Kang-Ai Injection Inhibits Gastric Cancer Cells Proliferation through IL-6/STAT3 Pathway

Objective:To explore the mechanisms underlying the proliferative inhibition of Chinese herbal medicine Kang-Ai injection(KAI)in gastric cancer ***:Gastric cancer cell lines MGC803 and BGC823 were treated by 0,0.3%,1%,3%and 10%KAI for 24,48 and 72 h,*** cell proliferation was evaluated by 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide(MTT)*** apoptosis and cell cycle were evaluated by flow ***(IL)-6 mRNA and protein expression levels were detected by quantitative real-time polymerase chain reaction(qRT-PCR)and enzyme-linked immune sorbent assay(ELISA),*** protein expression levels of cyclin A,cyclin E,cyclin B1,cyclin D1,p21,retinoblastoma(RB),protein kinase B(AKT),extracellular regulated protein kinases(ERK),signal transducer and activator of transcription(STAT)1 and STAT3 were detected by Western ***:KAI inhibited the proliferation of MGC803 and BGC823 gastric cancer cells in dose-and time-dependent *** treated with KAI for 48 h,the proportion of G,phase was increased,expression level of cyclin D1 and phosphorylation-RB were down-regulated,whereas the expression of p21 was up-regulated(all P<0.01).Furthermore,48-h treatment with KAI decreased the phosphorylation level of STAT3,inhibited the mRNA and protein expressions of IL-6(all P<0.01).IL-6 at dose of 10 ng/mL significantly attenuated the proliferative effect of both 3%and 10%KAI,and recovered KAI-inhibited STAT3 phosphorylation and cyclin D1 expression level(all P<0.01).Conclusion:KAI exerted an anti-proliferative function by inhibiting IL-6/STAT3 signaling pathway followed by the induction of G_(1)phase arrest in gastric cancer cells.

Increasing evidence has indicated that current tumor-node-metastasis(TNM) stage alone cannot predict prognosis and adjuvant chemotherapy benefits accurately for stage Ⅱ and Ⅲ gastric cancer(GC)patients after surgery. In order to improve the predictive ability of survival and adjuvant chemotherapy benefits of GC patients after surgery, this study aimed to establish an immune signature based on the composition of infiltrating immune cells. Twenty-eight types of immune cell fractions were evaluated based on the expression profiles of GC patients from the Gene Expression Omnibus(GEO) database using single-sample gene set enrichment analysis(ssGSEA). The immunoscore(IS) was constructed using a least absolute shrinkage and selection operator(LASSO) Cox regression model. Through the LASSO model, an IS classifier consisting of eight immune cells was constructed. Significant difference was found between high-IS and low-IS groups in the training cohort in DFS(P < 0.0001)and OS(P < 0.0001). Multivariate analysis showed that the IS classifier was an independent prognostic indicator. Moreover, a combination of IS and TNM stage exhibited better prognostic value than TNM stage alone. Further analysis demonstrated that low-IS patients who had more tumorinfiltrating lymphocytes had better response to adjuvant chemotherapy. More importantly, we found that patients with high-IS were more likely to benefit from a Xeloda plus cisplatin regimen after surgery. Finally, we established two nomograms to screen the stage Ⅱ and Ⅲ GC patients who benefitted from adjuvant chemotherapy after surgery. The combination of IS classifier and TNM stage could predict disease-free survival(DFS) and overall survival(OS) of GC patients. The IS model has been proven as a promising tool that can be used to identify the patients with stage Ⅱ and Ⅲ GC who may benefit from adjuvant chemotherapy.

Objective The prognostic value of PD-L1 in gastric cancer remains controversial, likely due to the complex of immune response involved, and the diversity of PD-L1 expression in gastric cancer cells. In addition to membraneassociated PD-L1, extracellular soluble and exosomal PD-L1 are also secreted in the microenvironment. Previous studies are limited to membrane-associated and soluble PD-L1. Meanwhile, the predictive effect of tumor-derived exosomal PD-L1 is unclear. Design We evaluated the prognostic value of Exosomal PD-L1 in the plasma of gastric cancer patients and the effect of Exosomal PD-L1-derived from gastric cancer cell lines. Results Exosomal PD-L1 was detected in plasma samples from 105 gastric cancer patients, and exosomal PD-L1 content was significantly associated with T stage(P=0.045). Although low exosomal PD-L1 resulted in better prognosis, statistical significance was not achieved(P=0.097). Overall Survival was significantly lower in the high exosomal PD-L1 group within two years after operation(p=0.037). Additionally, gastric cancer cells also secreted exosomal PD-L1, with the amounts positively associated with PD-L1 amounts in the corresponding GC cell lines. Besides, exosomal PD-L1 was more stable and showed stronger immunosuppressive effects in the microenvironment compared with soluble PD-L1. Conclusion Exosomal PD-L1 might predict long-term survival in GC, and induces higher T-cell apoptosis levels compared with soluble PD-L1.

Background: Good performance status is widely known as a superior prognostic *** purpose of this study was to explore baseline host-or tumor-related factors and to establish a nomogram-based prediction of survival in unresectable or metastatic gastric cancer(GC) patients with good performance status who received first-line ***: 309 patients from April 2007 and December 2013 at the First Hospital of China Medical University were involved in this study. We used a credible random split-sample method to divide patients into the training cohort(259 patients,80%) and the internal validation cohort(50 patients, 20%). Prognostic significance was determined in the training cohort using multivariate Cox regression analysis, and then was represented as a prognostic nomogram. We validated the nomogram in the internal validation cohort and the external cohort including 147 patients from January 2014 to October 2016. Results: The nomogram includes three covariates(the number of metastatic sites, fibrinogen and CA19-9) correlated significantly with survival. The median survival of the training cohort was 10.1 months(95% CI, 9.295–10.944). The nomogram showed good discrimination, with a c-index of 0.623(95% CI, 0.579-0.667). When applied to the internal validation and the external validation cohort, the nomogram achieved c-indices of 0.614(95% CI, 0.511-0.717) and 0.638(95% CI, 0.577-0.699), respectively. Calibration plots suggested that the nomogram was well calibrated for all ***: We developed and validated a reliable nomogram to predict the overall survival of unresectable or metastatic GC patients with good performance status who received first-line chemotherapy.

Objective : To investigate the association of Ki-67 with pathological characteristics and prognostic value in different genotype of NSCLC patients. Methods :347 cases of unresected local advanced or metastatic NCSCL patients were selected from the first affiliated hospital of China Medical University. Data about pathological features and survival were collected respectively. Genetic test results were achieved from plasma samples or tumor tissue samples by high throughput sequencing and Ki-67 proliferation index were analyzed by immunohistochemistry retrospectively. The relationship between Ki-67 and pathological features(such as gender, age, pathological type, tumor size, lymphatic metastasis, TMM stage, ECOG score and different gene status) was examined by Spearman’s rank correlation. Survival curves were calculated by the Kaplan-Meier *** analyses were performed using the Cox regression model to study factors that independently influenced overall survival. P<0.05 was considered statistically significant. Results :Ki-67 expression was significantly corrected with gender(61% in male vs 47% in female, P=0.009),tumor histology(82% in squamous cell cancer vs 51% in non-squamous cell cancer, P<0.001), and EGFR gene status(60% in wild type vs 48% in EGFR gene mutant type, P=0.037) in NSCLC *** survival analysis, low Ki-67 expression indicated better survival for PFS and OS in EGFR wild type(mPFS:6.0 months vs 5.0 months, P=0.004;mO S: not reached vs 27.0 months, P=0.020) 、 EGFR mutant type(mPFS:12.0 months vs 7.0 months, P<0.001;mO S: not reached vs 48.0 months, P=0.002),KRAS gene wild type(mPFS:10.0 months vs 5.0 months, P=0.024;mO S:53.0 months vs 24.0 months,P=0.031), PIK3 CA gene wild type(mPFS:7.0 months vs 5.0 months, P=0.015;mO S:not reached vs19.5 months, P=0.007) and PTEN gene wild type(mPFS:11.0 months vs 6.0 months, P=0.005;mO S:not reached vs 40.0 months, P=0.020) groups Furthermore, Ki-67 expression can also play as a great predictive
