H2S and kidney cystathionineγ-lyase decreased with elevated levels of serum *** vitro,increased concentrations and treatment times for Hcy were associated with increased expression of collagen type 1 and *** dose and time dependent response was also observed for p-STAT3 and p-ERK1/2 *** Endogenous H2S might mediate the process of altered blood pressure in response to changes in serum Hcy levels,in a process that is partly dependent on activated RAAS and ERK1/2-STAT3 signaling pathway.

hypertension has been found to be increased a risk of stroke in atrial fibrillation （AF）. Both the European and U.S. guidelines advocate the use of the CHA2DSz-VASc （congestive heart failure, hypertension, age 〉 75 years, diabetes mellitus, stroke/transient ischemic attack, vascular disease, age 65-74 years, sex category） scheme for risk stratification. Although vitamin K antagonists is more effective than acetyl-salicylic acid at preventing ischaemic stroke, its benefit is offs by an increased haemorrhage risk. The risk of ischemic stroke in patients with AF and a CHA2DS2-VASc score of 1 are considered to be low risk and may be not expected to benefit from anticoagulation therapy. Hyper-tension carries an increased risk of ischemic stroke, however, it is also a clear risk factor for hemorrhage in AF. Therefore, the optimal anti-thrombotic management is highlighted in patients with AF with only one risk factor especially hypertension.

The sodium and calcium cation transport in erythrocyte and their influencing factors were studied in essential hypertensive （EH） patients. The result showed that plasma sodium pump inhibitor,endogenous digitalis-like compound （EDLC）, rose in some patients and sodium pump was depressed in the others, but there were no parallel links between EDLC and sodium pump. The patients with normal sodium pump have mainly shown a decrease of their maximal CApump activity and calmodulin （CaM） content in erythrocyte. It indicated that there might be different types of ion transporting defect in EH, and the abnormalities of these cation transports might be an important pathogenesis in EH.
