Hearing Loss among Grain Millers in the Informal Sector in Benin

Introduction: Noise-induced hearing loss is a preventable health problem worldwide. However, it continues to affect workers especially in the informal sector, due to the lack of medical and environmental monitoring. In Benin, millers are highly exposed. The objective of the study was to assess the hearing health situation of grain millers in the Dantokpa market in 2020. Methods: This was a cross-sectional study that included by exhaustive recruitment 57 millers. The data were collected using a standardized questionnaire followed by blood pressure measurements, noise levels and the performance of audiometries. Descriptive and univariate analysis was performed. Results: The sample consisted only of men. The median age was 25 years with extremes of 18 and 50 years. Noise levels at the workstation ≥ 85 dB (A) were obtained for 94.74% of the workers. No worker was wearing hearing protection equipment. The prevalence of hearing loss was 87.72% (95% CI = [76.32%;94.92%]) and that of occupational deafness was 29.82% (95% CI = [18.43% - 43.40%]) which was associated with age over 28 and high blood pressure. An awareness campaign with donation of hearing pads was carried out. Conclusion: Hearing loss is important among millers who do not benefit from any supervision because they are in the informal sector. A restructuring of this sector is necessary for the hearing health of workers.

Introduction: In the general population as well as in the workplace, hypertension is one of the main causes of morbidity and mortality in the world. The particularities of hypertension in the world of work remain little studied. The objective of this study was to determine the prevalence and factors associated with hypertension among workers in a town hall in Benin in 2019. Methods: This was a descriptive, analytical cross-sectional study conducted from December 2018 to January 2019. A comprehensive recruitment was conducted. Data were collected during a face-to-face interview, supplemented by measurement of anthropometric parameters and blood pressure as well as a work stress assessment. Frequencies were compared using the chi-square test and Fisher’s test. The significance level was 5%. Results: A total of 86 workers were included. Their mean age was 41.55 ± 9.82 years with a sex ratio of 6.1. The prevalence of hypertension was estimated to be 18.6% [95% CI: 11.96% - 29.75%]. Hypertension was higher in workers under work stress (41.18%) as opposed to non-stressed workers (14.49%) (p = 0.032);in those who were overweight (30.77) as opposed to non-overweight (2.94%) (p Conclusion: Work stress and obesity are modifiable factors associated with hypertension in workers at the Bohicon city hall. These factors deserve to be corrected by occupational health promotion and prevention actions.

Factors Associated with Hypertensive Disorders of Pregnancy in Public Maternities of the City of Parakou In 2019

The purpose of this study was to evaluate the prevalence of Hypertensive Disorders in Pregnancy (HDP) and to determine the factors associated with them in the city of Parakou in Benin, a West African country. Method: It has been a descriptive cross-sectional study with a prospective collection of data using a questionnaire in prenatal consultation in the health center of Parakou’s commune for 4 months (May 1 to August 31, 2019). Result: A proportion of 8.2% (55/671) of the pregnancy had the hypertensive disorders of pregnancy. The Gestational HyperTension was the most common type of arterial hypertension (AHT) with a proportion of 49.1%. The factors associated with HDP were the age of 30 to 34 (OR: 11.9;95% CI: 1.54 - 92.29;p = 0.0346), the family history of Arterial HyperTension (AHT) (OR: 1.5;95% CI: 1.03 - 4.66;p = 0.0481), the BMI > 30 (OR:14.2;95% IC: 7.02 - 28.69;p = 0.0235), being married or in a common-law relationship (OR = 2.51;95% CI: 1.30 - 4.86;p = 0.0412), the stress (OR: 2.0;95% CI:1.09 - 3.32;p = 0.0209), the history of HDP (OR: 9.0;95% CI: 2.53 - 15.23;p are common in Parakou. Monitoring Adverse Events during Seasonal Malaria Chemoprevention Campaigns for Children Aged 3 - 59 Months in Benin in 2023

Background: Seasonal malaria chemoprevention (SMC) is crucial for reducing the burden of malaria in children. However, adverse events (AEs) can affect treatment adherence and efficacy. This study assesses the prevalence of AEs associated with SMC and identifies factors associated with treatment discontinuation. Methods: A cross-sectional study of 3115 children receiving SMC was conducted. The study was conducted in two departments in northern Benin. The prevalence of AEs and factors associated with discontinuation of treatment were analyzed. Results: Among the children, 578 (18.6%, 95% CI: 17.2 - 19.9) reported AEs, mainly vomiting (63.5%), fever (43.8%) and diarrhea (23.0%). Boys (51.9%) had slightly more AEs than girls (48.1%). The prevalence of AEs was higher in rural areas (64.0%) than in urban areas (36.0%). Only 2.8% of children stopped taking SMC because of AEs. There was no significant association between AEs and discontinuation of SMC (p = 0.608). Children referred to health centers for management of AEs were less likely to discontinue treatment (p = 0.015). Conclusion: AEs were common, but the rate of discontinuation of SMC due to AEs was low, indicating good treatment tolerance. Effective management of AEs in health centers reduces treatment interruptions.

Introduction: Seasonal malaria chemoprevention (SMC) was adopted in 2019 in two health zones in Benin where malaria transmission is very high. Positive results led to the extension of the intervention to other zones with additional financial support. Annual SMC campaigns from 2021 to 2023 were carried out in all six health zones in the Atacora and Alibori departments. In five years of implementation, various approaches have been developed on the basis of a communication plan facilitating buy-in and acceptance by all stakeholders. The aim of this study was to assess the effective coverage and acceptance of the SMC by their beneficiary populations in 2023. Methods: It was a cross-sectional study with an analytical focus. Data collection took place from November 30 to December 13, 2023. The study population consisted of children under 5 years of age residing in the departments of Atacora and Alibori in northern Benin. A total of 3573 children under 5 years of age were included in the study, and their parents or guardians were interviewed. Results: During the 2023 campaign, 87.7% of targets were reached by SMC administration and 100.00% of children had received at least one dose of SMC by the fourth visit. Effective therapeutic coverage of SMC was 70.55%, with 99.60% in BNK, 69.40% in KGS, 16.20% in MK, 56.10% in 2KP, 92.40% in NBT and 89.60% in TMC. This coverage was statistically related to child and respondent ages (p Conclusion: SMC is a strategy accepted by the population, and the main reasons for non-participation in SMC were dominated by the absence of mothers or babysitters when the agents visited.

Determinants of High Blood Pressure and Quality of Management in Three Regions of Benin

Objective: The aim of this work was to determine the prevalence, associated factors and quality of high blood pressure (HBP) management in three regions of Benin in 2015. Methodology: This was a cross-sectional study, with two components. The first component included adults aged from 18 to 69 years, selected using a three-stage random sampling within the households. Data were collected thanks to the French version of the WHO STEPS instrument. Anthropometric data, including blood pressure, capillary fasting glucose and total cholesterol were measured according to standard procedures. The second component included Public Health Centers (PHC) selected by a random stratified multi-stage sampling. Data were collected on the structures and the processes of HBP management using the standardized tool for assessing the capacities of management of non-communicable diseases in peripheral health centers provided by the World Health Organization. Results: A total of 4816 participants were included in the first component. The mean age was 35.8 ± 12.7 years. The weighted prevalence of HBP was 27.9% (95% Confidence Interval (CI) [25.6 - 30.2]). It was higher in the 60 - 69 years compared to the lower age groups (Adjusted Odd-ratio (ORa) = 5;95% CI [3.9 - 6.5)]). HBP was positively associated with urban residence (ORa = 1.26;95% CI [1.24 - 1.28]), obesity (ORa = 1.46;95% CI [1.43 - 1.50]), hyperglycemia (ORa = 1.13;95% CI [1.10 - 1.15)]) and hypercholesterolemia (ORa = 1.64;95% CI [1.59 - 1.70)]). A total of 27 PHC were included in the second component. Taking blood pressure and other anthropometric measurements was not routine in PHC. Several essential medicines were not available in the PHC. A low level of community involvement in the management of HBP was noted. Conclusion: This study confirms the high prevalence of HBP and shows inadequacies in its management in the targeted PHC. More appropriate prevention and control measures for HBP should be implemented.

Prevalence of Hypertension during Pregnancy in Benin: Results from STEPS Survey in 2015

Introduction: Hypertension disorders of pregnancy are one of the leading causes of maternal and fetal deaths. We aimed to estimate the prevalence of hypertension among pregnant women having participated in the STEPS survey in Benin in 2015. Methods: This was a database analysis. STEPS survey included consenting adults aged 18 to 69 years, living in Benin for at least 6 months prior to the survey. The participants were selected in households by a three-stage random sampling technique. The World Health Organization (WHO) STEPS instrument was used for data collection. After an individual interview with the participant at home, blood pressure was measured according to WHO recommendations. Pregnant women database was extracted for analysis. A multivariable logistic regression was done for associated factors identification. Results: A total of 2800 women (out of 5127 people) participated in the STEPS study of which 240 declared to be pregnant. The mean age was 28.9 ± 6.8 years. The prevalence of hypertension was 13.3%. Severe hypertension represented 2.9%. Hypertension during pregnancy was associated with primary school education level (compared to none, adjusted OR = 0.3, 95% CI [0.1 - 0.9], p = 0.042). No significant association was found with behavioral factors. Conclusion: The prevalence of hypertension during pregnancy is high in Benin. In-depth studies and targeted interventions for prevention should be implemented.

Background: Data on diabetes mellitus in general population in Benin is scarce. This study aimed therefore to assess prevalence and risk factors of diabetes mellitus in Benin. Methods: The study consisted of a cross-sectional survey, using the World Health Organisation (WHO)’s instrument for stepwise surveillance (STEPS) of non-communicable diseases risk factors. A five-stage random sample of 25 to 64 years old male and female adults living in Benin participated in structured interviews and their size, weight and blood pressure were measured according to standardized procedures. Glycaemia was measured using ACCUTREND? test strips for capillary blood glucose. Prevalence and means were computed with their 95% confidence interval and standard error respectively, taking into account the sampling design. Prevalence was compared by Khi2 and means by Student’s t test. Univariate and multivariate logistic regressions were performed to identify socio-demographic diabetes’s risk factors. Results: A total of 3772 adults participated in the study. The average age of respondents was 44.2 years. Overall, 68.3% of subjects were illiterate, 82.4% lived in rural areas, 82.57% were self-employed, 20% overweight, 7.51% obese and the prevalence of hypertension was 29.5 p.100. The prevalence of diabetes mellitus was 1.4 p.100 [CI95% = (0.94:2.05)]. This prevalence was higher in men than in women (2.1% vs. 0.8%, p = 0.0031). This prevalence was higher in urban areas than in rural areas. The Littoral department had the highest prevalence (3.4%), followed by the Collines department (2.7%), Donga (2.4%) and Borgou (2.3%). As many as 49.3% of subjects diagnosed with diabetes mellitus were not aware of their status. The age group of 56 - 64 years had the highest risk [OR = 6.6, IC95% = (1.6;22.4)]. Hepatitis B virus(HBV) infection has shown an intermediate or high endemicity level in low-income countries over the last five decades. In recent years, however, the incidence of acute hepatitis B and the prevalence of hepatitis B surface antigen chronic carriers have decreased in several countries because of the HBV universal vaccination programs started in the nineties. Some countries, however, are still unable to implement these programs, particularly in their hyperendemic rural areas. The diffusion of HBV infection is still wide in several low-income countries where the prevention, management and treatment of HBV infection are a heavy burden for the governments and healthcare authorities. Of note, the information on the HBV epidemiology is scanty in numerous eastern European and Latin-American countries. The studies on molecular epidemiology performed in some countries provide an important contribution for a more comprehensive knowledge of HBV epidemiology, and phylogenetic studies provide information on the impact of recent and older migratory flows.
