BACKGROUND The main goal of our research is to introduce transoral robotic surgery and laser resection(TLR)as a considerable way of treating patients with recurrent oropha-ryngeal *** To develop a foundation of minimally invasive transoral surgical technique for patients with oropharyngeal *** This study prospectively and retrospectively included patients with recurrent tumors from 2003 to *** were allocated into two groups:(1)Group I;underwent TLR;and(2)Group II(control);underwent open surgeries of varying *** was done with intraoperative blood loss,postoperative infection incidence,and quality of life using the scale for patients with head and neck tumors known as the Functional Assessment of Cancer Therapy-Head&Neck *** One-hundred and forty one patients were included(103 males and 38 females),in 82 cases(85.4%),a recurrent tumor developed earlier than a year after primary tumor therapy;forty-six were in group I and 69 in group II,age ranging from 18 years to 86 years(average:57.6 years).The first group showed a statistically significant less amount of blood loss and a decreased incidence of infectious complications(P<0.05).Additionally,there was a significant difference in functional outcomes(quality of life scores)but no significant difference in survival *** In properly elected patients,TLR is not just reasonable but tends to be a favorable alternative for recurrent oropharyngeal cancers compared to the outcomes of the open surgery group.

目的了解上海市西部三区男男性行为人群(Men who have sex with men,msM)艾滋病病毒(Humanimmunodeficiency virus,HIV)感染相关行为学特征,以及发生无保护男男性行为的影响因素。方法通过非政府社会小组和社会信息网络,对上海西部三区msM人群进行宣传和动员,并按照研究要求进行筛选,使用统一问卷,采用面对面一对一方式调查。结果共调查424名msM,平均年龄为(26.84±6.69)岁;未婚者占90.1%(382/424);教育程度较高;半数以上受访者月均收入≥3 000元。受访者初次和男性发生性行为的平均年龄为(21.15±4.46)岁;过去半年内受访者的平均性伴数为(2.99±3.77)人。目前有固定男友的受访者占60.6%(257/424),但在半年内和固定男友间在性行为上相互忠贞的只有38.1%(98/257)。调查发现,162名受访者半年内曾有过无保护男男性行为,其中有固定男友的受访者发生无保护男男性行为的比例高于无固定男友者。与和男性发生无保护性行为相关的影响因素为月均收入和半年内男性性伴人数。此外,受访者寻找性伴最常见的方式是通过网络。同时也发现,常在男浴室和公园、公厕寻找性伴的msM,发生无保护性行为的比例较在其他场所寻找性伴的msM更高。61.8%(262/424)的受访者认为自己有感染HIV的风险,424名msM中312人既往曾进行过HIV检测。结论 msM人群性行为特征方面存在多种与HIV感染相关的高危因素,浴室和公园、公厕应作为msM人群HIV干预的重点场所;以月均收入较低、性伴数量较多的msM,发生无保护性行为的概率较高。应加强对此类人群的HIV相关健康干预。

The Mesoamerican Ministers of Health have set 2020 as the target for malaria elimination to be achieved in the region. Imported malaria cases are a potential threat to countries attempting elimination or working to prevent resurgence. We report the first imported Plasmodium ovale infection with molecular confirmation in Central America, which occurred in a Guatemalan soldier that had been deployed in Africa. The obstacles for its diagnosis using the standard microscopy technique and the need to improve its detection are discussed.

2002年,Lee et al.提出了多波段准分析算法（multi-band Quasi-Analytical Algorithm-QAA）。多波段准分析算法目前只能用于光学深水情况,可以快速反演得到总吸收系数和粒子回向散射系数,以及浮游植物和CDOM（有色可溶有机物）的吸收系...

2002年,Lee et al.提出了多波段准分析算法（multi-band Quasi-Analytical Algorithm-QAA）。多波段准分析算法目前只能用于光学深水情况,可以快速反演得到总吸收系数和粒子回向散射系数,以及浮游植物和CDOM（有色可溶有机物）的吸收系数。但是在整个计算流程中并没有考虑非弹性散射的影响,例如Raman散射、CDOM荧光。当水体属于清澈的大洋水时,Raman散射的贡献变得不可忽视;当CDOM浓度较大时,CDOM荧光作用也同样值得考虑。本文通过辐射传递方程模拟的办法对Raman散射及CDOM荧光对多波段准分析算法的影响进行模拟。只有在比较清澈的大洋Ⅰ类水域,Raman散射贡献才值得考虑,所以进行数值模拟时需要设定比较小的叶绿素浓度。非弹性散射部分进行了以下四种情况的模拟:（1）既没有Raman散射也没有CDOM荧光、（2）只考虑Raman散射、（3）只考虑CDOM荧光、（4）同时考虑Raman散射和CDOM荧光。只有当CDOM浓度达到一定程度后,CDOM荧光作用才能表现出来。非弹性散射部分进行了以下两种情况的模拟:（1）既没有Raman散射也没有CDOM荧光、（2）只考虑CDOM荧光。结果表明,Raman散射对遥感反射比的影响（～9%～25%）主要集中在400-500nm。CDOM荧光的影响（～1%-15%）主要集中在400-550nm;Raman散射和CDOM荧光对多波段准分析算法的影响在2%左右,在反演过程中不需要对此进行校正。

Hydropower production is one of the greatest threats to fluvial ecosystems and freshwater *** that we have entered the Anthropocene,there is an opportunity to reflect on what might constitute a‘sustainable’Anthropocene in the context of hydropower and riverine fish *** elements of existing practices that promote favorable social-ecological outcomes(i.e.,‘bright spots’)is timely given that there are plans to expand hydropower capacity in previously undammed rivers,intensify dam development in some of the world's largest river systems,and re-license existing *** approach this from a pragmatic perspective:for the foreseeable future,hydropower will likely remain an important source of renewable *** offer support for moving toward a more‘sustainable’Anthropocene,we provide syntheses of best practices during the siting,design,construction,operation,and compensation phases of hydropower development to minimize impacts on inland *** each phase,we offer positive examples(or what might be considered‘bright spots’)pertaining to some of the approaches described within our syntheses,acknowledging that these projects may not be viewed as without ecological and(or)societal detriment by all *** findings underscore the importance of protecting critical habitat and free-flowing river reaches through careful site selection and basinscale planning,infrastructure designs that minimize reservoir effects and facilitate safe passage of fish,construction of hydropower plants using best practices that minimize long-term damage,operating guidelines that mimic natural flow conditions,and compensation that is lasting,effective,inclusive,and locally *** from these‘bright spots’may require engagement of diverse stakeholders,professionals,and governments at scales that extend well beyond a given site,river,or even ***,environmental planning that integrates hydropower development into broader discussions
