BACKGROUND One of the major perioperative complications for coronary artery bypass graft(CABG)is *** risk of perioperative stroke after CABG is approximately 2%.Carotid stenosis(CS)is considered an independent predictor of perioperative stroke risk in CABG *** optimal management of such patients has been a source of *** of the possible surgical options is synchronous carotid endarterectomy(CEA)and ***,we have presented 4 cases of successful synchronous CEA and *** center’s experience with 4 cases of significant carotid artery stenosis,which were successfully managed with combined CEA and CABG,are *** first case was a female who presented for CABG after a ST-elevation myocardial *** had right internal carotid artery(ICA)occlusion and 90%left ICA *** second case was a male who was electively admitted for *** was discovered that he had left ICA occlusion and 90%right ICA *** third case was a male with a history of stroke,two months prior to *** presented with non-ST-elevation myocardial ***,it was discovered that he had>90%right ICA *** final case was a male who was electively admitted for *** was discovered that he had bilateral>90%ICA *** have also reviewed the current evidence and guidelines for managing CS in patients undergoing *** Our case series demonstrated that synchronous CEA and CABG was safe.A multicenter study with additional patients is *** is necessary for clinicians to screen for CS in high-risk patients with features.

Upregulated hepatic expression of mitochondrial PEPCK triggers initial gluconeogenic reactions in the HCV-3 patients

Objective: To identify the differential expression of candidate gluconeogenic genes which may initiate hepatitis C virus(HCV) related metabolic disorder during early stages of disease. Methods: Patients of diverse age and sex, with positive HCV genotype 3(HCV-3) RNA in serum and with no history of other related infections, co-infections, alcoholism, diabetes or chemotherapeutic treatments were considered for this study. Semi-quantitative reverse transcriptase PCR analysis and quantitative fold change analysis of the fresh liver biopsies of eight chronically infected HCV-3 patients and six healthy individuals were evaluated for three potential biomarkers involved in glucose homeostasis induction, namely mitochondrial phosphoenolpyruvate carboxykinase 2(PCK2), glucose-6-phosphatase catalytic subunit(G6PC) and associated forkhead box protein 01(FOXO1). Results: Symptomatic evaluation, clinical history and blood test were conducted according to general disease prognosis procedures and reported here. Significantly upregulated expression of PCK2 independent of age, sex and viral infectivity levels in all HCV patients was observed, whereas no significant changes in the expression of G6 PC and FOXO1 were found. Conclusions: PCK2 triggers initial gluconeogenic reactions which ultimately result in the accumulation of glycogen in the liver hepatocytes. We therefore suggest that the overproduction of PCK2 has important physiological role in the onset of metabolic disorder in the HCV-3 patients.

Teaching writing to Arab EFL learners in general, and to students who have never been exposed to an Anglo-Saxon foreign language in particular in institutions of higher education, through incorporating culturally meaningful learning materials, can actually be enjoyable and fruitful. However, it can also be very challenging given the sociocultural and linguistic background of the learners as well as other factors related to the nature of the imported teaching materials from overseas. The challenge then is identifying and selecting materials for each particular class, as well as how to get students motivated and successfully engaged in the process of writing. This paper enlightens educators and language programs managers and planners about the mechanism of the writing process while incorporating the cultural components relevant to the learner, as well as drawing their attention to the benefits of engaging students in the actual process of writing and boosting their motivation via a meaningful socioculturally based approach.

Hepatocellular carcinoma(HCC)is one of the leading causes of death induced by cancer in the modern world and majority of the cases are related to chronic hepatitis B virus(HBV)***-encoded X protein(HBx)is known to play a pivotal role in the pathogenesis of viral induced *** is a multifunctional protein of17 kDa which modulates several cellular processes by direct or indirect interaction with a repertoire of host factors resulting in *** might interfere with several cellular processes such as oxidative stress,DNA repair,signal transduction,transcription,protein degradation,cell cycle progression and apoptosis.A number of reports have indicated that HBx is one of the most common viral ORFs that is often integrated into the host genome and its sequence variants play a crucial role in *** mutational or deletion analysis it was shown that carboxy terminal of HBx has a likely role in protein-protein interactions,transcriptional transactivation,DNA repair,cell,signaling and pathogenesis of *** accumulated evidence thus far suggests that it is difficult to understand the mechanistic nature of HBx associated HCC,and HBx mediated transcriptional transactivation and signaling pathways may be a major *** article addresses the role of HBx in the development of HCC with particular emphasis on HBx mutants and their putative targets.

Reno-protective effect of garlic extract against immobilization stress induced changes in rats

Objective: To examine immobilization stress-induced antioxidant defense alterations in rat kidney and the antioxidant effects of aqueous garlic extract in pre and post stress extract treatments. Methods: Albino rats were treated with aqueous extract of garlic both before and 6 h of immobilization stress. Pro-oxidant eminence of rat kidney was assessed by determining the levels of glutathione, thiobarbituric acid reactive substances, aspartate aminotransferase, alanine aminotransferase, glucose, uric acid, alkaline phosphatase and antioxidant enzymes activities. Results: In response to 6 h of immobilization stress, a significant rise in the level of kidney enzymes was recorded. However, antioxidant enzyme activities showed a sharp decline. Conclusions: The extract treatment before and after the stress reverted the activities of above mentioned enzymes towards their control values. Hence, garlic extract can be given as nutritional supplement for scavenging the free radicals generated in rat kidney.

MicroRNA and mRNA profiling of cerebral cortex in a transgenic mouse model of Alzheimer’s disease by RNA sequencing

Percutaneous Coronary Interventional Treatment for Coronary Artery Disease and the Role of Antiplaplatelets Therapy: A Review of the Literature

Uses of balloon catheters or BMS for the treatment of coronary artery lesions shows good short-term results but long-term follow up revealed restenosis in up to 20%-30% of patients. Thus new improvements to balloons and stents are always necessary to achieve the best results from percutaneous coronary intervention (PCI). Drug-eluting stents (DES) improved the principles of bare metal stents (BMS) by local drug release to inhibit neointimal growth. DES reduced the incidence of in-stent restenosis. These benefits and lower costs compared to surgical treatment make the DES an attractive alternative for the treatment of coronary artery disease. Different components of DES which include the polymers, drugs and the stents underwent progressive evolution, and these led to development of new generations of DES with variable types of drugs and polymers to fully absorbable stents. The concern of stent thrombosis still an issue and dual antiplatlets therapy (DAPT) is mandatory for variable time ranging from one month to one year. This article discusses the main available clinical trials in the developments of BMS, DES and the comparison between both with a prospective look at future technologies in the field, in addition to reviewing the current guideline in the uses of DAPT after PCI.

Prevalence of Infectious Complications in Children with Cancer

Background:?Infections are the most common complications during chemotherapy. The trends have been changing over time due to use of multi-agent intensive chemotherapy.?Procedure:?We looked?over?our data to see what complications we get in our patients. The data was collected on patients treated in king fahad medical City from July 2009 to Dec 2015.?Results:?We?found that 86 patients had one episode only while 92 had 2 or more episodes reaching up to 11 episodes in 1 patient. We found positive cultures in 17.3% of episodes with staphylococcus as common gram positive and Klebsiella pneumoniae as common gram negative bacteria, respiratory viral infections in 13.8% and GI infections in 9%. We found Candida albicans as the most common fungus while other yeasts followed. Acute lymphoblastic leukaemia was the most common diagnosis. The episodes were associated with neutropenia of 7 days in 1st?episode and 56.8% in cases with repeated episodes. We had 2 deaths one from recurrent infections with CNS (Central Nervous System) damage and one from Pulmonary infections causing pulmonary haemorrhage. We had one with persistent neurological sequele from prolonged fungal infection.?Conclusions:Neutropenia of 7 days came out to be significant risk factors for infection across all diseases. We have an acceptable level of morbidity and mortality due to good supportive services.?We need prospective studies to get an up-to-date picture of our practice.?We need prophylactic antibiotics and antifungals for high-risk patients like AML.
