Relationship between early structural changes at cornea incision sites and surgical outcomes after phacoemulsification

AIM: To assess the early structural changes at clear corneal cataract incision sites and surgical outcomes using anterior segment optical coherence tomography(AS-OCT).METHODS: We evaluated 80 eyes of 59 patients who underwent phacoemulsification with a clear corneal incision. All incisions were evaluated 1 wk, 1, and 3 mo postoperatively using AS-OCT and analyzed regarding angle, length of the incision, maximal corneal thickness at the incision, and if present, corneal endothelial gap length and incision gap area. The patients were divided into two groups according to whether or not an endothelial gap was present at 1 wk postoperatively(endothelial gap, group 1;no endothelial gap, group 2). We analyzed the difference in patient and surgical factors between the two groups, and compared the surgical outcome and the refractive ***: An endothelial gap was observed in 56(70.0%) of 80 eyes at 1 wk postoperatively but not at 3 mo postoperatively. The mean patient age was significantly higher in group 1 than in group 2. The longer the length of the corneal incision and the steeper the incision angle, the greater the length and area of the endothelial gap. In group 1, the mean change in mean keratometry of the anterior cornea was significantly greater than in group 2, and the spherical equivalent(SE) and mean numerical error indicated significant myopic changes at 1 wk postoperatively. CONCLUSION: The risk of an endothelial gap increases with patient age and a long corneal incision and steep incision angle. The presence of an endothelial gap aftersurgery may affect the early postoperative corneal curvature and SE.

Early prediction of myocardial viability after acute myocardial infarction by two-dimensional speckle tracking imaging

Background Identifying the transmural extent of myocardial necrosis and the degree of myocardial viability in acute myocardial infarction （AMI） is important clinically. The aim of this study was to assess myocardial viability using two-dimensional speckle tracking imaging （2D-STI） in patients with AMI. Methods 2D-STI was performed at initial presentation, three days, and six months after primary percutaneous coronary intervention （PCI） in 30 patients with AMI, who had a left anterior descending coronary artery （LAD） culprit lesion. In addition, 20 patients who had minimal stenotic lesions （〈 30% stenosis） on coronary angiography were also included in the control group. At six months dobutamine echocardiography was performed for viability assessment in seven segments of the LAD territory. According to the recovery of wall motion abnormality, segments were classified as viable or non-viable. Results A total of 131 segments were viable, and 44 were nonviable. Multivariate analysis revealed significant differences between the viable and nonviable segments in the peak systolic strain, the peak systolic strain rate at initial presentation, and peak systolic strain rate three days after primary PCI. Among these, the initial peak systolic strain rate had the highest predictive value for myocardial viability （hazard ratio： 31.22, P 〈 0.01）. Conclusions 2D-STI is feasible for assessing myocardial viability, and the peak systolic strain rate might be the most reliable predictor of myocardial viability in patients with AMI.

Base pair distance analysis in single DNA molecule by direct stochastic optical reconstruction microscopy

Precise fluorescence imaging of single l-DNA molecules for base pair distance analysis requires a superresolution technique, as these distances are on the order of diffraction limit. Individual l-DNA molecules intercalated with the fluorescent dye YOYO-1 were investigated at subdiffraction spatial resolution by direct stochastic optical reconstruction microscopy（d STORM）. Various dye-to-DNA base pair ratios were imaged by photoswitching YOYO-1 between the fluorescent state and the dark state using two laser sources. The acquired images were reconstructed into a super-resolution image by applying Gaussian fitting to the centroid of the point spread function. By measuring the distances between localized fluorophores, the base pair distances in single DNA molecules for dye-to-DNA base pair ratios of 1：50,1：100, and 1：500 were calculated to be 17.1 0.8 nm, 34.3 2.2 nm, and 170.3 8.1 nm[17_TD$IF], respectively,which were in agreement with theoretical values. These results demonstrate that intercalating dye in a single DNA molecule can be photoswitched without the use of an activator fluorophore, and that super-localization precision at a spatial resolution of 17 nm was experimentally achieved.

Magnetohydrodynamic flows tuning in a conduit with multiple channels under a magnetic field applied perpendicular to the plane of flow

In this study, three-dimensional liquid-metal magnetohydrodynamic flows in a conduit with multiple channels under a uniform magnetic field are numerically investigated. The geometry of the conduit is of a four-parallel-channels system including one inflow channel and three outflow channels. The liquid-metal flows into the inflow channel, then turns through 180° in the transi- tion segment, finally flows into three different outflow channels. This kind of channel system can induce counter flow and co-flow, which is rarely investigated before though the conceptual designs of duct flow in the blanket have suggested this type of flow. A structured grid system is chosen after a series of mesh independence tests in the present study. The axial velocity in the side layer near the first partitioning wall, located between the inflow channel and the first outflow channel, is the highest with the lowest electric potential formed therein. The pressure almost linearly decreases in the main flow direction, except in the transition segment. Moreover, the pressure gradient in the first outflow channel is the largest among the three outflow channels. The interdependency of the current, fluid velocity, pressure, electric potential is examined in order to describe the electromagnetic characteristics of the liquid-metal flows.

Comparative study of microarray and experimental data on Schwann cells in peripheral nerve degeneration and regeneration: big data analysis

A Schwann cell has regenerative capabilities and is an important cell in the peripheral nervous *** microarray study is part of a bioinformatics study that focuses mainly on Schwann cells. Microarray data provide information on differences between microarray-based and experiment-based gene expression analyses. According to microarray data, several genes exhibit increased expression(fold change) but they are weakly expressed in experimental studies(based on morphology, protein and mRNA levels). In contrast, some genes are weakly expressed in microarray data and highly expressed in experimental studies;such genes may represent future target genes in Schwann cell studies. These studies allow us to learn about additional genes that could be used to achieve targeted results from experimental studies. In the current big data study by retrieving more than 5000 scientific articles from PubMed or NCBI, Google Scholar, and Google, 1016(up-and downregulated) genes were determined to be related to Schwann cells. However,no experiment was performed in the laboratory; rather, the present study is part of a big data analysis. Our study will contribute to our understanding of Schwann cell biology by aiding in the identification of *** on a comparative analysis of all microarray data, we conclude that the microarray could be a good tool for predicting the expression and intensity of different genes of interest in actual experiments.

A NUMERICAL ANALYSIS OF THE BLOOD FLOW AROUND THE BILEAFLET MECHANICAL HEART VALVES WITH DIFFERENT ROTATIONAL IMPLANTATION ANGLES

The effects of implantation angles of Bileaflet Mechanical Heart Valves （BMHVs） on the blood flow and the leaflet motion are investigated in this paper. The physiological blood flow interacting with the moving leaflets of a BMHV is simulated with a strongly coupled implicit Fluid-Structure Interaction （FSI） method based on the Arbitrary-Lagrangian-Eulerian （ALE） approach and the dynamic mesh method （remeshing） in Fluent. BMHVs are widely used to be implanted to replace the diseased heart valves, but the patients would suffer from some complications such as hemolysis, platelet activation, tissue overgrowth and device failure. These complications are closely related to both the flow characteristics near the valves and the leaflet dynamics. The current numerical model is validated against a previous experimental study. The numerical results show that as the rotation angle of BMHV is increased the degree of asymmetry of the blood flow and the leaflet motion is increased, which may lead to an unbalanced force acting on the BMHVs. This study shows the applicability of the FSI model for the interaction between the blood flow and the leaflet motion in BMHVs.

Recurrent gastric neuroendocrine tumors treated with total gastrectomy

Gastric neuroendocrine tumors are rare; however,the incidence has recently increased due to the increasing use of upper endoscopy. Neuroendocrine tumors arise from the excess proliferation of enterochromaffinlike(ECL) cells. The proliferative changes of enterochromaffin cells evolve through a hyperplasiadysplasia-neoplasia sequence that is believed to underlie the pathogenesis of gastric neuroendocrine tumors. Endoscopic resection is recommended as the initial treatment if the tumor is not in an advanced stage. However,there is no definite guideline for the treatment of recurrent gastric neuroendocrine tumors following endoscopic resection. Here,we report a rare case of gastric neuroendocrine tumors in a 56-yearold male who experienced two recurrences within 11 years after endoscopic resection. The patient finally underwent a total gastrectomy. The pathological features of the resected stomach exhibited the full hyperplasia-dysplasia-neoplasia sequence of the ECL cells in a single specimen.

An Approach to Share Architectural Drawing Information and Document Information for Automated Code Checking System

The purpose of this study is to suggest a way of optimized managing and sharing information between standard architectural drawings and construction documents in Korea architectural industry for automated code checking system by linked STEP and XML. To archive this purpose, the authors have analyzed current research and technical development for STEP and XML link and developed a prototype system for sharing information between model based drawings and XML based construction documents. Finally, the authors have suggested practical use scenario of sharing information through linked STEP and XML using test case of automatic code checking. In the paper, the possibility of constructing integrated architectural computing environment through exchange and sharing of drawing information and external data for the whole building life-cycle, from the conceptual design stage to the construction and maintenance stage has been examined. Automated code checking through linked STEP and XML could be enhanced through col-laboration business, more completed code, improved building performance, and reduced construction costs.

Dyna-QUF:Dyna-Q based univector field navigation for autonomous mobile robots in unknown environments

A novel approach was presented to solve the navigation problem of autonomous mobile robots in unknown environments with dense obstacles based on a univector field method. In an obstacle-free environment, a robot is ensured to reach the goal position with the desired posture by following the univector field. Contrariwise, the univector field cannot guarantee that the robot will avoid obstacles in environments. In order to create an intelligent mobile robot being able to perform the obstacle avoidance task while following the univector field, Dyna-Q algorithm is developed to train the robot in learning moving directions to attain a collision-free path for its navigation. Simulations on the computer as well as experiments on the real world prove that the proposed algorithm is efficient for training the robot in reaching the goal position with the desired final orientation.

Numerical analysis of a magnetohydrodynamic duct flow with flow channel insert under a non-uniform magnetic field

This study performs a numerical analysis of three-dimensional liquid metal(LM) magnetohydrodynamic(MHD) flows in a square duct with an FCI in a non-uniform magnetic field. The current study predicts detailed information on flow velocity, Lorentz force, pressure, current and electric potential of MHD duct flows for different Hartmann numbers. Also, the effect of the electric conductivity of FCI on the pressure drop along the main flow direction in a non-uniform magnetic field is examined. The present study investigates the features of LM MHD flows in consideration of the interdependency among the flow variables.
