BACKGROUND ground Aspiration and evaluation of gastric residuals are commonly performed interventions before each feeding in intensive care units,especially in very low birthweight ***,there is no sufficient evidence about the necessity of routine gastric residual *** this study,we aimed to investigate the time to full enteral intake and the incidence of necrotizing enterocolitis(NEC)in preterm infants in the period with gastric residual aspiration performed before each feeding,and those in the period without gastric residual *** Preterm infants with a gestational week≤33 were included in the *** group with gastric residual control before each feeding consisted of 169 infants,and the group without routine gastric residual aspiration included 122 *** The mean gestational week was 30.37±2.58 and 29.31±3.37 in the group with gastric residual control and in the group without routine residual control,respectively(p<0.05).Birth weight,male gender,and mode of delivery were similar between both *** time to full enteral intake was shorter in the group without routine residual control(p<0.05).Total durations of parenteral nutrition,≥grade 2 NEC,weight at discharge and duration of hospitalization were similar between the *** of invasive mechanical ventilator support was shorter in the group without routine residual *** Avoidance of routine gastric residual aspiration in preterm infants shortens the time to full enteral intake without increasing the incidence of NEC.

Right lobe living donor liver transplantation(LDLT)is a major development in adult LDLT that has significantly increased the donor pool by providing larger graft size and by decreasing risk of small-for-size graft ***,right lobe anatomy is complex,not only from the inflow but also from the outflow *** reconstruction is one of the key requirements of a successful LDLT and venous drainage of the liver graft is just as important as hepatic inflow for the integrity of graft *** complications may cause acute graft failure which is not always easy to *** right lobe graft consists of two sections and three hepatic venous routes for drainage that require *** order to obtain a congestion free graft,several types of vascular conduits and postoperative interventions are needed to assure an adequate venous allograft *** review described the anatomy,functional basis and the evolution of outflow reconstruction in right lobe LDLT.

Pulmonary hypertension (PH) is a haemodynamic and pathophysiological condition defined as increase in mean pulmonary arterial pressure ≥ 25 mmHg at rest as assessed by right heart catheterization (RHC). It can be due to a primary elevation of pressure in the pulmonary arterial system alone (pulmonary arterial hypertension), or secondary to elevations of pressure in the pulmonary venous and capillary systems (pulmonary venous hypertension). PH can be a progressive, fatal disease if untreated, although the rate of progression is highly variable.

Solid-pseudopapillary tumor of the pancreas (SPTP) is an uncommon low grade exocrine pancreatic malignancy. We represented a 22 years old female with an abdominal mass of 4 years history and symptoms of weakness, loss of appetite, 7 kilograms weight loss and swelling for almost 2 months. She was suspected of pancreatic malignancy due her abdominal ultrasound and CT reports and therefore underwent explorative surgery that revealed a huge pancreatic tumor .The tumor was resected totally and hystopathological examination reported significant components of pancreatic solid pseudopapiller tumor features which was also the final diagnosis.

*** aimed to evaluate the relationships between acne severity, anxiety, depression and disease-specific quality of life in patients with ***.A total of 61 patients with acne vulgaris and 38 healthy volunteers were included in the *** severity was assessed by the Global Acne Grading *** patients were asked to complete the Acne Quality of Life Scale (AQOL), Dermatology Life Quality Index (DLQI) and Hospital Anxiety and Depression Scale (HAD), and healthy controls to fill only the *** mean HAD anxiety subscale (HAD-A) and HAD depression subscale (HAD-D) scores of the patients were significantly higher than those of the *** rates of subjects at risk for anxiety (26.2%) and for depression (29.5%) were significantly higher in the patient group than in the control group (0%and 7.9%, respectively).We found no correlations between acne severity and scores of AQOL, DLQI, HAD-A and *** and DLQI scores were positively correlatedwith HAD-Aand HADD scores in the patient *** patients at risk for anxiety had significantly higher scores on AQOL and DLQI compared to those who were not at *** were no statistically significant differences between the female and male patients with respect to AQOL, DLQI, HAD-A and HAD-D ***-sion.(1) Irrespective of the degree of severity, patients with acne are at increased risk for anxiety and depression compared to the normal population.(2) Acne negatively affects quality of life, and the greater the impairment of quality of life due to acne, the greater the level of anxiety and depression.(3) A greater impairment of dermatologic quality of life seems to put the patient at an increased risk for anxiety disorder.

Purpose: To investigate the risk factors that influence outcome of visual function in children with glaucoma. Design: Retrospective noncomparative interventional case series. Participants: One hundred twenty-six patients (204 eyes) who had childhood glaucoma observed over 30 years,with a mean follow-up of 11.6 years. Interventions: Full ophthalmologic examination,including measurement of corrected visual acuity (VA),slit-lamp and fundus examinations,intraocular pressure (IOP) measurement,and gonioscopic evaluation; periodic cycloplegic refraction and perimetry; and treatment of amblyopia. Main Outcome Measures: Type of glaucoma; final best-corrected VA of good (6/6-6/12),fair (6/15-6/30),or poor (≤ 6/60); patient age at time of development of glaucoma complications; and percentage of IOP measurements of ≤ 19 mmHg,perimetry results,and cup-to-disc (C/D) ratio during follow-up. Results: The most recently measured VAs of children treated for glaucoma were good in 29% ,fair in 24% ,and poor in 47% . The most favorable outcome was for patients with primary infantile glaucoma followed by secondary glaucoma. Amblyopia and optic nerve damage due to glaucoma were the most frequent complications affectingVA. Patients with an IOP of ≤ 19mmHg on 80% of determinations had stable optic nerve C/D ratios and visual fields. Conclusions: Vision sufficient to qualify for a motor vehicle driving license was attainable in almost 30% of affected eyes. Visual acuity achieved at 6 years of age remained stable over the study period. Treatment of amblyopia is important to achieve this result.

Objective: To differentiate benign from malignant ovarian tumors based on sonographic detection of a solid component. Method: Sixty-three women with ovarian masses were evaluated preoperatively by grayscale and power/color Doppler ultrasonographic examination, with specific predefined criteria for the solid component. Sensitivity, specificity, and positive and negative predictive values were calculated and assessed against the histopathologic outcome. The contribution of cancer antigen (CA) 125 levels to the diagnostic accuracy was also assessed. Result: Sensitivity, specificity, and positive and negative predictive values were 100%, 95.2%, 91.3%and 100%, respectively, with two false-positive results. Had an elevated CA 125 level ( > 35 U/mL) been included in the malignancy criteria, the false-positive results would have been eliminated, giving an accuracy of 100%. Conclusion: Sonographic evaluation with predefined specific criteria for the detection of a solid tumor component is an accurate method of preoperative discrimination between benign and malignant ovarian tumors. A serum CA 125 assay may assist in eliminating false-positive results.

Background. Although many histopathological, clinical, and imaging findings of ESS have been described, intense calcifications have not been reported previously. Case. A 54- year- old female was admitted with vaginal bleeding and abdominal pain. Gynecologic examination revealed a huge, firm pelvic mass extending up to the epigastrium. Transabdominal ultrasonography and computed tomography demonstrated a well-demarcated uterine mass containing heterogeneous solid and cystic areas and a centric curvilinear calcification. Histopathological diagnosis was established as undifferentiated high-grade endometrial stromal sarcoma. Conclusion. Endometrial stromal sarcomas should be considered in the differential diagnosis of uterine masses containing curvilinear calcifications.

偏头痛发作期间的QTc离散度和P波离散度

The aim of this study was to investigate increase of QTc dispersion and P-wave dispersion during migraine attacks. Fifty-five patients (16-65 years of age, 49 women, six men) with migraine were included in our study. Heart rate, QTc interval, maximum and minimum QTc interval, QTc dispersion, maximum and minimum P-wave duration and P-wave dispersion were measured from 12-lead ECG recording during migraine attacks and pain-free periods. ECGs were transferred to a personal computer via a scanner and then used for magnification of ×400 by Adobe Photoshop software. Maximum QTc interval (454 ±24 ms vs. 429 ±23 ms, P < 0.001), QTc interval (443 ±26 ms vs. 408 ±22 ms, P < 0.001) and QTc dispersion (63 ±18 ms vs. 43 ±14 ms, P < 0.001) were found significantly higher during migraine attacks compared with pain-free periods. Maximum P-wave duration (107 ±11 ms vs. 100 ±11 ms, P < 0.001) and P-wave dispersion (45 ±13 ms vs. 35 ±13 ms, P < 0.001) were found higher during migraine attacks than pain-free periods. We concluded that migraine attacks are associated with increased QTc and P-wave dispersion compared with pain-free periods.
