Optimal duration of the early and late recurrence of hepatocellular carcinoma after hepatectomy

AIM: To determine the best cut-off value between the early and late recurrence periods after the initial recurrence of hepatocellular carcinoma(HCC).METHODS: The clinical records of 404 patients who underwent macroscopic curative hepatectomy for HCC between 1980 and 2010 were retrospectively examined. We divided the 252 patients experienced a recurrence of HCC into two groups, the early and late recurrence groups using the "minimum P-value" approach. Factors for early recurrence were investigated using all 404 patients, and factors related to late recurrence were investigated in the patients who were confirmed to be recurrence free at the end of the early recurrence ***: For the 252 patients who experienced a recurrence, the optimal cut-off value for differentiating early and late recurrence based on the overall survival after initial recurrence was 17 mo(5-year overall survival after initial recurrence: 15.4% vs 36.3%, P = 0.000018). Cox proportional hazard analysis identified early recurrence(P = 0.003) as one of the independent prognostic factors associated with overall survival after initial recurrence. A logistic regression model showed that an alpha-fetoprotein level > 100 ng/m L(P < 0.001), multiple HCC(P < 0.001), serosal invasion(P = 0.031), and microvascular invasion(P = 0.012) were independent factors associated with early recurrence, whereas the only independent factor related to late recurrence was liver cirrhosis(P = 0.002).CONCLUSION: Seventeen months after hepatectomy is a useful cut-off value between early and late recurrence of HCC based on the prognosis and different etiologies.

AIM: To analyze hepatocellular carcinoma(HCC) patients with portal vein tumor thrombosis(PVTT) using the tumor-node-metastasis(TNM) staging ***: We retrospectively analyzed 372 patients with HCC who underwent hepatectomy between 1980 and *** studied the outcomes of HCC patients with PVTT to evaluate the American Joint Committee on Cancer TNM staging system(7th edition) for stratifying and predicting the prognosis of a large cohort of HCC patients after hepatectomy in a *** vein invasion(vp) 1 was defined as an invasion or tumor thrombus distal to the second branch of the portal vein,vp2 as an invasion or tumor thrombus in the second branch of the portal vein,vp3 as an invasion or tumor thrombus in the first branch of the portal vein,and vp4 as an invasion or tumor thrombus in the portal trunk or extending to a branch on the contralateral ***: The cumulative 5-year overall survival(5yr OS) and 5-year disease-free survival(5yr DFS) rates of the 372 patients were 58.3% and 31.3%,*** 5yr DFS and 5yr OS of vp3-4 patients(n = 10) were 20.0%,and 30.0%,respectively,which was comparable with the corresponding survival rates of vp1-2 patients(P = 0.466 and 0.586,respectively).In the subgroup analysis of patients with macroscopic PVTT(vp2-4),the OS of the patients who underwent preoperative transarterial chemoembolization was comparable to that of patients who did not(P = 0.747).There was a significant difference in the DFS between patients with stage Ⅰ HCC and those with stage Ⅱ HCC(5yr DFS 39.2% vs 23.1%,P 100 mg/dL was independently associated with ***: Hepatectomy for vp3-4 HCC results in a survival rate similar to hepatectomy for *** stratified the stage Ⅱ-Ⅲ HCC patients according to prognosis.

Direct cholangioscopy combined with doubleballoon enteroscope-assisted endoscopic retrograde cholangiopancreatography

Double-balloon enteroscope (DBE)-assisted endoscopic retrograde cholangiopancreatography (ERCP) is an effective endoscopic approach for pancreatobiliary disorders in patients with altered gastrointestinal anatomy. Endoscopic interventions via DBE in these postoperative settings remain difficult because of the lack of an elevator and the use of extra-long ERCP accessories. Here, we report the usefulness of direct cholangioscopy with an ultra-slim gastroscope during DBE-assisted ERCP. Three patients with choledocholithiasis in postoperative settings (two patients after Billroth Ⅱ gastrojejunostomy and one patient after Roux-en-Y gastrojejunostomy) were treated. DBE was used to gain access to the papilla under carbon dioxide insufflation, and endoscopic sphincterotomy was performed with a conventional sphincterotome. For direct cholangioscopy, the enteroscope was exchanged for an ultra-slim gastroscope through an incision in the overtube, which was inserted directly into the bile duct. Direct cholangioscopy was used to extract retained bile duct stones in two cases and to confirm the complete clearance of stones in one case. Bile duct stones were eliminated with a 5-Fr basket catheter under direct visual control. No adverse events were noted in any of the three cases. Direct cholangioscopy with an ultra-slim gastroscope facilitates subsequent treatment within the bile duct. This procedure represents another potential option during DBE-assisted ERCP.

A Test for Stabilization of an Oligomeric Protein by Introduction of Aromatic Residues into the Interface

The design of variants to enhance conformational stability of proteins is an important aspect of protein engineering. Oligomeric proteins are often stabilized by aromatic clusters located within the subunit interfaces. In the present study, the authors constructed five variants of Ps3aHSD （Pseudomonas sp. B-0831 3a-hydroxysteroid dehydrogenase） in which one or two residues at the dimer interface were replaced with aromatic residues, and examined the effects of introducing aromatic residues in this region on protein thermostability. Under their experimental conditions, all variants formed dimers, similar to wild-type Ps3aHSD. Thermal denaturation experiments indicated that Tm of all variants was 0.2-16.2 °C lower than that of wild-type protein, indicating less stable thanwild-type protein. The results collectively suggest that aromatic residues of natural oligomeric proteins are strictly posted in the interface to facilitate optimal interactions and avoid conformational strain.

An Authentication Method for Digital Audio Using a Discrete Wavelet Transform

Recently, several digital watermarking techniques have been proposed for hiding data in the frequency domain of audio files in order to protect their copyrights. In general, there is a tradeoff between the quality of watermarked audio and the tolerance of watermarks to signal processing methods, such as compression. In previous research, we simultaneously improved the performance of both by developing a multipurpose optimization problem for deciding the positions of watermarks in the frequency domain of audio data and obtaining a near-optimum solution to the problem. This solution was obtained using a wavelet transform and a genetic algorithm. However, obtaining the near-optimum solution was very time consuming. To overcome this issue essentially, we have developed an authentication method for digital audio using a discrete wavelet transform. In contrast to digital watermarking, no additional information is inserted into the original audio by the proposed method, and the audio is authenticated using features extracted by the wavelet transform and characteristic coding in the proposed method. Accordingly, one can always use copyright-protected original audio. The experimental results show that the method has high tolerance of authentication to all types of MP3, AAC, and WMA compression. In addition, the processing time of the method is acceptable for every-day use.

近红外光谱因为具有小成本、易操作、低耗时等优点,所以广泛用于食品领域。作为一种间接的检测方法,近红外光谱检测需要建立光谱和浓度之间的统计模型。但是,一种条件下建立的模型在另一种检测条件下会失效。针对此问题,重新建模可以加以解决,但是重新建立光谱与浓度之间的模型非常繁琐耗时。此时,模型转移可以在避免重新建模的情况下,通过光谱校正,保证预测精度。在模型转移中,已经建立好模型的光谱称为主光谱(A),不用建立模型,而只用主光谱模型预测的光谱称为从光谱(B)。模型转移方法的步骤是,先在校正集中选择一些样本作为主光谱的转移集(A_(t)),然后选择从光谱中浓度和A_(t)相同的光谱,以此作为从光谱的转移集(B_(t))。通过A_(t)和B_(t)构建模型转移矩阵。最后将需要校正的从光谱(B_(v))乘以上述的转移矩阵中,即可获得校正后的从光谱(B_(new))。此时,B_(new)就可以用主光谱的模型来直接预测。在模型转移中,转移集样本的选择对模型校正至关重要。目前,转移集的样本通常从光谱之间的距离而非模型转移误差获得。但是,转移误差对模型转移结果的验证至关重要,故该研究出了基于集群分析的集群优化法(ER)并将其用于优化KS方法产生的转移集样本。ER先用随机方法建立转移集的多个子集合,并计算每个子集合的转移误差。然后,对某一个样本,计算包含这个样本的子集合转移误差均值。最后,选择转移误差均值较低的样本作为新转移集样本进行模型转移。以玉米数据测试了ER算法。结果显示,对于典型相关分析-有信息成分提取法(CCA-ICE)、直接校正法(DS)、分段直接校正法(PDS)、光谱空间转化法(SST)这些常见的模型转移方法,相比于KS样本选择方法,ER方法可以找出重要的转移集样本,进而显著降低模型转移误差。

The introduction of serum prostate-specific antigen （PSA） in the 1980s has dramatically altered and benefited the initial diagnosis of prostate cancer. However, the widespread use of PSA testing has resulted in overdetection and overtreatment of potentially indolent disease. Thus, a clinical dilemma today in the management of prostate cancer is to discern men with aggressive disease who need definitive treatment from men whose disease are not lethal. Although several serum and tissue biomarkers have been evaluated during the past decade, improved markers are still needed to enhance the accuracy, with which patients at risk can be discerned and treated more aggressively. The cancer/testis antigens （CTAs） are a group of proteins that are restricted to the testis in the normal adult, but are aberrantly expressed in several types of cancers. Because of their restricted expression pattern, the CTAs represent attractive biomarker candidates for cancer diagnosis/prognosis. Furthermore, several studies to date have reported the differential expression of CTAs in prostate cancer. Here, we review recent developments that demonstrate the potential of the CTAs as biomarkers to discern the a^ressive Dhenotvoe of orostate cancer.

On a New Geometric Constant Related to the Modulus of Smoothness of a Banach Space

We shall introduce a new geometric constant A（X） of a Banach space X, which is closely related to the modulus of smoothness px （T）, and investigate it in relation with the constant As （X） by Baronti et al., the von Neumann-Jordan constant CNj（X） and the James constant J（X）. A sequence of recent results on these constants as well as some other geometric constants will be strengthened and improved.

Gastric carcinoma originating from the heterotopic submucosal gastric gland treated by laparoscopy and endoscopy cooperative surgery

Gastric carcinoma is derived from epithelial cells in the gastric mucosa. We reported an extremely rare case of submucosal gastric carcinoma originating from the heterotopic submucosal gastric gland(HSG) that was safely diagnosed by laparoscopy and endoscopy cooperative surgery(LECS). A 66-year-old man underwent gastrointestinal endoscopy, which detected a submucosal tumor(SMT) of 1.5 cm in diameter on the lesser-anterior wall of the upper gastric body. The tumor could not be diagnosed histologically, even by endoscopic ultrasound-guided fine-needle aspiration biopsy. Local resection by LECS was performed to confirm a diagnosis. Pathologically, the tumor was an intra-submucosal well differentiated adenocarcinoma invading 5000 μm intothe submucosal layer. The resected tumor had negative lateral and vertical margins. Based on the Japanese treatment guidelines, additional laparoscopic proximal gastrectomy was curatively performed. LECS is a less invasive and safer approach for the diagnosis of SMT, even in submucosal gastric carcinoma originating from the HSG.
