Although almost all primary colorectal lymphomas are of B-cell lineage in Western countries, primary colorectal T-cell lymphomas are not uncommon in the East. The aim of this study was to review the clinical characteristics and treatment outcomes of primary colorectal lymphomas, with special emphasis on the differences between T-cell and B-cell lymphomas. Ninety-five cases of primary colorectal lymphomas that satisfied Dawson‘s criteria were identified from the clinical databases of 13 university hospitals in Korea. The mean age at the time of presentation was 51.1 years and the male : female ratio was 64 ∶31. The clinical information, including endoscopic and histological characteristics, was retrospectively analyzed. Of the primary colorectal lymphomas, 78 cases (82.1%) were of B-lineage and 17 cases (17.9%) were of T-cell lineage. Patients with T-cell lymphomas presented at a younger age than patients with B-cell lymphomas (42.8 vs 52.9 years, respectively; P = 0.016). The most common presenting symptom was abdominal pain (87.1%) for B-cell lymphomas, whereas hematochezia or night fe ver was more common for T-cell lymphomas (52.9%and 35.3%, respectively). The most common endoscopic type was fungating mass (54.0%) for B-cell lymphomas and ulcerative/ ulcero-infiltrative lesions (80.0%) for T-cell lymphomas. Intussusception was more common in B-cell lymphomas than in T-cell lymphomas(30.8%vs 5.9%, respectively; P = 0.035), but perforation was more common in T-cell lymphomas than in B-cell lymphomas (23.5%vs 3.8%, respectively; P = 0.005). The prognosis was significantly worse for T-cell lymphomas than for B-cell lymphomas (P = 0.002). Primary colorectal T-cell lymphomas are characterized by multifocal ulcerative lesions in relatively young patients , a high rate of hematochezia, fever, or perforation, and a poor prognosis even for cases of localized disease.

To quantify mitochondrial DNA using real-time PCR in women with premature ovarian failure (POF) and a control group. Design: Prospective study. Setting: Genome Research Center for Reproductive Medicine and Infertility, Korea Ministry of Health & Welfare. Patient(s): Thrity patients with POF and 30 control individuals. Interventions(s): The mitochondrial DNA content was quantified using real-time PCR. The effectiveness of the assay was determined by relative quantification using the comparative threshold cycle (CT) method. Main Outcome Measure(s): Relative quantification of mitochondrial DNA content. Interventions(s): The mitochondrial DNA content was significantly lower in the POF group than in the control group (0.58 ± 0.38 vs. 1.15 ± 0.67; P < .01). In both groups, there was a significant positive correlation between the mitochondrial DNA/28S rRNA ratio and mitochondrial DNA CT (control group: r = 0.774; P < .001; POF group: r = 0.556; P = .001) and a significant negative correlation between the mitochondrial DNA/28S rRNA ratio and 28S rRNA CT (control group: r = - 0.677; P < .001; POF group: r = - 0.627; P = .001). Conclusion(s): This study has established the clinical feasibility of quantifying amounts of mitochondrial DNA, relative to an internal standard, using real-time PCR. Further studies are warranted to elucidate the roles of apoptosis and mitochondrial function in the pathogenesis of POF.

目的对于先前化疗耐受的非小细胞肺癌病人,评价为突变分析而进行重复活检的可行性和安全性。材料与方法这项前瞻性研究获得单位伦理委员会批准,并取得所有病人的知情同意书。选择126例对常规化疗或表皮生长因子受体(EGFR)-酪氨酸激酶抑制剂耐受的非小细胞肺癌病人,对其中94例病人[男31例,女63例,(57±10.3)岁]进行重复活检。32例病人在严格观察胸部CT影像后因各种原因被排除。采用C臂锥形束CT导向进行经皮胸穿肺活检。评价重复活检的技术成功率和进行突变分析的标本充足率。记录任何与活检相关的并发症。结果活检的技术成功率为100%。94例病人中的75例(80%)标本已足以进行突变分析。75例标本中,35例进行了EGFR突变检测,34例进行了间变性淋巴瘤激酶(ALK)基因重组检测,6例两者都进行。结果显示,20例EGFR光敏突变(外显子19或21)为阳性,5例出现EGFRT790M突变,而11例发现间变性淋巴瘤激酶基因重组。94例病人中13例(14%)出现术后并发症。结论当采用严格的CT标准进行操作时,对先前进行化疗的非小细胞肺癌病人进行突变分析时,重复活检是可行的,大约4/5的病人样品足够进行基因分析,而并发症的发生率可以接受。

Aim of the study: Several studies have shown the inferior performance of small prostheses in the narrowaortic root. However, modern low-profile mechanical prostheses have improved hemodynamic performance characteristics. By measuring the transprosthetic pressure gradient in vivo, we were able to characterize the hemodynamic features of two prostheses: the ATS Medical(ATS) and the Medtronic Hall(MH) valves. Methods: From October 1994 to April 2002, 113 patients received an aortic valve replacement(AVR) with either an ATS or a MH valve. The transprosthetic pressure gradients, calculated froma simplified Bernoulli equation during immediate postoperative Doppler echocardiographic examination, were compared for differently sized prostheses with respect to body surface area(BSA). Results: The mean pressure gradients and the mean BSAs were: 27.8±14.8 mm Hg and 1.50±0.10 m2 in ATS 19 mm(n=7), 20.4±8.5 mm Hg and 1.54±0.11 m2 in ATS 21 mm(n=22), 13.0±5.7 mm Hg, 1.70±0.13 m2 in ATS 23 mm(n=22), 10.9±3.5 mm Hg and 1.81±0.16 m2 in ATS 25 mm(n=19), 9.3±0.6 mm Hg and 1.72±0.17 m2 in ATS 27 mm(n=4), 13.5±6.5 mm Hg and 1.54±0.13 m2 in MH 20 mm(n=9), 10.9±4.7 mm Hg and 1.64±0.15 m2 in MH 22 mm(n=22), 9.3±3.1mmHg and 1.72±0.12 m2 in MH 24 mm(n=7). Conclusions: With the exception of the ATS 19mm valve, the variously sized prostheses have acceptable transprosthetic pressure gradient measurements. In addition, even-sized MH valves(20 and 22 mm) with a thinner sewing cuff showed better hemodynamic performances than similarly sized ATS valves.

Recently, the importance of plant maintenance (PM) was raised to provide efficient plant operation which highly affects the life extension as well as the productivity. For this reason, a number of engineering methodologies, such as fitness for service guidelines (FFS), plant lifecycle management (PLM) system, have been applied to improve the plant operation efficiency. Also, a network-based business operation system, which is called ERP (Enterprise Resource Planning), has been introduced in the field of plant maintenance. However, there hasn’t been any attempt to connect engineering methodologies to the ERP PM(Plant Maintenance) system. In this paper, a knowledge-based information system for the plant operation of steel making company has been proposed. This system, which is named as K-VRS(Knowledge-based Virtual Reality System), provides a connection between ERP PM module and knowledge-based engineering methodologies, and thus, enables network-based highly effective PM process. While the virtual plant is used for the management master of K-VRS, there are four expert modules attached;engineering document management module, real time lifetime estimation module, agent module and fitness for service module. Each module provides engineering knowledge based evaluation and judgment for more effective plant maintenance. K-VRS provides collaborative and concurrent working environment to workers and relevant experts. The developed system is expected to play a great role for plant life extension.

Partial hepatectomy has long been the standard treatment modality for patients with hepatocellular carcinoma(HCC),although the majority of patients with HCCs are not candidates for curative *** ablation(RFA) has been widely used as the preferred locoregional *** and hepatectomy can be complementary to each other for the treatment of multifocal *** hepatectomy with RFA permits the removal of larger tumors while simultaneously ablating any smaller residual *** using this combination treatment,more patients might become candidates for curative *** treating recurrent tumors involving the liver after hepatectomy,RFA has been performed recently instead of transcatheter arterial chemoembolization or ethanol *** retrospective studies on the combination of RFA and hepatectomy demonstrate favorable results of effectiveness and ***,further investigation of prospective design will be needed to confirm these encouraging results.
