Several diseases originate from bile duct *** studies on these diseases,certain etiologies of some of them still cannot be *** most common disease of the bile duct in newborns is biliary atresia,whose prognosis varies according to the age of surgical *** diseases such as Alagille syndrome,inspissated bile duct syndrome,and choledochal cysts are also time-sensitive because they can cause severe liver damage due to *** majority of these diseases present with cholestatic jaundice in the newborn or infant period,which is quite difficult to differentiate regarding clinical acumen and initial *** cholangiography is potentially necessary to make an accurate diagnosis,and further treatment will be performed synchronously or planned as findings *** article provides a concise review of bile duct diseases,with interesting cases.

Background:Burn first aid awareness has been shown to reduce morbidity and *** present a report on the knowledge and practices of the Saudi population with regard to burn first aid and the application of traditional ***:An internet-based survey was conducted to assess the public’s knowledge on first aid practices and home remedies applied for burn injuries among Saudi ***:A total of 2758 individuals responded to the *** were 1178(42.7%)respondents who had previously received burn first aid *** thousand five hundred fifty respondents had a history of burn exposure in which burn injury first aid was applied as follows:1118(72.1%)removed clothing and accessories from the injured area;water was applied by 990(63.9%);among those who applied water,877(88.6%)applied cold water;and only 57(5.8%)did so for more than 15 *** the burn area was performed by 526(33.9%),and 985(63.5%)sought medical *** it comes to traditional remedies,2134(77.4%)knew of and/or implemented these remedies as first aid or to treat *** and toothpaste were the commonest among these remedies with 1491(69.9%)and 1147(53.7%),*** was associated with female gender(r=0.87,Puniversity graduate(r=0.05,P=0.002).Nearly half of those who knew of traditional remedies did not have previous knowledge of burn first ***:Proper burn first aid is a simple,cheap,and accessible means of managing burns *** the majority of the respondents were university graduates(51.1%),knowledge and implementation of burn first aid was very *** healthcare agencies should review and promote a consistent guideline for burn first aid in an effort to tackle and minimize the effect of this grave injury.

BACKGROUND Children with esophageal atresia(EA)have risk of gastroesophageal reflux disease(GERD),suggesting reflux monitoring for prompt *** To evaluate GERD in children with EA and specific symptom association from combined Video with Multichannel Intraluminal Impedance and pH(MII-pH)*** Children diagnosed with EA with suspected GERD and followed up at king Chulalongkorn Memorial hospital between January 2000 and December 2018 were prospectively *** underwent esophagogastroduodenoscopy with esophageal biopsy and Video MII-pH study on the same *** of GERD which included both esophageal and extra-esophageal symptom were recorded from video monitoring and abnormal reflux from MII-pH study based on the statement from the European Paediatric Impedance *** of GERD was also reported by using histopathology as a gold *** appearance was recorded using Los Angeles Classification and esophagitis severity was graded using Esohisto *** Fifteen children were recruited with age of 3.1(2.2,9.8)years(40%,male)and the common type was C(93.3%).The symptoms recorded were cough(75.2%),vomiting(15.2%),irritability or unexplained crying(7.6%)and dysphagia(1.9%)with the symptom-reflux association of 45.7%,89%,71%and 0%,*** were abnormal endoscopic appearance in 52.9%,esophagitis in 64.7%and high reflux score in 47.1%.Video MII-pH study has high diagnostic value with the sensitivity,specificity and accuracy of 72.7%,100%and 82.4%,*** Prevalence of GERD in children with EA was *** MII-pH study to detect GERD in children with EA had high diagnostic value with the trend of specific symptom association.

Aim: The aims of this study were: 1) to estimate the prevalence and pattern of complications after Achilles tendon (AT) repair, and 2) to determine the significant predictors of post-operative infection. Methods: A retrospective cohort study of all patients who were operated at hamad General hospital (HGH) between June 2010 and June 2012 for AT rupture (n = 102), was conducted. Data was collected on 1) patient’ characteristics such as age, sex;2) disease characteristics such as mechanism of rupture, type of rupture (partial or complete), whether an anterior or posterior slab was applied, number of suture materials, number of antibiotics, surgical time, time to surgery and length of hospital stay (LOS), number of follow up visits, and 3) complications. Descriptive and analytical statistical analyses were applied. Receiver operating characteristic curve was applied to identify the validity of different LOS values, with a significance level at p ≤ 0.05. Results: Of the 102 patients with Achilles rupture, almost males (96.1%), with a mean age 31.07 ± 9.71 years, 52% with complete rupture, the majority were open ruptures (81.4%) and bathroom-related (70.6%). Anterior slab was applied to 58.2% and 2 types of suture materials to 71.6% of cases. Fifteen cases (14.7%, 95% CI: 7.8% - 21.6%) presented with one or more complications (9.8% post-operative infections, 5.9% stiffness and 2% re-rupture). Post-operative infections were significantly associated with: old age (z = 2.11, p = 0.035), longer LOS (z = 2.01, p = 0.04), and presence of diabetes (Fisher exact test: p = 0.003). After adjustment for age, LOS (p = 0.04) and diabetes (p = 0.017) remained as significant predictors of post-operative infections. LOS of 2.5 days was the optimum cut-off point above which post-operative infection is more likely to occur, with sensitivity of 80% and specificity of 54%. Evidence shows that pulmonary problems in coronavirus disease 2019(COVID-19)may set off from vascular injury that progresses to physiological disturbances through a compromised gas exchange,following an infection with the severe acute respiratory syndrome coronavirus *** this process,inefficient gas exchange in the alveolar could precipitate silent nonclinical ***,patients with“silent hypoxemia”do not necessarily experience any breathing difficulty(dyspnea)at the early stage of COVID-19 while the disease *** a result,several asymptomatic,presymptomatic and patients with mild symptoms may escape quarantine measure and thus continue to spread the virus through ***,early diagnosis of“silent hypoxemia“,which attracts no clinical warnings,could be an important diagnostic measure to prevent acute respiratory distress syndrome from the risk of pulmonary failure among the presymptomatic and as a screening tool in the asymptomatic who are hitherto potential spreaders of the virus.

Clinical features and visual outcomes of scleritis patients presented to tertiary care eye centers in Saudi Arabia

AIMTo describe the clinical features, systemic associations, treatment and visual outcomes in Saudi patients with *** retrospective chart review was performed for patients with scleritis presenting to two tertiary care eye hospitals in Riyadh, Saudi Arabia, from 2001 to 2011. Data were collected on the clinical features of scleritis, subtypes of scleritis, associated systemic disease, history of previous ocular surgery and medical therapy, including the use of immunosuppressants. Treatment outcomes were evaluated based on best-corrected visual acuity (BCVA) and response to *** the 52 patients included in the study, non-necrotizing anterior scleritis was the most common type of scleritis in 22 patients (42.3%), followed by posterior scleritis in 14 patients (26.9%). The majority of cases, 31 patients (59.6%), were idiopathic in nature. Systemic associations were present in 12 patients (23.1%). Infectious scleritis was confirmed in 6 patients (11.5%): 3 with bacterial scleritis after pterygium excision, 2 patients with scleritis related to tuberculosis and 1 patient with scleritis resulting from herpes simplex infection. For the various subtypes of scleritis, BCVA values after treatment and time to remission significantly differed (P<0.05, all cases). Systemic immunosuppressive therapies in addition to steroids were administered to 46.2% of all patients. The T-sign was present on B-scan ultrasonography in 9 (64.3%) of the 14 posterior scleritis ***-necrotizing anterior scleritis was the most common subtype of scleritis. Final visual outcome and time to remission differed among the various scleritis subtypes.
