Transcatheter Closure of Postoperative Residual Atrial or Ventricular Septal Shunts in Patients with congenital heart disease

Background:Transcatheter closure(TCC)has emerged as the preferred treatment for selected congenital heart disease(CHD).While TCC offers benefits for patients with postoperative residual shunts,understanding its mid-and long-term efficacy and safety remains ***:This study aims to assess the mid-and long-term safety and efficacy of TCC for patients with residual atrial or ventricular septal shunts following CHD ***:In this consecutive retrospective study,we enrolled 35 patients with residual shunt who underwent TCC or surgical repair of CHD between June 2011 to October *** candidacy was determined based on established *** and electrocardiogram were conducted during the perioperative period and continued as part of long-term ***:Among the patients,5(14.3%)exhibited interatrial shunt-ing,while 30(85.7%)had interventricular *** was successfully implemented in 33 of 35 patients,with exceptions in two cases of post-ventricular septal defect repair due to anatomical challenges involving the shape and aortic *** resulted in a TCC success rate of 94.3%.Trace residual shunt was detected in two interventricular shunting cases and a mild residual shunt in one interventricular shunting case;all resolved by the three-month follow-up after *** complications included one hematoma at the puncture site and one transient junctional rhythm during the perioperative *** a median follow-up of 73 months,there were no instances of residual shunt,device embolization,occluder displacement,valve insufficiency,malignant arrhythmia,infective endocarditis,death,or other serious ***:TCC is an effective and safe therapy for patients with residual atrial or ventricular septal shunts following CHD *** support the consideration of TCC as the preferred treatment option for appropriate patient populations.

Selexipag as Add-on Therapy for Patients with Pulmonary Arterial Hypertension Associated with congenital heart disease:A Single-Center Retrospective Study

Purpose:This study examined the efficacy and safety of selexipag in treating pulmonary arterial hypertension(PAH)associated with congenital heart disease(CHD).Materials and Methods:We conducted a retrospective study of patients with CHD-associated PAH,treated with selexipag since December *** adult patients(mean age,45.4 years;women,77%)were treated with selexipag as add-on *** characteristics,World Health Organization functional class,6-minute walking distance(6MWD)test results,N-terminal pro-B-type natriuretic peptide levels,echocardiographic data,and incidence of side effects were ***:The majority of patients(12/13,92.3%)experienced more than one treatment-associated complication;one patient dropped out of the study due to intolerable *** results of 6MWD test(from 299.2±56.2 m to 363.8±86.5 m,p=0.039)and tricuspid regurgitation(TR)pressure gradient(from 84.7±20.5 mmHg to 61.6±24.0 mmHg,p=0.018)improved and remained improved after selexipag treatment in 12 *** on the results of a non-invasive risk assessment,8(66.7%)patients showed improvement,3(25.0%)showed no interval change,and the status of one patient(8.3%)***,compared to patients treated with a low dosage,patients treated with a medium-to-high dosage showed a greater increase in 6MWD results(88.3±26.4 m vs.55.3±27.6 m,p=0.043)and a greater reduction in the TR pressure gradient(-33.7±10.9 mmHg vs.-12.5±12.0 mmHg,p=0.015).Conclusion:Selexipag is an efficient pulmonary vasodilator as add-on therapy in treating CHD-associated PAH.

congenital heart disease （CHD） is the most fi＇equent birth defect （0.8% 1% of all live births）. Due to the advance in prenatal and postnatal early diagnosis and treatment, more than 90% of these patients survive into adulthood today. However, several mid- and long-term morbidi- ties are dominating the follow-up of these patients. Due to the rarity and heterogeneity of the phenotypes of CHD, multicenter registry-based studies are required. The CHD-Biobank was established in 2009 with the aim to collect DNA from patients and their parents （trios） or fi＇om affected families, as well as cardiovascular tissues from patients undergoing corrective heart surgery for cardiovascular malformations. Clinical/phenotype data are matched to the International Paediatric and congenital Cardiac Code （IPCCC） and the International Statistical Classification of diseases and Related Health Problems 10th Revision （ICD-10）. The DNA collection currently comprises samples from approximately 4200 participants with a wide range of CHD phenotypes. The collection covers about 430 trios and 120 families with more than one affected member. The cardiac tissue collection comprises 1143 tissue samples [Yore 556 patients alter open heart surgery. The CHD-Biobank provides a comprehensive basis for research in the field of CHD with high standards of data privacy, IT management, and sample logistics.

Neonatal surgical outcomes after prenatal diagnosis of complex congenital heart disease: experiences of a perinatal integrated diagnosis and treatment program

Background This study aimed to evaluate neonatal surgical outcomes of patients diagnosed with complex congenital heart disease(CHD)during pregnancy and treated by the newly initiated"perinatal integrated diagnosis and treatment program(PIDTP)".Methods We reviewed clinical data of 207 neonates(surgical age≤28 days)who underwent cardiac surgeries in a single center from January 2017 to December 2018,including 31 patients with referrals from the"PIDTP"(integration group)and 176 patients with routine referral treatment(non-integrated group).Results In the integration group,median admission age was 0 days and median age at surgery was 4 *** the non-integrated group,median admission age was 8 days(P=0.001)and median age at surgery was 13 days(P=0.001).The emergency surgery rate in patients with duct-dependent defects was 36%in the integration group and 59%(P=0.042)in the non-integrated group,*** in-hospital mortality was 16%in the integration group and 14%(P=0.78)in the non-integrated *** 2-year cumulative survival rate after surgery was 83.9%±6.6%in the integration group and 80.3%±3.1%(P=0.744)in the non-integrated *** to multivariable regression analysis,independent risk factors for early mortality of overall neonatal cardiac surgery were low body weight,high serum lactate level,postoperative extracorporeal membrane oxygenation(ECMO)support and prolonged cardiopulmonary bypass(CPB)*** PIDTP shortens the postnatal transit interval,reduces the emergency operation rate of neonatal critical CHD,and provides better preoperative status for *** treated by the PIDTP tend to have more complicated anatomical deformity and a greater requirement for the operation and postoperative management,but early outcome and follow-up prognosis are satisfactory.

Cardiopulmonary Response to Exercise at High Altitude in Adolescents with congenital heart disease

Objective:To extend our knowledge on tolerance of acute high-altitude exposure and hemodynamic response to exercise in adolescents with congenital heart disease(AscCHD)without meaningful clinical or functional ***:A symptom limited cardiopulmonary exercise stress test and a non-invasive cardiac output measurement during steady state exercise were performed at 540 m and at 3454 m *** of acute mountain sickness were ***:We recruited 21 healthy controls and 16 AscCHD(59%male,mean age 14.7±1.1 years).Three subjects(2 controls,1 AscCHD)presented light symptoms of acute mountain sickness(dizziness and headache).During the symptom limited exercise test at lowland,control subjects showed a significantly higher power to weight index(3.5±0.6 W/kg vs.3.0±0.7 W/kg,pheart rate(188.8±10.41/min vs.179.4±13.11/min,p<0.050)and ventilation(92.8±22.9 l/min vs.75.4±18.6 l/min,<0.050).At altitude,power to weight index only remained significantly higher in the control group(2.8±0.6 W/kg vs.2.6±0.6 W/kg,p<0.001).Pulmonary blood flow(PBF)at lowland showed no difference between the control and the AscCHD group,neither at rest(5.4±0.8 l/min vs.5.1±0.9 l/min,p=0.308),nor during the steady state test(10.6±2.4 l/min vs.10.5±2.0 l/min,p=0.825).At high altitude,PBF increased by 110%and 112%,respectively(12.8±2.32 l/min vs.12.5±3.0 l/min;intergroup difference:p=0.986).Conclusions:High altitude exposure was well tolerated in an unselected group of *** significant difference in the cardio-pulmonary adaptation to a control group was noted during a steady state *** of minor acute mountain sickness did occur,which should however not be misinterpreted as signs of hemodynamic maladaptation.
