BACKGROUND:trauma-induced coagulopathy(TIC)due to serious injuries significantly leads to increased mortality and morbidity among elderly ***,the risk factors of TIC are not well *** study aimed to explore the risk factors of TIC in elderly patients who have major ***:In this retrospective study,the risk factors for TIC in elderly trauma patients at a single trauma center were investigated between January 2015 and September *** demographic information including gender,age,trauma parts,injury severity,use of blood products,use of vasopressors,need of emergency surgery,duration of mechanical ventilation,length of stay in the intensive care unit(ICU)and hospital,and clinical outcomes were extracted from electric medical *** logistic regression analysis was performed to differentiate risk factors,and the performance of the model was evaluated using receiver operating characteristics(ROC)***:Among the 371 elderly trauma patients,248(66.8%)were male,with the age of 72.5±6.8 years,median injury severity score(ISS)of 24(IQR:17-29),and Glasgow coma score(GCS)of 14(IQR:7-15).Of these patients,129(34.8%)were diagnosed with TIC,whereas 242(65.2%)were diagnosed with *** severity scores such as ISS(25[20-34]vs.21[16-29],P16(OR:3.404,95%CI:1.471-7.880;P=0.004),SI>1(OR:5.641,95%CI:1.700-18.719;P=0.005),low BE(OR:0.868,95%CI:0.760-0.991;P=0.037),hypocalcemia(OR:0.060,95%CI:0.009-0.392;P=0.003),and hypofibrinogenemia(OR:0.266,95%CI:0.168-0.419;P<0.001)were independent risk facto

Development of extramural health care for chronic wounds is still in its infancy in China,and thus it is urgent and vital to establish a correct concept and practicable *** authors reviewed recent domestic and international literature and summarized the following treatment procedures and principles for extramural health care of chronic wounds.(1)The patient needs to do self-assessment of the wound by using available simple methods;(2)The patient consults with professional physicians or nurses on wound care to define the severity and etiology of the non-healing wound;(3)Professionals evaluate the existing treatment strategies;(4)Etiological treatments are given by professionals;(5)Patients buy needed dressings via the more convenient ways from pharmacies,e-commerce platform or others;(6)Professionals provide a standardized and reasonable therapeutic plan based on the patient's wound conditions;(7)Both professionals and the patient pay attention to complications to prevent adverse outcomes;(8)Professionals strengthen the public education on wound care and integrated *** review expected to provide new perspectives on the therapeutic strategies for chronic wounds in an extramural setting.

severe trauma has the characteristics of complicated condition, multiple organs involved, limitq auxiliary examinations, and difficulty in treatment. Most of the trauma patients were sent to prima hospitals to receive treatments. But the traditional mode of separate discipline management can easi lead to delayed treatment, missed or wrong diagnosis and high disability, which causes a high mc tality in severe trauma patients. Therefore, if the primary hospitals, especially county-level hospit~ （usually the top general hospital within the administrative region of a county）, can establish a sc entific and comprehensive trauma care system, the success rate of trauma rescue in this region can ] greatly improved. On March 1st, 2013, Tiantai People＇s Hospital of zhejiang province, China set up trauma care center, which integrated the pre-hospital and in-hospital trauma treatment procedur~ and has achieved good economic and social benefits. Till March 1st, 2017, 1265 severe trauma patier （injury severity score 〉16） have been treated in this trauma center. The rescue success rate reach~ 95~ and the delayed and]or missed diagnosis rate was less than 5%. Totally 86 severe cases of peK fractures with unstable hemodynamics were treated, and the success rate was 92%. The in-hospil emergency rescue response time is less than 3 min, and the time from definite diagnosis to surge is within 35 min.

Invasive fungal infection is one of the major complication of severe burns which can induce local or systemic inflammatory response and cause serious substantial damage to the patient. The incidence of fungal infection for burn victims is increasing dramatically during recent years. This guideline, organized by Chinese Society of burn Surgeons, aims to standardize the diagnosis, prevention and treatment of burn invasive fungal infection. It can be used as one of the tools for treatment of major burn patients.
