Greenlight Photoselective Vaporisation of the Prostate in 133 High Surgical Risk Patients: A 5-Year Outcome Study

Greenlight photoselective vaporisation of the prostate (GPVP) is progressively becoming an established treatment in patients with LUTS because it is a minimally invasive technique that achieves efficient haemostasis, making it the ideal technique for patients at high surgical risk. Material and Methods: To study of 133 patients with an ASA surgical risk score of 3 or 4, undergoing GPVP, with an analysis of perioperative outcome, IPSS, Qmax, IIEF-5 and complications during a five-year follow-up. Results: At 5 years the mean annual improvement in IPSS was stable, and at 5 years there was a 15.2 point improvement versus the preoperative score (p 0.05). The Qmax showed an improvement of 14.9 ml/sec and was maintained at five years after surgery (p 0.05). No patients were transfused or suffered urinary incontinence. 2.25% suffered major complications and there were no deaths. 3.1% of patients suffered de novo urgency. In the 5-year follow-up, five patients had to be reoperated. The quality of sexual health assessed by IIEF-5 before the procedure was scored at 14 points;the 5-year follow-up covering the preoperative period and all revisions did not show any worsening in the IIEF-5 score (p > 0.05). Conclusions: Due to its physical characteristics, in our opinion GPVP is now the treatment of choice in patients at high surgical risk. In our series, the risk of major/minor complications and transfusions was much lower than the same risks in conventional techniques. The objective results (Qmax and quality of life questionnaire) are equivalent to conventional techniques and persist over a 5-year follow-up.

Integral Real-time Locomotion Mode Recognition Based on GA-CNN for Lower Limb Exoskeleton

The wearable lower limb exoskeleton is a typical human-in-loop human–robot coupled system,which conducts natural and close cooperation with the human by recognizing human locomotion timely.Requiring subject-specific training is the main challenge of the existing approaches,and most methods have the problem of insufficient recognition.This paper proposes an integral subject-adaptive real-time Locomotion Mode Recognition(lmr)method based on GA-CNN for a lower limb exoskeleton system.The lmr method is a combination of Convolutional Neural Networks(CNN)and Genetic Algorithm(GA)-based multi-sensor information selection.To improve network performance,the hyper-parameters are optimized by Bayesian optimization.An exoskeleton prototype system with multi-type sensors and novel sensing-shoes is used to verify the proposed method.Twelve locomotion modes,which composed an integral locomotion system for the daily application of the exoskeleton,can be recognized by the proposed method.According to a series of experiments,the recognizer shows strong comprehensive abilities including high accuracy,low delay,and sufficient adaption to different subjects.

BACKGROUND Colorectal cancer(CRC)accounts for 9.4%of overall cancer deaths,ranking second after lung cancer.Despite the large number of factors tested to predict their outcome,most patients with similar variables show big differences in survival.Moreover,right-sided CRC(RCRC)and left-sided CRC(LCRC)patients exhibit large differences in outcome after surgical intervention as assessed by preoperative blood leukocyte status.We hypothesised that stronger indexes than circulating(blood)leukocyte ratios to predict RCRC and LCRC patient outcomes will result from combining both circulating and infiltrated(tumour/peritumour fixed tissues)concentrations of leukocytes.AIM To seek variables involving leukocyte balances in peripheral blood and tumour tissues and to predict the outcome of CRC patients.METHODS Sixty-five patients diagnosed with colon adenocarcinoma by the Digestive Surgery Service of the La Paz University Hospital(Madrid,Spain)were enrolled in this study:43 with RCRC and 22 with LCRC.Patients were followed-up from January 2017 to March 2021 to record overall survival(OS)and recurrence-free survival(RFS)after surgical interventions.Leukocyte concentrations in peripheral blood were determined by routine laboratory protocols.Paraffin-fixed samples of tumour and peritumoural tissues were assessed for leukocyte concentrations by immunohistochemical detection of CD4,CD8,and CD14 marker expression.Ratios of leukocyte concentration in blood and tissues were calculated and evaluated for their predictor values for OS and RFS with Spearman correlations and Cox univariate and multivariate proportional hazards regression,followed by the calculation of the receiver-operating characteristic and area under the curve(AUC)and the determination of Youden’s optimal cutoff values for those variables that significantly correlated with either RCRC or LCRC patient outcomes.RCRC patients from the cohort were randomly assigned to modelling and validation sets,and clinician-friendly nomograms were developed to predict OS and RFS from the

Past and Future Changes in Climate and Water Resources in the Lancang–Mekong River Basin: Current Understanding and Future Research Directions

The Lancang–Mekong River(lmr)is an important transboundary river that originates from the Qinghai–Tibet Plateau,China and flows through six nations before draining into the South China Sea.Knowledge about the past and future changes in climate and water for this basin is critical in order to support regio-nal sustainable development.This paper presents a comprehensive review of the scientific progress that has been made in understanding the changing climate and water systems,and discusses outstanding challenges and future research opportunities.The existing literature suggests that:①The warming rate in the Lancang–Mekong River Basin(lmrB)is higher than the mean global warming rate,and it is higher in its upper portion,the Lancang River Basin(LRB),than in its lower portion,the Mekong River Basin(MRB);②historical precipitation has increased over the lmrB,particularly from 1981 to 2010,as the wet season became wetter in the entire basin,while the dry season became wetter in the LRB but drier in the MRB;③in the past,streamflow increased in the LRB but slightly decreased in the MRB,and increases in streamflow are projected for the future in the lmrB;and④historical streamflow increased in the dry season but decreased in the wet season from 1960 to 2010,while a slight increase is projected during the wet season.Four research directions are identified as follows:①investigation of the impacts of dams on river flow and local communities;②implementation of a novel water–energy–food–ecology(WEFE)nexus;③integration of groundwater and human health management with water resource assessment and management;and④strengthening of transboundary collaboration in order to address sustainable development goals(SDGs).

Introduction:Ankylosing spondylitis(AS)is a chronic inflammatory disorder involving the sacroiliac joints,lumbar spine,thoracic spine and even cervical spine.The affected spine not only suffers from back pain, but also progressive joint and spinal mobility limitation,untimely treatment of AS can lead to disability.The lymphocytemonocyte ratio(lmr) is a systemic inflammatory and immunological indicator for prediction of disease diagnosis,development and prognosis.Recently,many studies proved lmr was associated with kinds of diseases,including carcinomas and autoimmune diseases.However,the role of lmr in AS patients remains unclear.Method:78 AS patients and 78 sex-age-matched healthy controls(HC) were enrolled in this study.Magnetic resonance imaging(MRI) determined the staging of sacroiliac arthritis of AS patients.The diagnosis of AS was performed by the New York criteria.lmr level and other laboratory results were compared between AS patients and healthy controls.Results:A higher level of NLR,RDW,PLR,MPV,ESR,CRP and lower level of RBC,Hb,Hct,lmr,ALT,AST,TBIL and A/G in the AS group were noted as compared to the healthy controls.A positive correlation was observed between lmr and RBC,Hb,Hct and A/G,while lmr was negative correlated with NLR,PLR,AST,TBIL(Plmr was0.803(95%CI=0.734-0.872),and it showed 62.8% sensitivity and 87.2% specificity.Biomarkers between low MRI stage group and high MRI stage group were compared.Levels of WBC and NLR were higher in high MRI stage group and levels of lmr was lower(Plmr values among different stages(Plmr.Conclusions:This study suggests that lmr is an important inflammatory marker that can be used to identify disease activity and MRI stage of sacroiliac arthritis in AS patients.

Background:We investigated the prognostic value of the preoperative lymphocyte-to-Mononuclear ratio (lmr)and platelet-to-lymphocyte ratio (PLR) in a large cohort of patients with Non-small cell lung cancer (NSCLC).Methods:Clinic pathological data from 507 NSCLC patients,between 2010 and 2016 at Taizhou Hospital of Zhejiang Province,were evaluated retrospectively X tile was applied to assess the optimal cutoff levels for lmr and PLR.Univariate and multivariate COX regression models was performed to assess the prognostic factors.

【目的】研究水曲柳(Fraxinus mandschurica)幼苗对不同光照强度的响应及适应机理,为育苗造林和林下天然更新研究提供参考。【方法】以2年生盆栽水曲柳幼苗为材料进行不同光照处理,1年后测定幼苗在4种强度日光处理(相对光强100%、60%、30%和15%)下的生长、光合生理和水力性状等指标。【结果】遮阴处理显著影响了水曲柳幼苗的生长、光合和水分生理特性。与60%和30%相对光强处理相比,100%相对光强下水曲柳具有更高的气孔导度(Gs)、净光合速率(Pn)、根系水力导度(Kroot)、地上部分水力导度(Kshoot)和整株水力导度(Kplant)。随光照强度的减弱,水曲柳的生长速率显著降低,根生物量占比(RMR)减少,最大净光合速率(Pn, max)、光补偿点(LCP)和光饱和点(LSP)降低,而茎、叶生物量占比(SMR、lmr)以及表观量子效率(AQY)增加;枝条木质部解剖结构在不同光照处理下存在显著差异,相对光强100%处理时的导管密度显著高于相对光强30%和60%处理,而导管直径显著低于30%和60%处理。【结论】水曲柳幼苗通过调节形态、光合和水分生理特性来适应一定程度的弱光环境,但总体上对光强有较高的需求。水曲柳的光合和水力性状随光照强度的改变都具有较大的可塑性,二者对光照强度梯度变化的响应存在显著耦合关系。这些性状的可塑性响应有利于提高水曲柳幼苗在异质性光环境下的生存适合度,对于其更新阶段在林下不同光照条件下的生存有重要意义。
