Rotational stability of plate haptic toric intraocular lenses after combined 25-gauge vitrectomy and cataract surgery

AIM:To evaluate the postoperative intraocular lens(IOL)rotational stability and residual refractive astigmatism following combined 25-gauge vitrectomy and cataract surgery with implantation of a plate haptic toric IOL.METHODS:In this retrospective case series,32 eyes of 32 patients underwent a combined 25-gauge vitrectomy and phacoemulsification for vitreoretinal diseases and cataract with regular corneal astigmatism of at least 1 diopter(D).A plate haptic toric IOL(AT Torbi 709M,Carl Zeiss Meditec AG)was implanted in all eyes.The outcome measures were rotational stability and refractive astigmatism up to 6mo postoperatively as well as the best corrected visual acuity(BCVA).RESULTS:Preoperative refractive astigmatism was 2.14±1.17 D,which was significantly reduced to 0.77±0.37 D six to eight weeks postoperatively and remained stable throughout the observation period(0.67±0.44 D at three months and 0.75±0.25 D at six months;for all groups:P<0.0001 compared to baseline).BCVA improved significantly from 0.36±0.33 logMAR preoperatively to 0.10±0.15 logMAR following surgery(P=0.02).Mean IOL axis deviation from the target axis was 3.4°±2.9°after six to eight weeks and significantly decreased over time(2.4°±2.6°six months after surgery;P=0.04).In one patient IOL,re-alignment was performed.CONCLUSION:Corneal astigmatism is significantly reduced following combined 25-gauge vitrectomy and cataract surgery.The plate haptic toric IOL position and axis remain stable during the observation period of six months.

Dear Editor:Brain metastasis(BM)has long been recognized as a prognostic factor associated with poor prognosis for non-small cell lung cancer(NSCLC)in the era of con-ventional chemotherapy and targeted therapy[1].In the era of immunotherapy,controversial findings have been reported regarding the prognostic significance of BM in patients with NSCLC treated with programmed death-1/programmed death-ligand 1(PD-1/PD-L1)inhibitors.

Drug-induced liver injury(DILI)is a rare but potentially life-threatening condition,which accounts for the majority of acute liver failure cases in the US and EU.Unlike direct hepatoxicity,which is mainly caused by acetaminophen,idiosyncratic DILI is unpredictable and occurs unrelated to the dose or frequency of the medication.Interestingly,DILI cannot only be caused by a large variety of prescription drugs,but also by herbal and dietary supplements(HDS).While HDS-DILI has already played a role in Asian countries like China or Korea for a long time,there is also an increasing incidence of HDS-DILI in countries with formerly less HDS-DILI cases such as the US,presumably due to a rising usage of those remedies(1).

Photosynthesis is central to life on Earth,employing sunlight,water,and carbon dioxide to produce chemical energy and oxygen.It is generally accepted that boosting its efficiency offers one promising way to increase crop yields under agronomically realistic conditions.Since the components,structure,and regulatory mechanisms of the light reactions of photosynthesis are well understood,concepts for enhancing the process have been suggested and partially tested.These approaches vary in complexity,from targeting single components to comprehensive redesign of the whole process on the scales from single cells or tissues to whole canopies.Attempts to enhance light utilization per leaf,by decreasing pigmentation,increasing levels of photosynthetic proteins,prolonging the lifespan of the photosynthetic machinery,or massive reconfiguration of the photosynthetic machinery and the incorporation of nanomaterials,are discussed in this review first.Secondly,strategies intended to optimize the acclimation of photosynthesis to changes in the environment are presented,including redesigning mechanisms to dissipate excess excitation energy(e.g.,non-photochemical quenching)or reduction power(e.g.,flavodiiron proteins).Moreover,schemes for improving acclimation,inspired by natural or laboratory-induced adaptation,are introduced.However,all these endeavors are still in an early exploratory phase and/or have not resulted in the desired outcome,largely because photosynthesis is embedded within large networks of closely interacting cellular and metabolic processes,which can vary among species and even cultivars.This explains why integrated,systems-wide approaches are required to achieve the breakthroughs required for effectively increasing crop yields.

COG1 CANENHANCE PHOTOSYNTHESIS AND PLANT GROWTH Recently,Wei et al.(2023)report that overexpression or a gain-offunction mutation of COGWHEEL1(COG1)gene,which encodes a transcription factor belonging to the DNA binding with one finger(Dof)protein family,can increase growth and biomass in Arabidopsis thaliana.Similarly,overexpression of any of its six paralogs,CYCLING DOF FACTORS 1-6,also leads to increased growth and biomass production,implying that these seven Dof proteins have partially redundant functions.Accordingly,several of the seven Dof genes must be knocked out to significantly reduce growth and biomass production;the septuple mutants show severely reduced growth but are still viable.COG1 positively modulates the accumulation of transcripts of several photosynthetic genes and,in turn,the corresponding proteins and binds to the promoters of most LHC genes.Changes in gene expression related to photosynthesis increase the efficiency of photosynthesis.Overexpression of two closely related Brassica rapa COG1 homologs in A.thaliana or Arabidopsis COG1 in Nicotiana benthamiana,could cause an increase in growth and biomass production.These results indicate that the effects of COG1 are not confined to a specific species.

Pharmacological drug screening to inhibit uveal melanoma metastatic cells either via EGF-R, MAPK, mTOR or PI3K

AIM: To screen five potential pharmacological substances specifically targeting EGF-R, MAPK, mTOR, or PI3K for their antiproliferative effects, possible impact on cell viability, as well as cell death rates on three different uveal melanoma metastasis cell lines in vitro. METHODS: Three different uveal melanoma metastasis cell lines(OMM2.5, OMM2.3, and OMM1), that originated from human hepatic and subcutaneous metastasis, were exposed to inhibitors of different targets: erlotinib(EGF-R), everolimus(mTOR), selumetinib(MAPK), trametinib(MAPK) or the alkylphosphocholine erufosine(PI3K). Cell viability was assessed with a 2,3-bis-(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carboxanilide(XTT) dye reduction assay after 24 h of treatment. Antiproliferative effects were evaluated separately after a 72-hour incubation of the cells with the pharmacological substance. Subsequently, the IC_(50) was calculated. Tumor cell death was investigated using a double stain apoptosis detection assay. RESULTS: Selumetinib, trametinib, and erufosine significantly decreased cell viability of all OMM cell lines(P<0.04). In addition, selumetinib and trametinib showed a significant inhibition of cell proliferation(P<0.05). Everolimus and erlotinib solely inhibited cell proliferation at the used concentrations(P<0.05). Besides an increase of necrotic cells after erufosine treatment(P<0.001), no changes in the number of dead cells for the other substances were observed.CONCLUSION: The preliminary drug screening demonstrates five new candidates, successfully targeting the canonical MAPK/ERK and PI3K/AKT/m TOR pathways in uveal melanoma metastasis cells in vitro. Hence, these findings provide an experimental basis to explore future single or combined therapy strategies for metastatic uveal melanoma.

Recent results emphasize the supportive effects of adipose-derived multipotent stem/progenitor cells(ADSPCs)in peripheral nerve recovery.Cultivation under hypoxia is considered to enhance the release of the regenerative potential of ADSPCs.This study aimed to examine whether peripheral nerve regeneration in a rat model of autologous sciatic nerve graft benefits from an additional custom-made fibrin conduit seeded with hypoxic pre-conditioned(2%oxygen for 72 hours)autologous ADSPCs(n=9).This treatment mode was compared with three others:fibrin conduit seeded with ADSPCs cultivated under normoxic conditions(n=9);non-cell-carrying conduit(n=9);and nerve autograft only(n=9).A 16-week follow-up included functional testing(sciatic functional index and static sciatic index)as well as postmortem muscle mass analyses and morphometric nerve evaluations(histology,g-ratio,axon density,and diameter).At 8 weeks,the hypoxic pre-conditioned group achieved significantly higher sciatic functional index/static sciatic index scores than the other three groups,indicating faster functional regeneration.Furthermore,histologic evaluation showed significantly increased axon outgrowth/branching,axon density,remyelination,and a reduced relative connective tissue area.Hypoxic pre-conditioned ADSPCs seeded in fibrin conduits are a promising adjunct to current nerve autografts.Further studies are needed to understand the underlying cellular mechanism and to investigate a potential application in clinical practice.

A recent research article published in Science by Ritter et al.reported that endosomal sorting complexes required for transport(ESCRT)proteins mediate repair of lesions in the cell membrane caused by the pore-forming toxin perforin at sites of cytotoxic T cell engagement.1 Subsequent entry of granzymes and initiation of apoptosis of cancer cells is thus limited,leading to reduced sensitivity against T cell‑mediated cytotoxicity.1 Along these lines,this could constitute a new critical resistance mechanism for T cellbased treatment approaches.

Dear Editor,Hatching is a crucial moment in the life of an animal.The animal stops being an embryo protected by the egg and becomes a post-embryonic immature(see Haug,2020 for difficulties in naming these stages).This must have been the case in the past as well,but the fossil record is rather sparse in the preservation of this very moment.The fossil preservation of such a case needs to facilitate not only preservation of the animal,but also of the egg.As these are often of rather different material,making the preservation of both together in many cases unlikely.Yet,there are preservation types that seem to favor such incidents.Amber has become famous for preserving cases of“frozen behaviour",preserving a specific moment in the life of an animal in an almost life-like manner(Arillo,2007),including:copulations,parasite-host interactions,aggregations,group defense,brood care,egg-laying(although likely stress-induced).
